Incidental Detection of Glutamate Formiminotransferase Deficiency Through Newborn Screening in a Clinically Asymptomatic Infant: Molecular Findings and Counseling Considerations.

We report a seven-week-old male infant, born to consanguineous (double cousin) parents, incidentally through routine newborn screening, following an abnormal acylcarnitine profile caused by elevated formiminoglutamate (FIGLU), with subsequent biochemical confirmation of glutamate formiminotransferase cyclodeaminase (FTCD) deficiency. The infant was born at 37 weeks' gestation via spontaneous vaginal delivery, was vigorous at birth, and required no resuscitation. He remained clinically asymptomatic at the time of metabolic and genetic evaluation. Plasma acylcarnitine analysis demonstrated eleva

Ovarian reserve and oxidative stress in sickle cell anaemia: a comparative cross-sectional study.

Sickle cell anaemia (SCA), the most common monogenic disorder, affects 20-25 million people globally and is prevalent in Sub-Saharan Africa, especially Nigeria, where the prevalence is 1-3%. Beyond its haematological complications, SCA is associated with chronic anaemia and oxidative stress, which may adversely affect ovarian function; however, this relationship remains poorly understood. This study aimed to evaluate ovarian reserve in women with SCA using Anti-Mullerian Hormone (AMH) and Antral Follicle Count (AFC) and assess the role of oxidative stress. This was a comparative cross-sectiona

Role of FOXO3 in regulation of redox homeostasis and promotion of red blood cell longevity in sickle cell disease.

Sickle cell disease (SCD) is a severe monogenic red blood cell (RBC) disorder that follows a simple Mendelian mode of inheritance. SCD is characterized by hemoglobin S polymerization under deoxygenated conditions, leading to chronic hemolysis and vaso-occlusive complications. Increasing evidence indicates that oxidative stress plays a central pathogenic role in SCD complications. Forkhead box O3 (FOXO3) is a key transcription factor involved in different cellular mechanisms, including apoptosis, autophagy, and cytoprotective signaling cascades. FOXO3, through its transcriptional modulation of

Bovine Lactoferrin Compared With Ferrous sulfate for Treating Iron-Deficiency Anemia in Bangladeshi Women-A Randomized Controlled Trial.

BACKGROUND: Lactoferrin is an iron-binding glycoprotein present in secretions, including breast milk, with anti-inflammatory and antimicrobial properties. Previous studies suggest bovine lactoferrin (bLF) may improve hemoglobin and iron status; however, most evidence comes from high-income countries, and data from resource-limited settings remain limited. OBJECTIVES: This study aimed to determine whether oral bLF is noninferior to ferrous sulfate for improving hemoglobin among nonpregnant women with iron-deficiency anemia (IDA) in Bangladesh. We hypothesized that 200 mg or 400 mg bLF would be

VEXAS (Vacuoles, E1 Enzyme, X-linked, Autoinflammatory, Somatic) Syndrome in a Patient Presenting With Auricular Chondritis.

VEXAS (vacuoles, E1 enzyme, X-linked, autoinflammatory, somatic) syndrome is a recently described autoinflammatory disorder caused by somatic UBA1 mutations in hematopoietic stem and progenitor cells. It typically presents in older males with treatment-refractory systemic inflammation and hematologic abnormalities. We report a case of a 92-year-old male who presented with auricular chondritis and other systemic symptoms, including inflammatory arthritis, conjunctivitis, and a diffuse rash. Laboratory evaluation revealed macrocytic anemia and elevated inflammatory markers, with largely negative

Heme and hemozoin induce platelet cell death through UPR-induced apoptosis and ferroptosis in vivax malaria.

Malaria is a highly prevalent infectious disease caused by Plasmodium parasites. Plasmodium intraerythrocytic replication leads to hemolysis-driven intermittent febrile crises in patients. In addition, the lysis of unparasitized red blood cells contributes to anemia and endotoxemia. Because thrombocytopenia is an important feature of vivax and severe falciparum malaria, we hypothesized that increased hemolysis in malaria contributes to severe thrombocytopenia by releasing endogenous and parasite toxins (ie, heme and hemozoin) capable of inducing programmed cell death in platelets. Using comple

Folic acid supplementation and malaria susceptibility and severity among people taking antifolate antimalarial drugs in endemic areas.

BACKGROUND: Malaria is an infectious disease transmitted by female Anopheles mosquitoes, with ongoing transmission in over 80 countries. The malaria parasite requires folate for survival and growth; antifolate antimalarial medications used for prevention and treatment target enzymes in folate metabolism as their mechanism of action. Periconceptional folic acid (synthetic form of folate) supplementation (400 μg/day) is the standard of care for neural tube defect prevention. Concerns have been raised about the potential effects of folic acid (including above the tolerable upper intake level (UL)

Diagnostic potential of a multi-antigen ELISA for feline leishmaniosis.

BACKGROUND: Leishmania infantum is a sand fly-transmitted zoonotic protozoan, endemic in the Mediterranean basin and responsible for human, canine (CanL), and feline (FeL) leishmaniosis. While dogs are the primary reservoir host, a growing number of FeL cases have been reported in this region despite the absence of pathognomonic clinical signs and limited diagnostic tools. Herein, we evaluate the performance of seven serological tools for CanL in detecting antibodies to Leishmania in cats, aiming to improve FeL diagnosis. METHODS: Five ELISAs based on Leishmania-specific antigens (soluble pro

Hookworm infestation in the biliary tract: a case report.

BACKGROUND: Hookworms are parasitic nematodes that primarily infect humans in impoverished communities. They attach to the intestines to feed on blood, which results in anemia and other health complications. While parasitic infections of the biliary tract can occur and lead to significant issues such as cholangitis, pancreatitis, and biliary obstruction, hookworms are not documented to be associated with these biliary infestations. CASE PRESENTATION: A 53-year-old female Tibeto-Burman ethnicity following Buddhism, with a significant smoking history and no alcohol consumption presented to Trib

Rapidly Progressive Cytophagic Histiocytic Panniculitis in a Child Triggered by Trauma and Scrub Typhus: A Dramatic Response to Cyclosporine.

Cytophagic histiocytic panniculitis (CHP) is a rare and potentially life-threatening inflammatory panniculitis characterized by lobular lymphohistiocytic infiltration and cytophagic "bean-bag" histiocytes. Because its early features overlap with infectious panniculitis and hemophagocytic lymphohistiocytosis (HLH), diagnosis is often delayed, contributing to significant morbidity. We describe a rapidly progressive pediatric case of CHP triggered by antecedent trauma and scrub typhus infection. A 10-year-old girl presented with seven days of high-grade fever and painful erythematous nodules over

Beyond Hemolysis: Babesiosis-Associated Atraumatic Splenic Infarction in an Immunocompetent Patient.

Babesiosis is a protozoan infection transmitted by the tick, Ixodes scapularis. The disease presentation varies from asymptomatic to fatal illness. Usually, severe illness occurs in asplenic and immunocompromised patients. In rare cases, the complication of splenic injury, including infarcts and rupture, has been observed in young individuals. We present the case of a middle-aged woman who presented with abdominal pain and fatigue. She was found to be febrile and hypotensive with thrombocytopenia, hemolytic anemia, and elevated transaminases. Imaging of the abdomen demonstrated multiple spleni

IL-1-targeted therapy in dermatologic conditions.

Interleukin-1 (IL-1) plays a key role in inflammasome activation, keratinocyte signaling and neutrophil recruitment, and is a driving force behind many neutrophil-rich and suppurative dermatoses. There is robust evidence supporting the use of IL-1 blockade as a disease-modifying therapy in cryopyrin-associated periodic syndromes and other monogenic periodic fever syndromes, as well as in the treatment of Schnitzler syndrome, where it can rapidly control urticarial and neutrophilic eruptions. Cohort- and series-level data demonstrate high efficacy in IL-1 receptor antagonist deficiency and ster

Prevalence of Helicobacter pylori Exposure and Risk Factors Among BRCA1 and BRCA2 Carriers.

Recent evidence suggests that carriers of a pathogenic germline variant (PGV) in BRCA1 or BRCA2 may have an increased risk of gastric cancer (GC). However, the specific mechanisms underlying gastric carcinogenesis remain uncertain. Strikingly, recent data have demonstrated that Japanese BRCA1 and BRCA2 PGV carriers with Helicobacter pylori (Hp) exposure have a substantially increased risk of GC compared to non-carriers with Hp exposure, indicating that Hp may be an important risk factor for BRCA1 and BRCA2-associated GC. However, rates of Hp exposure amongst broader BRCA1 and BRCA2 PGV cohorts

Atypical Presentation of Pediatric Cat Scratch Disease: A Case Report.

Cat scratch disease is a zoonotic infection caused by Bartonella henselae and typically presents with regional lymphadenopathy and mild systemic symptoms in immunocompetent children. Atypical disseminated disease with visceral involvement is uncommon and may occur without classic clinical findings, leading to delayed diagnosis, particularly in patients with altered immune responses. We describe the case of an 11-year-old girl with type 1 diabetes mellitus who presented with persistent fever and progressive abdominal pain. Initial evaluation suggested streptococcal pharyngitis, and she was trea

Distinguishing Leptospirosis from Other Causes of Acute Encephalitis Syndrome: Clinical Insights.

INTRODUCTION: Acute encephalitis syndrome (AES) is a serious public health problem in India, especially in Uttar Pradesh, where children and adolescents are particularly affected. Although viral diseases such as Japanese encephalitis (JE) are the main cause, many cases remain unexplained. AES could be caused by a zoonotic leptospirosis, but its role is uncertain. AIM: The aim is to assess the prevalence and clinical features of leptospirosis in children with AES in a tertiary care hospital and compare the etiology with other causes. MATERIALS AND METHODS: Children aged 1-14 years with AES or

Aumolertinib with or without chemotherapy in EGFR-mutated advanced non-small-cell lung cancer (AENEAS2): an open-label, multicentre, randomised, controlled, phase 3 trial.

BACKGROUND: Although third-generation epidermal growth-factor receptor (EGFR)-tyrosine-kinase inhibitors (TKIs) are standard first-line therapies for patients with advanced EGFR-mutated non-small-cell lung cancer (NSCLC), their effectiveness is often limited by the emergence of drug resistance and subsequent disease progression. Given the previously established clinical efficacy and adverse event profile of aumolertinib, we aimed to evaluate the efficacy and adverse event profile of aumolertinib in combination with platinum-based chemotherapy versus aumolertinib monotherapy as first-line treat

Lysosomal metabolism in pancreatic cancer and its crosstalk with host physiology.

Transcription factors from the family of Signal Transducers and Activators of Transcription (STAT) are activated by numerous cytokines. Two members of this family, STAT5A and STAT5B (collectively called STAT5), have gained prominence in that they are activated by a wide variety of cytokines such as interleukins, erythropoietin, growth hormone, and prolactin. Furthermore, constitutive STAT5 activation is observed in the majority of leukemias and many solid tumors. Inactivation studies in mice as well as human mutations have provided insight into many of STAT5's functions. Disruption of cytokine
