Moyamoya syndrome associated with hereditary spherocytosis: pathogenesis, management, and an illustrative case with rapid radiological progression.

BACKGROUND: The association between moyamoya syndrome (MMS) and hereditary spherocytosis (HS) is exceedingly rare, with only eight pediatric cases reported to date. This review explores the clinical intersection, pathophysiological theories, and management challenges of this dual pathology. CASE DESCRIPTION: We present a 13-year-old boy with known HS who presented with acute left limb weakness. Imaging confirmed MMS. Over a 3-year follow-up, serial magnetic resonance imaging (MRI) and digital subtraction angiography (DSA) demonstrated significant progression of cerebrovascular steno-occlusive

In-depth analysis of osmotic gradient ektacytometry parameters across different genotypes in hereditary spherocytosis.

Hereditary spherocytosis (HS) is a hereditary haemolytic anaemia, caused by pathogenic variants in genes encoding red blood cell membrane proteins. Osmotic gradient ektacytometry evaluates red cell deformability and hydration and is increasingly used in the diagnosis of HS. We retrospectively evaluated laboratory data from 233 HS patients focusing on osmotic gradient ektacytometry parameters, including two novel parameters, Omin-width and Omax-width. We found that the maximum elongation index (EImax), representing the maximum deformability, was decreased in SPTB relative to SLC4A1 and SPTA1 (0

De novo mutations in ANK1 and SPTB cause hereditary spherocytosis: three case reports and literature review.

Hereditary spherocytosis (HS) is characterized primarily by jaundice, anemia, splenomegaly (enlarged spleen), increased numbers of spherocytes in peripheral blood, and elevated erythrocyte osmotic fragility. However, these manifestations are nonspecific, resulting in misdiagnosis or underdiagnosis. Here, we report three Chinese patients with a family history of HS. Case 1 was an infant who presented with jaundice and anemia during the neonatal period, without splenomegaly; case 2 was a woman with hepatitis C and mild anemia; and case 3 was a child with no obvious clinical symptoms. Genetic tes

Abnormal Liver Function with Low Glycated Hemoglobin: a Case of Hereditary Spherocytosis Concealed by Liver Disease Symptoms.

BACKGROUND: Hereditary spherocytosis (HS), a familial hemolytic disorder caused by red blood cell membrane defects, leads to increased red blood cell destruction. It manifests as jaundice, hemolytic anemia, and splenomegaly. HS patients, due to chronic hemolysis, are prone to developing bile duct or gallbladder stones, and some primarily present with 'cholelithiasis' as their main clinical manifestation. METHODS: We reported a case of a patient who was admitted for biliary colic due to choledocholithiasis and was ultimately diagnosed with HS. We conducted a retrospective analysis of this hosp

In silico analysis of the invasion mechanics and invasiveness of the Plasmodium falciparum merozoite.

Although there has been considerable progress in understanding the factors that determine the invasiveness of Plasmodium falciparum merozoites, the collective role of the biophysical characteristics of erythrocyte deformability in the invasion process is poorly understood. Cell shape, cytoplasmic viscosity, and membrane stability are the main determinants of erythrocyte deformability, but it remains unknown how these properties affect the merozoite invasiveness. This study aimed to investigate computationally (i) the role of erythrocyte morphology and merozoite-induced erythrocyte membrane dam

On-Chip Evaluation of Red Blood Cell Deformability Through Transit Velocity Index in Hematological Diseases.

This study introduces a novel method for evaluating red blood cell (RBC) deformability using a microfluidic platform equipped with constriction channels that mimic capillary vessels. The normalized transit velocity of RBCs at 50% compression ratio (V^ε = 0.5) is proposed as a quantitative index of RBC deformability. Experimental validation is conducted in two key phases. First, to validate the proposed index, a controlled reduction in RBC deformability is induced using varying concentrations of diamide. V^ε = 0.5 progressively decrease as the diamide concentration increased, confirming that th

From Splenectomy to Partial Splenic Embolization, Which is Better for Hereditary Spherocytosis?

Hereditary Spherocytosis (HS), with characteristic red blood cells (RBCs), is a group of genetic hemolytic disorders that occur in an autosomal-dominant manner. It is clinically characterized by anemia, splenomegaly, jaundice, and mutations in key genes. Splenectomy is the standard treatment for HS. Alternatively, partial splenic embolization (PSE) balances efficacy and safety, making it a promising solution. To determine how to choose surgery and ensure correct management, we reviewed the indications, techniques, efficacies, complications, and progress of alternative solutions to reach a comp

Evaluation of the Eosin-5'-Maleimide Flow Cytometry Test for the Diagnosis of Hereditary Spherocytosis.

INTRODUCTION: Hereditary spherocytosis (HS) is a congenital hemolytic anemia caused by erythrocyte membrane protein defects. While the eosin-5'-maleimide (EMA) binding test by flow cytometry is the established first-line assay, this study evaluates its optimal cut-offs and performance within a large, real-world cohort. METHODS: We retrospectively analyzed 181 consecutive samples (2017-2024) with suspected HS. EMA binding was performed by flow cytometry using conventional and standardized protocols; the latter (implemented in 2023) utilized fixed erythrocyte counts and FL1 bead calibration. Ac

Case Report: Abnormally low hemoglobin A1c in a diabetic patient with SLC4A1 gene mutation.

BACKGROUND: Hemoglobin A1c (HbA1c) is a critical biomarker used for the diagnosis and management of diabetes. However, nonglycemic genetic variations may affect the accuracy of HbA1c measurements. METHODS: We presented a clinical evaluation of a type 2 diabetic patient with an SLC4A1 (solute carrier family 4 member 1) gene mutation, characterized by high blood glucose and low HbA1c, and estimated the carrier frequency of SLC4A1 variants in Chinese population. RESULTS: A 56-year-old patient with type 2 diabetes presented with a low HbA1c level, an elevated glycated albumin percentage (GA), no

Diagnostic performance of flow cytometric osmotic fragility testing in hereditary spherocytosis.

Hereditary spherocytosis (HS) is the most common congenital red blood cell membrane disorder, characterized by structural protein defects that lead to hemolytic anemia. Although several diagnostic tests exist, including osmotic fragility tests (OFTs), acidified glycerol lysis test (AGLT), and the EMA-binding test (EMA), each presents specific limitations regarding sensitivity, specificity, or technical requirements. Flow cytometric osmotic fragility testing (OFT-FCM) emerges as a promising complementary assay, offering a standardized workflow and rapid turnaround time. We conducted a retrospec

Comorbidity of Dengue and Hereditary Spherocytosis in an 18-Year-Old Patient: A Case Report.

Hereditary spherocytosis (HS) is one of the most common non-immune hemolytic anemias, and it's characterized by defects in erythrocyte membrane proteins that result in increased red blood cell fragility and hemolysis. Dengue is an acute viral infection frequently associated with hematologic alterations, including thrombocytopenia and plasma leakage. The coexistence of dengue infection and HS has rarely been reported in the literature. In this report, we describe the case of an 18-year-old male with a history of HS, diagnosed during childhood, who presented with a five-day history of fever and

A novel heterozygous mutation in ANK1 solves a mystery of a patient with hyperbilirubinemia and splenomegaly.

Patients presented with jaundice and splenomegaly are easily diagnosed as having liver diseases. Actually, some atypical cases belong to hemolytic diseases leading to secondary hemochromatosis, which is easily misdiagnosed or never diagnosed. Here, we describe an atypical case in a 27 years old man presenting with unconjugated hyperbilirubinemia and splenomegaly since early childhood, who was hospitalized to check for any possible causes related to liver diseases. However, the probable etiology related to hemolytic diseases and liver disease was ruled out by negative traditional tests, which l
