Background/Objectives: We wished to raise awareness of Hereditary Spherocytosis (HS) as a potential cause of early and significant hemolytic newborn jaundice. Methods: We utilized three recent cases from our experience to discuss hyperbilirubinemia etiologies to be considered when a baby has hemolytic hyperbilirubinemia, including HS, and presented a review of the literature about this disorder including presentation and evaluation in the neonate. Results: We found that ABO hemolytic disease of the newborn (HDN) is often considered as the etiology for presumed hemolytic hyperbilirubinemia even

Introduction: Hereditary spherocytosis (HS) is a congenital hemolytic anemia, often under-recognized in neonates. Co-inheritance with other genetic disorders like Gilbert syndrome (GS) and beta-thalassemia trait (BTT) can complicate the diagnosis. Case Report: We report a neonate presenting with significant unconjugated hyperbilirubinemia and anemia. Genetic testing revealed a triple diagnosis- HS due to a heterozygous deletion in the SPTB gene, BTT with a splice-site variant in the HBB gene, and heterozygosity for UGT1A1 promoter polymorphism associated with GS. The father, previously diagnos

Hemoglobin A1c (HbA1c) is widely utilized for monitoring glycemic control in diabetes mellitus. Growth hormone (GH) therapy necessitates regular glucose monitoring due to an increased risk of diabetes. However, hemolytic anemia may result in spuriously low HbA1c levels due to a shortened erythrocyte lifespan, potentially leading to misinterpretation of glycemic status. We report an 11-year-old girl with GH deficiency undergoing GH therapy who presented with an abnormally low HbA1c level of 3.9% despite normal glucose tolerance. Laboratory tests revealed mild anemia (hemoglobin: 11.8 g/dL), ele

BACKGROUND: Hereditary spherocytosis (HS) is a genetically inherited hemolytic anemia resulting from erythrocyte membrane defects, predominantly associated with genetic mutations in membrane protein genes such as ANK1 and SPTB. The disease exhibits considerable heterogeneity in both clinical manifestations and age of onset, presenting substantial diagnostic challenges for clinicians, particularly in pediatric cases.
CASE PRESENTATION: The patient was a 29-day-old boy who had experienced persistent anemia and a medical history of neonatal hyperbilirubinemia since birth. Upon admission, the inf

BACKGROUND: Severe neonatal unconjugated hyperbilirubinemia is associated with the risk of neurotoxicity and hence warrants prompt diagnostic and therapeutic interventions, including phototherapy and exchange transfusions. Although relatively rare, Hereditary spherocytosis (HS) is one of the common non-alloimmune hemolytic disorders associated with severe neonatal hyperbilirubinemia and hemolytic anemia during childhood. HS results from a mutation in genes encoding 1 or more proteins in the erythrocyte membrane, which leads to a loss of membrane structure and dysfunction of the cell. The clini

To screen hereditary spherocytosis (HS), we first introduced the novel flow cytometric osmotic fragility test (FC-OFT) based on the two-point kinetic assay principle (FC-OFTKinetic). With the introduction of FC systems with automatic tube loaders, we updated the FC-OFT protocol to follow the endpoint assay principle (FC-OFTEndpoint). This study aims to evaluate the updated FC-OFT protocol (FC-OFTEndpoint) and compare its assay performance with that of FC-OFTKinetic. We investigated factors influencing the FC-OFTEndpoint assay, optimized its protocol, and defined the cutoff index using 152 nega

BACKGROUND: Neurological compromise due to extramedullary hematopoiesis is a rare occurrence, typically associated with myeloproliferative disorders such as thalassemia or hereditary spherocytosis. Few cases have been reported in the setting of sickle cell anemia, and there is no established consensus on optimal treatment. Management strategies range from exchange transfusion to radiotherapy and surgical decompression. This case report presents a rare case of stenosis compression caused by extramedullary hematopoiesis in a patient with sickle cell anemia and highlights the therapeutic efficacy

Hereditary spherocytosis (HS) is a genetic hemolytic disorder primarily characterized by hemolytic anemia, jaundice, splenomegaly, and frequent complications, including cholelithiasis, accompanied by the presence of spherocytes in the peripheral blood. This disorder predominantly follows an autosomal dominant inheritance pattern; however, certain cases exhibit an autosomal recessive mode of inheritance. HS is the most prevalent disorder associated with defects in the red blood cell membrane. Primary myelofibrosis (PMF), a chronic myeloproliferative neoplasm (MPN) characterized by splenomegaly

INTRODUCTION: Numerous algorithms based on recent developments of additional parameters on last generation haematology analysers have opened new perspectives for hereditary spherocytosis (HS) screening. Although some studies have demonstrated significant age-related variation for reticulocyte-derived parameters, these algorithms did not consider patients' age. Moreover, the apparent lack of short-term stability of some parameters could represent an obstacle to their use if not performed as first-line testing. This study aimed to identify robust and discriminating parameters that could be used

OBJECTIVE: The flow cytometric osmotic fragility test (OFT) screens for hereditary spherocytosis (HS) and other red blood cell (RBC) membrane disorders. It has been suggested that the proportion of residual RBCs at the midpoint region of the time/forward scatter plot would be just as informative for HS diagnosis as that at the terminal region. We established reference intervals for both regions in healthy Korean adults using one of the most recently available flow cytometry systems.
METHODS: This cross-sectional study included 249 adults (129 men and 120 women) with normal complete blood cell

BACKGROUND: Anecdotal Literature regarding hereditary spherocytosis, a rare hemolytic anemia, points to an early cerebrovascular involvement that would imply early strict patients’ monitoring and management. However, this issue is under-investigated. METHODS: Clinical history regarding cerebrovascular events in the young ( < 55 years) was investigated in a referral Center for hereditary spherocytosis (190 patients; mean-age 23.5 ± 17.4 years; range 1–77; 93 females). By means of 3T-MR scanner, intracranial artery stenoses, aneurysms, brain infarctions, vascular-like white matter lesions and si

OBJECTIVE: Hereditary spherocytosis (HS) is an inherited disorder characterized by spherical erythrocytes and abnormalities of several erythrocyte membrane proteins with extreme genotypic and phenotypic heterogeneity. HS patients were clinically diagnosed by the presence of spherical erythrocytes on the peripheral blood smear, hemolytic anemia, jaundice, and splenomegaly, with or without cholelithiasis or gallstones. To date, mutations of five genes (ANK1, EPB42, SLC4A1, SPTA1, and SPTB) have been reported to be associated with different subtypes of HS. Germline mutations of the SPTB gene caus

OBJECTIVES: Pediatric gallstone disease is increasing in incidence, possibly due to rising rates of childhood obesity. There are concerns about delayed presentations and the need for surgery and endoscopic management. The aim of this project is to assess rates of cholecystectomy in pediatrics, distribution of pathology, impact of body mass index (BMI), and need for endoscopic retrograde cholangiopancreatography (ERCP).
METHODS: We performed a retrospective cohort study of pediatric patients undergoing cholecystectomy and compared baseline characteristics and outcomes between different patholo

INTRODUCTION: Gallstones are a significant complication in individuals with hereditary spherocytosis (HS) and related hemolytic disorders. Chronic hemolysis leads to elevated bilirubin levels, a precursor for pigment gallstone formation. Despite advancements in understanding HS, the mechanisms driving gallstone pathogenesis remain incompletely understood, particularly in the presence of genetic factors such as Gilbert syndrome, beta-thalassemia trait, and G6PD deficiency.
OBJECTIVE: This study aims to investigate gallstone patterns in individuals with HS and related hemolytic conditions, focu

Parvovirus B19 (PVB19) is a common virus that usually causes a mild and self-limiting illness. However, in patients with hereditary spherocytosis (HS), it can lead to severe complications such as aplastic crisis and, rarely, hemophagocytic lymphohistiocytosis (HLH). We report a six-year-old boy with HS who presented with persistent fever and severe anemia. Laboratory tests revealed a progressive pancytopenia, including a sudden drop in hemoglobin from 10 to 6.8 g/dL, hyperferritinemia (5,129 ng/mL), elevated soluble IL-2 receptor (1,179 U/mL), mild hypofibrinogenemia, and positivity for PVB19

Hereditary spherocytosis (HS) is an inherited disease characterized by the presence of spherical erythrocytes in the peripheral blood. Beta-spectrin (SPTB) encodes a cytoskeletal protein previously associated with spherocytosis. Mutations in SPTB are one of the most common causes of HS. We have reported here the case of a 2-year-old boy who presented with severe anemia, jaundice, and hyperbilirubinemia at Kunming Children's Hospital. His osmotic fragility tests (OFT) and eosin maleimide (EMA) tests were positive. Genetic molecular analysis was performed to identify the underlying cause of the
