Exploring the latest emerging drugs for the treatment of sickle cell disease.

INTRODUCTION: Sickle cell disease (SCD) is an inherited autosomal recessive hemoglobinopathy that causes significant morbidity and mortality in children and adults. The availability of novel therapeutic agents that are safe, effective, and affordable and new cell therapy techniques remains highly desirable for the treatment of SCD. AREAS COVERED: After a brief reminder of the main SCD complications, this review summarizes emerging disease-modifying agents and promising cell therapy strategies for the treatment of patients with SCD. A comprehensive search to look for the efficacy and safety of

Endothelium-derived endothelin-1 mediates sickle cell nephropathy.

Sickle cell nephropathy (SCN) significantly shortens the life expectancy of patients with sickle cell disease (SCD). We previously reported that endothelin-1 (ET-1) and endothelin A receptor (ETA) are upregulated in SCN, and ETA antagonism mitigates SCN early in the disease progression in a humanized mouse model of SCD. We hypothesized that endothelium-derived ET-1 mediates the progression of SCN and T cell inflammation in the kidney of SCD mice. To test this hypothesis, we first used allogenic bone marrow transplantation from humanized sickle cell mice (HbSS) into endothelial-derived ET-1 kno

Technology Access and Preferences for Smartphone App Interventions to Optimize Iron Chelation Therapy Adherence Among Adolescents, Young Adults, and Parents of Children Receiving Chronic Transfusions: Cross-Sectional Survey Study.

BACKGROUND: Iron chelation therapy (ICT) is essential for people with hematological disorders requiring chronic transfusions to minimize the risk of iron overload, yet suboptimal adherence is prevalent. Widespread use of personal technology makes mobile health (mHealth) an attractive platform to promote adherence. OBJECTIVE: This study aimed to examine access to mobile technology and preferences for an mHealth intervention to improve adherence to ICT. METHODS: A cross-sectional survey that included 63 items assessing technology access, mHealth preferences, and demographics was administered t

Utilization of Model-Informed Drug Development in Early Clinical Development: Case Study of a Hb Modulator for Treatment of Sickle Cell Disease.

Model-informed drug development (MIDD) has an important role to play at various stages of drug development. In this case study, the application of MIDD in the early clinical development of a small-molecule inhibitor of sickle hemoglobin (Hb) polymerization is highlighted. Starting from the setting of quantitative criteria, to using a combination of PKPD and semi-mechanistic RBC modeling, and key biomarker analysis, based on data from a healthy participant first-in-human (FIH) study and historical data, we chart the strategic approaches planned and taken to inform decisions for the program, and

Predictors of Critical Care Interventions and Length of Stay in Pediatric Sickle Cell Acute Chest Syndrome: A Five-Year Pediatric Intensive Care Unit (PICU) Cohort Study.

Introduction Acute chest syndrome (ACS) remains one of the most severe complications of pediatric sickle cell anemia (SCA) and a leading cause of Pediatric Intensive Care Unit (PICU) admission. Despite its clinical significance, regional data describing PICU-level characteristics, interventions, and outcomes in children with SCA are limited. This study aimed to evaluate the clinical profile, management strategies, and outcomes of pediatric SCA patients admitted to the PICU at a tertiary care center in Tabuk, Saudi Arabia. Methods We conducted a retrospective cohort study of pediatric patients

Cerebral microhemorrhages in a mouse model of sickle cell disease.

OBJECTIVES: Stroke in sickle cell disease (SCD) is often attributed to large vessel involvement in the disorder, whereas the contribution of cerebral microvascular disease has been less explored. In this study, we investigated the formation of cerebral microvascular lesions and the involvement of mast cells in a humanized SCD mouse model. METHODS: We studied hemorrhagic microvascular disease in a well-characterized mouse model of humanized transgenic sickle (HbSS-BERK) expressing >99% human sickle hemoglobin (HbS) and a control (HbAA-BERK) mouse model expressing normal human hemoglobin A (HbA

Decoding Sickle Cell Disorders Using Flow Injection Analysis Mass Spectrometry.

RATIONALE: Chromatographic and electrophoretic methods reliably distinguish the heterozygous and homozygous sickle cell states with hemoglobin variants appearing at consistent percentages with specific retention or migration times. However, co-migrating variants alter the overall hemoglobin profile, complicating interpretation in compound heterozygous conditions such as βS/βD-Punjab since these variants electrophoretically migrate close to each other. These cases often require hematological correlation or molecular confirmation. As molecular testing is not available in all laboratories, mass s

Implications of changes in WHO haemoglobin elevation adjustment guidelines on global, regional, and national anaemia burden, 1990-2023: a population-based modelling study.

BACKGROUND: Adjustment of haemoglobin concentrations for elevation is essential for anaemia assessments of both individuals and populations. Whereas the previous WHO anaemia definitions released in 2001 used an elevation adjustment derived from small sample sizes with limited representativeness, the 2024 WHO global anaemia definitions reflect empirical assessments across several geographies and settings derived from population-representative surveys. We aimed to assess the effect of these changes on estimates of global anaemia burden. METHODS: In this population-based modelling study, using a

In Utero HSC Transplantation for Sickle Cell Disease: A Potential Therapeutic Approach That Overcomes Complications of Current Therapies.

Sickle cell disease (SCD) affects millions worldwide but has limited treatment options, most of which carry significant side effects. At present, the only curative treatment for SCD is allogeneic or gene-modified autologous hematopoietic stem cell (HSC) transplantation (Tx). HSC-Tx carries considerable challenges/risks to the patient largely due to the highly toxic conditioning required to achieve sufficient HSC engraftment. Furthermore, existing treatments often only commence in late childhood or adolescence, yet SCD's clinical manifestation start during infancy, placing the child at risk of

A novel activating somatic mutation in EPAS1, coding for HIF-2α, in a patient with a paraganglioma and sickle cell disease.

Pheochromocytomasand paragangliomas (collectively referred as PPGLs) are highly heritable neoplasms arise from chromaffin cells of neural crest tissues; 40% of patients with PPGLs harbour germline pathogenic variants (PV), which up to 45% of patients exhibit somatic mutations in similar susceptibility genes. Endothelial PAS domain-containing protein-1 [also known as hypoxia inducible factor-2α, HIF-2α] is encoded by EPAS1, and along with other hypoxia-inducible factors (HIFs) acts as a key mediator in the cellular response to hypoxia. Gain-of-function mutations in EPAS1 have been linked to the

Negative Descriptors of Patients With Sickle Cell Disease in the Electronic Health Record.

IMPORTANCE: Patients with sickle cell disease (SCD) may experience clinician biases due to intersecting stigmatizing factors because most patients with SCD racially identify as Black, have pain as the most common presenting symptom, and are often treated with opioids. OBJECTIVE: To examine intersecting stigmatizing identities of patients with SCD-identifying as Black, presenting with pain, and being treated with opioids-and compare differences in negative descriptors in the electronic health record relative to SCD. Movement behavior in adults with sickle cell disease compared to healthy adults: a cross-sectional study.

BACKGROUND: Sickle cell disease (SCD) is one of the most common monogenic disorders in the world. Patients with SCD have chronic hemolytic anemia and experience episodic pain due to vaso-occlusion resulting in, amongst other, musculoskeletal and cardiopulmonary complications. We hypothesize that adults with SCD are less physically active, but limited information is available on the movement behavior of adults with SCD. In the current study, we aimed to evaluate movement behavior in patients with SCD as compared to healthy controls. METHODS: Movement behavior of patients with SCD (≥16 years) w

Illness Identity in Symptomatic Adults with Sickle Cell Disease: Preliminary Findings and Considerations.

Illness identity, or how an individual integrates their disease into their sense of self, is an important psychological concept for medical populations. According to one model, there are four proposed dimensions of illness identity: Acceptance, Rejection, Enrichment, and Engulfment. Acceptance and Enrichment describe an individual's ability to embrace their illness and regard it as a source of positive meaning-making, while Rejection and Engulfment describe an individual's inclination to disregard or be preoccupied by their illness in unhelpful ways. This study aimed to describe illness identi

Prevalence estimates of sickle cell disease among children and adolescents in sub-Saharan Africa: a systematic review and modelling analysis.

BACKGROUND: There is a scarcity of data reporting on the burden of sickle cell disease across many African settings, particularly among children, who have the highest risk of preventable morbidity and mortality in the absence of early diagnosis and care. We aimed to estimate the prevalence of sickle cell disease and the absolute number of paediatric cases in sub-Saharan Africa to inform policy and service responses. METHODS: For this systematic review and modelling analysis, we searched MEDLINE, Embase, Global Health (CABI), and African Journals Online (AJOL) for studies published from Jan 1,

Associations Between Sickle Cell Disease, Pica, and Enuresis in Pediatric Neurodevelopmental Disorders.

Background: Sickle cell disease (SCD) is a hereditary disorder affecting red blood cells' shape and functional capacity. Individuals with SCD report relatively high co-occurrence of neurodevelopmental disorders (NDDs). In addition, these children also have higher rates of enuresis (incontinence) and pica, disorders prevalent in children with developmental delays. Both enuresis and pica can have negative effects on mental health, but their pathophysiology, especially in SCD, remains unclear. Objectives: The objective of this study was to determine the rates of pica and enuresis in a pediatric S

Acute kidney injury: incidence, risk factors, and early outcomes in hospitalized children with sickle cell disease attending a tertiary hospital in Eastern Uganda.

BACKGROUND: Many sickle cell disease (SCD) patients experience renal damage that impairs function, with acute kidney injury (AKI) significantly affecting both immediate and long-term outcomes. The only Ugandan study on this subject reported a high incidence and mortality of AKI among children with SCD, but did not assess other outcomes. METHODS: This hospital-based prospective study was conducted between January and June 2024 at Jinja Regional Referral. AKI was diagnosed using KDIGO criteria, and participants were followed until discharge or death. Outcomes included length of hospital stay, mo

Dissecting polycomb complexes for enhanced fetal hemoglobin production.

Polycomb repressive complexes PRC1 and PRC2 regulate diverse developmental processes, including the fetal-to-adult switch in hemoglobin production, a process whose reversal is a goal for the treatment of sickle cell disease and β-thalassemia. PRC inhibitors show promise for various disorders, but use is limited because of pleiotropic PRC activities. We explored whether fetal hemoglobin (HbF) can be reactivated in adult erythroid cells by selective perturbations of PRC1 or PRC2 components without complete loss of PRC function. A high-density CRISPR-Cas9 mutagenesis screen identified a region in
