Biclonal gammopathy with coexistent proximal tubulopathy and cast nephropathy in multiple myeloma: a rare case report.

BACKGROUND: Multiple myeloma is a hematological malignancy originating from neoplastic plasma cells, typically characterized by the presence of a monoclonal immunoglobulin. Light chain cast nephropathy is the most common cause of acute kidney injury in myeloma patients, typically presenting with elevated serum creatinine. Light-chain proximal tubulopathy is a manifestation of monoclonal light chain-associated renal injury, characterized by the accumulation of free light chains within proximal tubular cells. However, the co-occurrence of κ light-chain MM with an additional IgG-λ monoclonal band

Casdatifan shows durable response linked to HIF-2α biology in kidney cancer.

Clear cell renal cell carcinoma (ccRCC) is largely driven by the transcription factor hypoxia-inducible factor 2α (HIF-2α)1. Here we show that monotherapy with casdatifan-an orally bioavailable, potent and selective HIF-2α inhibitor2-produces meaningful, durable antitumour activity with manageable safety in individuals with refractory metastatic ccRCC. Dose-expansion data from the ARC-20 study ( NCT05536141 ) are presented, including for the 100 mg once daily (QD) cohort (n = 32) and the total cohort (n = 127). Treatment discontinuation from casdatifan-related adverse events was infrequent (3%

Fibrocytes drive JAK2V617F-mutated myelofibrosis: pitavastatin reverses marrow fibrosis and anemia.

Bone marrow (BM) fibrosis in primary and post-polycythemia vera/essential thrombocythemia myelofibrosis (MF) has traditionally been considered a reactive process driven by cytokines, such as transforming growth factor β1 (TGF-β1), primarily produced by neoplastic megakaryocytes and platelets. These cytokines promote the differentiation of wild-type mesenchymal stromal cells into collagen- and fibronectin-producing myofibroblasts, thereby inducing BM fibrosis. However, hematopoietic-derived collagen-producing cells of monocyte lineage, termed fibrocytes, have also been implicated in this proces

Small-Bowel Lymphoma: The Hidden Cause of Obscure Gastrointestinal Bleeding.

Obscure gastrointestinal bleeding (OGIB) accounts for approximately 5% of all gastrointestinal bleeding cases, with the small intestine being the most common source of origin. Primary small-bowel lymphoma is a rare malignancy that can present as intestinal bleeding in 2% to 22% of cases.  A 67-year-old male presented with a four-day history of diarrhea and melena, accompanied by severe anemia with a hemoglobin level of 5 g/dL. While a colonoscopy only revealed internal hemorrhoids and an ascending colon polyp, a subsequent non-contrast whole-abdomen CT scan demonstrated a 6-cm segmental small

Analysis of indocyanine green-guided sentinel lymph node mapping for cervical cancer during pregnancy.

OBJECTIVE: Cervical cancer is the most common gynecological malignancy during pregnancy. Management must balance maternal oncologic control with fetal safety. Sentinel lymph node mapping using indocyanine green may offer a minimally invasive alternative to pelvic lymphadenectomy, but its feasibility and safety during pregnancy remain unclear. We evaluated the feasibility, detection outcomes, and obstetric safety of indocyanine green-guided sentinel lymph node mapping in early-stage cervical cancer during pregnancy. METHODS: This multi-center retrospective case series included pregnant patient

Disruption of microhomology-mediated end joining in Ewing sarcoma.

Ewing sarcoma (EwS) is a group of bone and soft-tissue cancers in children and young adults. Because EwS cells have pronounced sensitivity to radiation and chemotherapy-induced DNA damage, the oncoprotein EWS-FLI1 is likely to be involved in DNA repair. Here, we demonstrate that EWS-FLI1 causes a defect in microhomology-mediated end joining (MMEJ) repair. EWSR1 is a splicing factor that promotes the faithful splicing of the POLQ pre-mRNA, required for the expression of Polθ, a critical protein in the MMEJ pathway. Expression of EWS-FLI1 or depletion of EWSR1 causes increased POLQ exon 25 skipp

Methylene dianiline-induced liver toxicity supporting cytotoxicity-regenerative repair key events of parenchymal and non-parenchymal cells.

Lifetime 4,4'-methylenedianiline (MDA) administration culminates in hepatic neoplastic nodules in male F344 rats. Numerous published studies demonstrate MDA-induced periportal/bile duct region pathology that eventually spreads throughout the acinus. Severe reductions in body weight gain, and food and water intake accompany MDA-induced liver injury. This paper presents the results of a 90-day drinking water and a 14/28-day oral gavage study of MDA in rats. The 90-day study demonstrated decreased drinking water and food consumption, resulting in severe body weight gain reductions. Male and femal

m6A-methylated circFANCB Promotes Gastric Cancer Progression by Regulating Cellular Ferroptosis Through miR-454-3p/CEACAM5.

Gastric cancer (GC) is a major global digestive malignancy with high incidence and mortality. Circular fanconi anemia complementation group B (circFANCB) expression is elevated in cancer, and its suppression suppresses tumor progression. However, research on the function of circFANCB in GC has not been reported yet. The role and mechanism of circFANCB in GC were investigated through circular RNA (circRNA) sequencing, database prediction, and experimental validation, including quantitative real-time PCR (qRT-PCR), ribonuclease R assay, nuclear-cytoplasmic fractionation, 5-ethynyl-2'-deoxyuridin

Spontaneous Splenic Rupture Associated With Cytomegalovirus Infection: A Case Report.

Atraumatic splenic rupture is a rare but potentially life-threatening condition, most commonly associated with infectious, hematological, neoplastic, inflammatory, or iatrogenic disorders. Cytomegalovirus (CMV)-associated splenic rupture is an exceptionally uncommon complication in immunocompetent individuals. We report the case of a 46-year-old immunocompetent man who presented to the emergency department with acute abdominal pain and a low-grade fever. Laboratory investigations revealed mild microcytic anemia, thrombocytopenia, elevated inflammatory markers, and mild transaminase elevation.

Neuroendocrine Tumor of Unknown Origin Leading to the Diagnosis of Advanced Celiac Disease: A Case Report Exploring a Possible Association.

INTRODUCTION: Celiac disease (CD), if left untreated, can predispose a wide range of intestinal malignancies including enteric lymphoma and adenocarcinoma. On rare occasions, intestinal neuroendocrine tumors (NETs) were reported. We present a case of metastatic NET that led to the diagnosis of CD. CASE PRESENTATION: A 58-year-old woman presented with a lump in the left axilla without fever or weight loss. Physical examination showed a single tender and rubbery lymph node (LN) of 2.5 cm in size in the left axilla. Laboratories showed mild iron-deficiency anemia. An excisional LN biopsy showed

FANCM is required for the PAX3::FOXO1-driven oncogenic program in rhabdomyosarcoma.

Many cancers are driven by mutationally altered transcription factors (TFs) that rewire cells to an oncogenic state. Cells must activate specific mechanisms to tolerate the burden of oncogenic TF activity. To define such mechanisms, we focused on a canonical oncogenic fusion protein-driven cancer, alveolar rhabdomyosarcoma (ARMS), where the PAX3::FOXO1 fusion protein hyperactivates and mislocalizes PAX3 and FOXO1 TF functions. Employing sequential functional genomic CRISPR-Cas9 screens, we identified FANCM, a DNA translocase in the Fanconi anemia pathway, as a selective dependency in PAX3::FOX

Atypical Kaposi Sarcoma in HIV Infection with Predominant Nodal Involvement.

Kaposi sarcoma (KS) is an angioproliferative neoplasm driven by human herpesvirus 8 (HHV-8), most commonly occurring in immunocompromised individuals, particularly in association with HIV infection. Although it classically presents with characteristic cutaneous or mucocutaneous lesions, nodal-predominant disease without skin involvement is uncommon and may pose a significant diagnostic challenge. We report the case of a 26-year-old man with recently diagnosed HIV infection and no prior antiretroviral therapy (ART) who presented with a two-week history of persistent fever, constitutional sympto

A Case of Mitochondrial Myopathy, Lactic Acidosis and Sideroblastic Anemia (MLASA Syndrome) and Long QT Interval in a 10-Year-Old Saudi Child.

MLASA syndrome is a rare mitochondrial disorder that presents in three distinct genetic forms: MLASA1, MLASA2, and MLASA3; MLASA1 is the most common form. The clinical features include mitochondrial myopathy, lactic acidosis, and sideroblastic anemia. Although presence of other features is not uncommon, its association with long QT (LQT) syndrome has not been described before. In addition, while MLASA syndrome has been reported from several countries worldwide, we present the first patient with MLASA1 syndrome from the Kingdom of Saudi Arabia in this case report. The 10-year-old girl with hist

A novel frameshift deletion in SLC25A38 and its role in mitochondrial dysfunction: A case study of sideroblastic anemia in a child from Iran.

Sideroblastic anemia (SA) is a rare hematological condition characterized by the accumulation of iron in the mitochondria of erythroid precursor cells, resulting in the formation of sideroblastic rings. It occurs in both inherited and acquired forms. A prevalent subtype of congenital sideroblastic anemia (CSA) results from autosomal recessive mutations in the SLC25A38 gene. This investigation included a clinical and genetic evaluation of a family with a child diagnosed with pyridoxine-refractory SA. Whole Exome Sequencing (WES) was performed on the patient to detect the genetic variation. Afte

SLC25A38 gene modification mediated by CRISPR/Cas9 in HEK293T cell line.

Sideroblastic anemia is a rare disease and one of the heterogeneous bone marrow disorders. Gene therapy is considered as a method for treating these patients. The CRISPR/Cas9 system can recognize specific genomic DNA sequences and create Double-Strand Breaks (DSBs) in DNA. In this study, we attempted to investigate the effect of different sgRNAs in creating targeted cleavage in the desired region using the CRISPR/Cas9 system and inducing changes in the disease-related gene. Following the design of six sgRNAs, recombinant pX458 and pX459 plasmids harboring the respective sgRNA sequences were co

Case Report: A Previously Healthy Young Woman With Lethal, Unremitting Metabolic Acidosis: Could a Novel Variant in ALAS2 Be the Culprit?

BACKGROUND: Heme synthesis is critical for several biological processes, including mitochondrial energy production and oxygen delivery via hemoglobin. The initial and rate-limiting step in heme synthesis is the conjugation of glycine with succinyl-CoA to form 5-aminolevulinic acid (ALA), which is catalyzed by two closely related enzymes that are coded by highly homologous genes, known as ALAS1 and ALAS2. Loss-of-function variants in ALAS2 result in sideroblastic anemia, whereas gain-of-function variants result in porphyria. We present a case of a woman who died with severe metabolic acidosis a

Potential Vitamin B6 Depletion During Carbidopa/Levodopa Therapy in Parkinson's Disease: A Scoping Review of Direct Evidence.

Vitamin B6 depletion during carbidopa/levodopa therapy has been linked to polyneuropathy, seizures, and anemia, but the direct evidence is heterogeneous, and causality remains uncertain. We conducted a focused scoping review of direct empirical evidence on vitamin B6 biology during carbidopa/levodopa exposure and on vitamin B6-related clinical consequences supported by biochemical assessment. We also included studies of biomarker-guided monitoring, supplementation, and direct carbidopa-vitamin B6 chemistry.
