A Case of Refractory Thrombocytopenia in Pregnancy.

Immune thrombocytopenia is an acquired autoimmune disorder characterized by isolated thrombocytopenia due to antibody-mediated platelet destruction and impaired platelet production. Thrombocytopenia in pregnancy is common, but severe thrombocytopenia requires prompt evaluation to distinguish benign gestational thrombocytopenia from immune thrombocytopenia, hypertensive disorders of pregnancy, hemolysis, elevated liver enzymes, low platelet count syndrome, and thrombotic microangiopathies. We present the case of a 19-year-old gravida 1 para 0 patient with sickle cell trait and iron deficiency a

Eligibility of blood donors with thalassaemia trait, sickle cell trait or glucose-6-phosphate-dehydrogenase deficiency in Italy: Results of a national survey and practical guidance.

BACKGROUND AND OBJECTIVES: Thalassaemia trait (ThalT), glucose-6-phosphate dehydrogenase (G6PD) deficiency and sickle cell trait (SCT) are prevalent inherited red blood cell disorders among blood donors in Italy, particularly in southern and insular regions, and among donors of non-European origin. Despite their frequency, harmonized national guidance on eligibility of carriers for blood and apheresis donation has been lacking, resulting in heterogeneous practices across Italian transfusion centres. MATERIALS AND METHODS: Between 17 January and 16 February 2024, the Italian National Blood Cen

An Atypical Finding in a Patient With Acute Left Flank Pain.

Hemorrhagic adrenal infarction is a rare and potentially under-recognized cause of acute flank pain. We report the case of a 29-year-old woman presenting with acute left flank pain of two days' duration. Initial contrast-enhanced abdominal CT, including unenhanced and portal venous phases, was interpreted as normal. Retrospective review demonstrated subtle enlargement of the left adrenal gland with mildly reduced enhancement compared with the contralateral gland. Following clinical deterioration with fever, a repeat CT performed five days later showed marked enlargement of the left adrenal gla

A Rare Case of Mononucleosis-Induced Splenic Infarction in a Patient With Sickle Cell Trait.

Splenic infarction is a rare complication of Epstein-Barr virus (EBV) mononucleosis, particularly in patients with underlying hematologic conditions. We present a rare case of EBV-associated splenic infarction in a 20-year-old male with sickle cell trait (SCT), highlighting the potential overlap between SCT and infection-related ischemic complications. The patient presented to the emergency department with one week of left upper quadrant pain, fever, night sweats, dyspnea, and concentrated urine following suspected dehydration after outdoor activity. Initial Monospot testing was negative. CT i

Major features of parasite adaptation revealed by genomes of Plasmodium falciparum population samples archived for over 50 years.

Understanding evolution of human pathogens requires looking beyond the effects of recent interventions. To study malaria parasites prior to widespread drug selection, Plasmodium falciparum genomes were sequenced from the oldest population-based set of archived research samples yet identified, placental blood collected in the Gambia between 1966 and 1971. High-quality data were obtained from 54 infected samples, showing that genomic complexity within infections was high, most infections were genetically unrelated, and no drug resistance alleles were detected. Strong signatures of positive selec

A maximal-coverage approach to prioritizing newborn sickle cell screening sites in Uganda using a sickle cell-malaria co-risk surface.

BACKGROUND: Uganda has a high, spatially heterogeneous burden of sickle cell disease (SCD) with national screening indicating sickle cell trait at 13.1 percent and disease at 0.7 percent, concentrated in the northern and eastern regions where Plasmodium falciparum transmission remains intense. Newborn sickle cell screening and follow up care programmes need a robust method to prioritize the selection of screening hubs that aligns sickle cell genetic risk and malaria transmission intensity with travel-time access to health facilities where newborn screening and early sickle cell anaemia (SCA) c

Kinetics of antimalarial antibodies in children with common haemoglobinopathies in a Tanzanian population.

INTRODUCTION: Haemoglobin gene mutations have long been associated with protection against malaria, as evidenced by the concordant geographic distribution of these mutations with malaria parasite prevalence and by the reduced risk of severe disease among individuals carrying thalassaemia or sickle cell trait alleles. However, the mechanisms underlying this protection remain poorly understood. Although the precise correlates of immunity to malaria are still debated, antibody-mediated responses are widely considered to play a critical role. In this study, we investigated changes in putatively pr

Natural History and the Burden of Malaria During the First Year of Life in the High-Transmission Setting of Uganda.

Infants under 1 year of age are considered partially protected from malaria because of maternal antibodies and fetal hemoglobin. However, emerging evidence suggests that the malaria burden in this age group may be underestimated. A cohort of 855 infants in Busia District, Uganda, was enrolled in the present study to characterize malaria incidence and parasite prevalence during the first year of life and to identify risk factors for these outcomes. The study was conducted from 2021 to 2025, before the malaria vaccine roll-out. Infants born to HIV-uninfected women were enrolled at 4-8 weeks of a

Haematological responses in children under 5 years with malaria and soil-transmitted helminth coinfections in Delta State, Nigeria: a cross-sectional study.

BACKGROUND: Malaria and soil-transmitted helminth (STH) infections often co-occur in Nigerian children under 5 years of age, affecting haematological functions. However, local data on their combined effect are limited. OBJECTIVES: To determine the prevalence and impact of malaria, STH and malaria-STH co-infection on haematological parameters in children under five in the Delta South Senatorial District in Nigeria. DESIGN: Hospital-based cross-sectional study. METHODS: A total of 269 children under 5 years of age attending selected health centres were recruited across the Delta South Senator

Sickle Trait Hemoglobin Does Not Influence Anopheles Biting Rates.

Children with sickle cell trait (HbAS) are protected against severe and symptomatic Plasmodium falciparum (P. falciparum) malaria. Although several within-host resistance mechanisms have been investigated, it is unknown whether this protection is attributable in part to reduced exposure to P. falciparum parasites via mosquito bites. In a 15-month cohort in Western Kenya, mosquito blood meals were matched to human hosts on the basis of short tandem repeat genotypes to determine individual mosquito biting rates. Using a multilevel multivariable model, mosquito biting behavior was assessed with r

Resveratrol alleviates osteoarthritis progression by inhibiting NLRP3 inflammasome response via the AMPK/PPARγ signaling pathway.

PURPOSE: It is not clear whether vasopressin versus norepinephrine changed mortality in clinical practice in the Vasopressin and Septic Shock Trial (VASST) coordinating center hospital after VASST was published. We tested the hypothesis that vasopressin changed mortality compared to norepinephrine using propensity matching of vasopressin to norepinephrine-treated patients in the VASST coordinating center hospital before (SPH1) and after (SPH2) VASST was published. METHODS: Vasopressin-treated patients were propensity score matched to norepinephrine-treated patients based on age, APACHE II, re

Active learning for the accelerated discovery of complex concentrated NiCoCr alloys in additive manufacturing.

BACKGROUND: According to the World Health Organization, air pollution is closely associated with climate change and, in particular, with global warming. In addition to melting of ice and snow, rising sea level, and flooding of coastal areas, global warming is leading to a tropicalization of temperate marine ecosystems. Moreover, the effects of air pollution on airway and lung diseases are well documented as reported by the World Allergy Organization. METHODS: Scientific literature was searched for studies investigating the effect of the interaction between air pollution and climate change on

The authority and politics of epiphanic experience.

BACKGROUND: Attention deficit hyperactivity disorder (ADHD) is a common and growing problem and a leading cause of child referrals to Child and Adult Mental Health Services (CAMHS). It is a drain on resources across nationally funded support agencies and associated with negative outcomes such as early criminality, school disruption and antisocial behaviour. Mainstream interventions (pharmacological and behavioural) demonstrate effectiveness whilst implemented, but are costly, often have unwanted side effects and do not appear to be affecting long-term outcomes.Development of a robust evidence

Melioidosis in sickle cell hemoglobinopathies in eastern India: Cavitating pneumonia and immune susceptibility - a case series-based insight.

Melioidosis, caused by Burkholderia pseudomallei, is an emerging infection in the tropics. Most of the melioidosis patients have underlying co-morbidities or occupational exposures. Hemoglobinopathies, such as thalassemia, are established risk factors for melioidosis; however, the association of SCD with melioidosis is infrequently reported. We report two culture-confirmed melioidosis cases from eastern India: a 58-year-old male with SCD on hydroxyurea and another 44-year-old with sickle cell trait, both presenting with cavitary lung lesions mimicking tuberculosis. SCD-related functional asple

Mechanism of activation of an ancestral TEC kinase by PIP3.

BACKGROUND: The introduction of second-generation antipsychotics (SGAs) over the past 2 decades generated considerable optimism that better antipsychotic treatments for schizophrenia and bipolar disorder were possible. SGAs offer several tolerability benefits over first-generation antipsychotics (FGAs), particularly with respect to extrapyramidal symptoms. However, SGAs can induce serious metabolic dysregulations, especially in drug-naive, first-episode, and child and adolescent populations, with olanzapine and clozapine having the highest propensity to cause these abnormalities. Characteristics of patients hospitalized for diabetic foot in a national hospital in Peru, 2024.

INTRODUCTION: Expansive processes around the third ventricle have always been a major neurosurgical challenge. Despite all the technological and scientific progress recorded over the last few years, third ventricle tumors are still a very difficult pathology to approach due to their formidable complexity.Treating such a tumor demands a good knowledge of local anatomy, pathophysiology, pathology and a good capacity to integrate all the data gathered from the patient prior to and during surgery. Last but not least, the correct means to approach such a tumor involves using modern neuronavigation

Prevalence, knowledge, awareness, attitudes, perceptions and practices of G6PD deficiency among mothers, children and pregnant women.

BACKGROUND: Glucose-6-phosphate dehydrogenase (G6PD) deficiency is one of the most common inherited enzymatic disorders worldwide, with significant clinical implications for mothers, children, and pregnant women. Despite its prevalence, knowledge, awareness, and preventive practices remain inadequate, particularly in high-burden regions. This review aimed to systematically synthesise evidence on the prevalence of G6PD deficiency and the knowledge, attitudes, perceptions, and practices surrounding the condition among pregnant women, mothers, and children. METHOD: A systematic review was conduc
