[Neonatal screening for sickle cell disease in South Kivu, eastern Democratic Republic of Congo : a hospital-based cross-sectional study].

Newborn screening for sickle cell disease (SCD) is a key public health intervention. Data from eastern Democratic Republic of the Congo (DRC) are limited. This study assessed the prevalence of SCD and the feasibility of screening in maternity units in South Kivu. Cord blood was analyzed by mass spectrometry. Among 1.300 newborns, 128 had sickle cell trait (9.8 %; 95 % CI: 8.2-11.5 %), and no homozygous cases. Screening is feasible at birth and findings support its integration into routine maternal and child health services.

Probing Red Blood Cell Deformability in Sickle Cell Disease, Sickle Cell Trait, and Healthy Controls by Ektacytometry.

INTRODUCTION: Red blood cell (RBC) deformability is a key determinant of sickle cell disease (SCD) pathophysiology. We evaluated RBC deformability across HbSS, HbAS, and HbAA genotypes and investigated its associations with hemolysis, fetal hemoglobin (HbF), coinherited α-thalassemia, hydroxyurea treatment, and the PIEZO1 E756del variant. METHODS: We used Lorrca ektacytometry to compare RBC deformability across 174 participants with HbSS, HbAS, and HbAA genotypes. Deformability was evaluated across a range of shear stresses, osmotic pressures, and oxygen tensions. Associations between deforma

APOL1 Risk Genotypes and Sickle Cell Trait are Associated with Kidney Disease Progression in an Afro-Brazilian Population.

BACKGROUND: Apolipoprotein L1 (APOL1) risk alleles and sickle cell trait (HbAS) are established risk factors for CKD, but their distribution and clinical impact in admixed populations remain incompletely defined. We conducted a Brazilian population-based study integrating APOL1 genotypes, sickle cell trait, kidney histopathology, and longitudinal clinical outcomes. METHODS: We studied 889 newborns from a population-based survey and 424 patients with biopsy-proven kidney disease. APOL1 genotypes and hemoglobin variants were determined by PCR-based sequencing and high-performance liquid chromat

Hepatitis A-associated oxidative haemolysis in a patient with glucose-6-phosphate dehydrogenase deficiency with co-inherited sickle cell trait.

An adult male patient in his late teens presented with pruritus, nausea and progressive jaundice after travel to West Africa. He did not get a pre-travel hepatitis A vaccination or malaria chemoprophylaxis. Laboratory testing demonstrated hepatocellular injury with alanine aminotransferase (ALT) 4002 U/L (reference <60), marked hyperbilirubinaemia (total bilirubin 632 µmol/L, reference <20) and a mildly prolonged international normalised ratio (INR). Concurrently, he developed non-immune haemolysis with undetectable haptoglobin, elevated lactate dehydrogenase and reticulocytosis, as well as bi

Acoustic probing of new biomarkers for rapid sickle cell disease screening.

Sickle cell disease (SCD) remains a critical global health issue, with high child mortality in low-resource regions. Early diagnosis is essential for improving health outcomes, but conventional diagnostic methods are unsuitable for widespread use due to the high costs of laboratory equipment. There is an urgent need for portable, cost-effective, and user-friendly point-of-care diagnostic tools that can quickly assess blood health. Here, we explore two new biomarkers enabled by acoustic probing for rapid SCD diagnosis: cell membrane stability from measuring red blood cell (RBC) lysis temperatur

Whole Blood Transcriptomic Analysis of Sickle Cell Trait.

Sickle cell trait (SCT) is the heterozygous carrier state for the HBB missense variant which causes sickle cell disease (SCD). SCT has been associated with increased risk of venous thromboembolism and chronic kidney disease as well as alterations in clinical laboratory parameters. To investigate differential gene expression in SCT, we used RNA sequencing of whole blood samples collected from 805 African American female participants (143 SCT; 660 controls) from the Women's Health Initiative Long Life Study (mean age = 76). We identified 226 differentially expressed genes (DEGs) in SCT compared

Carbon monoxide treatment rescues leukofiltration and preserves storage quality of red blood cell concentrates from sickle cell trait donors.

BACKGROUND: Filtration failures in sickle cell trait (SCT, AS) blood donations limit the availability of antigen-matched red blood cell concentrates (RBCCs) for transfusion. Carbon monoxide (CO), by stabilizing hemoglobin in its high-affinity relaxed state, may prevent filter clogging and restore leukofiltration efficiency. However, the storage quality and stability of CO-treated RBCCs remain to be evaluated. STUDY DESIGN AND METHODS: RBCCs from normal (AA) donors and AS donors with prior leukofiltration failure were categorized as AA-NC (untreated AA), AA-CO (CO-treated AA), and AS-CO (CO-tr

Effectiveness of preimplantation genetic testing in sickle cell disease: insights from a single-center experience.

PURPOSE: Sickle-cell disease (SCD) is a severe autosomal recessive disorder. At-risk couples may prevent transmission either through prenatal diagnosis with possible termination of pregnancy or preimplantation genetic testing for monogenic disease (PGT-M). Data on PGT-M outcomes in this population remain scarce. METHODS: We conducted a monocentric retrospective study (2006-2021). To assess ovarian response to stimulation, each PGT-M cycle for SCD was matched with two control cycles. RESULTS: Sixty couples underwent at least one ovarian stimulation for PGT procedure for SCD. Eight couples (13

Clinicopathological features of SMARCB1/INI1-deficient medullary-like renal cell carcinoma: Report of 3 cases and literature review.

SMARCB1/INI1-deficient medullary-like renal cell carcinoma (MLRCC), also used to be known as unclassified renal cell carcinoma with medullary phenotype (RCCU-MP), is an extremely rare and highly aggressive kidney tumor. The 2022 World Health Organization (WHO) classification proposes that high-grade renal cell carcinomas exhibiting morphological and immunophenotypic features identical to renal medullary carcinoma, but without evidence of sickle cell trait or disease, should be diagnosed as SMARCB1/INI1-deficient medullary-like renal cell carcinoma(MLRCC) and classified as a subtype of SMARCB1-

Recurrent Priapism Associated With Occupational Cocaine Exposure in a Patient With Sickle Cell Trait: A Case Report.

Cocaine is an uncommon but recognized cause of ischemic priapism due to its potent sympathomimetic and vasoactive properties. Case reports involving ischemic priapism related to intentional parenteral, intranasal, intracavernosal, and topical cocaine have been documented. Recurrent priapism resulting from occupational exposure to cocaine powder, however, has not been previously described. We report the case of a 37-year-old African American man with sickle cell trait who presented to the emergency department at least six times within a 12-month time period with recurrent priapism. Each episode

Is sickle cell trait truly benign? A case-control study of physical and mental health outcomes from the sickle cell belt of Eastern India.

Sickle cell trait (SCT) has long been considered a benign carrier condition; however, emerging evidence suggests that individuals with SCT may experience adverse health outcomes, both physical and mental. These findings challenge the conventional notion of SCT as clinically insignificant. Therefore, the present study aims to capture the physical and mental health of SCT individuals among scheduled caste (SC) and scheduled tribe (ST) populations of Odisha, India. This cross-sectional study was conducted in the hard-to-reach regions, primarily located in the mountainous terraces of Koraput distr

Radiomic Features Extracted from Magnetic Resonance Imaging to Identify Arteriopathy in a Humanized Mouse Model of Sickle Cell Anemia and Sickle Cell Trait.

PURPOSE: Sickle cell disease (SCD) is a hereditary blood disorder that increases stroke risk in children. Previously, we showed that SCD caused accelerated arteriopathy in a sickle cell transgenic mouse model compared to sickle cell trait controls. We sought to determine whether radiomics analysis of label-free carotid artery magnetic resonance angiography (MRA) can differentiate between SCD mice (SS) from heterozygous sickle cell trait control mice (AS). Radiomics analysis of MRI data was used to extract quantitative imaging features, then tested for discrimination between SS and AS mice. ME

Pregnancy-Triggered Vaso-Occlusive Crises in Hemoglobin SE Disease Complicated by Glucose-6-Phosphate Dehydrogenase Deficiency: A Case Report.

Hemoglobin SE (HbSE) disease is a rare hemoglobinopathy that may be associated with significant clinical complications despite its traditional characterization as a mild condition, and concomitant glucose-6-phosphate dehydrogenase (G6PD) deficiency can further complicate diagnosis and management. We report the case of a 40-year-old Saudi woman with known G6PD deficiency who experienced recurrent vaso-occlusive crises triggered by pregnancy. She first presented postpartum in 2017 with severe joint pain, and initial hemoglobin electrophoresis suggested sickle cell trait (HbS: 27.1%, HbA: 67.9%).

Alterations of Plasma Metabolites Associated with Sickle Cell Trait.

KEY POINTS: We identified 69 plasma metabolites associated with sickle cell trait, including markers of eGFR and/or related to oxidative stress pathways. Twenty-five percent or 39% of the sickle cell trait-associated metabolites were replicated in the Atherosclerosis Risk in Communities study. Sickle cell trait-associated metabolites individually or in aggregate were associated with better prediction of incident kidney failure in those with sickle cell trait. BACKGROUND: Sickle cell trait (SCT) is the heterozygous carrier state for sickle cell disease (SCD) and is common among individuals of

Exertional Rhabdomyolysis, Hyposthenuria, and Acute Kidney Injury: The Non-benign Side of Sickle Cell Trait.

Sickle cell trait (SCT) is largely understood to be a clinically silent disease that typically does not require intensive clinical monitoring or counseling of patients. In fact, many patients with SCT are unaware that they have this genetic condition. However, emerging studies, case reports, and reviews increasingly demonstrate that severe clinical pathology can be associated with SCT, showcasing the need for improved counseling and education. We present the case of a healthy young male patient who was admitted to the hospital with rhabdomyolysis, acute liver injury, extreme electrolyte distur

A population-scale Red Blood Cell proteome atlas of 13,000 donors uncovers genetically encoded aging clocks predicting hemolysis, transfusion efficacy, and donor activity a decade later.

UNLABELLED: As the most abundant human cell and the foundation of transfusion medicine, red blood cells (RBCs) offer a unique readout of systemic health, yet they have never been characterized at population scale. We generated a proteome atlas of 13,091 blood donors with multi-omics longitudinal phenotyping, characterizing the influence of demographics and genetic variation on the reproducibility of RBC proteomes across donations. Elastic-net aging clocks captured biological aging with high accuracy and uncovered genetic regulators of ΔAge at FN1, C4/IKZF1, CRAT, PFAS, TRIM58. Across independe

Birth Prevalence of Sickle Cell Disease in India: A Systematic Review and Meta-Analysis.

Newborn screening helps identify sickle cell disorder (SCD) early and to promptly initiate effective measures. It is estimated that India accounts for approximately 16% of global annual births with SCD. Multiple reports of screening for SCD in India have emerged in the last decade. Our aim was to pool the birth prevalence of SCD and sickle cell trait (SCT). A systematic review of published evidence on nontargeted, universal screening for SCD or SCT in newborns was performed (16 studies). The pooled prevalence of SCD was 1100 per 100,000 (10 studies, 88,276 neonates, 95% CI: 432, 1768), while t
