A longitudinal study of the mental and physical development of 200 children with normal haemoglobin and 21 with the sickle-cell trait was carried out in a small rural community in Jamaica. At about 2 and 10 years of age heights and weights showed no significant differences. At about 10 years of age classroom behaviour, sociability, and educational achievement were similar. The results suggest that the sickle-cell trait does not affect growth and mental development.

In a controlled study in Ghana, the hemoglobin electrophoretic pattern in 112 patients with Burkitt's lymphoma was compared to that of their nearest neighbor controls of the same age, sex, and tribe, as well as their sibling controls. Analysis of the data obtained did not show any statistically significant protective advantage for sickle cell trait against Burkitt's lymphoma. Hemoglobin C trait appeared to offer a slight protective advantage (p less than 0.1), but this did not reach statistical significance. These results do not disprove the malaria co-factor hypothesis in the etiology of 0urk

The reproductive outcome of pregnancies complicated by severe sickle cell hemoglobinopathies is not usually satisfactory. In this study, prophylactic partial exchange transfusions have been used in 35 patients with sickle hemoglobinopathies. There was significant improvement in results, both maternal and neonatal, as compared to those in 29 pregnancies complicated by similar disease processes who did not receive this therapy. The former group was also compared to 61 normal pregnant patients and 31 patients with sickle cell trait, none of whom received blood as part of their therapy. The result

This article describes a rapid, simple, and inexpensive method for testing the cord blood of infants for the common hemoglobinopathies by electrophoresis using cellulose polyacetate strips. In a study of 7,500 umbilical cord blood specimens, 11% were found to contain an abnormal hemoglobin. While the importance of evaluating cord blood in a screening program for hemoglobinopathies is evident, the procedure must be accurate, verified, and followed by proper counseling.

Hematuria secondary to sickle-cell trait has on occasion proved refractory to medical management. We have adopted a new six-drug regimen to deal with this difficult problem. Herein is reported our experience with this protocol in 2 patients. Our results have been excellent, with hematuria clearing within twenty-four hours. The drug regimen is used in an attempt to alter the conditions that are favorable for sickling in the renal medulla.

Renal acidification was evaluated in patients with sickle-cell disease (HvSS) with both oral NH4CI and NaHC03 and the results were compared to those of subjects with sickle-cell trait (HbAS) and controls. The pH of arterial blood was normal in HbSS subjects but their PC02 and [HC03] were lower than those of controls. In response to NH4CI, six of 20 HbSS subjects had an abnormal minimal urine pH (greater than 5.3) and the entire HbSS group had a higher mean value than did either controls or HbAS subjects. Since none of the six HbSS subjects had evidence of proximal tubular abnormalities, it was

Occasional complications and even death in subjects with sickle cell trait have been attributed to severe physical exertion. However, the extent to which sickling actually occurs during exercise has not been reported. This study examined the red blood cell morphological features immediately following near maximal upright graded bicycle exercise in five asymptomatic black subjects with hemoglobin AS. Exercise produced minimal sickling in vivo, which was not proportional to the intensity of exercise. The amount of sickling in vivo was small in comparison to that observed in the presence of sever

Incubation of sickle cells with cystamine, a thiol reagnet, resulted in the formation of an intracellular S-ethylamine derivative. The rate of the reaction was dependent upon the cystamine concentration, the temperature, and the duration of the incubation. The cystamine-treated cells demonstrated a marked inhibition of sickling under hypoxic conditions, a decrease in their mean corpuscular hemoglobin concentration, and a significant increase in their oxygen affinity. The oxygen affinity of these cells was less dependent on their mean corpuscular hemoglobin concentration than that of untreated

Anti-haemoglobin S antibodies were raised in horses and purified by affinity chromatography. These antibodies recognize beta6 valine, while they fail to bind to haemoglobins with a glutamy1 (Hb A) or a lysy1 (Hb C) residue in this position. The purified anti-Hb S antibodies were composed of equine IgG(a, b) and IgG(T) subclasses and failed to cause precipitation with Hb S, evidently because of the bivalencey of both antibodies and antigen. Identification of Hb S in single erythrocytes was achieved by reacting the fluorescein isothiocyanate-conjungated anti-Hb S antibodies with the haemoglobin

A study of the osmotic resistance of red cells from normal subjects (AA), patients with the sickle cell trait (heterozygous subjects) (AS) and homozygous subjects (SS) was carried out using a dynamic automatic micro-method: Danon's fragiligraph, permitting one to obtain simultaneous recording of cumulative and derivative curves of the hemolytic process. 15 samples of red cells of each of the three categories were studied. Fragiligrams obtained from SS red cells showed that they became hemolysed later and in a more progressive fashion than the normal red cells AA. The AS red cells occupied an i

An epidemiological survey of adults aged 35--64 years living in a rural area of Jamaica was started in 1962--63 and continued until 1975. Information was available on 856 subjects with AA and 119 subjects with AS haemoglobin genotypes. The mortality-rates over 12 or 13 years and the number of clinic attendances in 10 years did not differ significantly between subjects with AA and those with AS haemoglobin. The patterns of disease as well as the life expectancy of the two groups appeared to be the same.

Sodium para-aminosalicylate (sodium salt of 2-hydroxy-4-aminobenzoic acid, Na-PAS) lowers the oxygen affinity of normal adult human placental, heterozygous and homozygous sickle cell anemic whole blood at 37 degrees C. The reduction of oxygen affinity is related to the type of hemoglobin in the blood. The mean P50 +/- S.E. at pH 7.40 for normal, placental, heterozygoud and homozygous sickle cell anemic blood in 26.2 +/- 0.1, 20.8 +/- 0.3, 26.8 +/- 0.3 and 31.0 +/- 0.5 mm Hg; in the presence of 5.7 mmol of Na-PAS per liter of blood the P50 values are increased to 28.0 +/- 0.3, 22.9 +/- 0.8, 30.

Sickle-trait red blood cells respond to freezing and thawing like normal cells, but during deglycerolization by centrifugal methods, they pack and hemolyze. This appears to be the result of changes in surface characteristics with suspension in hyperosmotic saline and even modest increases in osmolality result in packing with centrifugation. Modifications to permit deglycerolization of sickle-trait cells have been designed for use with both the Haemonetics and the IBM cell washing systems. Sickle-trait cells do not agglomerate satisfactorily but we were unable to devise a modification of this d
