DICER1-mutated renal neoplasia: A series of 5 cases demonstrating the spectrum of cystic nephroma, Wilms tumor, and anaplastic sarcoma.

OBJECTIVES: The DICER1 gene encodes a ribonuclease essential for microRNA processing and regulates diverse cellular functions. Germline DICER1 alterations underlie DICER1-related tumor predisposition syndrome, which can lead to renal neoplasia. Herein, we studied the spectrum of DICER1-altered renal neoplasia. METHODS: Our institutional archives were queried for DICER1-mutated renal neoplasia. Relevant clinicopathologic features and molecular profiling results were summarized. RESULTS: We report five renal tumors that reinforce the clinicopathologic spectrum of DICER1-associated renal neopla

Assessment of Knowledge, Awareness, and Attitude toward Premarital Screening for Sickle Cell Disease among Young Adults in Central India - A Cross-sectional Study.

INTRODUCTION: Sickle cell disease (SCD) is a prevalent hemolytic disorder stemming from structurally abnormal hemoglobin, inducing intermittent sickle-shaped red blood cell formation. Its hereditary transmission follows an autosomal recessive pattern and is notably widespread in central India. SCD poses a substantial health challenge, potentially leading to organ failure and fatality in severe cases. Advocating for SCD awareness and premarital screening among young adults assumes critical importance, aiming to prevent unions between carriers of sickle cell trait (SCT) and thus reduce the likel

Renal medullary carcinoma: a systematic review of clinical and radiological findings.

BACKGROUND AND AIMS: Renal medullary carcinoma (RMC) is a rare subtype of renal cell carcinoma, typically described in young patients with sickle cell trait, which follows an aggressive course. We aimed to perform a systematic review of all published cases of RMC from 1995 to 2024, to summarise the clinico-pathological and radiological features. METHODS: A systematic review of all published cases of RMC from 1995 to 2024 was performed. Patient demographics, clinical presentation, radiological features, metastatic profile, management, and overall survival were recorded. Sub-analysis was perfor

Distribution of Abnormal Hemoglobins in Saudi Arabia: Analysis of Private Sector Laboratory Data.

BACKGROUND: Hemoglobinopathies are inherited blood disorders causing both a global and regional burden on health care systems and on affected individuals. Our study describes the epidemiological and statistical distribution of hemoglobin patterns in individuals referred to a large private-sector laboratory network in Saudi Arabia. OBJECTIVE: Our study investigates the distribution of hemoglobin patterns among individuals tested at Alborg laboratories, their Complete Blood Count (CBC) parameters as well as regional distributions. METHODS: This is a retrospective analysis of a 10-year database t

Lower efficacy of transfusion of red blood cells from donors with sickle cell trait.

Sickle cell trait (SCT), the heterozygous state for the hemoglobin S (HbS) mutation, affects roughly 1 in 13 African American individuals and is common among blood donors recruited for antigen-matched transfusions in sickle cell disease (SCD). While individuals with SCT are typically asymptomatic, it is unclear whether red blood cells (RBCs) from SCT donors have impaired storage quality and transfusion efficacy. Here, we integrate multi-omics to characterize RBCs from donors SCT and evaluate their performance post-transfusion. We first profiled RBCs from 174 HbAS and 248 HbAA volunteers, ident

Bilateral Avascular Necrosis of the Femoral Head in a Patient With Sickle Cell Trait: A Case Report.

Sickle cell trait (SCT), historically regarded as a benign carrier state, may nonetheless be associated with severe complications such as bilateral avascular necrosis (AVN) of the femoral head in select individuals. This risk is often underrecognized, particularly in regions with high SCT prevalence, leading to delayed diagnosis and management. We report the case of a 43-year-old Yemeni man who presented with progressive bilateral hip pain over eight months without traditional risk factors except smoking. His family history was notable for sickle cell disease and trait. Physical examination re

Evaluating blood donation safety for donors with sickle cell trait.

BACKGROUND AND OBJECTIVES: Finding compatible blood donors for patients from minority racial and ethnic populations can be challenging due to differences in blood group antigen frequencies with the existing donor pool. Individuals with sickle cell trait (SCTrait) are allowed to donate in some countries, but in the Netherlands, carriers are currently not permitted to donate. This study estimates the incidence of adverse events among SCTrait donors compared with non-SCTrait donors, to inform potential policy decisions. MATERIALS AND METHODS: A retrospective study was performed at Sanquin, inclu

Major Discordance in Hemoglobin A1c Measurement in Sickle Cell Trait: Effect of Analytical Methods.

Hemoglobin A1c (HbA1c) is widely used for diagnosing and monitoring diabetes mellitus and reflects long-term glycemic control. However, hemoglobin variants may interfere with certain assay methods, leading to misleading results. We report the case of a 54-year-old woman with sickle cell trait and long-standing diabetes who presented with markedly discordant HbA1c values obtained using two different methods. High-performance liquid chromatography (HPLC) showed a significantly elevated HbA1c level, whereas a fluorescence immunoassay yielded a substantially lower value, despite persistently eleva

Evaluation of Implementation of Newborn Screening for Sickle Cell Disease Program in Selected Hospitals in Dar es Salaam, Tanzania.

Sickle cell disease (SCD) is a major public health concern in Tanzania where approximately 11,000 children are born with the condition annually. Newborn screening (NBS) enables early diagnosis and timely intervention. Despite the proven effectiveness of NBS in reducing early mortality from SCD, implementation in Tanzania remains limited to pilot programs at facilities such as Temeke and Amana Regional Referral Hospitals (RRHs) in the city of Dar-es-salaam. This study evaluated the implementation of NBS for the SCD Program at Temeke and Amana RRHs. An explanatory mixed-methods process evaluatio

Kidney function among adult sickle cell disease patients at Ahmadu Bello University Teaching Hospital, Zaria, Nigeria.

BACKGROUND: Sickle cell disease (SCD) patients are certainly experiencing improved longevity with improved care and it is not unexpected that long-term complications including kidney diseases are being seen more often than before. Unfortunately, late diagnosis is not unusual. This can in part be explained by some aspects of pathogenesis of kidney disease in SCD including body compensatory mechanisms such as hyperfiltration with its associated lower creatinine levels and exaggerated estimated glomerular filtration rate (eGFR). Worse still, no single eGFR formula has been validated across board

A Chatbot to Meet Parents' Information Needs for Sickle Cell Trait Newborn Screening Results: Multiple Methods Formative Study.

BACKGROUND: Newborn screening (NBS), a mandated public health intervention, allows the identification of babies with potentially life-threatening disorders and facilitates disease diagnosis and management before the onset of symptoms. While NBS saves lives, the process can be fraught with anxiety and unanswered questions from parents or guardians of newborns, especially as they wait for an appointment with a clinician. OBJECTIVE: This study aimed to describe the development and testing of an educational chatbot (NBSchat) to address the emotional support and information needs of parents of new

Heat-Related Illnesses.

Heat-related illnesses encompass a spectrum of disorders resulting from exposure to high temperatures and the body's inability to efficiently dissipate heat. These syndromes range from mild illnesses, such as heat edema, heat rash, and heat cramps, to more concerning manifestations such as exercise-associated collapse, heat exhaustion, and exertional rhabdomyolysis. The most severe and potentially life-threatening heat-related illnesses include exercise collapse associated with sickle cell trait, severe exercise-associated hyponatremia, and heat stroke. Common risk factors of heat-related illn

No overall increased risk of death in individuals with sickle cell trait: a study of 467 779 general population adults.

Uncertainty remains about whether sickle cell trait (SCT) shortens life or predisposes to disease. Therefore, we examined whether SCT carriers had an increased risk of death or diseases linked to SCT. We studied 467 779 general population individuals from the UK Biobank. All had undergone whole-exome sequencing and were observed prospectively for a median of 15 years. Individuals with SCT (n = 1253) were not at an increased risk of all-cause death (hazard ratio [HR], 1.04; 95% confidence interval [CI], 0.84-1.29) or cardiovascular death (1.34; 0.90-2.00) or for having myocardial infarction (0.

Suzetrigine for Vaso-Occlusive Pain in Sickle Cell Disease: A Three-Patient Case Series.

BACKGROUND: Sickle cell disease (SCD) is characterized by recurrent vaso-occlusive crises and chronic pain, often requiring multimodal analgesia that frequently includes opioids. Given the risks associated with long-term opioid therapy, there is a growing need for effective non-opioid alternatives. Suzetrigine, a selective NaV1.8 sodium channel inhibitor, represents a novel analgesic approach targeting peripheral nociceptive signaling. METHODS: We present a case series of three patients with sickle cell-related pain treated with suzetrigine as part of a multimodal pain management strategy. Th

A Novel F5 Gene Variant Associated With Early-Life Ischemic Brain Injury in a Child With a Familial Coagulation Disorder.

Perinatal and early-life arterial ischemic stroke are important causes of hemiparesis and lifelong neurodevelopmental morbidity. While many cases are considered idiopathic, inherited thrombophilia is increasingly implicated in selected patients. We report an infant who developed right-sided motor impairment recognized at nine months of age. He was born at term after an uncomplicated pregnancy, although the neonatal period was complicated by transient respiratory distress and a small atrial septal defect requiring brief neonatal intensive care admission. Examination demonstrated right spastic h

Factors associated with anemia among adolescent girls in Western India: insights from a multi-centric cross-sectional study.

BACKGROUND: Anemia remains a persistent public health challenge among adolescent girls in India, with states like Gujarat continuing to report a high burden despite ongoing national nutrition and anemia control programs. Regional disparities, particularly across rural and tribal populations, further exacerbate the problem. This study aimed to estimate the prevalence of anemia among adolescent girls in Gujarat, India, and examine its sociodemographic, nutritional, behavioral, hematological, and biochemical associated factors. METHODS: A community-based, multi-centric cross-sectional study was

"If People Know, They Will Think Something Is Wrong with Me": An Ethnography of Sickle Cell Hemoglobinopathy, Stigma, and Health Disparities in Tribal Odisha, India.

Sickle cell hemoglobinopathy is not merely a genetic disorder but a deeply embedded socio-cultural phenomenon in the remote regions of Odisha. Drawing on long-term ethnographic fieldwork in Koraput and surrounding districts, this study explores the lived experiences of individuals with sickle cell trait (SCT) and sickle cell disease (SCD), focusing on social discrimination, medical uncertainty, and healthcare accessibility. The findings reveal reluctance toward screening, particularly among unmarried women, due to fears of marital exclusion and family stigma. Misconceptions about SCT, includin
