OBJECTIVE: To assess the clinical manifestations and incidence of infection associated with Shiga-like toxin-producing Escherichia coli (SLTEC).
STUDY DESIGN: Children with diarrhea within a defined geographic area during a 12-month period were examined for the presence of SLTEC in their stools by polymerase chain reaction with the use of primers that were complementary to sequences of Shiga-like toxins types I and II and to other virulence factors.
RESULTS: There were 13 SLTEC infections among 468 children with diarrhea. Besides Shiga-like toxin sequences, the virulence genes eae and EHEC-h

Human parvovirus B19 is linked with a broadening spectrum of hematologic disorders, including aplastic crises in the context of hemolytic anemias, neutropenia, thrombocytopenia, and hemophagocytic syndromes. Children with any of these cytopenias should be screened for the presence of B19 because treatment with intravenous gamma globulin may provide resolution of abnormal blood counts if other therapeutic options, such as transfusion, are not adequate or desired.

OBJECTIVE: Thrombotic microangiopathies have been increasingly recognised in HIV infection. The contribution of haemolytic uraemic syndrome (HUS) has not received as much emphasis as other members of the thrombotic microangiopathies. We describe the clinical features and prognosis of HUS in a group of patients with AIDS.
SETTING: St Bartholomew's and The Middlesex Hospitals, London.
PATIENTS: Five HIV seropositive individuals with clinical and histological features consistent with HUS.
INTERVENTIONS: Blood transfusion, fresh frozen plasma, haemodialysis, renal biopsy, autopsy.
CONCLUSIONS:

An 8-year-old boy had been suffering from chronic autoimmune neutropenia for more than 5 years. The neutropenia proved to be resistant to high-dose steroids and intravenous (either low-or high-dose) immunoglobulin (Ig) therapy. The chronic autoimmune thrombocytopenia and recurrent phases of autoimmune haemolytic anaemia did, however, respond to high-dose prednisone. Other signs of immune dysregulation in this patient consisted of insulin-dependent diabetes mellitus type I (IDDM) and an acquired hypogammaglobulinaemia, most compatible with common variable immunodeficiency (CVI). Prior to rhG-CS

There has been an increase in the indications for splenectomy for hematologic diseases. It is a consequence of expanding the list of disorders and liberalizing the indications for splenectomy for many diseases. Hereditary spherocytosis is the most frequently encountered congenital anemia for which splenectomy is curative. Splenectomy is generally advised for warm antibody acquired hemolytic anemia if conventional medical therapy fails. Idiopathic thrombocytopenic purpura (ITP) remains the most common hematologic disorder for which splenectomy is therapeutic. A variety of thrombocytopenias are

The incidence and time course of the immune response to Vero toxin-producing Escherichia coli (VTEC) other than O157 (O26, O55, O111, O128) were determined in 107 children with enteropathic hemolytic uremic syndrome (HUS) by ELISA and compared to antibody profiles of 125 healthy pediatric controls and 100 children with community-acquired uncomplicated diarrhea. Six of 8 HUS patients with non-O157 VTEC isolates exhibited a serologic response to the homologous lipopolysaccharide antigen similar to that of patients infected with E. coli O157. In addition, elevated IgM or IgG antibodies were demon

Initially used as replacement therapy in patients with hypogammaglob-ulinemia, intravenous gamma-globulin (IVIg) preparations are increasingly being used as treatment for various autoimmune disorders. Although expensive, IVIg has become first-line or adjunctive therapy in the treatment of disorders as diverse as autoimmune or post-transfusion thrombocytopenia, autoimmune hemolytic anemia, Kawasaki's disease, inflammatory myositis, myasthenia gravis, pediatric acquired immune deficiency syndrome, bone marrow transplantation, and inflammatory de-myelinating polyneuropathy. IVIg remains the first

BACKGROUND: Acquired deficiency in C1-inhibitor (C1-INH) associated with malignancy is often asymptomatic because clinical manifestations are not dependent on a critical complement threshold (in contrast to hereditary C1-INH deficiency). Increased complement consumption involving different kinds of antibodies is the postulated mechanism for this disease, but other factors must play an important role.
CASE REPORT: A 76-year-old woman with unremarkable medical history experienced three episodes of angioedema over 6 months. Investigations revealed a complement profile characteristic of acquired

Escherichia coli O.157:H7 is a serious and common human pathogen that can cause diarrhea, hemorrhagic colitis, and the hemolytic uremic syndrome (HUS). During a massive outbreak of infection with E coli O157:H7 in January 1993 in Washington State, more than 600 people, mostly children, acquired symptomatic infection, and 37 were hospitalized with HUS at Children's Hospital and Medical Center in Seattle, and six at other hospitals in Washington. Twenty-one (57%) required dialysis. Nineteen (51%) had significant extrarenal pathology: gastrointestinal in 14 patients (38%), cardiovascular in 13 (3

Glomerulocystic kidney disease is a rare condition, usually seen in infants and young children, characterised by cystic dilatation of the glomeruli. It may be sporadic or represent the congenital expression of dominant polycystic kidney disease. Glomerular cysts may also be seen in association with various syndromes and as a component of dysplastic kidneys. Only two cases of acquired glomerulocystic kidneys have been described, both in adults. The cystic change followed haemolytic-uraemic syndrome in one patient and systemic sclerosis in the other. These two conditions are closely related and

BACKGROUND: Hemolytic uremic syndrome (HUS) is an acquired disorder largely affecting infants and young children. It is characterized by the triad of microangiopathic hemolytic anemia, acute renal failure, and thrombocytopenia. Although its etiology is unknown, viral and bacterial infections, disseminated malignancies in adults, and a variety of chemotherapeutic agents including cisplatin, have been implicated in its occurrence. The association of HUS with chemotherapeutic agents after its detection in a pediatric patient treated with cisplatin is reviewed.
METHODS: A 16-year-old male with os

Recently there have been increasing reports of HIV infection acquired through transfusion of HIV seronegative blood in Thailand due to high incidence of HIV new infection in blood donors. Blood or blood components (BC) prepared from HIV seronegative blood donation pose significant hazards to recipients because of the risk of viremia during the "window period" of HIV infection. This paper presents the HIV seroprevalence in hematologic patients other than hemophiliacs who received multiple blood transfusion at Ramathibodi Hospital. The retrospective analysis was done on 167 patients: 132 thalass

The erythrocyte membrane, in the broad sense of the term, comprises a lipid bilayer and below it the erythrocyte skeleton responsible for the mechanical properties of red cells. The lipid bilayer contains numerous proteins including the anion transporter. The underlying portion contains spectrin, actin and protein 4.1. At the interface between these two portions, ankyrin, protein 4.2 and other proteins are found. The structure and mode of expression of the corresponding genes are roughly known, as are the structure-function relationships within the proteins. In the wake, or sometimes ahead of

OBJECTIVE: Paroxysmal nocturnal hemoglobinuria (PNH) is an acquired hemolytic disorder, characterized by hemosiderin deposition in the proximal renal tubules.
MATERIALS AND METHODS: We examined six cases of PNH with MRI including gradient echo imaging.
RESULTS: On T2- and T2*-weighted imaging a characteristic hypointense pattern was noted in five of six patients with PNH. The extent of the hypointense area roughly correlated with the time elapsed from the latest hemolytic attack.
CONCLUSION: T2*-weighted imaging (gradient echo imaging) is superior to T2-weighted imaging in the detection of

Plasma-derived coagulation factor concentrates, prepared using traditional manufacturing processes, have transmitted viral diseases, especially AIDS, hepatitis B and hepatitis C to patients. To date, more extensive selection of blood donors, improved screening procedures of each plasma donation for direct and indirect viral markers, and newly developed virucidal procedures, especially pasteurization and solvent-detergent, together with extraction technologies of plasma proteins based on high-resolution chromatographic separations, have diminished considerably the risks of transmitting these pa

Bleeding problems in the cancer patient may result from the effects of the tumor on hemostatic mechanisms or from the treatment of the tumor by cytotoxic and other agents. Among the tumor-related bleeding problems are disseminated intravascular coagulation, primary fibrinolysis, thrombocytopenia, acquired platelet dysfunction, and circulating inhibitors or anticoagulants. Disseminated intravascular coagulation in most solid tumors is associated with hypercoagulability, whereas in acute promyelocytic leukemia bleeding is the most common presentation. Treatment-related bleeding disorders include

In evaluating pregnant women with anemia, it is essential to do a complete history and physical examination, as well as a complete blood count with indices and a blood smear examination. Based on these findings, other tests such as ferritin and serum or red cell folate may be ordered. Because of the normal physiologic changes in pregnancy that affect the hematocrit, indices, and some other parameters, diagnosing true anemia, as well as the etiology of anemia, is challenging. Because of the increased nutritional requirements of the mother and fetus, the most common anemias are iron deficiency a

Retyculocytopenia and neutropenia or both abnormalities are very infrequent findings during the evolution of patients with acquired autoimmune hemolytic anemia (AIHA). In this paper we describe the clinical cases of three patients with AIHA in whom six different periods of reticulocytopenia were identified; the first two patients also had neutropenia. At the moment of the diagnosis of the cytopenias, the patients did not have a systemic disease, a viral infection or received immunosuppression able to produce them. In every patient the bone marrow cellularity was increased mostly at the expense

Platelet IgG has been studied primarily to develop an aid for clinical diagnosis--a search that has proven elusive (48). However, studies of the interaction of IgG and other plasma proteins with platelets have provided new insight into the intracellular pathways for the acquisition of proteins into regulated secretory granules. From a clinical perspective, it is important not to make the assumption that all platelet IgG is antiplatelet antibody. Data on platelet IgG must be distinguished between measurements of alpha-granule IgG, which comprises almost all of the total platelet IgG content, an
