Auto-immune haemolytic anaemia (AIHA) has been found in a case of alpha-interferon treatment. Serum antibody and eluate were positive in the absence of the drug. Although the patient recovered after the treatment was stopped, DAGT remained positive for at least 8 months. The mechanism proposed to explain why this drug induced AIHA is similar to that proposed for alpha-methyl-dopa. Drugs could alter the red cell membrane and impair the immune system. Such changes have been observed with alpha-interferon and were related with increased autoimmunity.

Drug allergies can be classified into three subgroups which differ in their pathophysiology and require different diagnostic steps: 1. classical drug allergies, which are directed against the drug itself, a reactive metabolite or some contaminant of the drug, 2. pseudoallergic reactions, which are caused by non-immune mediated degranulation of mast cells and basophils, and 3. autoimmune reactions, in which the drug elicits an immune reaction to autologous structures. A very detailed accurate history is of the greatest importance in the clarification of a suspected drug allergic reaction, as we

The serological investigation of immunohaemolytic anaemias is based on the direct antiglobulin test, on the study of the serum and on the eluate, thus allowing a classification, which is still valid and correlates well with the clinical features. The use of more sensitive techniques and of reagents made from monoclonal antibodies should improve the study of these anaemias, but emphasizes the issue of the limits between physiological and pathological states. The addition of new therapeutical approaches should improve their prognosis. The increasing consumption of drugs and the development of th

What conclusions can be drawn concerning specificity of drug-induced immune reactions? We have seen that specificity of these reactions depends on several molecular features including the chemical nature of the drug, specific domains of particular membrane components, and as yet unidentified characteristics that determine selectivity for one or more cell types. This latter property does not seem to be related to shared membrane components because, for example, Rh antigens on RBCs, the peptide tail region of GPIb alpha on platelets, and the 85-kd GP on neutrophils are clearly not part of the sa

A patient who expired during an episode of gross intravascular hemolysis had a complex medical history, including renal disease, Coombs positive anemia of unclear etiology, recent transfusion, and cholecystectomy. Drug history included 21 different medications, including penicillin, acetaminophen, procainamide, furosemide, sulindac, and tolmetin, all of which have been associated with a positive direct antiglobulin test or drug-induced hemolytic anemia. The patient had a history of recent use of three chemically similar nonsteroidal anti-inflammatory drugs: tolmetin (Tolectin), sulindac (Clino

Immune hemolytic anemia due to a drug-adsorption mechanism has been described primarily in patients receiving penicillins and first-generation cephalosporins. We describe a patient who developed anemia while receiving intravenous cefotetan. Cefotetan-dependent antibodies were detected in the patient's serum and in an eluate prepared from his red blood cells. The eluate also reacted weakly with red blood cells in the absence of cefotetan, suggesting the concomitant formation of warm-reactive autoantibodies. These observations, in conjunction with clinical and laboratory evidence of extravascula

Anemia is a common finding in children with systemic lupus erythematosus (SLE). Anemia in SLE may result from several mechanisms and more than one may be operative at any time. Anemia of chronic disease is the most common, but a low hemoglobin can be caused by auto-antibodies to red cells as part of the auto-immunity, or it may be the result of impaired erythropoietin production by kidneys involved in the SLE, gastrointestinal blood loss from anti-inflammatory therapy, increased red cell destruction from hypersplenism or a drug-induced immune phenomenon. We briefly review the important process

This review presents a unifying hypothesis that provides a connection between several types of hypersensitivity reactions associated with several types of drugs and explains some of the therapeutic effects (antiinflammatory activity and antithyroid effects) of these same drugs. This hypothesis centers on the oxidation of these drugs to chemically reactive metabolites by peroxidases. The drugs of interest have functional groups that are easily oxidized. The major peroxidase involved in this hypothesis is MPO because of its critical location in leukocytes which play a key role in the function of

Methyldopa is an antihypertensive medication which is available generically and under the trade name Aldomet that is widely prescribed in the adult population and infrequently used in children. Methyldopa causes an autoimmune hemolytic anemia in a small percentage of patients who take the drug. We report a case of methyldopa-induced hemolytic anemia in a 15-year-old boy who presented to the emergency department with near-syncope. The boy had been treated with intravenous methyldopa during a trauma admission seven weeks prior to presentation. Evaluation revealed a hemoglobin of three grams, 3+

A case of haemolytic anaemia secondary to oral ingestion of a Chelidonium majus extract is presented. It coursed with intravascular haemolysis, renal failure, liver cytolysis and thrombocytopenia. The direct Antiglobulin test was positive (4+), IgG+C'. The patient was treated with steroids, red cell and platelet transfusion and also twice with an haemodialytic treatment, with complete resolution of the clinical features about the 12th day. A complete bibliographic revision from 1966-1989 was performed and no cases like this one were found during that period, so this could the first one reporte

Drugs are responsible for the causative antibodies in 10-20% of all cases with immunocytopenias (immune hemolytic anemias, immune thrombocytopenias or immune granulocytopenias). Usually only one cell type is affected in the same patient, but rarely two cell types can be simultaneously involved. The immunocytopenia can precipitously occur within minutes to hours, or it develops gradually throughout days and weeks. In the former case the reactions are usually due to complement-activating IgG- and/or IgM-antibodies, and in the latter case to non-complement-activating IgG-antibodies without comple

Immunotoxicologic studies have demonstrated that autoimmune responses and/or autoimmune diseases are induced in humans and experimental animals by chronic exposure to various chemicals. The present review is focused on seven groups of chemically induced human disorders, i.e. systemic lupus erythematosus, autoimmune hemolytic anemia, myasthenia gravis, pemphigus, glomerulonephritis, thyroiditis and hepatitis. Results obtained from studies of the available experimental counterparts of these diseases, i.e. those models obtained from the exposure of laboratory animals to various chemicals, are the

The authors report the clinical and laboratory findings of a patient who had severe immune hemolytic anemia due to hydrochlorothiazide (HCTZ). In this case, the HCTZ antibody reacted not only with other thiazide and thiazide-like drugs, but also with a chemically unrelated diuretic, ethacrynic acid. These results indicate that HCTZ antibody activity is not restricted solely to the thiazides and imply that therapy with any of the reactive drugs would be contraindicated for this patient. The serologic screening for drug reactivity may be useful for selecting alternative therapy for patients with

A 70-year-old man developed severe immune intravascular hemolysis and renal failure following ingestion of fenoprofen, a nonsteroidal, anti-inflammatory drug. The patient's red blood cells were sensitized with both IgG and C3d. The serum reacted with normal red blood cells in the presence and absence of the drug. Addition of albumin to the serum inhibited the reactivity with both neat and drug-treated serum. These atypical serologic findings for drug-related immune hemolytic anemia were explained by (1) the measurement of fenoprofen by high performance liquid chromatography (HPLC) in the neat

Cold agglutinin disease is a normo- or macrocytic anemia due to antibodies, active under body temperature, mostly belonging to the immunoglobulin class M. Initially the agglutination of erythrocytes with acrocyanosis is reversible at body temperature. High antibody activity or long lasting period of coldness lead to intravascular or intrahepatic hemolysis, but high risk anemia is rare. Beside the idiopathic form, infection induced (especially infectious mononucleosis, cytomegalovirus, Mycoplasma pneumoniae, Klebsiella), and drug induced (especially quinidine, alpha methyldopa, penicillin, para
