Diagnostic Limitations of Hemoglobin A1c in the Setting of Compound Hemoglobinopathy: A Case Report of Sickle Cell Disease, Alpha Thalassemia, and Occult Diabetes.

This case report illustrates the diagnostic limitations of hemoglobin A1c (HbA1c) in patients with coexisting hemoglobinopathies. Sickle cell disease (SCD) and alpha thalassemia are inherited disorders characterized by chronic anemia and altered red blood cell turnover, both of which can significantly interfere with the accuracy of HbA1c measurements. In such cases, HbA1c may appear falsely low, potentially delaying the diagnosis and treatment of diabetes mellitus. We describe a 63-year-old female with previously undiagnosed SCD and alpha thalassemia whose longstanding hyperglycemia and diabet

The emerging role of complement inhibitory approaches in the treatment of acute sickle cell disease complications.

INTRODUCTION: Sickle cell disease (SCD) is a common and life-threatening hematological disorder that affects almost 100,000 people in the United States and millions worldwide. The wide phenotypic variability of SCD results from a complex pathophysiology involving coagulation, inflammation, leukocyte and endothelial cell activation, and other cellular and humoral mechanisms. To improve the prognosis of SCD patients, we must gain a better understanding of the underlying molecular pathogenesis and offer new therapies. AREAS COVERED: In SCD and other hemolytic disorders, continual red blood cell

Exploring the temporal relationship between psychosocial risk factors and recurrent ischemic priapism.

BACKGROUND: The development of recurrent ischemic priapism (RIP) is associated with mental health disorders, psychiatric medications, and substance use, but the temporal relationship between the onset of RIP and the development of these risk factors is poorly understood. AIM: This study aimed to investigate the chronology of psychosocial and substance use risk factors relative to the initial clinical presentation of RIP. METHODS: We conducted a retrospective cohort study of 73 adult patients diagnosed with RIP at a single academic center, identified via manual chart review of the electronic

Newborn Screening for Hemoglobinopathies and Thalassemias: Brief History, Recent Activities, and Global Status-2026.

Newborn bloodspot screening (NBS) began in Guthrie's laboratory in 1961 for phenylketonuria. A federal study the following year formed the basis for expanding NBS as a public health function. Diseases detectable through NBS gradually expanded, eventually including sickle cell anemia, which was included in the screening panel in New York in 1975. Universal inclusion of full population screening for sickle cell anemia was included in all US NBS programs by 2006. Through the years, NBS for sickle cell anemia has expanded to include other clinically significant hemoglobin disorders (both hemoglobi

Newborn screening for sickle cell disease in Angola: Implementation challenges and emerging data on hemoglobinopathy prevalence.

Sickle Cell Disease (SCD) is an autosomal recessive disorder with a substantial global burden. Despite its particularly high incidence in sub-Saharan Africa, early diagnosis remains limited in many countries. The objective of this study was to implement a newborn screening programme for SCD in one of the largest maternity hospitals in Angola and to support the subsequent pediatric follow-up of the affected children. Between June 2023 and December 2024, all children born in or attending the main hospital for vaccination were screened after parental or guardian consent. Blood was collected by he

Non-aneurysmal Subarachnoid Hemorrhage in an Adult With Sickle Cell Anemia: A Case Report and Review of the Literature.

Moyamoya disease (MMD) is a rare, idiopathic cerebrovascular disorder characterized by bilateral stenosis of the internal carotid arteries (ICAs) and the formation of collateral vessels at the base of the brain. In patients with sickle cell disease (SCD), chronic vaso-occlusive changes can produce moyamoya-like vasculopathy, resulting in moyamoya syndrome (MMS) and increasing the risk of aneurysm formation and rupture. Nonetheless, presentation of MMS as non-aneurysmal subarachnoid hemorrhage (NASAH) is extremely rare, with few reported cases. Our case is about a 28-year-old male, a known case

Genetic Contribution to Asthma Informs Acute Chest Syndrome Pathophysiology and Risk Stratification.

Acute chest syndrome (ACS) is a severe complication of sickle cell disease (SCD) occurring in ~50% of patients, some presenting frequent episodes. We lack tools to identify patients at high risk of ACS occurrence or frequent episodes. Epidemiological studies have found an association between asthma and ACS, but whether this link is causal is unclear. We used polygenic scores (PGS) to analyze whether the genetic propensity for asthma was associated with ACS and could be used to stratify the risk of ACS. We identified that PGS for asthma (PGSasthma) was associated with ACS rate (number of episod

Current Status of Clinical Gene Therapy for Hemophilia and Globin Disorders.

In the last few years, gene therapy, holding the promise for long-term disease correction through a one-time treatment, has transitioned from experimental research to approved medicine. Several gene therapies are now available for congenital blood disorders, notably for hemophilia and hemoglobinopathies. In this review, we discuss each of the six therapies that now have regulatory approval for treatment in the United States: Roctavian (valoctocogene roxaparvovec) for hemophilia A, Beqvez (fidanacogene elaparvovec) and Hemgenix (etranacogene dezaparvovec) for hemophilia B, Lyfgenia (lovotibeglo

Gene expression profiling identifies potential biomarkers for vaso-occlusive episodes in sickle cell disease.

Vaso-occlusive episodes (VOEs) or acute pain events, involving complex interactions between sickle erythrocytes and other blood cells, are a hallmark of sickle cell disease (SCD). In this study, we analyzed changes in peripheral blood transcriptomes between steady state and VOEs in individuals with SCD. We followed a cohort of 174 individuals with SCD with or without chronic pain and collected peripheral blood at clinic visits (steady state) and during hospitalizations (VOEs). We performed RNA-Seq profiling of CD45+ leukocytes and CD71+ erythroid cells. Pathways linked to complement activation

Understanding stroke risk phenotypes in pediatric patients with sickle cell disease and concurrent moyamoya arteriopathy: insights from 61 cases at a single institution.

OBJECTIVE: Children with sickle cell disease (SCD) are at risk of developing cerebral vasculopathies, increasing the risk of stroke. While some vasculopathies are medically managed, surgical revascularization might be needed in patients who also have moyamoya arteriopathy (MMA). The aim of this study was to characterize stroke timing and phenotype between children with homozygous SCD (HbSS) alone and those with HbSS and concurrent MMA. METHODS: A retrospective review of clinical and radiological information for pediatric patients (age < 21 years) with HbSS and cerebrovascular sequelae at a si

The Epidemiology of Sickle Cell Disease in Sub-Saharan Africa: Current Knowledge and Gaps to be Filled.

Sickle Cell Disease (SCD) is highly prevalent in sub-Saharan Africa. Epidemiological data remain sparse, but regional screening and research initiatives are expanding. Due to genetic, environmental, and socioeconomic factors, the disease course differs markedly from that in high-income countries. Although mortality is improving and can be further lowered with simple interventions, it remains high, especially among undiagnosed children. Genetic factors, poor healthcare infrastructure, and poverty contribute to disease severity. While recent collaborative programs like SickleInAfrica offer hope,

Exploring Affordable Curative Therapy for Sickle Cell Disease in Africa: A Comprehensive Overview.

The practical aspects of developing curative treatments for sickle cell disease (SCD) in Africa, such as gene therapy and hematopoietic stem cell transplantation, involve strengthening healthcare infrastructure, training healthcare professionals, establishing regional treatment centers, and creating national SCD programs. The costs associated with gene therapy and stem cell transplants can be prohibitive, especially in low- and middle-income countries. Strategies to address affordability, including local manufacturing, government funding, and partnerships with global health agencies, are essen

Predictors of Acute Chest Syndrome in Patients With Sickle Cell Disease: A Cross-Sectional Observational Study.

Background Acute chest syndrome (ACS) is a major cause of mortality and hospitalization in patients with sickle cell disease (SCD). While clinical features often overlap with uncomplicated vaso-occlusive crises (VOC), early differentiation is critical for survival. Data regarding specific predictors of ACS in the Indian population remains limited. The objective of this study was to identify clinical and laboratory factors associated with the diagnosis of ACS in hospitalized patients with sickle cell disease. Methods This cross-sectional analytical study was conducted at a tertiary care teachin

Action taken upon the detection of sickle cell trait (heterozygous hemoglobinopathy S) in newborn screening in Spain.

INTRODUCTION: Since 2021, sickle cell disease (SCD) has been included in all 17 neonatal screening programmes in Spain. The carrier state for hemoglobin S (HbS) is also detected collaterally; although clinically benign, it appears to increase the risk of certain complications, with genetic counselling being the main reason for its recognition. The Red Cell Disorders Working Group of the Spanish Society of Pediatric Hematology and Oncology (SEHOP) conducted this study to examine current practices in the management of its detection. MATERIALS AND METHODS: A survey was carried out among represen
