This case report describes a previously healthy 43-year-old woman who developed haemolytic uraemic syndrome (HUS) following Escherichia coli sepsis. She presented with vomiting, diarrhoea, fever, right upper quadrant pain, and altered mental status after returning from travel to Bali and Australia. Initial investigations revealed stage 3 acute kidney injury, deranged liver function, anaemia, thrombocytopenia, and raised inflammatory markers. Blood and urine cultures confirmed E. coli, and she was treated with intravenous antibiotics and supportive care. A peripheral blood film showed features

Thrombotic thrombocytopenic purpura (TTP) is a rare but potentially life-threatening complication associated with systemic lupus erythematosus (SLE). This condition is characterized by microangiopathic hemolytic anemia and thrombocytopenia, which can result from either an acquired deficiency of the ADAM metallopeptidase with thrombospondin type 1 motif 13 (ADAMTS13) or through complement-mediated mechanisms. We present the case of a 55-year-old female with a history of Sjögren's disease and a recent diagnosis of SLE. She exhibited severe anemia, thrombocytopenia, and symptoms suggestive of imm

In England, Shiga toxin-producing Escherichia coli (STEC) serogroup O26 has recently emerged as a public health concern, despite fewer than half of diagnostic laboratories in England having the capability to detect non-O157 STEC. STEC O26 cases frequently report exposure to farms or nurseries. We describe the epidemiology of STEC O26 and examine evidence for a relationship between O26 and exposure to these settings. We analysed national surveillance data describing laboratory-confirmed STEC cases and public health incidents over the past 10 years to explore the incidence, clinical outcomes, an

Shiga-toxin producing Escherichia coli (STEC) infections can cause severe gastrointestinal bleeding and haemolytic uraemic syndrome (HUS). This report highlights diagnostic and management challenges in a rare case of prolonged per rectal (PR) bleeding caused by STEC. A woman in her 40s presented with recurrent PR bleeding and haemodynamic instability. Extensive investigations including upper and lower endoscopy, capsule endoscopy, nuclear scintigraphy and CT angiography failed to identify a definitive bleeding source. Scintigraphy suggested active bleeding near the caecum, and CT angiogram rev

There are at least six well-established pathotypes of Escherichia coli (E. coli), including extraintestinal E. coli (ExPEC), enteroinvasive E. coli (EIEC), enteropathogenic E. coli (EPEC), enteroaggregative E. coli (EAEC), enterotoxigenic E. coli (ETEC), and Shiga toxin-producing E. coli (STEC). Here, we reviewed the role hybrid STEC pathotypes have played in causing outbreaks of gastrointestinal infectious disease (GIID) from the epidemics of dysentery at the end of the 19th century, to recent foodborne outbreaks and outbreaks of haemolytic uraemic syndrome (HUS). We assessed genomic and epid

BACKGROUND: Atypical haemolytic uraemic syndrome (aHUS) leads to acute kidney injury, necessitating dialysis in about half of patients. A certain proportion of patients treated with C5 inhibitors discontinue dialysis; however, little is known about the patient characteristics and clinical courses relating to discontinuation.
METHODS: We compared the characteristics and clinical courses of patients with aHUS on dialysis at the initiation of eculizumab during post-marketing surveillance in Japan, stratified by those who did (Group A) and did not (Group B) discontinue dialysis within 26 weeks of

Implementation of molecular detection methodology of Shiga toxin-producing Escherichia coli (STEC) in Danish patients began in 1997. Since then, changes in molecular detection methods and diagnostic criteria have led to the present situation, in which almost all diarrhoeal stool specimens are examined for STEC. Whole genome sequencing (WGS) of STEC isolates referred to the national reference laboratory has increased the detailed characterisation, and revealed a large spectrum, of STEC types, including cross-over pathotypes typically associated with extraintestinal disease or traveller's diarrh

INTRODUCTION: Acquired thrombotic thrombocytopenic purpura (aTTP) is a severe microangiopathic hemolytic anemia (MAHA) characterized by profound ADAMTS13 deficiency. MAHA can also manifest as a paraneoplastic syndrome, particularly in advanced malignancies, with cancer-associated MAHA (CA-MAHA) linked to signet ring cell carcinoma being exceptionally rare.
CASE DESCRIPTION: We present the case of a 42-year-old Arab male who initially presented with classic features of aTTP, including severe MAHA, profound thrombocytopenia, and renal and neurological dysfunction, confirmed by an ADAMTS13 activ

Lupus nephritis (LN) is a severe manifestation of systemic lupus erythematosus with approximately 50% of patients having renal involvement. Patients who fail to respond to appropriate induction immunosuppression are considered to have refractory disease. Refractory LN is associated with increased risk of progression to end-stage chronic kidney disease and mortality. Despite the development of modern therapeutics, refractory LN remains challenging in clinical practice and continues to be associated with treatment failure. Complement has a well-described role in the pathogenesis of LN; however,

We managed a complex outbreak of Shiga toxin-producing Escherichia coli (STEC) and stx-negative E. coli O157 (non-STEC) cases in four nurseries in two neighbouring towns in south-east Scotland, with epidemiological links through shared management, staff or other visits. There were 57 confirmed cases between July and October 2022. Nurseries voluntarily closed to support outbreak control. Subsequent whole genome sequencing identified two separate, unlinked outbreaks of stx2a-positive E. coli O157:H7 (19 confirmed cases in Nursery 1) and stx-negative E. coli O157:H39 (17 confirmed cases in Nurser

BACKGROUNDShiga toxin-producing Escherichia coli (STEC) can cause illnesses ranging from self-limiting diarrhoea to severe manifestations such as haemolytic-uraemic syndrome (HUS). In 2023, an increase in notified STEC cases was observed in the German federal state of Lower Saxony and nationwide.AIMWe aimed to investigate possible reasons for the observed increase.METHODSWe analysed data on notified STEC cases at federal and state level. All available STEC isolates from Lower Saxony from 2023 were whole genome sequenced. We sent a survey on detection and identification methods to 25 clinical m

Shiga toxin-producing Escherichia coli (STEC) O157:H7 is a globally dispersed zoonotic pathogen capable of causing severe disease outcomes, including bloody diarrhoea and haemolytic uraemic syndrome. While variations in Shiga toxin subtype are well-recognized drivers of disease severity, many unexplained differences remain among strains carrying the same toxin profile. We applied explainable machine learning (ML) approaches - Random Forest and Extreme Gradient Boosting - to whole-genome sequencing data from 1,030 STEC O157:H7 isolates to predict patient clinical outcomes, using data collected o

Hypertriglyceridaemia is a well-documented cause of pancreatitis. Although rare, pancreatitis can result in life-threatening complications such as atypical haemolytic uremic syndrome (aHUS) and secondary haemophagocytic lymphohistiocytosis (sHLH). Our case presents a female in her late 30s with right upper abdominal pain, elevated triglycerides and lipase levels in the background of an undiagnosed genetic hypertriglyceridaemia (HTG). These findings are consistent with the diagnosis of HTG pancreatitis. Her condition deteriorated a week following admission with the development of rhabdomyolysis

Background Pure red cell aplasia (PRCA) is an uncommon disorder characterized by severe normocytic normochromic anemia, reticulocytopenia, and an absence of erythroblasts from otherwise normal bone marrow. PRCA can present as either a congenital disorder or an acquired syndrome. Methodology A total of six cases of PRCA diagnosed in a tertiary care hospital from January 2016 to December 2019 were retrospectively studied. The diagnosis of PRCA was made based on bone marrow examinations showing erythroblastopenia with essentially normal myelopoiesis and megakaryopoiesis. Results Of the six patien

BACKGROUND: Atypical Haemolytic Uraemic Syndrome is a rare disease, associated with high morbidity and mortality. Eculizumab, a monoclonal complement inhibitor, is an effective treatment but the optimal way to use this high-cost medication has not been determined. The SETS aHUS trial aimed to establish the safety of eculizumab withdrawal and the effectiveness of a monitoring protocol to detect disease relapse and reintroduction of treatment if relapse occurs.
METHODS: The SETS aHUS multicentre, open label, prospective, single-arm trial enrolled participants from 15 UK hospitals. Patients over

BACKGROUND: There is no consensus on the classification of lupus arthritis (LA). In this study, we aimed to investigate patients with LA who were clinically and serologically very similar to those patients with rheumatoid arthritis (RA).
METHODS: The electronic medical files of systemic lupus erythematosus (SLE) patients from a single tertiary rheumatology department between 2017 and 2022 were reviewed. The inclusion criteria were being age ≥ 18 years, having nonerosive peripheral arthritis lasting longer than six months, and having dual seropositive (rheumatoid factor (RF) and anti-citrullin

Contrast medium is frequently associated with allergic reactions and kidney dysfunction. However, contrast media can induce hemolytic anemia with a broad spectrum of hemolytic manifestations. We report a 38-year-old patient with very severe immune hemolytic anemia after the application of iomeprol due to a CT of the thorax/abdomen. In this case report, we illustrate the diagnostics and treatment of life-threatening hemolytic anemia induced by a contrast medium that was successfully treated with eculizumab.

Autoimmune liver diseases (AiLDs), including autoimmune hepatitis (AIH), primary biliary cholangitis (PBC), and primary sclerosing cholangitis (PSC), are immune-mediated conditions associated with significant hepatic and systemic manifestations. Among these, cytopenias-defined as reductions in blood cell counts affecting single or multiple lineages-represent a clinically important, though often under-recognized, complication. Cytopenias in AiLDs arise from diverse mechanisms, including immune-mediated destruction, hypersplenism due to portal hypertension, bone marrow suppression, and nutrition
