Hereditary spherocytosis (HS) is the most common hereditary hemolytic disease with defects in red blood cells (RBC) membrane proteins caused by mutations in membrane protein genes, like SPTB, SPTA1 and ANK1. Gilbert syndrome (GS) is a disease characterized by a mild deficiency of uridine diphosphate-glucuronosyltransferase 1A1 (UGT1A1) enzyme activity and unconjugated hyperbilirubinemia, largely caused by UGT1A1 mutations. The two inherited diseases HS and GS are rarely occurred in the same patient and are easy to be misdiagnosed, resulting in excessive diagnosis and treatment. Here, we report

Hereditary spherocytosis (HS) is a common variant of inherited hemolytic anemia, in which abnormalities of red blood cells (RBC) structural proteins lead to loss of erythrocyte membrane surface area, resulting in spherical, hyperdense, weakly deformable RBCs. It is characterized by the presence of osmotically fragile erythrocytes, known as spherocytes in peripheral blood smear (PBS). The mutations that cause HS occur in genes encoding for red blood cell membrane/cytoskeletal proteins, predominantly, ankyrin-1, α- and β-spectrins, band-3, and protein 4.2. This results in defective cell membrane

Defects in erythrocyte membrane proteins can cause the most common type of inherited hemolytic anemia, so called hereditary spherocytosis (HS). It is characterized by the appearance of spherocytes in peripheral blood, hemolytic anemia, splenomegaly, jaundice and gallstones. Due to difficulty of diagnosis solely based on aforementioned parameters, the addition of genetic testing seems to be effective and most acknowledged. Up to date, pathogenic variations in five genes encoding membrane proteins (ANK1, SPTA1, SPTB, SLC4A1, EPB42) are identified to cause HS. Here, we have studied the genetic sp

Hereditary spherocytosis (HS) is a common inherited hemolytic disease caused by mutations in genes encoding proteins crucial to the red blood cell (RBC) membrane, leading to a change in RBC shape from biconcave to spherical. There are five distinct types of hereditary spherocytosis, with types III and V being autosomal recessive and types I, II, and IV autosomal dominant. X-linked agammaglobulinemia (XLA) is a common inborn error of immunity that impairs B cell maturation and differentiation. We report a case of a two-year-old Saudi boy with persistent anemia, recurrent chest infections, and a

The clearance of senescent and altered red blood cells (RBCs) in the red pulp of the human spleen involves sequential processes of prefiltration, filtration, and postfiltration. While prior work has elucidated the mechanisms underlying the first two processes, biomechanical processes driving the postfiltration phagocytosis of RBCs retained at interendothelial slits (IES) are still poorly understood. We present here a unique computational model of macrophages to study the role of cell biomechanics in modulating the kinetics of phagocytosis of aged and diseased RBCs retained in the spleen. After

Hereditary spherocytosis (HS) is a genetic disorder characterized by the presence of spherocytes, which are abnormally shaped red blood cells, leading to hemolytic anemia. While HS is not uncommon in hematology, it can present significant diagnostic and therapeutic challenges in its late stages, particularly when complicated by severe cholestasis. We report a case of a 48-year-old male presenting with jaundice and abdominal pain, initially diagnosed with cholecystolithiasis and moderate splenomegaly. Subsequent investigations confirmed HS through clinical observation and eosin-5'-maleimide (EM

Stiff person syndrome (SPS) is a rare autoimmune neurological disorder characterized by muscle rigidity and episodic spasms that involve axial and limb musculature. It has important implications during anesthesia, as it leads to gamma-aminobutyric acid (GABA)-mediated inhibitory networks malfunction. This report describes the anesthetic management of a 56-year-old patient with SPS and hereditary spherocytosis undergoing emergent splenectomy due to splenic hematoma and hemoperitoneum after a fall. Total intravenous anesthesia (TIVA) was performed with the adjunctive administration of rocuronium

Hereditary spherocytosis (HS) is the most prevalent form of congenital hemolytic anemia, being caused by genetic mutations in genes encoding red blood cell cytoskeletal proteins. Mutations in the ANK1 and SPTB genes are the most common causes of HS.; however, pathogenicity analyses of these mutations remain limited. This study identified three novel heterozygous mutations in 3 HS patients: c.1994 C > A in ANK1, c.5692 C > T, and c.3823delG in SPTB by whole-exome sequencing (WES) and validated by Sanger sequencing. To investigate the functional consequences of these mutations, we studied their

Hereditary Spherocytosis (HS) is a type of hemolytic anemia characterized by a defect in the membrane of red blood cells, which causes them to assume a spherical shape, become osmotically fragile, and be prone to early hemolysis. While it is relatively well understood in the medical field, there is limited evidence regarding the dental management of patients with this condition. Therefore, this report aims to present a case involving the extraction of permanent teeth before splenectomy in a child with HS who had been hospitalized. Additionally, treatment guidelines for these patients are propo

BACKGROUND: Hereditary spherocytosis is a rare genetic disorder of the red blood cell membrane that is characterized by anemia, jaundice, and splenomegaly; however, in the absence of family history and with unusual clinical presentation, the diagnosis might not be made until later in life.
CASE PRESENTATION: Here, we present a challenging case of genetically proven hereditary spherocytosis that involves the SPTB gene in a 23-year-old female patient from Ethiopia who had repeated medical visits for episodic jaundice and hepatosplenomegaly, with unusual features of conjugated hyperbilirubinemia

Non-transfusional iron overload (IOL) in hereditary spherocytosis (HS) is poorly documented compared with other red blood cell disorders. We studied 13 HS adults with confirmed IOL to identify potential genetic factors. Using a next-generation sequencing panel of 46 genes related to HS, anaemia and iron metabolism, we found no association between IOL and the genes involved in HS nor the HFE:p.(Cys282Tyr) variant responsible for hereditary haemochromatosis. However, potential genetic factors contributing to IOL were identified in some patients, including variants in HJV (haemojuvelin), SLC40A1

BACKGROUND: Gallstones are among the most common complications of hereditary spherocytosis (HS). In previous treatments, gallbladder-preserving cholelithotomy (GPC) has remained a subject of significant debate due primarily to potential risks of stone recurrence. However, past studies have often overlooked the impact of specific disease conditions on GPC. In this study, we reviewed the clinical data of GPC in HS pediatric patients with concurrent gallstones over a period of seven years in a single center.
METHODS: From December 2016 to April 2024, 32 pediatric patients with HS who underwent s

Patients with hereditary spherocytosis (HS) often require red blood cell transfusions for the treatment of hemolytic anemia. Iron overload is a known complication of frequent transfusions. Deferasirox, an oral iron chelator, can cause transient elevations in serum aminotransferase levels. There have been a few cases demonstrating Deferasirox-associated liver injury in patients with sickle cell anemia and thalassemia. In this case report, we present a 13-year-old male with transfusion-dependent HS treated with Deferasirox who presented with jaundice and was found to have evidence of acute hepat

INTRODUCTION: Membranopathies encompass haemolytic disorders arising from genetic variants in erythrocyte membrane proteins, including hereditary spherocytosis and stomatocytosis. Congenital dyserythropoietic anaemia type II (CDA II) is associated with the SEC23B gene and can exhibit phenotypic similarities to membranopathies. Current treatment options for these conditions, apart from splenectomy, are primarily supportive. Mitapivat, a novel pyruvate kinase (PK) activator, has demonstrated efficacy in increasing haemoglobin levels and reducing haemolysis in patients with PK deficiency, thalass
