Global population structure of Shiga toxin-producing Escherichia coli O103:H2 and the variation in their major virulence factor-encoding genetic elements.

Shiga toxin (Stx)-producing Escherichia coli (STEC) is a major cause of serious gastrointestinal illness, including diarrhoea, haemorrhagic colitis and life-threatening haemolytic-uraemic syndrome. Although O157:H7 STEC strains are the most prevalent, the incidence of STEC infections caused by several other serotypes has recently increased. O103:H2 STEC is one of these major non-O157 STEC strains, but systematic whole-genome sequence (WGS) analyses have not yet been conducted. To gain a global phylogenetic overview of O103:H2 STEC based on WGSs, we analysed 2,701 WGSs of O103:H2 strains, inclu

From dog bite to dialysis: complement-mediated haemolytic uraemic syndrome .

A previously healthy woman in her late 40s presented with rapidly progressive sepsis 4 days after a dog bite. She developed disseminated intravascular coagulation, pancytopenia and acute kidney injury, ultimately progressing to complete anuria and dialysis dependence. Blood cultures confirmed Capnocytophaga canimorsus bacteraemia. Despite an initial clinical improvement with antibiotics and supportive care, she developed sudden isolated haemolysis and persistent thrombocytopaenia. Laboratory and renal biopsy findings confirmed the presence of a thrombotic microangiopathy (TMA), and complement-

Pathogenic potential and phylogenomic analysis of stx2-carrying O26:H11 Shiga toxin-producing Escherichia coli isolated from dairy products in France (2014-2024).

Shiga toxin-producing Escherichia coli (STEC) are major foodborne pathogens causing sporadic enteric disease and paediatric haemolytic uraemic syndrome (HUS) cases globally. In France, STEC O26:H11 strains carrying the stx2 gene have become the leading cause of paediatric HUS over the last decade. Concurrently, data from the French National Reference Laboratory show an increased proportion of stx2-carrying O26:H11 strains isolated from food, including raw milk products (RMPs). The consumption of RMP contaminated by this specific genotype has been linked to three out of four nationwide outbreak

Clinical and cost-effectiveness of eculizumab withdrawal in atypical haemolytic uraemic syndrome: the SETS aHUS multi-centre, open-label, prospective and single-arm study.

BACKGROUND: Atypical haemolytic uraemic syndrome is a rare disease (incidence: 0.4 cases per million per year) which, without treatment, is associated with high morbidity and mortality. Eculizumab, a monoclonal complement inhibitor, is an effective treatment but the optimal way to use this high-cost medication (£360,000 per year for an adult) has not been established. OBJECTIVE: Establish the safety of eculizumab withdrawal and the effectiveness of a monitoring protocol to detect disease relapse and reintroduction of treatment if relapse occurs. SETTING: Fifteen hospitals in the United Kingd

Prevalence of selected Shiga toxin-producing Escherichia coli vaccine antigen genes among two geographically distinct ruminant populations.

Introduction. Shiga toxin-producing Escherichia coli (STEC) are a group of bacteria that cause severe bloody diarrhoea and haemolytic uraemic syndrome and are associated with neurological complications and potentially life-threatening infections. Children under five and elderly individuals are particularly vulnerable to STEC infections. The majority of STEC infections have been traced back directly or indirectly to ruminants, and there is an unmet need for a vaccine to protect these vulnerable cohorts.Gap Statement. We previously identified a series of STEC proteins that are involved in attach

Erythrodermic psoriasis complicated by immune complex-mediated crescentic glomerulonephritis and atypical hemolytic uremic syndrome: a case report.

BACKGROUND: Erythrodermic psoriasis (EP) is a severe, systemic inflammatory dermatosis. While kidney involvement is rare, the massive cytokine release associated with EP can induce systemic complications. We report a rare case of EP complicated by the simultaneous onset of immune complex-mediated crescentic glomerulonephritis (GN) and atypical haemolytic uraemic syndrome (aHUS), highlighting a severe form of complement-mediated renal injury in the context of dermatological inflammation. CASE PRESENTATION: A 52-year-old male presented with a three-month history of diffuse erythrodermic psoriasi

Cocaine-induced thrombotic microangiopathy mimicking TTP in a young patient.

A young woman presented with acute right flank pain, fever and thrombocytopenia. Initial evaluation suggested pyelonephritis, supported by flank tenderness, elevated C-reactive protein, and Eschericia coli in urine culture. Broad-spectrum antibiotics were initiated. Over the following 2 days, she developed progressive anaemia and severe thrombocytopenia with biochemical evidence of haemolysis. Differential diagnoses included infection-related haemolysis, atypical haemolytic uraemic syndrome, thrombotic thrombocytopenic purpura (TTP) and drug-induced thrombotic microangiopathy (TMA). Haemolytic Uraemic Syndrome Triggered by Non-Shiga Toxin-Producing Enteropathogenic Escherichia coli in a Child: Difficulties in Diagnosis and Treatment.

Haemolytic uraemic syndrome (HUS) is an important cause of acute kidney injury (AKI) in children, most commonly triggered by Shiga toxin-producing Escherichia coli (STEC). In contrast, HUS associated with non-Shiga toxin-producing enteropathogenic E. coli (EPEC) is rare, particularly when accompanied by neurological involvement. Optimal management in such cases remains uncertain, especially concerning the role of complement inhibition. A previously healthy three-year-old boy presented with bloody diarrhoea, abdominal pain, and fever. He developed biochemical features of HUS and neurological ma

Practical approach to pregnancy-associated thrombotic microangiopathies: navigating diagnostic challenges in resource-limited settings.

Pregnancy can trigger thrombotic microangiopathy (TMA), a condition associated with high morbidity and mortality if treatment is delayed. Timely diagnosis remains challenging, particularly in resource-limited settings due to limited access to specialised assays. We present three cases of maternal TMA admitted to our tertiary care unit located in a developing country. One patient was a known case of hereditary thrombocytopenic purpura. Two other cases presented with TMA post-caesarean section complicated by sepsis and postpartum haemorrhage, respectively, and were diagnosed as pregnancy-associa

Properdin deficiency or anti-properdin treatment ameliorates disease in the C3 gain-of-function mouse model of atypical haemolytic uraemic syndrome.

INTRODUCTION: We have previously shown that the C3D1115N mouse, engineered around a single point mutation in C3 associated with atypical haemolytic uraemic syndrome (aHUS) in man, fully recapitulates the clinical disease. In this study, we investigated the role of properdin in aHUS. METHODS: C3D1115N mice were crossed onto properdin-deficient mice or C3D1115N mice were treated with anti-properdin monoclonal antibody therapy and survival tracked. Kidney function, serum biomarkers and kidney pathology was carefully assessed at set end points. C3 and Fibrin deposition was assessed using immuno-f

Dialysis Toolkit for Severe Thrombocytopenia in Patients with Acute Kidney Injury.

Acute kidney injury (AKI) in the setting of severe thrombocytopenia represents a formidable challenge in nephrology, where both the underlying disease and therapeutic interventions carry a high risk of bleeding. Conditions such as lupus nephritis, thrombotic thrombocytopaenic purpura (TTP), haemolytic uraemic syndrome (HUS), haematological malignancies, chemotherapy-induced marrow suppression, and severe viral infections including dengue or sepsis-related disseminated intravascular coagulation often present with simultaneous kidney dysfunction and profound thrombocytopenia. 50 years of estimating exposures for different jobs: the past, present and future of developing and using job exposure matrices (JEM).

INTRODUCTION: During treatment for malignant lymphoma, cytopenia can develop for several reasons. This can range from mild cytopenias leading to infection and bleeding to full-blown drug-induced aplastic anaemia. While aplastic anaemia affects individuals of all genders and ages, here, we describe aplastic anaemia after chemotherapy exposure to bendamustine in a 65-year-old female with non-Hodgkin's lymphoma. CASE DESCRIPTION: A 65-year-old woman with recurrent indolent marginal zone lymphoma and post-chemotherapy with bendamustine and rituximab, presented with a neutropenic fever and was adm

Risk ranking and antimicrobial resistance of Shiga toxin-producing Escherichia coli of bovine origin.

Shiga toxin-producing Escherichia coli (STEC) are zoonotic pathogens commonly carried by cattle and responsible for severe human diseases, as bloody diarrhoea (BD) and haemolytic uraemic syndrome (HUS). In an Italian slaughterhouse, 150 dairy cows were tested by swabbing the hides after stunning and the carcasses before refrigeration. STEC prevalence was 6.7% on hides and 4.0% on carcasses. No hide-to carcass contamination in the same animal was observed. Twenty isolates belonging to the serotypes O26:H11, O55:H9, O55:H12, O109:H10, O116:H28, O127:H12, O150:H2 (hides only), O157:H7 and O177:25

Sustained Terminal Complement Inhibition on the Endothelium with Ravulizumab in Patients with Atypical Haemolytic Uraemic Syndrome.

INTRODUCTION: Atypical haemolytic uraemic syndrome (aHUS) is a rare and severe form of thrombotic microangiopathy caused by dysregulation of the alternative complement pathway, leading to sustained complement activation at the endothelial level and microvascular thrombosis, predominantly affecting the kidney. Anti-C5 therapy with eculizumab has markedly improved patient outcomes, with ex vivo studies showing suppression of endothelial C5b-9 deposition by sera from treated patients, in contrast to the marked C5b-9 formation induced by acute-phase sera. Neutrophil extracellular traps and acute kidney injury: a narrative review.

Neutrophil extracellular traps (NETs) are complex, network-like structures comprising chromatin filaments bound to various antimicrobial proteins. While they play a role in host defence under physiological conditions, there is evidence to suggest that excessive production or impaired clearance of NETs may contribute to the pathological progression of acute kidney injury (AKI). This review analyses the heterogeneous roles of NETs composed of various components, such as histones, DNA and associated enzymes, in mediating renal tissue damage. It also explores their involvement in ischaemia-reperfu

Beyond complement: comprehensive assessment of factor XIII plasma dynamics and genetics in atypical hemolytic uremic syndrome.

BACKGROUND: Atypical hemolytic uremic syndrome (aHUS) is a distinct form of thrombotic microangiopathy characterized by formation of fibrin-rich thrombi. Factor XIII (FXIII), a key fibrin-stabilizing enzyme, may therefore represent an unexplored contributor to the pathophysiology of aHUS, and a comprehensive evaluation of its plasma dynamics and genetic background is lacking. OBJECTIVES: This study aimed to evaluate plasma FXIII levels and F13A1/F13B genetic variants in patients with aHUS. METHODS: The cohort comprised 102 patients with aHUS and 73 healthy controls. FXIII activity was measur

Complement diagnostics and therapeutics for the pediatric population: early successes and opportunities for further advancement.

Complement-mediated diseases are individually rare, often rapidly progressive, and historically associated with high rates of permanent organ injury or death but are now among the most therapeutically actionable conditions in pediatric nephrology, hematology, and rheumatology. As demonstrated by thrombotic microangiopathy, and hemolytic uremic syndrome in particular, diagnostic delay can contribute directly to irreversible kidney injury. Complement inhibition has substantially improved outcomes in these conditions that previously carried a substantial risk of end-stage kidney disease. This rev
