A Less Common but Lethal Encounter: Mixed Autoimmune Hemolytic Anemia Unmasking Angioimmunoblastic T-cell Lymphoma.

Autoimmune hemolytic anemia (AIHA) is a spectrum of acquired hemolytic disorders caused by autoantibodies targeting red blood cells, leading to their destruction and anemia. Mixed AIHA, characterized by both warm and cold autoantibodies, is a rare and complex condition, often presenting diagnostic challenges. Angioimmunoblastic T-cell lymphoma (AITL), a subtype of peripheral T-cell lymphoma, is also uncommon and can be associated with autoimmune cytopenias. We present a case of a 48-year-old male patient diagnosed with mixed AIHA and AITL after presenting with generalized weakness, jaundice, a

The toll of exclusion from work or preschool due to toxin-producing Escherichia coli positivity in Dublin and South East Ireland, 2022/2023.

BACKGROUND: Ireland has one of the highest rates of Shiga toxin-producing Escherichia coli (STEC) infections in Europe. As part of the public health response, those from high-risk groups must be excluded from work or pre-school until microbiological clearance is achieved. METHODS: This study analysed STEC cases from Dublin and the South-East public health region in Ireland in 2022 and 2023, focusing on the duration of exclusion of patients in high-risk groups. RESULTS: There were 383 STEC cases notified during the study period. Ninety-seven (25.3%) were excluded from work or pre-school, 76 h

C3d-targeted complement inhibitors to correct complement dysregulation in aHUS patients.

Atypical hemolytic uremic syndrome (aHUS) is a rare and severe thrombotic microangiopathy caused by genetic or acquired abnormalities leading to activation of the complement alternative pathway on cell surfaces. This process leads to endothelial dysfunction and microvascular thrombosis. The introduction of anti-C5 antibodies has dramatically improved aHUS prognosis; however, these treatments require regular intravenous infusions and block systemic complement activity, exposing the patient to risk of infections. Recently complement inhibitors have been developed to selectively bind injury-assoc

The epidemiology of haemolytic uraemic syndrome in England, 2009-2023: An analysis of national surveillance data.

OBJECTIVES: Haemolytic uraemic syndrome (HUS) is a serious multi-system disorder, predominantly caused by Shiga toxin-producing Escherichia coli (STEC). We aimed to describe the epidemiology of HUS in England, including characteristics associated with HUS among STEC cases. METHODS: Enhanced surveillance questionnaire (ESQ) and microbiological data were extracted for HUS and laboratory-confirmed STEC cases reported through national surveillance between 2009-2023. HUS incidence rates were computed. Trends in demographic, clinical, microbiological and exposure characteristics were described for

Gene-Environment Interaction: Lessons From Complement-Mediated Kidney Disease.

Atypical hemolytic uremic syndrome (aHUS) or complement-mediated thrombotic microangiopathy (CM-TMA) and C3 glomerulopathy are two prototypical diseases of complement dysregulation occurring due to genetic variants in complement proteins or acquired factors such as autoantibodies. Despite the presence of an underlying genetic etiology, an environmental trigger is often necessary to manifest disease, a phenomenon known as incomplete penetrance. These triggers could include infections, pregnancy, medication, cancers, or ischemia-reperfusion injury and antibody-mediated rejection in the setting o

Paroxysmal nocturnal hemoglobinuria masquerading as hemolytic uremic syndrome: a Case Report.

Paroxysmal nocturnal hemoglobinuria (PNH) is a rare acquired disorder of intravascular hemolysis caused by a somatic mutation in the gene responsible for glycosylphosphatidylinositol (GPI)-anchored complement regulatory proteins. This mutation leads to the production of abnormal blood cell clones lacking CD55 and CD59. PNH can result in renal damage. The challenge of early identification and diagnosis leads to misdiagnosis as other intravascular hemolytic conditions. This paper presents a case that began with fever, diarrhea, and acute renal failure, initially misdiagnosed as atypical hemolyti

Shiga toxin-producing Escherichia coli O26:H11 associated with a cluster of haemolytic uraemic syndrome cases following the mid-July 2021 floods in Belgium.

BACKGROUND: Flood-related waterborne diseases are an acute consequence of natural disasters, primarily due to contamination of drinking water supplies. With climate change driving an increase in extreme weather events, such as floods, the risk of waterborne disease outbreak is expected to rise. This report summarizes an outbreak investigation of Shiga toxin-producing Escherichia coli (STEC) O26:H11 in the Belgian-Luxembourg following the mid-July 2021 floods. METHODS: In summer 2021, STEC O26 isolates referred to the Belgian National Reference Centre (NRC) were first characterized with IS621-

IgG-Fc Glycosylation and a Novel Flowcytometric Assay to Predict Hemolytic Disease of the Fetus and Newborn.

INTRODUCTION: Timely and accurate referral of pregnancies with red blood cell alloantibodies is essential to prevent perinatal death or long-term adverse consequences in hemolytic disease of the fetus and newborn (HDFN). Classically, antibody titers are used to select high-risk pregnancies. Research suggested that also IgG-Fc fucosylation could be an important pathological feature. We evaluated the diagnostic potential of IgG-Fc fucosylation in identifying high-risk pregnancies, and we report on a flowcytometric assay that integrates antibody quantity, antibody subclass and fetal FcγRIIIa affi

Management of Atypical Haemolytic Uraemic Syndrome With Triggers: Diagnostic and Treatment Algorithms From an Asia-Pacific Perspective.

Complement-amplifying events/conditions associated with thrombotic microangiopathy (TMA) include pregnancy/postpartum period, severe hypertension, autoimmune diseases, drug exposures, infections and organ transplantation. Some of these 'triggers' may exist comorbidly with atypical haemolytic uraemic syndrome (aHUS; a complement-mediated form of TMA), unmask previously undiagnosed aHUS, or occur secondary to aHUS, thus creating a considerable diagnostic challenge. A major goal in patients presenting with TMA is to differentiate complement-mediated aHUS from other causes of TMA such that appropr

A novel enrichment-free, low-volume filtration and rapid lysis (ELR) method in combination with real-time PCR for detection of Shiga toxin-producing Escherichia coli (STEC) in water.

Consequences of Shiga toxin-producing Escherichia coli (STEC) infection can range in severity from asymptomatic infection to haemolytic uraemic syndrome, renal failure and death. Groundwater-derived drinking water is an important route for STEC transmission. Detection of STEC in water is crucial for timely response and public health interventions; however, currently used culture-based methods are time-consuming and laborious. Therefore, there is a need for rapid methods that maintain high sensitivity and specificity [1]. We describe a novel, sensitive, enrichment-free water filtration method u

Assessing Alloimmunization in Sickle Cell Disease Patients: Insights From a Tertiary Care Center Study on Multiple Blood Transfusion Recipients in Central India.

INTRODUCTION: Alloimmunization is defined as a delayed transfusion reaction, which occurs days to months after transfusion, in which a patient may produce antibodies against a red cell antigen that the patient lacks. This alloimmunization process is influenced by genetic as well as acquired patient-related factors. This study aims to determine the pattern and frequency of allo-antibodies in patients with sickle cell disease (SCD) who have undergone multiple transfusions. MATERIAL AND METHODS: We carried out a prospective observational study at a tertiary care hospital in central India. Our fo

A Tale of Immune Fragility: Severe Mycoplasma pneumoniae Infection With Mixed Autoimmune Hemolytic Anemia in Selective IgM Deficiency Disorder.

Mycoplasma pneumoniae is widely recognized as a leading cause of community-acquired atypical pneumonia and can trigger a wide spectrum of extra-pulmonary manifestations. This report discusses a rare clinical presentation of a young female patient who was admitted with an acute-onset, severe febrile illness. Her initial symptoms included a persistent cough with minimal purulent expectoration and mild difficulty in breathing, which rapidly progressed to severe dyspnea at rest. Her condition deteriorated swiftly, resulting in acute hypoxemic respiratory failure that required emergent endotracheal

Iptacopan/LNP023 and rituximab as rescue therapy in a patient with systemic lupus erythematosus-associated atypical haemolytic uraemic syndrome.

Systemic lupus erythematosus (SLE) is rarely associated with atypical haemolytic uraemic syndrome, leading to a poor prognosis with multi-system involvement. The pathogenesis seems to be driven by hyperactivation of alternate pathways of complement cascade and patients should be treated with prompt administration of complement inhibitors and immunosuppressive therapy. Iptacopan/LNP023, a novel, orally administered C3 convertase inhibitor that has shown efficacy in C3 glomerulopathy and paroxysmal nocturnal haemoglobinuria, is going to be tested in patients with atypical haemolytic syndrome. Relevance of the interactions between the complement and contact coagulation systems on renal pathology.

The complement system is a network of soluble and cell surface proteins primarily involved in innate immune responses. Complement signaling is essential for pathogen defence and homeostasis, but an activation-regulation imbalance can lead to tissue damage. This phenomenon has been implicated in kidney diseases such as atypical Haemolytic Uraemic Syndrome (aHUS), frequently associated with dysfunction of the complement regulator Factor H (FH). Physiologically, complement interacts with the coagulation, fibrinolysis, renin-angiotensin and kallikrein-kinin systems (KKS). The KKS is a proinflammat

[Exploring critical thinking in the management of diagnosis and treatment of fulminant pregnancy-associated atypical haemolytic uraemic syndrome].

Critical care emphasizes critical thinking, focuses on the triggers that lead to disease progression, and attaches great importance to early diagnosis of diseases and assessment of the compensatory capacity of vital organs. Pregnancy-associated atypical hemolytic uremic syndrome (P-aHUS) is relatively rare in the intensive care unit (ICU). Most cases occur within 10 weeks after delivery. Severe cases can be life-threatening. It characterized by microangiopathic hemolytic anemia, decreased platelet count (PLT), and acute kidney injury (AKI). Early clinical diagnosis is difficult due to its simi

Rotational Thromboelastometry (ROTEM)-Assisted Anaesthetic Management of a Parturient With Paroxysmal Nocturnal Haemoglobinuria and Aplastic Anaemia for Caesarean Section: A Case Report.

Paroxysmal nocturnal haemoglobinuria (PNH) is a rare, acquired, clonal haematopoietic disorder. It is life-threatening and is characterized by intravascular haemolytic anaemia, large vessel thrombosis and bone marrow failure. Aplastic anaemia (AA) and PNH are intimately linked; however, concurrent treatment of PNH and AA is uncommon. Pregnancies in patients with PNH are rare and associated with increased maternal and fetal morbidity and mortality. Nevertheless, with the advent of eculizumab, a monoclonal antibody used to treat PNH, pregnancy outcomes have become more favourable. The obstetric
