Red blood cell transfusion to treat or prevent complications in sickle cell disease: an overview of Cochrane reviews.

BACKGROUND: Sickle cell disease (SCD) is one of the most common severe monogenic disorders globally. SCD can cause severe pain, significant end-organ damage, pulmonary complications, and premature death. Red blood cell (RBC) transfusions are used to treat complications of SCD, e.g. acute chest syndrome (ACS), or they can be part of a regular long-term transfusion programme to prevent SCD complications. OBJECTIVES: To summarise the evidence from Cochrane reviews of the benefits and harms of RBC transfusions versus no transfusion, or restrictive transfusion (to increase the total haemoglobin) v

Sickle cell anaemia with and without crises: An observational study of pregnancy outcomes.

We assessed the pregnancy outcomes of patients with sickle cell crises. We carried out a retrospective study of 2 698 556 pregnancies in Quebec, Canada between 1989 and 2022. The primary exposure was sickle cell anaemia with crisis during or outside of pregnancy. Outcomes included severe maternal morbidity and other pregnancy complications. We used log-binomial regression models to estimate risk ratios (RRs) and 95% confidence intervals (CIs) for the association between sickle cell crises and pregnancy outcomes. We found that patients with sickle cell crises were particularly at risk of severe

Sickle Cell Disease: From Ancient Origins to Modern Breakthroughs in Gene Therapy.

Sickle Cell Disease (SCD) is a hereditary hemoglobinopathy arising from a single-nucleotide transversion (GAG → GTG) at codon six of the HBB gene on chromosome 11, substituting glutamic acid with valine in the β-globin chain and producing hemoglobin S (HbS). Under hypoxic conditions, HbS polymerizes and distorts erythrocytes into the characteristic sickle shape, initiating a cascade of vaso-occlusion, chronic hemolytic anemia, and progressive multi-organ damage that defines the clinical burden of this disease. Although SCD has ancient origins in sub-Saharan Africa, the Indian subcontinent, the

Modelling irregular nonlinear waves: potential flow and volume-of-fluid comparison with experiment.

UNLABELLED: Cancer remains one of the leading causes of death worldwide and is characterized by the dysregulation of multiple signalling pathways involved in cell survival, proliferation, differentiation, and migration. Among these, fibroblast growth factor 2 (FGF2) serves as a key regulator that promotes tumor growth and metastasis and is frequently upregulated in several cancers, including glioblastoma, gastric and breast cancer, acute myeloid leukemia, nasopharyngeal carcinoma, and non-small cell lung cancer. Many cancers, such as glioblastoma where FGF2 plays a key role, remain incurable.

Structured missingness in biomedical research.

Antipathogen defense; b-caryophyllene; vector-virus mutualism Plants respond to insect herbivory by producing and releasing distinctive blends of volatile compounds. These herbivore-induced plant volatiles (HIPVs) serve several roles, including direct defense through toxic or repellent effects on herbivores and indirect defense by attracting the herbivores' natural enemies (Bleeker et al. 2009; Veyrat et al. 2016). HIPVs also act as warning signals, informing undamaged tissues within the same plant and nearby plants of a pending insect attack (Heil and Bueno 2007). HIPVs are mostly composed of

Multifocal osteomyelitis and abscess due to Bacteroides caccae in a child with sickle cell disease.

BACKGROUND: Diagnosing osteomyelitis in pediatric patients with sickle cell disease (SCD) is challenging as bone pain is often attributed to vaso-occlusive episodes ("pain crisis"). Such episodes can result in bone infarcts which can subsequently become sites of infection. Among non-SCD patients, Staphylococcus species are the most common cause of osteomyelitis whereas Salmonella spp. are more frequently the causative organisms in SCD patients [1]. We report the first case of Bacteroides caccae osteomyelitis in a SCD pediatric patient. CASE PRESENTATION: Our patient is a 6-year-old non-verbal

Liposomal amphotericin B (AmBisome) is a lipid-associated formulation of the broad-spectrum polyene antifungal agent amphotericin B. It is active against clinically relevant yeasts and moulds, including Candida spp., Aspergillus spp. and filamentous moulds such as Zygomycetes, and is approved for the treatment of invasive fungal infections in many countries worldwide. It was developed to improve the tolerability profile of amphotericin B deoxycholate, which was for many decades considered the gold standard of antifungal treatment, despite being associated with infusion-related events and nephr

Harnessing Artificial Intelligence to Restore Standards in Medical and Dental Education in Pakistan.

The month of March is dedicated to Colon Cancer Awareness. Worldwide, colorectal cancer (CRC) incidence has been on the rise. It is currently the third most common cancer in men (746,000 cases, 10.0% of the total) and the second in women (614,000 cases, 9.2% of the total).1 Arecent meta-analysis reported a 61% risk reduction in CRC incidence with colonoscopy.2 Unlike screening programs for breast and prostate cancers, not only has CRC screening reduced mortality from colon cancer and detected early CRC, it has also decreased the incidence of CRC through detection and removal of pre-cancerous l

Registry-based surveillance of paediatric home respiratory support in Switzerland: methodology and initial findings.

INTRODUCTION: The use of long-term home respiratory support in children has increased dramatically worldwide in recent decades. However, no national data are available in Switzerland since the last survey in 2001. In 2022, the national prospective Swiss Paediatric Home Respiratory Support (SwissPedHRS) registry was created on behalf of the Swiss Society of Paediatric Pulmonology, involving all centres caring for children with home respiratory support. Its main goal is to prospectively describe and study the population of children requiring home respiratory support in Switzerland. This first pu

Evaluating Zygomatic Implants in Orbital Floor Complications: A Clinical and Anatomical Study.

BACKGROUND: Sickle cell disease (SCD) is an inflammatory disease that can result in both chronic and acute inflammation. Immature granulocytes (IG) are not-yet-mature white blood cells that can be easily detected in complete blood count (CBC) tests. In recent studies it has been suggested that IG may play a role in determining the prognosis of inflammatory diseases. The aim of our study was to investigate the role of IG percentage on predicting acute chest syndrome (ACS) and the severity of vaso-occlusive crisis (VOC) in patients with SCD. METHODS: The study cohort consisted of 49 SCD patient

Targeting P-selectin and interleukin-1β in mice with sickle cell disease: effects on vaso-occlusion, liver injury and organ iron deposition.

Continuous vaso-occlusive and inflammatory processes cause extensive end-organ damage in adults with sickle cell disease (SCD), and there is little evidence that long-term hydroxyurea therapy prevents this. In initial trials, P-selectin blockade with crizanlizumab reduced SCD vaso-occlusive crisis frequency, and interleukin (IL)-1β inhibition in SCD patients, using canakinumab, lowered inflammatory markers. We used murine SCD models to examine the effects of acute and chronic blockade of P-selectin and of IL-1β on vaso-occlusive events, their inflammatory profile and organ health. Both approac

The Management of Postpartum Cardiorespiratory Failure in a Patient with COVID-19 and Sickle Cell Trait Requiring Extraorporeal Membrane Oxygenation Support and Airflight Transportation.

Acute cardiovascular disorders are incriminated in up to 33% of maternal deaths, and the presence of sickle cell anemia (SCA) aggravates the risk of peripartum complications. Herein, we present a 24-year-old Caribbean woman with known SCA who developed a vaso-occlusive crisis at 36 weeks of gestation that required emergency Cesarean section. In the early postpartum period, she experienced fever with rapid onset of acute respiratory distress in the context of COVID-19 infection that required tracheal intubation and mechanical ventilatory support with broad-spectrum antibiotics and blood exchang

An efficient heuristic for geometric analysis of cell deformations.

Sickle cell disease causes erythrocytes to become sickle-shaped, affecting their movement in the bloodstream and reducing oxygen delivery. It has a high global prevalence and places a significant burden on healthcare systems, especially in resource-limited regions. Automated classification of sickle cells in blood images is crucial, allowing the specialist to reduce the effort required and avoid errors when quantifying the deformed cells and assessing the severity of a crisis. Recent studies have proposed various erythrocyte representation and classification methods (Jennifer et al., 2023 [1])

Sickle cell trait does not cause "sickle cell crisis" leading to exertion-related death: a systematic review.

Globally, an estimated 300 million individuals have sickle cell trait (SCT), the carrier state for sickle cell disease (SCD). Although SCD is associated with increased morbidity and shortened life span, SCT has a life span comparable with that of the general population. However, "sickle cell crisis" has been used as a cause of death for decedents with SCT in reports of exertion-related death in athletes, military personnel, and individuals in police custody. To appraise this practice, the American Society of Hematology convened an expert panel of hematologists and forensic pathologists to cond
