Incidence and risk factors for vaso occlusive crisis among children living with sickle cell disease in Uganda: a cohort study.

BACKGROUND: Vaso-occlusive crises (VOC) are the most frequent complications among people living with sickle cell disease. VOC lead to frequent hospitalisations, reduce quality of life and contribute to long-term sequelae such as stroke, acute chest syndrome, and renal impairment. There are limited contemporary data on the incidence and risk factors for VOC among children with sickle cell anaemia (SCA) after the introduction of hydroxyurea in Uganda. METHODS: A prospective cohort study conducted among children living with SCA enrolled in the Uganda Sickle Pan-African Research Consortium Regist

Positive neurological outcome after acute encephalopathy and multifocal stroke after early red blood cell exchange transfusion in a patient with hemoglobin SC sickle cell disease.

Sickle cell disease is a genetic disorder characterized by the presence of abnormal hemoglobin, leading to various complications, including vaso-occlusive crises and organ damage. While much research has focused on the homozygous hemoglobin SS genotype, less is known about the heterogeneous hemoglobin SC genotype (HbSC), particularly in adults. This case report details the presentation, clinical course, and management of a 27-year-old African American man with HbSC sickle cell disease, who presented with severe acute encephalopathy and multifocal acute ischemic stroke and had a positive neurol

Cyclic Fevers in Pregnancy: A Case of Parvovirus-Related Hemophagocytic Lymphohistiocytosis.

Parvovirus B19 typically causes mild symptoms in healthy adults, including fever, rash, and arthralgias. Certain immunocompromised adult populations, including those with immunodeficiencies or hematologic disease, are at risk of clinically significant complications, including transient aplastic crisis. Further, in pregnant patients, there is a risk of transplacental viral transmission, which may lead to severe fetal anemia, nonimmune hydrops fetalis, and intrauterine fetal demise. The clinical course of parvovirus B19 in pregnant patients with underlying hematologic disease has not been well r

Spontaneous calvarial regeneration after craniectomy for sickle cell-related skull infarction and spontaneous epidural hematoma: implications for delayed cranioplasty.

BACKGROUND: Spontaneous epidural hematoma is a rare but recognized neurological complication of sickle cell disease and is often associated with infarction of the calvarial bone. Surgical evacuation may necessitate craniectomy when the involved bone is infarcted and nonviable. Spontaneous regeneration of calvarial defects after craniectomy is uncommon but has been reported in children. CASE PRESENTATION: We report a 10-year-old boy with homozygous sickle cell disease (HbSS), diagnosed at the age of three years, who presented with headache and progressive right hemiparesis two weeks after a va

Safety, efficacy and feasibility of GCSF and plerixafor based CD34 mobilization for backup autologous stem cell collection in patients undergoing alternate donor hematopoietic stem cell transplant for sickle cell disease: Take away lessons for gene therapy.

A retrospective analysis was performed on 70 patients with SCD aged <30 years, who underwent alternate donor HSCT, in whom autologous stem cell was collected as backup. Patients received 3-4 days of 10 mcg/kg GCSF with plerixafor 0.24 mg/kg 6-8 h prior to PBSC harvest and a desired target CD34 + count of > 3 million/ Kg recipient body weight was collected. Median age of cohort was 10 years (1-27 years). Median CD34 dose per collection was 7.7 × 106/ kg recipient body weight (range, 1.7-33.7 × 106/ kg recipient body weight). Sixteen patients had veno-occlusive crisis (VOC); 11, mild, managed wi

Lung function among children and adolescents with sickle cell disease living in Lake Victoria basin in Kenya.

INTRODUCTION: Sickle cell disease (SCD) is a group of inherited blood diseases in which the affected person inherits two abnormal haemoglobin genes from both parents and characterized by lifelong symptoms of anemia, vaso-occlusive crisis (VOCs) and end organ damage. Pulmonary complications that precipitate decline in lung function are contributors to morbidity and mortality. Determination of lung function can enable early detection of derangements allowing for timely institution of appropriate interventions to delay progression of symptoms. AIMS: This study evaluated the lung function pattern

Hydroxyurea (hydroxycarbamide) use in adults with haemoglobin SC disease: A real-world study in Quebec.

Haemoglobin SC (HbSC) disease is the second most prevalent form of sickle cell disease, but evidence for hydroxyurea (hydroxycarbamide; HU) to prevent pain episodes was limited until the prospective identification of variables as outcomes for treatment (PIVOT) trial. To assess HU effectiveness and safety in adults with HbSC, we conducted a retrospective, single-centre, pre-post cohort study at the Centre hospitalier de l'Université de Montréal. Patients initiated HU between January 2010 and July 2025. Effectiveness analysis required ≥12 months of pre-HU follow-up; patients with shorter follow-

Aplastic Crisis Triggered by Parvovirus B19 in an Adult Man With Sickle Cell Disease.

Patients with sickle cell disease (SCD) are vulnerable to acute hematologic complications due to chronic hemolysis and dependence on increased erythropoiesis. Parvovirus B19 infection can suppress erythroid progenitor cells, precipitating a transient aplastic crisis characterized by severe anemia and reticulocytopenia. Early recognition is critical to distinguish this condition from other causes of acute anemia in patients with SCD. A 22-year-old man with hemoglobin SS SCD and prior splenectomy presented with a three-day history of severe headache, non-bloody emesis, fevers, fatigue, and gener

Ratio of Postdischarge Emergency Department Visits to Readmissions by Diagnosis.

BACKGROUND: Pediatric readmissions and postdischarge emergency department (ED) visits are common measures of postdischarge use and vary across pediatric conditions. Although often studied separately, these outcomes represent a continuum of hospital reuse. We introduce the ED/READ ratio (the ratio of postdischarge ED visits to readmissions) to characterize diagnosis-specific patterns of hospital reuse across common pediatric conditions. METHODS: In this single-center retrospective cohort study, we included children aged 0 to 17 years discharged from an urban tertiary children's hospital betwee

Hydroxyurea (hydroxycarbamide) in haemoglobin SC disease: Moving from therapeutic hesitation to evidence-guided care.

Girard et al. report a retrospective, single-centre, real-world study evaluating hydroxyurea (hydroxycarbamide) use in adults with haemoglobin SC disease in Quebec. Hydroxyurea was associated with reduced vaso-occlusive crisis composite events, mostly mild adverse events and no observed hyperviscosity-related complications, while highlighting adherence and prescription-renewal barriers in routine care. Drug Repurposing in Sickle Cell Disease: Evaluating Imatinib as a Therapeutic Candidate.

Sickle cell disease (SCD) is an inherited hemolytic hemoglobinopathy characterized by chronic hemolysis, vaso-occlusive pain crises, and progressive organ damage. Hydroxyurea, often combined with analgesics, remains a mainstay therapy, but may provide insufficient relief during active disease episodes. Drug repurposing offers a practical alternative where de novo drug development is limited, providing faster and more cost-effective therapeutic options. Imatinib (Gleevec), a tyrosine kinase inhibitor primarily used for chronic myeloid leukemia, has gained attention as a potential repurposed the

Blood Pressure Abnormalities Amongst Children with Sickle Cell Anaemia in Steady State and in Crises Attending a Tertiary Hospital in South East Nigeria.

BACKGROUND: Sickle cell anaemia (SCA) is a hereditary haemoglobin disorder associated with significant cardiovascular complications. Blood pressure abnormalities, including both hypertension and hypotension, are increasingly recognized as important contributors to morbidity in affected children. This study aims to determine the prevalence of hypertension and hypotension among children with SCA in steady state and during vaso-occlusive crisis, and to compare these findings with age- and sex-matched controls with normal haemoglobin genotype (HbAA). METHODOLOGY: This cross-sectional study includ

Vitamin D Deficiency as a Potential Modifier of Hematological Outcomes and Vaso-Occlusive Crisis Frequency in Sickle Cell Disease: Evidence from a Sudanese Cohort.

Background: Vitamin D deficiency is highly prevalent in patients with sickle cell disease (SCD) and may contribute to hematological dysregulation and increased vaso-occlusive crisis (VOC) burden. However, data from Sudan, a region with a substantial SCD burden, remain limited. This study investigated the prevalence and determinants of vitamin D deficiency and its association with hematological parameters and VOC frequency in Sudanese patients with SCD. Methods: A cross-sectional observational study was conducted among 50 patients with confirmed SCD attending a hematology outpatient clinic in S

HOPE Kids 2: Phase 3, Randomized Trial of Voxelotor in Children With SCD and Conditional Cerebral Blood Flow Velocities.

Children with SCD have elevated stroke risk, correlated with cerebral blood flow velocity (CBFV). HOPE Kids 2 was a phase 3, multicenter, double-blind, placebo-controlled trial evaluating the effect of voxelotor on CBFV. Participants aged 2 to < 15 years with SCD (HbSS/HbSβ0) and conditional CBFV (170 to < 200 cm/s) were randomized 1:1 to voxelotor (N = 120) or placebo (N = 116) for 96 weeks; 195 (83%) were in sub-Saharan Africa. The primary endpoint, CBFV change from baseline to Week 24, was significantly greater with voxelotor (least-squares mean change: voxelotor [n = 114] -12.06 cm/s, plac

Vaso-occlusive crisis in the cervicofacial region in sickle cell anemia.

BACKGROUND: Sickle cell anemia (SCA) is an autosomal recessive disorder characterized by a point mutation in the β-globin chain of the hemoglobin protein, causing abnormal folding. This can lead to sickling of red blood cells, agglutination, and ultimately vascular obstruction, which rarely occurs in the cervicofacial region. CASE DESCRIPTION: This report reviews a rare case of vaso-occlusive crisis (VOC) in the cervical vertebrae and mandible, manifesting as severe mandibular pain of unknown origin in a patient with SCA. The patient experienced diffuse body pain, 3 days in duration, and most

Systematic review of machine learning approaches for predicting sickle cell crisis and mortality risk at the climate-health nexus.

BACKGROUND: Sickle cell anemia (SCA) is a severe genetic blood disorder characterized by recurrent vaso-occlusive crises and increased mortality, with the greatest burden occurring in low- and middle-income countries. Climatic and environmental conditions, including temperature variability, humidity, rainfall, air pollution, and seasonal changes, have been associated with disease exacerbation. However, the extent to which these factors have been incorporated into predictive models remains unclear. This study systematically reviews the application of machine learning (ML) models for predicting

Splenic Sequestration and Suspected Transient Aplastic Crisis in Sickle-Cell SC/Hemoglobin-G Disease in an Adult: A Case Report.

Acute Splenic Sequestration Crisis (ASSC) and Transient Aplastic Crisis (TAC) are severe, but distinct, hematologic complications of sickle cell disease (SCD). ASSC typically presents with acute splenomegaly and anemia due to red blood cell (RBC) pooling, while TAC, often secondary to Parvovirus B19 infection, results in a sudden halt in erythropoiesis and reticulocytopenia. The concurrence of these two conditions is rare, particularly in adults and non-Hb-SS genotypes. We describe a 36-year-old woman with Hb-SC and Hb-G Philadelphia variant SCD who presented with non-specific symptoms who was
