BACKGROUND: Pregnancy in women with Sickle Cell Disease (SCD) is associated with an increased risk in severe complications, such as eclampsia, pre-eclampsia, maternal death, intrauterine growth restrictions, perinatal mortality, and low birth weight. Since 50% of these patients are now living to reproductive age, the management of SCD during pregnancy has become pertinent. Searching for prophylactic and affordable measures for early prevention of these complications is urgently needed.
METHODS: Daily low dosage of Aspirin is widely used during pregnancy to prevent pre-eclampsia and other vasc

Nonmyeloablative-related haploidentical allogeneic stem cell transplantation (NMAC-HID allo-HSCT) has emerged as an additional treatment to achieve durable remission in sickle cell disease (SCD), a prevalent blood disorder characterized by painful vaso-occlusive crises and chronic anemia. The standard of care (SOC) for SCD includes hydroxyurea, pain management, and blood transfusion, but patients with SCD still lose several decades of life expectancy. Gene therapy (GT) for SCD is the other treatment for lifelong disease amelioration in SCD, with accessibility limited by cost and manufacturing

UNLABELLED: Sickle cell disease has a profound impact on the physical, mental and social health of affected children. Currently, there is considerable variability among the available patient reported outcomes measures (PROMs) used in children with sickle cell disease, and no consensus has yet been achieved. We aim to assess the psychometric properties of the generic pediatric and proxy Patient-Reported Outcomes Measurement Information System (PROMIS®) measures in children with sickle cell disease living in the Netherlands. Dutch children with sickle cell disease aged 5-17 years old and their c

Iron overload is a significant cause of transfusion-related morbidity and mortality. Deferiprone (DFP) is an established oral iron chelator approved for the treatment of transfusional iron overload in patients with thalassemia, sickle cell disease (SCD), or other anemias. Although DFP was initially approved as a thrice daily formulation, a twice daily formulation was developed to improve patients' treatment adherence. Here, we assess the safety of DFP twice daily in patients with thalassemia, SCD, or other anemias in real-world clinical practice in the United States. Data for patients referred

Hydroxyurea (HU) is prescribed to reduce systemic complications of sickle cell disease; however, its association with sickle cell retinopathy (SCR) remains unclear. This meta-analysis (PROSPERO: CRD420251079006) evaluates the pooled association between HU use and SCR. In the primary analysis, HU use was not associated with a statistically significant reduction in SCR (OR = 0.90, 95% CI [0.41, 1.97], p = 0.79; I² = 77%). In a prespecified leave-one-out analysis, exclusion of the outlier study did not produce a statistically significant association, although the estimate shifted toward a stronge

INTRODUCTION: Hemoglobinopathies, the most prevalent recessive monogenic disorders globally, encompass thalassemia syndromes and structural hemoglobin variants, affecting approximately 5% of the world's population as carriers, with around 315,000 affected births annually. These conditions are especially common in regions including the Mediterranean Basin, parts of Africa, the Middle East, India, and Southeast Asia. Traditional screeningcomplete blood count (CBC), high-performance liquid chromatography (HPLC), and capillary electrophoresis (CE)followed by molecular genetic confirmation, require

Patients with paroxysmal nocturnal hemoglobinuria (PNH) on anti-C5 often experience extravascular hemolysis with anemia. Iptacopan, the first oral proximal complement inhibitor targeting factor B, has shown efficacy and safety in PNH patients. APPULSE-PNH (NCT05630001), a phase 3b, single‑arm, open-label trial, enrolled adult patients with PNH and hemoglobin ≥10 g/dL on stable anti-C5 for ≥6 months. Patients switched to iptacopan monotherapy (200 mg twice daily; 24 weeks). Primary endpoint: mean hemoglobin change from baseline across four visits (Days 126-168). At baseline, 57.7% of patients h

Sickle cell disease (SCD) is characterized by chronic hemolysis, ischemia-reperfusion injury, and progressive end organ damage. Although survival for children with SCD in the United States (US) is excellent the life expectancy for adults is approximately two decades shorter compared to those without SCD. Consequently, those with severe SCD, estimated to be about 20% of individuals with SCD in the US deserve to be offered treatment choices with the potential for cure. Two treatment options with curative intent are available. The first, hematopoietic cell transplantation is limited by donor avai

Sickle cell disease (SCD) is characterized by both acute and chronic complications. The clinical manifestation of these complications differs between genotypes. Given the large amount of research already published, this systematic review aims to offer a complete overview of types of sickle cell complications between adults in the most common genotypes Hemoglobin SS (HbSS) and Hemoglobin SC (HbSC), putting options for further research into perspective. An extensive literature search was performed to study all available evidence on these complications. This review was performed according to the

Prior surveys of chronic automated red blood cell exchange (RCE) for patients with sickle cell disease (SCD) have identified considerable procedural variability, especially with the use of isovolemic hemodilution red blood cell exchange (IHD-RCE). We conducted a survey of chronic RCE practices among American Society for Apheresis (ASFA) members to identify opportunities for practice harmonization and future studies. The ASFA SCD Research Subcommittee developed a 72-item survey of chronic RCE practices, with a focus on IHD-RCE. The survey was validated internally and distributed by email to all

BACKGROUND: Sickle cell disease (SCD) is a chronic and life-limiting hemoglobin and systemic vascular disease. While over 1000 people have undergone hematopoietic cell transplantation (HCT) over the last 40 years, long-term disease-specific and health-related quality of life data are lacking. The American Society of Hematology 2021 Guidelines for SCD emphasized the need for more detailed registry data to inform patients and providers with decision-making and practice recommendations.
PROCEDURES: In January 2021, the Sickle Cell Transplant Advocacy and Research Alliance (STAR) launched Project

Adult patients with sickle cell disease may present to the emergency department with a broad spectrum of acute clinical manifestations. These acute complications can be life-threatening and require a structured, time-sensitive emergency management approach. Although vaso-occlusive pain crises represent the most frequent cause of presentation, other severe complications-including cerebrovascular events, acute chest syndrome, priapism, and hepatobiliary disorders-may also require immediate medical evaluation. Prompt diagnostic assessment and initiation of appropriate therapy are essential to pre

BACKGROUND: Depression is a common mental health comorbidity among patients with sickle cell disease (SCD) and is associated with increased health care utilization, increased costs, and poorer quality of life. Although depression and antidepressant adherence have been shown to be associated with medication adherence in other chronic conditions, research examining these associations in SCD is lacking.
OBJECTIVE: To determine whether comorbid depression and antidepressant medication adherence are associated with adherence to SCD disease-modifying therapies (DMTs) among Texas Medicaid patients w

BACKGROUND: Sickle cell disease (SCD) is characterized by chronic inflammation, yet the inflammatory landscape defining the clinically defined steady state remains poorly characterized, particularly in African populations where the disease burden is highest. Understanding baseline cytokine and chemokine signatures during the steady state is important for distinguishing physiological disease equilibrium from transitions toward acute complications.
METHODS: We conducted a cross-sectional study of 129 children aged 2-17 years in Ghana, including 63 children with SCD in steady state and 66 health

Artificial intelligence (AI) augmentation of routine hematological tests offers a promising strategy to improve hereditary hemolytic anemia (HHA) carrier detection in premarital screening, especially in resource-limited settings. HHA in this review specifically encompasses β-thalassemia, α-thalassemia, sickle cell disease (including HbS and HbC variants), and other hemoglobinopathies with autosomal recessive inheritance patterns requiring carrier detection for prevention. These conditions share hemolytic phenotypes but differ in hematological signatures, necessitating separate subgroup analyse

Sickle cell disease (SCD) is an inherited hemoglobinopathy characterized by chronic hemolytic anemia, painful vaso-occlusive episodes, and end-organ damage. Population-level studies indicate individuals with SCD are at an increased risk for myeloid neoplasia (MN). While the absolute risk of MN remains low, fatal cases of myelodysplastic syndrome and acute myeloid leukemia suggest MN risk is enhanced in the setting of curative cellular therapies for SCD, including allogeneic hematopoietic cell transplantation and autologous gene therapy. Clonal hematopoiesis (CH), the expansion of a genetically

Pain is the most common complication experienced by individuals living with sickle cell disease (SCD). While episodes of acute pain, also known as vaso-occlusive crises, are the hallmark of the disease, chronic pain is prevalent in SCD and is associated with many comorbidities. We review current understanding of pain in SCD across the lifespan, integrating definitions, mechanistic categories, and management within biologic, psychosocial, and environmental contexts. Acute vaso-occlusive pain, which emerges as early as infancy, evolves from intermittent nociceptive events driven by multicellular

Sickle cell anemia (SCA) is associated with chronic anemia and cerebrovascular dysfunction, leading to progressive brain atrophy. Hydroxyurea (HU), a disease-modifying therapy, improves hematologic parameters and reduces stroke risk, but its long-term effects on brain development remain unclear. Even though clinician guidelines recommend initiation of HU in infancy, there is a persistent underuse of HU. We examined whether HU use initiated during childhood and long-term HU use can mitigate the changes in gray matter volume (GMV), white matter volume (WMV), total brain volume, and cortical thic

Sickle cell disease (SCD) is defined by important heterogeneity between patients, but this clinical presentation mutability is not well understood. Therefore, this immense phenotypic variability calls for research into risk factors associated with greater chance of developing stroke in a specific population at Amazon forest. This study sought to explore the relationship between genetic markers of inflammatory process and the clinical risk of stroke in people with sickle cell anemia. The authors presented 70 patients (87.5%) were classified in group 1, without a stroke clinic, and 10 patients (
