Oral squamous cell carcinoma risk and magnitude of association in inherited cancer predisposition syndromes: evidence from a large real-world cohort.

OBJECTIVE: Inherited cancer predisposition syndromes (ICPS) are rare genetic disorders associated with an elevated cancer risk. This study evaluates oral squamous cell carcinoma (OSCC) prevalence across selected ICPS, including Fanconi anemia (FA), Plummer-Vinson syndrome, Cowden syndrome, Li-Fraumeni syndrome, dyskeratosis congenita, and xeroderma pigmentosum, quantifies risk magnitude, examines age at diagnosis, and assesses tobacco's modifying effect on OSCC risk in these populations. STUDY DESIGN: We conducted a retrospective cohort study using the TriNetX Research Network, including pati

When sickle cell disease strikes the orbits: a rare pediatric case of bilateral subperiosteal hematomas.

Bone infarctions can occasionally involve the orbit, presenting with subperiosteal hematomas and mimicking infectious or neoplastic processes. We report a case of bilateral orbital bone infarctions complicated by subperiosteal hematomas in a child with sickle cell disease during a vaso-occlusive crisis. The diagnosis of subperiosteal orbital hematoma was established by computed tomography. Differentiating infection on imaging was essential to guide management. Surgical decompression was performed, leading to marked clinical improvement and relief of optic nerve compression. This case illustrat

Genetic Features of Tumours Arising in the Context of Suspected Hereditary Cancer Syndromes with RAD50, RAD51C/D, and BRIP1 Germline Mutations, Results of NGS-Reanalysis of BRCA/MMR-Negative Families.

BACKGROUND AND OBJECTIVES: Despite the well-established role of the BRCA and mismatch repair (MMR) genes in DNA damage repair pathways, a substantial proportion of familial cancer cases still lack pathogenic variants in those genes. Next Generation Sequencing (NGS) panels have emerged as a powerful tool to identify hereditary cancer at-risk individuals and subsequently provide them with accurate management. MATERIALS AND METHODS: Families harbouring PVs in RAD50, RAD51C, RAD51D, and BRIP1 were identified by analysing a cancer-predisposing genes panel using Ion S5 system technology. A retrospe

SLC25A38 is required for mitochondrial pyridoxal 5'-phosphate (PLP) accumulation.

Many essential proteins require pyridoxal 5'-phosphate, the active form of vitamin B6, as a cofactor for their activity. These include enzymes important for amino acid metabolism, one-carbon metabolism, polyamine synthesis, erythropoiesis, and neurotransmitter metabolism. A third of all mammalian pyridoxal 5'-phosphate-dependent enzymes are localized in the mitochondria; however, the molecular machinery involved in the regulation of mitochondrial pyridoxal 5'-phosphate levels in mammals remains unknown. In this study, we used a genome-wide CRISPR interference screen in erythroleukemia cells an

Case report: A novel 11-bp deletion in exon 11 causing a frameshift in the C-terminal of the ALAS2 gene leading to X-linked sideroblastic anemia-a family study.

X-linked sideroblastic anemia (XLSA) (MIM 300752) is the most common genetic form of sideroblastic anemia, a heterogeneous group of disorders characterized by iron deposits in the mitochondria of erythroid precursors. It is due to mutations of the erythroid-specific enzyme ALAS2, the first enzyme of the heme biosynthetic pathway. Herein, we report a novel 11-bp deletion in exon 11 leading to a frameshift in the C-terminal region of the ALAS2 gene with a non-functional longer polypeptide of 614 amino acids leading to a loss-of-function mutation manifested as an X-linked sideroblastic anemia phe

Sideroblastic anemia in children: challenges in diagnosis and management in three cases.

Sideroblastic anemias (SAs) represent a heterogeneous group of rare hematological disorders characterized by iron accumulation in mitochondria of erythroblasts with ineffective erythropoiesis. SAs are categorized into acquired and congenital forms. Acquired, secondary, and clonal, SA is rare in pediatric populations. Congenital SA (CSA) is classified into syndromic and non-syndromic forms. Herein, we describe three cases of pediatric patients with SA. The diagnosis of SA was based on the presence of type 3 sideroblasts on BM aspirate smear (greater than 15%) and genetic tests. In the first cas

Rare association between hepatotoxicity and sideroblastic anaemia after taking Garcinia cambogia-based dietary supplement.

We report a rare case of lead poisoning after taking a dietary supplement which was bought online. A woman in her 60s presented with symptomatic anaemia and hepatitis for 2 weeks. Bone marrow examination showed sideroblastic anaemia and subsequent blood tests confirmed lead poisoning. She was successfully treated with a course of the chelating agent succimer. Further history and investigations showed that she had taken a Garcinia cambogia (GC)-based multi-ingredients dietary supplement in the form of capsules for weight reduction and that the lead content within the capsules was very high. Hyperferritinemia as a Clue to Neuroendocrine Carcinoma.

Ferritin is the protein that serves as the main mechanism for iron storage. It can also be an acute-phase reactant, which may rise in response to inflammation, infection, injury, autoimmune disease, or malignancy. Therefore, when evaluating ferritin levels, it is important to consider both iron storage and potential underlying conditions that could cause hyperferritinemia. We report a case of an elderly man in his late 80s who had elevated ferritin levels for years before ultimately being diagnosed with metastatic neuroendocrine carcinoma. The initial diagnosis was suspected to be due to iron

A Case of Myelodysplastic Syndrome-Induced Acquired Sideroblastic Anemia.

Hematological disorders are frequently encountered at the doctor's office and in the emergency room. Sideroblastic anemia, a rare hematological malady, is characterized by ring sideroblasts in the red blood cells due to accumulation of poorly transported and underutilized iron. Unlike primary sideroblastic anemia which can be conclusively diagnosed through genetic testing, the more common secondary form of the disease requires intricate cascading of several preliminary and confirmatory laboratory testing to determine the accurate etiology and provide the appropriate management regimen. The sco

A comprehensive analysis of transfusion trends and patient outcomes in blood utilization practices at Jimma university medical center.

In resource-limited settings like Ethiopia, ensuring adequate blood availability and optimizing its use remain a significant challenge. Comprehensive studies on blood utilization practices are essential for minimizing unnecessary transfusions and enhancing transfusion services, yet such research remains limited. This study aimed to systematically evaluate the efficiency of blood utilization, analyse transfusion trends, and assess patient outcomes. A prospective cross-sectional study was conducted at the largest referral hospital in Southwest Ethiopia, from June 1 to August 30, 2023. Data were

[Emerging perspectives on sideroblastic anemia].

Sideroblastic anemias (SAs) represent a diverse group of congenital and acquired disorders, characterized by anemia and the presence of ring sideroblasts in the bone marrow. Congenital sideroblastic anemia (CSA) arises from genetic mutations that disrupt heme and iron metabolism within mitochondria. The most common form of CSA is X-linked sideroblastic anemia (XLSA), caused by mutations in the erythroid-specific aminolevulinate synthase 2 (ALAS2) gene, a key enzyme in the heme biosynthesis pathway in erythroid cells. On the other hand, the most common form of acquired SA is myelodysplastic syn

Angioid streaks and optic disc drusen in a patient with congenital dyserythropoietic anaemia.

A man in his 60s with dry eye symptoms was noted to have angioid streaks under the peripapillary retina. Congenital dyserythropoietic anaemia was the major health issue throughout his life, requiring venesection for iron overload, but no transfusions for many years. Close inspection of the eyes with optical coherence tomography also detected small subclinical optic disc drusen, which have not been reported in association with congenital dyserythropoietic anaemia. Together, optic disc drusen and angioid streaks represent an ectopic calcification phenotype in the eye and can be seen in the more

Congenital-onset MLASA2 from a novel YARS2 variant: A literature review.

IntroductionMLASA (myopathy, lactic acidosis and sideroblastic anemia) is a rare, autosomal recessive mitochondrial disorder. Symptom onset typically occurs in childhood and differs considerably in disease severity. Congenital-onset disease is uncommon.MethodsWhole genome sequencing was performed which identified two heterozygous, rare YARS2 (NM_001040436.3) variants in trans, one of which was novel. The diagnosis was confirmed with muscle biopsy and mitochondrial enzyme activity testing. To compare the clinical phenotype of our patient to those previously described in the literature, we revie

Spectrum of Pure Red Cell Aplasia in a Tertiary Care Hospital in Northeast India.

Background Pure red cell aplasia (PRCA) is an uncommon disorder characterized by severe normocytic normochromic anemia, reticulocytopenia, and an absence of erythroblasts from otherwise normal bone marrow. PRCA can present as either a congenital disorder or an acquired syndrome. Methodology A total of six cases of PRCA diagnosed in a tertiary care hospital from January 2016 to December 2019 were retrospectively studied. The diagnosis of PRCA was made based on bone marrow examinations showing erythroblastopenia with essentially normal myelopoiesis and megakaryopoiesis. Results Of the six patien

Mutational heterogeneities in STAT3 and clonal hematopoiesis-related genes in acquired pure red cell aplasia.

Dysregulation of T cell-mediated immunity is considered a major pathophysiological mechanism in acquired pure red cell aplasia (PRCA), including idiopathic PRCA, large granular lymphocytic leukemia-associated PRCA, and thymoma-associated PRCA. Although STAT3 mutations are frequently detected in PRCA patients, the roles of other mutational profiles and their impact on clinical characteristics remain unclear. In this study, whole-exome sequencing and targeted sequencing using a custom-designed panel were performed on 53 PRCA patients. The most frequently mutated genes were STAT3 (36%), PCLO (9%)

Successful treatment by systemic corticosteroids and intravenous cyclophosphamide in acquired pure red cell aplasia associated with systemic lupus erythematosus.

Commonly, acquired pure red cell aplasia (PRCA) can be associated with an underlying autoimmune disease. Reports of PRCA associated with systemic lupus erythematosus (SLE) are rare, and no treatment strategies have been documented. We report a case of PRCA associated with SLE that responded to intravenous cyclophosphamide (IVCY) and systemic corticosteroids. A 62-year-old Japanese female was diagnosed with PRCA and SLE. At the time of the initial visit, the patient simultaneously presented with severe proteinuria, massive pleural effusion, and interstitial changes. Notably, cyclosporine and co

Comprehensive analysis of the clinical feature, myeloid neoplasm-related gene mutation profiles and T cell diversity acquired pure red cell aplasia.

PURPOSE: Acquired pure red cell aplasia (PRCA) is a kind of rare bone marrow failure disease characterized by destruction of erythrocyte by the immune system. However, the diverse etiologies of acquired PRCA make its pathogenesis largely unclear. MATERIALS AND METHODS: We portrayed the clinicopathologic, mutation and TCR rearrangement profiles of 64 primary PRCA cases and 104 large granular lymphocytic leukemia (LGLL)-associated PRCA, and tried to reveal the association factors of CsA response. RESULTS: We found that gene mutations were detected in 39.7% of acquired PRCA who were older than

[Acquired pure red cell aplasia].

Acquired pure red cell aplasia (PRCA), characterized by anemia, reticulocytopenia, and erythroid hypoplasia in the marrow, is a bone marrow failure syndrome that mainly affects elderly adults. Underlying T-cell dysregulations, often associated with clonality and/or STAT3 gene mutation, have been a rationale for using cyclosporin and other directed immunosuppressive therapies in most of the disease subtypes, namely, thymoma-associated PRCA, large granular lymphocytic leukemia-associated PRCA, and idiopathic PRCA. Although the epidemiologic rarity of PRCA has precluded comparative clinical trial
