Sterile neutrinos at MAPP in the B - L model.

BACKGROUND: Congenital anomalies of kidney and urinary tract (CAKUT) are the leading causes of chronic kidney disease (CKD) in childhood. Determining the clinical course, outcome, and prognostic factors of this heterogeneous disease group is important to provide appropriate management and follow-up. Therefore, we aimed to identify the risk factors of CKD in CAKUT and the differences in clinical courses between disease subgroups. METHODS: Three hundred patients (M/F: 203/97) divided into 16 CAKUT categories were enrolled in the study. Logistic regression and survival analyses were performed to

Ocular Complications Among Patients with Beta-Thalassaemia Major and Intermedia: A Comparative Study from Pakistan.

Beta-thalassemia is associated with systemic complications from chronic anemia, iron overload, and long-term transfusion therapy, which may also affect ocular structures. This study aimed to compare the frequency and patterns of ocular complications between patients with β-thalassemia major and β-thalassemia intermedia. This comparative cross-sectional study included 30 patients (60 eyes) with β-thalassemia major or β-thalassemia intermedia, recruited via convenience nonprobability sampling. All participants underwent comprehensive optometric and ophthalmic evaluation, including assessments of

BK virus-associated haemorrhagic cystitis following allogeneic haematopoietic stem cell transplantation in a paediatric patient with β-thalassaemia major: A case report.

BK virus (BKV) can reactivate in immunosuppressed patients after stem cell transplantation. We report an 8-year-old boy with β-thalassemia major who developed Grade III BKV-associated haemorrhagic cystitis just 17 days post-transplant. Bladder ultrasound showed significant wall thickening with clot formation, and urine BKV Polymerase Chain Reaction(PCR) was markedly elevated, though blood PCR remained negative, meaning infection stayed localised to the urinary tract. He recovered fully with IV fluids, platelet support, and levofloxacin; no antivirals needed. This case highlights that urine BKV

Leg Ulcers in Thalassaemia: A Comparative Retrospective Review of Transfusion-Dependent and Non-Transfusion-Dependent Patients.

To compare clinical and laboratory characteristics of transfusion-dependent and non-transfusion-dependent beta thalassaemia patients with leg ulcers, and to identify contributing and protective factors We retrospectively reviewed 12 patients with beta thalassaemia and leg ulcers (seven transfusion-dependent and five non-transfusion-dependent) at a tertiary centre. We analysed demographics, haemoglobin levels, ferritin, foetal haemoglobin, transfusion status, comorbidities, and ulcer characteristics. Transfusion-dependent patients were predominantly younger males (mean age: 31.9 years), while n

The β-goblin gene architecture in individuals with and without sickle cell disease in Nigeria: Implications for β-thalassaemia trait diagnosis.

BACKGROUND: β-thalassaemia is considered rare in Africa; however, recent screening-based studies suggest a β-thalassaemia trait prevalence of 6% - 10% among individuals with sickle cell disease (SCD) and up to 25% in those without SCD. Co-inheritance with SCD may modify disease severity, highlighting the need for molecular confirmation. OBJECTIVE: To ascertain the prevalence and genetic basis of β-thalassaemia trait in Nigerians with and without SCD. METHODS: We recruited 260 participants (130 per group; aged 3 years - 69 years, median [interquartile range] = 16 [9-29]). Haemoglobin fraction

Co-Occurring Genetic Mutations in Rett Syndrome and MECP2-Related Disorders-Clinical and Diagnostic Implications from a Case Series.

Background/Objectives: Factors modulating phenotypic variability in Rett syndrome (RTT, OMIM 312750) include X chromosome inactivation (XCI), type of MECP2 variant, and/or disease modifiers. Emerging evidence also points to multi-locus genetic variants. Understanding the phenotypic variability associated with multi-locus genetic diagnoses in individuals with RTT and MECP2-related disorders would be important not only for accurate diagnosis, risk stratification and clinical management but also to explain symptoms that might not be typically associated with RTT. Methods: We present a case series

Differentiating β-Thalassaemia From Iron Deficiency: Performance and Cut-Off Dependency of More Than 40 Red Cell Discrimination Indices in a Large German Cohort.

BACKGROUND: Since the 1970s, more than 40 red blood cell-based formulas have been proposed to differentiate β-thalassaemia trait (BTT) from iron deficiency (ID) and iron deficiency anaemia (IDA), the main causes of microcytic anaemia. In Germany, where haemoglobinopathies are rare in the autochthonous population but increasingly encountered due to migration, rapid, low-cost screening tools are of growing importance. This study benchmarked more than 40 published discrimination indices (1973-2024) in a large laboratory dataset and explored new formulas. METHODS: In a retrospective analysis, 131

Gene-based screening using GAP-PCR and MALDI-TOF MS versus haemogram-based screening for thalassaemia in early pregnancy in Vietnam.

BACKGROUND: Thalassaemia is a common inherited disorder in Southeast Asia, including Vietnam. Conventional haemogram-based screening is widely used in routine prenatal care but has limited specificity and may be confounded by other conditions. This study compared gene-based screening using gap polymerase chain reaction (GAP-PCR) and matrix-assisted laser desorption ionization time-of-flight mass spectrometry (MALDI-TOF MS) with haematologic indices for thalassaemia carrier detection in early pregnancy. METHODS: A prospective diagnostic accuracy study was conducted amongst 572 first-trimester

Alpha-thalassaemia early eluting peak for alpha-thalassaemia --SEA carrier screening: a multicentre diagnostic comparison with immunochromatographic strip test and haemoglobin H inclusion test.

While high-performance liquid chromatography (HPLC) is well-established for β-thalassaemia and haemoglobinopathies, phenotypic screening for α0-thalassaemia has been limited. To address this limitation, we aimed to translate the discovery of the α-thalassemia early eluting peak (αEEP) in HPLC into clinical practice by comparing its diagnostic performance with other existing methods (haemoglobin H inclusion test [HbHi] and immunochromatographic strip test [ICT]) in a multicentre setting, and elucidating the nature of the αEEP by liquid chromatography-tandem mass spectrometry (LC-MS/MS). Thalassaemia.

The thalassaemia syndromes, which primarily include α-thalassaemia and β-thalassaemia, are a complex group of inherited disorders affecting haemoglobin production. They are prevalent throughout the most populated parts of the world and span a wide range of severity from mild to fatal. Advances in the management of these syndromes, including blood transfusion and iron chelation, have led to substantial improvements in the life expectancy and quality of life of many patients worldwide. Nevertheless, major forms of thalassaemia are still associated with chronic comorbidities and remain an importa

Longstanding Microcytosis with a Thalassaemia-Like Phenotype and Inconclusive Molecular Testing: A Case Report.

BACKGROUND: Persistent microcytosis in adults with normal or only mildly reduced haemoglobin usually prompts investigation for iron deficiency or thalassaemia trait. In some patients, however, the cause remains uncertain despite haemoglobin fractionation and targeted molecular testing. CASE PRESENTATION: An asymptomatic adult male had longstanding microcytosis, with haemoglobin of 13.6 g/dL, a red blood cell count of 5.67 × 106/µL, and a mean corpuscular volume of 72 fL. Serum iron was low, but ferritin remained within the reference range at 100.5 ng/mL. Haemoglobin fractionation showed HbA2

Efficacy and safety of mitapivat in adults with transfusion-dependent α-thalassaemia or β-thalassaemia (ENERGIZE-T): a double-blind, randomised, multicentre, placebo-controlled, phase 3 trial.

BACKGROUND: The absence of disease-modifying therapies for patients with α-thalassaemia and oral disease-modifying therapies for patients with β-thalassaemia has been a substantial unmet need in these patients. We assessed the efficacy and safety of mitapivat, an oral allosteric activator of pyruvate kinase, in adults with transfusion-dependent thalassaemia. METHODS: ENERGIZE-T is a global, double-blind, randomised, placebo-controlled, phase 3 trial, conducted across 19 countries in North America, Europe, Asia-Pacific, South America, and the Middle East. Alfimeprase, a fibrolase derivative with thrombolytic activity produced by recombinant DNA technology, was discovered by Amgen and was in development with Nuvelo for the treatment of stroke and catheter occlusion. However, development has been discontinued. Fibrolase is a zinc-containing metalloendopeptidase that was first isolated from the venom of the Southern copperhead snake, Agkistrodon contortrix contortrix. Alfimeprase directly degrades fibrin to break down clots. Alfimeprase is infused directly into the thrombus (side-hole catheter pushed through the entire clot) via multiple manual pu

Disparity in Coronal Deformity Measurements: Standing Radiographs Versus Navigation in Varus Total Knee Arthroplasty.

BACKGROUND: The third generation of bisphosphonates is clinically in use for patients of osteoporosis or malignancy-linked hypercalcemia. The agents can also produce anti-tumor effects on bone metastasis of several types of tumors. We recently found that one of the agents achieved cytotoxicity to mesothelioma in vitro and in an orthotopic animal model. Mesothelioma is resistant to a number of chemotherapeutic agents, and suppression of local tumor growth is beneficial to the patients since metastasis to extra-thoracic organs is relatively infrequent until a late stage. METHODS/DESIGN: We demo

Characteristics of patients hospitalized for diabetic foot in a national hospital in Peru, 2024.

INTRODUCTION: Expansive processes around the third ventricle have always been a major neurosurgical challenge. Despite all the technological and scientific progress recorded over the last few years, third ventricle tumors are still a very difficult pathology to approach due to their formidable complexity.Treating such a tumor demands a good knowledge of local anatomy, pathophysiology, pathology and a good capacity to integrate all the data gathered from the patient prior to and during surgery. Last but not least, the correct means to approach such a tumor involves using modern neuronavigation

Harnessing Artificial Intelligence to Restore Standards in Medical and Dental Education in Pakistan.

The month of March is dedicated to Colon Cancer Awareness. Worldwide, colorectal cancer (CRC) incidence has been on the rise. It is currently the third most common cancer in men (746,000 cases, 10.0% of the total) and the second in women (614,000 cases, 9.2% of the total).1 Arecent meta-analysis reported a 61% risk reduction in CRC incidence with colonoscopy.2 Unlike screening programs for breast and prostate cancers, not only has CRC screening reduced mortality from colon cancer and detected early CRC, it has also decreased the incidence of CRC through detection and removal of pre-cancerous l

Iron induces blood-brain barrier alteration contributing to cognitive impairment in β-thalassaemia mice.

Iron overload has been associated with cerebrovascular disease and cognitive impairment in β-thalassaemia patients, typically appearing earlier than in the general population. However, the mechanisms of iron overload on cerebrovascular pathology remain unclear. This study investigated the effects of heavy iron overload on the blood-brain barrier and neurohistology, particularly in the CA3 region of hippocampus and its contribution to cognitive impairment in β-thalassaemia mice. Three-month-old female β-thalassaemia and wild-type mice received iron dextran via intraperitoneal injection to induc
