Congenital dyserythropoietic anaemia type-I (CDA-I) is a rare autosomal recessive disease characterised by ineffective erythropoiesis, haemolysis and non-haematological developmental abnormalities. Its treatment is multifactorial, including the management of anaemia, iron overload and prevention of osteoporosis. The only treatment specific to CDA-I is subcutaneous interferon alpha (IFNα) 2A. This study presents the first summary of all published cases of CDA-I patients (n = 33) treated with IFNα and categorises their outcome. We also present new unpublished cases (n = 7). Overall, we find that

Congenital Dyserythropoietic Anaemia type I (CDA-I) is a rare inherited disorder of erythropoiesis, in which erythroid cells display a unique nuclear phenotype referred to as 'spongy' heterochromatin. The molecular basis of CDA-I remains unknown, with most cases of CDA-I caused by mutations in CDAN1, encoding Codanin-1, or CDIN1, encoding for Codanin-1-interacting nuclease 1 (CDIN1). To date, very little is known about the function of CDA-I disease proteins and the mechanism by which their associated mutations cause disease. Here, we demonstrate that endogenous CDIN1 interacts with Codanin-1,

BACKGROUND: Congenital dyserythropoietic anaemia type I (CDA-I) is a rare inherited bone marrow failure syndrome characterized by ineffective erythropoiesis and distinct morphological abnormalities of erythroblasts. CDA-I is primarily caused by biallelic pathogenic variants in CDAN1, while mutations in C15ORF41 account for a much rarer subtype, classified as CDA-Ib. Reports of C15ORF41-associated CDA-I from the Indian population are extremely limited.
METHODS: We investigated a transfusion-dependent male child presenting with neonatal jaundice and persistent anaemia. Hematological evaluation

A heterozygous Kruppel-like factor 1 (KLF1) E325K mutation causes a severe form of congenital dyserythropoietic anaemia (CDA), designated CDA-IV (Online Mendelian Inheritence in Man, OMIM 613673), a rare disease with only 15 cases known. In addition to haemolytic anaemia, manifest often at birth, four of seven (57%) of 46,XY children with KLF1 E325K mutation exhibit a range of dysmorphic external genitalia including 46,XY complete sex reversal (2/7 or 28.5%). No genital abnormalities were reported in a boy with double heterozygous KLF1 null mutations, suggesting that the KLF1 E325K variant cau

Parvovirus B19 (B19V) is a common single-stranded DNA virus that infects humans and is its only host. Although infection is often asymptomatic, B19V can cause significant hematologic disease due to its tropism for erythroid progenitor cells. Immunocompromised individuals, including those with human immunodeficiency virus (HIV), are susceptible to persistent infection and may develop pure red cell aplasia (PRCA) and chronic anemia. Since B19V infection is not routinely monitored in the United States, and its symptoms can overlap with other causes of anemia in HIV, diagnosis may be overlooked. E

BACKGROUND: Cyclosporine A (CsA) is the standard first-line treatment for acquired pure red cell aplasia (PRCA), but outcomes vary. Effective salvage strategies for CsA-refractory patients, particularly those with complex secondary subtypes, remain inadequately defined. Additionally, the clinical utility of STAT3/5b mutations in guiding treatment selection remains unclear.
OBJECTIVES: To evaluate the efficacy of second-line sirolimus for the treatment of PRCA and explore the possible genetic predictors for the response.
DESIGN: This study was a retrospective, single center, comparative study

INTRODUCTION: During treatment for malignant lymphoma, cytopenia can develop for several reasons. This can range from mild cytopenias leading to infection and bleeding to full-blown drug-induced aplastic anaemia. While aplastic anaemia affects individuals of all genders and ages, here, we describe aplastic anaemia after chemotherapy exposure to bendamustine in a 65-year-old female with non-Hodgkin's lymphoma.
CASE DESCRIPTION: A 65-year-old woman with recurrent indolent marginal zone lymphoma and post-chemotherapy with bendamustine and rituximab, presented with a neutropenic fever and was adm

RATIONALE: Multiple endocrine neoplasia type 1 (MEN1) is characterized by the coexistence of multiple endocrine tumors, most commonly due to autosomal dominant mutations in the tumor suppressor gene MEN1. Because this neoplastic disease is rare, diagnosis is often challenging. Some patients remain undiagnosed because of absent or nonspecific symptoms, while others may die before a correct diagnosis is established.
PATIENT CONCERNS: An 88-year-old man presenting with progressive dementia and heat stroke was admitted to our hospital. Close examinations revealed syndrome of inappropriate secreti

We describe a patient with long-standing rheumatoid arthritis (RA) who developed severe refractory transfusion-dependent anaemia lasting several months following treatment with a JAK1 inhibitor (Filgotinib). The patient proceeded with extensive investigations, including bone marrow assessment, and was initially diagnosed with Filgotinib-associated anaemia. However, despite stopping this agent, the anaemia persisted with fluctuating reticulocyte count resulting in multiple hospital admissions. Although the anaemia was initially attributed to Filgotinib, its persistence alongside delayed reticul

Vacuoles in hematopoietic cell precursors have garnered significant attention in recent years due to their association with a newly characterized clonal hematopoiesis with acquired mutations of ubiquitin like modifier activating enzyme 1 (ubiquitin-activating enzyme E1, UBA1) gene associated with clinical systemic autoinflammatory manifestations, known as VEXAS (vacuoles, E1 enzyme, X-linked, autoinflammatory, somatic) syndrome. The cytomorphologic hallmark of this rare disorder is vacuolization involving the granulocytic and erythroid precursors. While affected vacuolated cells are seen in mo

Uveal melanoma is a hard-to-treat arginine-dependent cancer secondary to argininosuccinate synthetase 1 (ASS1) loss with half of patients succumbing to liver-dominant metastases. Arginine deprivation with pegargiminase is a novel antimetabolite strategy for patients with uveal melanoma. We investigated the preclinical rationale for combining pegargiminase with melphalan, an alkylating agent approved recently for the treatment of hepatic-centric disease. Drug sensitivity of ASS1-deficient uveal melanoma cell lines was performed in 2D culture using proliferation and cytotoxicity assays, with ana

BACKGROUND: Pearson syndrome (PS) is a rare multisystem mitochondrial disorder characterized by single large-scale mitochondrial DNA deletions (SLSMDs). It typically presents in infancy with refractory sideroblastic anemia, exocrine pancreatic insufficiency, and failure to thrive. Due to its heterogeneous manifestations and resemblance to other hematological conditions, early diagnosis remains a clinical challenge.
CASE PRESENTATION: A south asian male infant presented with persistent pancytopenia, severe anemia unresponsive to intravenous and oral iron and multivitamin supplements, exocrine

Gene therapy is revolutionizing treatment paradigms for inherited haematological and immunological conditions. Recent successes, including the United States Food and Drug Administration (FDA) approval of gene therapy products for sickle cell disease and beta-thalassaemia, highlight the translational path of gene therapies for erythroid-specific disorders. In contrast, gene therapy development for other inherited erythroid disorders remains largely preclinical. Here, we examine the emerging landscape of gene therapies for inherited non-haemoglobinopathy erythroid disorders, focusing on the stat

Plasma cell gingivitis (PCG) is an uncommon plasma-cell-rich inflammatory disorder of the gingiva that may clinically resemble plaque-associated enlargement, granulomatous disease, hematologic disease, or plasma-cell neoplasia. Its association with cheilitis is rare, and diagnosis requires clinicopathologic correlation. An 18-year-old female presented with a 1-year history of progressive gingival enlargement and upper-lip swelling. Intraoral examination showed generalized erythematous marginal and papillary gingival enlargement with loss of stippling, bleeding on probing, pseudopocketing, and

Previous studies in Sri Lanka have explored the prevalence and causes of anaemia, mainly emphasizing iron deficiency while overlooking other important factors such as enzymopathies, membranopathies, and haemoglobinopathies. Moreover, many studies reported unexplained cases of anaemia even after detailed evaluation. This study aimed to determine the prevalence and underlying causes of anaemia among community-based young adults in Sri Lanka. A descriptive cross-sectional study was conducted from January 2023 to December 2024 among young adults aged 18-30 years. Data and blood samples were collec

BACKGROUND: Hypoxic microenvironment is an important characteristic of solid tumors. A series of adaptations associated with hypoxia occur in cancer cells, including reprogramming of multiple metabolic pathways. Gastrointestinal bleeding is a common clinical symptom of CRC, resulting in anemia and iron deficiency. CRC patients have unbalanced iron homeostasis due to blood loss, chronic inflammation, etc. However, the molecular mechanisms underlying the effect of hypoxia-induced reprogramming of iron metabolism on chemosensitivity in CRC remain unclear.

MicroRNAs (miRNAs) and their isoforms, known as isomiRs, are important regulators of tumorigenesis that act as post-transcriptional modulators of gene expression. Among these, 5'-isomiRs-generated through imprecise cleavage during miRNA biogenesis-exhibit altered seed regions compared to their canonical counterparts, potentially leading to distinct targetomes. Consequently, 5'-isomiRs may exert biological functions that differ substantially from those of the corresponding canonical miRNAs. Despite growing recognition of their potential significance, the functional roles of 5'-isomiRs remain la

Cholangiocarcinoma (CCA) is a malignant tumor arising from the epithelial cells of the bile ducts. Its incidence is increasing, and it is associated with a high mortality rate due to its aggressive behavior and frequent late diagnosis. We report the case of a 52-year-old man with a history of dyslipidemia and active smoking who was brought to the emergency department following a suicide attempt. The patient suspected he had a neoplastic disease, similar to the CCA that had affected his mother. He reported a three-month history of weight loss, asthenia, fever, and night sweats. Malignancy remains a major cause of late morbidity and mortality in kidney transplant recipients, largely due to chronic immunosuppression and impaired tumor immune surveillance. Mammalian target of rapamycin inhibitors have antiproliferative and antiangiogenic properties and are frequently used in recipients considered to be at increased oncologic risk. However, their protective effect against de novo malignancy is not absolute. Here, we report 2 cases of severe malignancies that developed in kidney transplant recipients after conversion from calcineurin inhibitors to sirolimus following poly
