Transition from Pediatric to Adult Care in Patients with Transfusion-Dependent Beta-Thalassemia in France: A National Study Concerning a Rare Disease.

Background/Objectives: Transfusion-dependent β-thalassaemia (TDT) is a lifelong condition requiring coordinated multidisciplinary care. In France, where the disease is rare, transition from pediatric to adult care remains poorly structured, potentially compromising adherence and long-term outcomes. Methods: This national retrospective study evaluated current transition practices and their clinical impact among young adults with TDT. Patients aged 20-25 years in December 2022 were identified from the national NaThalY registry. Those diagnosed and managed in France before age 15 were included. C

Efficacy and safety of thalidomide in β-thalassaemia: a systematic review and meta-analysis.

Thalidomide has shown promise as an adjunct therapy for β-thalassaemia, yet its effectiveness and safety are uncertain. This systematic review and meta-analysis evaluate the efficacy and safety of thalidomide in β-thalassaemia; in both transfusion-dependent and non-transfusion-dependent patients. Scopus, PubMed and Cochrane databases were searched for studies assessing the efficacy and safety of thalidomide in β-thalassaemia. Primary outcomes included transfusion response and haemoglobin increases whereas adverse events and effects on serum ferritin and spleen size were secondary outcomes. Stu

Clinical application of base editing for treating β-thalassaemia.

β-Thalassaemia is caused by reduced or absent production of β-haemoglobin1-4. Previously, we performed laboratory-scale electroporation of CD34+ haematopoietic stem and progenitor cells from patients with β-thalassaemia using a transformer base editor5,6. The aim was to target the binding motif of the transcription repressor BCL11A in the HBG1 and HBG2 promoters7 to reactivate fetal haemoglobin (HbF) production. Here we present results of a phase 1 clinical trial (ClinicalTrials.gov identifier: NCT06024876) of five patients who received autologous CD34+ cells modified using a transformer base

Red blood cell membrane proteome as a reporter of disease severity, transfusion impact and genetic background in transfusion-dependent β-thalassaemia.

Omics technologies have transformed research in haemoglobinopathies, yet the proteome of RBCs remains largely unexplored in transfusion-dependent thalassaemia (TDT). In this proteomic analysis, Red blood cell (RBC) membranes from 48 adults with TDT were compared with healthy controls. The resulting profile, corresponding to patient and donor RBCs, captured disease-linked features and exposure to donor blood. Certain differentially expressed proteins were plasma-absorbed or remnants of erythroid precursors, recapitulating findings in non-transfusion-dependent thalassaemia. Structural proteins w

Advances in CRISPR Base Editing: From Molecular Evolution to Therapeutic Applications in Genomic Medicine.

CRISPR-Cas9 systems revolutionized gene editing, but inherent drawbacks, namely DNA double-strand breaks (DSBs) and the difficulty of achieving precise repairs (due to low HDR efficiency), led researchers to invent new, more accurate gene editing tools. Base editing represents a significant leap forward, enabling targeted single-nucleotide conversions directly on the DNA without DSBs or donor templates. The core technology involves fusing catalytically dead or nickase Cas proteins to DNA deaminase enzymes. Cytosine base editors (CBEs) convert C•G to T•A pairs, while adenine base editors (ABEs)

Case Report: life-threatening haemothorax due to extramedullary haematopoiesis in a patient with Beta-thalassaemia major.

INTRODUCTION: Extramedullary haematopoiesis is a rare compensatory phenomenon occurring in haematologic disorders such as thalassaemia. While typically asymptomatic, it can rarely lead to life-threatening complications, including spontaneous haemothorax. CASE PRESENTATION: A 60-year-old man with beta-thalassaemia major and a history of extramedullary haematopoiesis presented to the emergency department with acute chest pain, dyspnoea, and hypotension. Imaging revealed a massive left haemothorax secondary to bleeding from a paravertebral extramedullary haematopoiesis mass. The patient was mana

Triple Iron Chelation in Transfusion Dependent Thalassemia: A Case Report.

Background: Iron overload and its associated complications are major concerns in patients with transfusion-dependent β-thalassaemia (TDT). Iron chelation is an important part of TDT therapy with monotherapy or dual iron chelation being the most commonly used strategies. Evidence regarding the efficacy and safety of triple iron chelation therapy remains limited. Case presentation: We present the case of a 21-year-old immigrant from the Middle East with TDT and a history of irregular transfusion management without chelation therapy, leading to clinically significant iron overload. She was succes

The effect of limited donor phenotyped blood transfusion on alloimmunisation in children with thalassaemia and sickle cell anaemia.

BACKGROUND: Chronic transfusion therapy has improved outcomes in children with Beta-Thalassaemia Major (BTM) and sickle cell disease (SCD). However, exposure to multiple blood transfusions introduces challenges such as alloimmunisation, which hinders further transfusions and makes finding matching units more difficult. As a measure to limit exposure of multi-transfused patients to multiple donors, the limited donor exposure program (LDEP) was introduced, where phenotyped blood from committed voluntary 3-5 donors is donated at intervals to each patient. In this study, we aimed to compare the fr

Neutrophil extracellular traps induced by activated platelets as a cause of neutrophil-platelet aggregation in β-thalassaemia/haemoglobin E patients.

The hypercoagulable state is a major contributor to thromboembolic events and mortality in β-thalassaemia. The mechanisms underlying platelet-induced neutrophil activation leading to immunothrombosis remain poorly understood. Three-dimensional confocal microscopy demonstrated that platelets induced neutrophil extracellular trap (NET) formation through P-selectin- or high mobility group box 1 protein (HMGB1)-mediated binding to P-selectin glycoprotein ligand-1 (PSGL-1) or receptor for advanced glycation end products (RAGE), respectively, on neutrophils. Molecular signalling pathways involved in

Epidemiology of imported malaria in an area of North-Eastern Italy (2016-2025): clinical focus on cerebral malaria.

BACKGROUND: In non-endemic settings, imported malaria poses diagnostic challenges due to non-specific presentations and lack of immunity, especially in visiting friends and relatives (VFRs). Focusing on the Western Friuli (Italy) observatory, this work aims to optimize triage by identifying early severity predictors. Through the analysis of clinical parameters, paediatric cerebral cases, and host genetics - including an illustrative case of HbS/β-thalassaemia - we evaluate the drivers of Plasmodium falciparum progression to improve patient management. METHODS: This retrospective study (2016-2

Outcomes with luspatercept in patients with β-thalassemia: a systematic review and meta-analysis.

β-thalassaemia is a genetic blood disorder characterized by ineffective erythropoiesis and chronic anaemia, often requiring lifelong red blood cell (RBC) transfusions. Luspatercept, an erythroid maturation agent, has emerged as a potential therapy to reduce transfusion burden in affected adults. To assess the clinical efficacy and safety of luspatercept in managing adult patients with β-thalassemia. We conducted a systematic review and meta-analysis of randomized controlled trials (RCTs) evaluating luspatercept in adult patients with β-thalassemia. A comprehensive search of PubMed, Scopus, Emb

Non-genotoxic transplantation and in vivo selection through epitope editing.

The short-term and long-term effects of genotoxic pre-transplant conditioning remain barriers to the broader application of haematopoietic stem/progenitor cell (HSPC) transplantation and gene therapies1-4. Although monoclonal antibodies targeting KIT have been proposed as alternatives to chemotherapy or radiotherapy5-7, their pharmacokinetics hinder clinical applications owing to the risk of depleting transplanted HSPCs. Here, to address this issue, we identified amino acid changes in the extracellular domain of KIT that disrupt the binding of two therapeutic monoclonal antibodies8,9, which im

Red lines and green lights: Gene therapy for inherited erythroid disorders beyond the haemoglobinopathies.

Gene therapy is revolutionizing treatment paradigms for inherited haematological and immunological conditions. Recent successes, including the United States Food and Drug Administration (FDA) approval of gene therapy products for sickle cell disease and beta-thalassaemia, highlight the translational path of gene therapies for erythroid-specific disorders. In contrast, gene therapy development for other inherited erythroid disorders remains largely preclinical. Here, we examine the emerging landscape of gene therapies for inherited non-haemoglobinopathy erythroid disorders, focusing on the stat

The Prevalence of Anxiety in Patients With Beta-Thalassaemia Major: A Systematic Review and Meta-Analysis.

AIM: Beta-thalassaemia major (BTM) is a chronic hemoglobinopathy associated with a substantial psychosocial burden. Reported rates of anxiety in BTM vary widely, hindering service planning and intervention design. We performed a systematic review and meta-analysis to estimate the pooled prevalence of anxiety in patients with BTM. DESIGN: A comprehensive search of PubMed/MEDLINE, Scopus and Web of Science (studies from 2005 to 30 April 2025) identified observational studies reporting anxiety prevalence in BTM. Pooled prevalence and 95% confidence intervals (CIs) were calculated using random-ef

Gene Therapy for β-Haemoglobinopathies: From Molecular Correction to Curative Medicine.

Background: β-haemoglobinopathies, including sickle cell disease and transfusion-dependent β-thalassaemia, are among the most common monogenic disorders worldwide and represent a major global health burden. Conventional treatments, such as blood transfusions, iron chelation, fetal haemoglobin induction, and allogeneic haematopoietic stem cell transplantation, have improved outcomes but remain limited by treatment-related toxicity, donor availability, and incomplete curative potential. Methods: A narrative literature review was conducted using PubMed up to 2025. Search terms included "sickle ce
