Arterial infections due to Listeria monocytogenes: report of four cases and review of world literature.

Early diagnosis and successful antimicrobial therapy have diminished the frequency of embolomycotic aneurysms, but infected aortic and small vessel aneurysms, arteriosclerotic plaques, and prosthetic grafts are becoming more common. A broad spectrum of pathogens, including Staphylococcus, Salmonella, Enterobacteriaceae, Pseudomonas aeruginosa, and some unusual organisms, are associated with this change. We treated four patients (three with abdominal aortic aneurysms and one with a prosthetic graft) with arterial infections caused by Listeria monocytogenes. Only seven other cases have previousl

Listeriosis and AIDS: case report and literature review.

Listeriosis is a not uncommon infection in humans, usually associated with immunodeficient states and with newborns. However, relatively few cases have been reported in HIV-infected patients. This scarcity of reported cases has aroused interest in the association of listeriosis and AIDS. In this paper we present a case of meningitis and septicemia caused by Listeria monocytogenes in a female patient with AIDS. A review of recent medical literature indicates that association of listeriosis and AIDS may be more common than it seems. Recent research in host-parasite interaction in listerial infec

Listeria monocytogenes infections in patients with AIDS: report of five cases and review.

Five patients with AIDS and Listeria monocytogenes infection (three cases of bacteremia and two of meningitis) are reviewed. Four patients had prior or concurrent gastrointestinal illness. Two patients received corticosteroids. A 7- to 21-day course of ampicillin was administered with or without a 7- to 14-day course of gentamicin. This regimen was effective, with no evidence of relapse 7-8 months after therapy was discontinued. The relative infrequency of infection with L. monocytogenes in AIDS patients is unexpected. Tumor necrosis factor (TNF) appears to be essential in the inhibition of Li

[The potentiating action of Listeria monocytogenes in relation to endotoxic shock].

The infection of mice with Listeria cells 15 hours prior to the inoculation of endotoxin sharply increased its lethal action. Blood taken from the mice at the peak of the development of shock was toxic for adrenalectomized recipients. This potentiating effect was blocked by hydrocortisone. Carrageenan prolonged the survival time of such animals, but did not change the course of endotoxin shock, as well as toxicosis, caused by listerial aqueous extract in adrenalectomized mice. This potentiating phenomenon plays supposedly a certain role in the genesis of toxicosis in mixed infections (listerio

The treatment of Listeria monocytogenes septicaemia.

Thirty cases of Listeria monocytogenes septicaemia occurred in Denmark between 1981 and 1986. The aim of this study was to consider the treatment of these patients, 18 males and 12 females aged from 20 to 87 years: average (AV) 65 years. One or more predisposing factors (PF) were found in 90% of the patients, mainly cancer (16), steroid treatment (12), cirrhosis and/or alcoholism (8), and diabetes mellitus (3). Follow-up varied from 3 months to 5 years. Ampicillin (AMP) alone or with an aminoglycoside (AMI) was the treatment in 9 and 16 cases, respectively. One patient was successfully treated

Perinatal listeriosis in Dresden 1981-1986: clinical and microbiological findings in 18 cases.

Between 1981 and 1986 Listeria monocytogenes was isolated from blood cultures, CSF, meconium/stools or external swabs from 18 newborn infants of two neonatal intensive care units (ICU) in adjacent pediatric clinics of Dresden. The epidemiological and clinical data of infants and their mothers, as well as microbiological and laboratory, x-ray, EEG and ultrasonic findings, are presented. All infants had an early onset of their disease. Cases were classified as granulomatosis infantiseptica (three cases), sepsis (three cases), meningitis (eight cases) and listerial infection without distinct orga

A randomised prospective comparison of cefotaxime versus netilmicin/penicillin for treatment of suspected neonatal sepsis.

In an open prospective study performed in 2 neonatal units, infants with suspected neonatal sepsis (SNS) of unknown microbial cause were randomly allocated to receive treatment with either cefotaxime (CTX) or netilmicin plus penicillin (N + P). 236 patients were entered into the trial, of whom 222 were evaluable. The number of 'definitely' and 'probably' infected babies was similar in both groups. There was no difference in clinical outcome between patients in the 2 treatment groups and no side effects were recorded for either of the antibiotic regimens. Antibiotic sensitivity testing of bacte

Listeria monocytogenes: a rare cause of opportunistic infection in the acquired immunodeficiency syndrome (AIDS) and a new cause of meningitis in AIDS. A case report.

A forty-two year-old male homosexual with the acquired immunodeficiency syndrome (AIDS) developed Listeria monocytogenes septicemia and meningitis. The gastrointestinal tract was the likely portal of entry. The patient was treated with intravenous ampicillin with complete and permanent resolution of his listerial infection. Although L. monocytogenes infection has been reported as an uncommon complications of AIDS, we are unaware of Listeria meningitis being previously reported in an AIDS patient. It is hoped that this case report will alert health care workers to the possibility of Listeria in

Perinatal listeriosis. A report of six cases.

The clinical and pathological findings in six patients with perinatal listeriosis are presented. One pregnancy resulted in a live-born infant who developed listerial septicaemia but made a complete recovery following prompt treatment. The other pregnancies ended in intrauterine death. Often antecedent maternal prodromal illness preceded expulsion of a macerated fetus by only a matter of hours making early diagnosis difficult. In all cases the diagnosis of listerial infection was apparent only after the birth of the fetus or new born.

Brain abscess due to Listeria monocytogenes: case report and literature review.

Listeria monocytogenes is an uncommon cause of brain abscess. Of a total of 14 cases of L. monocytogenes brain abscess (one described for the first time and 13 reported previously in the English-language literature), seven (50%) occurred in patients with leukemia and recipients of renal transplants; four (29%) of the cases occurred in previously healthy individuals. Common clinical findings were similar to those in brain abscess due to other causes and included fever (57%), headache (57%), and focal neurologic signs (64%). Distinctive, however, was the unusually high frequency of associated me

Listeria monocytogenes cerebritis, bacteremia, and cutaneous lesions complicating hairy cell leukemia.

Hairy cell leukemia is a lymphoreticular malignancy characterized by a chronic course and multiple defects in host defense mechanisms. Infections are the major cause of morbidity and mortality in this malignancy. Opportunistic infections due to pathogens normally controlled by cell-mediated immune mechanisms have been increasingly described but have not included listerial infections. This report describes a case of disseminated Listeria monocytogenes infection including the uncommon manifestations of cerebritis and cutaneous lesions in a patient with hairy cell leukemia.

Epidemic listeriosis in the newborn.

Within a three-month period 13 cases of listeriosis in the newborn were seen at the National Women's Hospital, Auckland. Eleven presented in the first 24 hours of life, the most common feature being respiratory difficulty in low birth weight infants. Meconium-stained liquor was noted in nine cases. The constant finding in all cases was an aspiration pneumonia which appeared to be of intrauterine origin from an infected amniotic cavity. There was also evidence of septicaemia in nine cases, and two infants survived meningitis which developed at 1 week. Maternal symptoms were mild and variable, a

Listeria monocytogenes infections in Metropolitan Toronto. A clinicopathological study.

The clinical and laboratory findings in 21 patients with listeriosis are described and the subject is reviewed. Eleven of the infections were septicemias of newborns, eight were meningitis in infants or adults, and two other children had unusual manifestations.Neonatal septicemia was rapidly fatal; one of 11 infants survived. The disease often seemed traceable to mild maternal infection during the third trimester usually leading to premature delivery of critically ill babies. Only awareness of the possible presence of listeriosis and early antibiotic therapy seem capable of reducing this high
