Phase 3, randomised, placebo-controlled trial of voxelotor for the resolution of leg ulcers in sickle cell disease: RESOLVE.

Leg ulcers are a debilitating complication of sickle cell disease (SCD, including sickle cell anaemia and other sickle cell disorders) and have no proven treatments. Resolution of Sickle Cell Leg Ulcers with Voxelotor (RESOLVE) was a Phase 3, randomised, double-blind, placebo-controlled trial evaluating voxelotor for leg ulcer resolution in participants aged ≥12 years with SCD (homozygous haemoglobin S [HbSS]/haemoglobin S β0-thalassaemia [HbSβ0]) and ≥1 active cutaneous lower-extremity ulcer. Participants from Kenya, Nigeria and Brazil were randomised 1:1 to voxelotor 1500 mg (N = 45) or plac

Case Report: Recurrence of recipient-derived erythropoiesis in hematopoietic stem cell transplant for sickle-cell anemia: multi-lineage chimerism analysis as a tool to distinguish atypical pure red cell aplasia from poor erythroid engraftment.

Experience in hematopoietic stem cell transplant (HSCT) as a long-term curative treatment for sickle-cell anemia (SCA) has been gathered over the last 30 years. Sustained engraftment to restore normal erythropoiesis remains the primary aim of HSCT in this disease. Yet, with the increasing use of reduced intensity conditioning as a toxicity-sparing alternative, a significant proportion of transplanted patients develop mixed chimerism. While a myeloid donor chimerism above 20% is deemed sufficient to prevent pain crisis, patients with donor chimerism below 50% can present recurrence of hemolytic

Clinical pathway implementation for adult sickle cell vaso-occlusive crises: Impact on healthcare utilization, opioid prescribing, and costs.

BACKGROUND: Vaso-occlusive crisis (VOC) is the leading cause of emergency department (ED) visits and hospitalization in sickle cell disease (SCD). Standardized clinical pathways may improve care quality and reduce healthcare utilization, yet regional evidence from Saudi Arabia remains limited. OBJECTIVES: To evaluate the clinical and economic impact of a multidisciplinary clinical pathway on VOC management, opioid prescribing, and direct healthcare costs. DESIGN: Retrospective before-and-after cohort study. SETTING: Single-center, 500-bed tertiary referral hospital in Jeddah, Saudi Arabia.

Evaluation of non-opioid therapies for pain management of sickle cell disease: a literature-based review.

Pain is a debilitating complication of sickle cell disease (SCD), driven by recurrent vaso-occlusive crises and chronic pain syndromes involving central sensitization and neuropathic mechanisms. Although opioid medications remain first-line therapy for acute pain, there are limitations that highlight the need for effective adjunctive approaches. Available data support non-opioid pharmacologic therapies including acetaminophen and nonsteroidal anti-inflammatory drugs as components of multimodal analgesia, with emerging evidence for therapies targeting neuropathic and centralized pain. Non-pharm

Arginine Therapy for Sickle Cell Disease Acute Pain Episodes: The STArT Randomized Clinical Trial.

IMPORTANCE: Acute pain episodes are the leading cause of emergency department visits and hospitalizations for patients with sickle cell disease (SCD), yet US Food and Drug Administration-approved drugs for acute pain episodes are lacking. During acute pain episodes, patients develop acute arginine deficiency associated with longer time to crisis resolution and greater total parenteral opioid use. Multiple single-center, phase 2 randomized clinical trials have shown that arginine is safe, is opioid sparing, improves cardiopulmonary function, and reduces length of hospital stay. OBJECTIVE: To d

Parvovirus B19 infections in paediatric sickle cell disease patients: Genotype and hydroxyurea treatment influence disease severity.

In patients with sickle cell disease (SCD), parvovirus B19 infection (B19V) leads to acute anaemia (aplastic crisis), but may also be associated with other serious complications. We retrospectively analysed clinical data from paediatric SCD patients with B19V infections between 2023 and 2025, including symptoms, laboratory parameters, diagnostic procedures and treatment details to detect differences between SCD genotypes (HbSS and HbS/β0-thalassaemia, i.e. HbSS/Sβ0, and HbSC) and to determine the effect of hydroxyurea (HU; hydroxycarbamide) treatment in HbSS/Sβ0 patients. Ninety-eight patients

Updated Review of Current Therapeutic Approaches for the Management of Sickle Cell Disease.

Sickle cell disease is a severe genetic blood disorder marked by the production of abnormal hemoglobin (HbS), leading to sickle-shaped red blood cells that obstruct blood flow and cause various problems, such as the increased risk of infection, persistent anemia, acute pain episodes, and organ damage. Roughly 100,000 Americans suffer from SCD, with approximately 40,000 of them being children. Black people have the highest frequency of the disease. There are six Food and Drug Administration (FDA)-approved drugs, hydroxyurea, L-glutamine, crizanlizumab- TMCA, voxelotor, Casgevy, and Lyfgenia, th

Intranasal fentanyl and impact of an emergency department pain protocol for sickle cell disease on hospitalization rates.

Sickle cell disease (SCD) is an inherited red blood cell disorder with significant complications, including vaso-occlusive crisis (VOC), the most common cause of emergency department (ED) visits and hospitalizations. Pain in children with SCD is often underestimated. We evaluated the impact of a standardized institutional nurse-initiated ED SCD pain protocol, using intranasal (IN) fentanyl (INF) as the initial agent, on hospital admission rates. We conducted a preintervention/postintervention study of patients with SCD (aged 0-21 years) presenting to a quaternary pediatric ED for uncomplicated

Evaluating the Current Opioid Misuse Measure (COMM) as a Tool to Inform Management of Vaso-occlusive Episodes Among Hospitalized Sickle Cell Disease Patients.

OBJECTIVES: Sickle cell disease (SCD) is associated with vaso-occlusive episodes (VOEs) that often require inpatient care. Parenteral opioids are recommended as first-line VOE treatment in hospitalized patients. The Current Opioid Misuse Measure (COMM) survey was used to screen for aberrant opioid use in SCD patients hospitalized for VOE. Goals were to estimate the proportion of positive screens and evaluate associations with length of stay (LOS), electronic health record (EHR) data, and Prescription Drug Monitoring Program (PDMP) assessments. METHODS: SCD patients hospitalized for VOE were a

Comprehensive analysis of complement activation in a hydroxyurea-treated patient cohort with sickle cell disease.

Sickle cell disease (SCD) is an understudied, life-threatening genetic disorder affecting ∼300 000 infants yearly with limited treatment options. The complement system, a critical part of innate immunity, has emerged as a contributor to SCD pathophysiology, thus presenting a potential new treatment target. Our aim was to assess complement activity in children with SCD receiving hydroxyurea (HU) therapy during vaso-occlusive crisis (VOC) and steady state. Blood samples were collected from 46 pediatric patients with SCD during VOC (early and late) and steady state, with control samples from heal

Ultrasound-Guided Erector Spinae Plane Block for Refractory Chest and Epigastric Pain Control in Sickle Cell Crisis: A Case Report.

This report aims to demonstrate the potential role of ultrasound-guided erector spinae plane block (ESPB) in managing refractory epigastric and chest pain due to vaso-occlusive crises (VOCs). An 18-year-old girl with sickle cell disease (SCD) presented to the emergency department with refractory epigastric and chest pain due to VOC. An ultrasound-guided left-sided erector spinae plane block (ESPB) using 30 mL of 0.2% ropivacaine provided rapid and sustained analgesia, reducing her Numerical Rating Scale (NRS) pain score from 8/10 at baseline to 0/10 at 2 hours postblock. She remained pain-free

Wearable Device Photoplethysmography As a Viable Tool to Longitudinally Monitor Vasoconstriction Biomarkers for Predicting Vaso-Occlusive Crisis in Sickle Cell Disease: Feasibility and Validation Study.

BACKGROUND: Entrapment of sickled red blood cells in the microvasculature leads to sudden painful vaso-occlusive crises (VOCs) in sickle cell disease (SCD). This is potentially triggered by autonomic nervous system-mediated vasoconstriction in the microvasculature. Indeed, vasoconstriction biomarkers derived from a single night of laboratory-based fingertip photoplethysmography (PPG) recording were predictive of a higher frequency of future VOC in SCD. Noninvasive, remote, and longitudinal monitoring of autonomic vasoreactivity will facilitate the development of predictive biomarkers of immine

Implicit bias in pain management: The intersection of stigma, race, and chronic disease in sickle cell disease.

Healthcare disparities in pain management represent one of the most persistent and pernicious manifestations of systemic bias within medicine. This comprehensive review examines the complex intersection of implicit bias, racial stigmatization, and clinical decision-making in the context of sickle cell disease, a predominantly African descent genetic disorder characterized by severe recurrent acute pain episodes and chronic pain that worsens with age. Through analysis of empirical research spanning psychosocial functioning, pain perception, healthcare utilization patterns, and provider attitude

Vitamin D deficiency in sickle cell disease: a neglected comorbidity in Tunisia.

Sickle cell disease (SCD) is a hereditary hemoglobinopathy that imposes a significant burden on global health. Individuals with SCD often experience multiple nutritional deficiencies, among which vitamin D deficiency has emerged as a critical yet under recognized factor contributing to disease complications. This study aimed to assess the vitamin D status in patients with SCD compared to a matched healthy control group, and to explore its correlation with clinical and biological parameters, including the frequency of vaso-occlusive crises (VOC). A cross-sectional study was conducted between Ja

Arginine therapy in sickle cell disease: A systematic review and meta-analysis of clinical outcomes.

BACKGROUND & AIMS: Vaso-occlusive crises (VOCs) in sickle cell disease involve nitric oxide deficiency, creating a rationale for l-arginine. This meta-analysis evaluates its efficacy on clinical VOC outcomes including pain, opioid use, and hospitalization by synthesizing evidence from RCTS. METHODS: The PubMed, Embase, and Cochrane databases were searched for RCTs comparing l-arginine with placebo or standard care in SCD patients. Primary outcomes were pain scores, opioid consumption, time to crisis resolution, and length of hospital stay. Pooled estimates were calculated using random-effects

Apohemoglobin-haptoglobin complex is a therapeutic against vaso-occlusive crisis: scavenging of hemoglobin and heme.

Sickle cell disease (SCD) leads to vaso-occlusive episodes (VOEs) releasing cell-free hemoglobin (Hb) and heme that drive oxidative stress, inflammation, and microvascular dysfunction. Current treatments for VOEs remain limited with no targeted therapies addressing these hemolytic byproducts. This study investigated the potential of an apohemoglobin-haptoglobin (ApoHb-Hp) complex (protein scavenger of both Hb and heme) vs Hp (scavenger of only Hb), in transgenic SCD mice exposed to hypoxia-reoxygenation. Briefly, HbSS-Townes mice instrumented with a dorsal skinfold window chamber received ApoH

Non-pharmacological interventions for managing sickle cell crisis pain: A qualitative study on adolescents' and caregivers' experiences.

Sickle cell crisis (SCC), characterized by recurrent acute pain episodes, is the most common reason for hospitalization among adolescents with sickle cell disease (SCD), a common hemoglobinopathy in Nigeria. SCC pain is challenging to manage, undertreated, and adolescents are often stigmatized and labelled as drug seekers. The combined use of analgesics and non-pharmacological interventions (NPIs) is highly recommended for optimal pain management. Studies indicate high usage of NPIs among adolescents, including unsafe interventions. This study aims to explore the experiences of adolescents and
