A Case of Mitochondrial Myopathy, Lactic Acidosis and Sideroblastic Anemia (MLASA Syndrome) and Long QT Interval in a 10-Year-Old Saudi Child.

MLASA syndrome is a rare mitochondrial disorder that presents in three distinct genetic forms: MLASA1, MLASA2, and MLASA3; MLASA1 is the most common form. The clinical features include mitochondrial myopathy, lactic acidosis, and sideroblastic anemia. Although presence of other features is not uncommon, its association with long QT (LQT) syndrome has not been described before. In addition, while MLASA syndrome has been reported from several countries worldwide, we present the first patient with MLASA1 syndrome from the Kingdom of Saudi Arabia in this case report. The 10-year-old girl with hist

A novel frameshift deletion in SLC25A38 and its role in mitochondrial dysfunction: A case study of sideroblastic anemia in a child from Iran.

Sideroblastic anemia (SA) is a rare hematological condition characterized by the accumulation of iron in the mitochondria of erythroid precursor cells, resulting in the formation of sideroblastic rings. It occurs in both inherited and acquired forms. A prevalent subtype of congenital sideroblastic anemia (CSA) results from autosomal recessive mutations in the SLC25A38 gene. This investigation included a clinical and genetic evaluation of a family with a child diagnosed with pyridoxine-refractory SA. Whole Exome Sequencing (WES) was performed on the patient to detect the genetic variation. Afte

SLC25A38 gene modification mediated by CRISPR/Cas9 in HEK293T cell line.

Sideroblastic anemia is a rare disease and one of the heterogeneous bone marrow disorders. Gene therapy is considered as a method for treating these patients. The CRISPR/Cas9 system can recognize specific genomic DNA sequences and create Double-Strand Breaks (DSBs) in DNA. In this study, we attempted to investigate the effect of different sgRNAs in creating targeted cleavage in the desired region using the CRISPR/Cas9 system and inducing changes in the disease-related gene. Following the design of six sgRNAs, recombinant pX458 and pX459 plasmids harboring the respective sgRNA sequences were co

Case Report: A Previously Healthy Young Woman With Lethal, Unremitting Metabolic Acidosis: Could a Novel Variant in ALAS2 Be the Culprit?

BACKGROUND: Heme synthesis is critical for several biological processes, including mitochondrial energy production and oxygen delivery via hemoglobin. The initial and rate-limiting step in heme synthesis is the conjugation of glycine with succinyl-CoA to form 5-aminolevulinic acid (ALA), which is catalyzed by two closely related enzymes that are coded by highly homologous genes, known as ALAS1 and ALAS2. Loss-of-function variants in ALAS2 result in sideroblastic anemia, whereas gain-of-function variants result in porphyria. We present a case of a woman who died with severe metabolic acidosis a

Potential Vitamin B6 Depletion During Carbidopa/Levodopa Therapy in Parkinson's Disease: A Scoping Review of Direct Evidence.

Vitamin B6 depletion during carbidopa/levodopa therapy has been linked to polyneuropathy, seizures, and anemia, but the direct evidence is heterogeneous, and causality remains uncertain. We conducted a focused scoping review of direct empirical evidence on vitamin B6 biology during carbidopa/levodopa exposure and on vitamin B6-related clinical consequences supported by biochemical assessment. We also included studies of biomarker-guided monitoring, supplementation, and direct carbidopa-vitamin B6 chemistry. PubMed, Scopus, and Web of Science were searched without date or language limits on Jul

Harnessing Artificial Intelligence to Restore Standards in Medical and Dental Education in Pakistan.

The month of March is dedicated to Colon Cancer Awareness. Worldwide, colorectal cancer (CRC) incidence has been on the rise. It is currently the third most common cancer in men (746,000 cases, 10.0% of the total) and the second in women (614,000 cases, 9.2% of the total).1 Arecent meta-analysis reported a 61% risk reduction in CRC incidence with colonoscopy.2 Unlike screening programs for breast and prostate cancers, not only has CRC screening reduced mortality from colon cancer and detected early CRC, it has also decreased the incidence of CRC through detection and removal of pre-cancerous l

SLC25A38 is required for mitochondrial pyridoxal 5'-phosphate (PLP) accumulation.

Many essential proteins require pyridoxal 5'-phosphate, the active form of vitamin B6, as a cofactor for their activity. These include enzymes important for amino acid metabolism, one-carbon metabolism, polyamine synthesis, erythropoiesis, and neurotransmitter metabolism. A third of all mammalian pyridoxal 5'-phosphate-dependent enzymes are localized in the mitochondria; however, the molecular machinery involved in the regulation of mitochondrial pyridoxal 5'-phosphate levels in mammals remains unknown. In this study, we used a genome-wide CRISPR interference screen in erythroleukemia cells an

Case report: A novel 11-bp deletion in exon 11 causing a frameshift in the C-terminal of the ALAS2 gene leading to X-linked sideroblastic anemia-a family study.

X-linked sideroblastic anemia (XLSA) (MIM 300752) is the most common genetic form of sideroblastic anemia, a heterogeneous group of disorders characterized by iron deposits in the mitochondria of erythroid precursors. It is due to mutations of the erythroid-specific enzyme ALAS2, the first enzyme of the heme biosynthetic pathway. Herein, we report a novel 11-bp deletion in exon 11 leading to a frameshift in the C-terminal region of the ALAS2 gene with a non-functional longer polypeptide of 614 amino acids leading to a loss-of-function mutation manifested as an X-linked sideroblastic anemia phe

Sideroblastic anemia in children: challenges in diagnosis and management in three cases.

Sideroblastic anemias (SAs) represent a heterogeneous group of rare hematological disorders characterized by iron accumulation in mitochondria of erythroblasts with ineffective erythropoiesis. SAs are categorized into acquired and congenital forms. Acquired, secondary, and clonal, SA is rare in pediatric populations. Congenital SA (CSA) is classified into syndromic and non-syndromic forms. Herein, we describe three cases of pediatric patients with SA. The diagnosis of SA was based on the presence of type 3 sideroblasts on BM aspirate smear (greater than 15%) and genetic tests. Rare association between hepatotoxicity and sideroblastic anaemia after taking Garcinia cambogia-based dietary supplement.

We report a rare case of lead poisoning after taking a dietary supplement which was bought online. A woman in her 60s presented with symptomatic anaemia and hepatitis for 2 weeks. Bone marrow examination showed sideroblastic anaemia and subsequent blood tests confirmed lead poisoning. She was successfully treated with a course of the chelating agent succimer. Further history and investigations showed that she had taken a Garcinia cambogia (GC)-based multi-ingredients dietary supplement in the form of capsules for weight reduction and that the lead content within the capsules was very high. Hyperferritinemia as a Clue to Neuroendocrine Carcinoma.

Ferritin is the protein that serves as the main mechanism for iron storage. It can also be an acute-phase reactant, which may rise in response to inflammation, infection, injury, autoimmune disease, or malignancy. Therefore, when evaluating ferritin levels, it is important to consider both iron storage and potential underlying conditions that could cause hyperferritinemia. We report a case of an elderly man in his late 80s who had elevated ferritin levels for years before ultimately being diagnosed with metastatic neuroendocrine carcinoma. The initial diagnosis was suspected to be due to iron

A Case of Myelodysplastic Syndrome-Induced Acquired Sideroblastic Anemia.

Hematological disorders are frequently encountered at the doctor's office and in the emergency room. Sideroblastic anemia, a rare hematological malady, is characterized by ring sideroblasts in the red blood cells due to accumulation of poorly transported and underutilized iron. Unlike primary sideroblastic anemia which can be conclusively diagnosed through genetic testing, the more common secondary form of the disease requires intricate cascading of several preliminary and confirmatory laboratory testing to determine the accurate etiology and provide the appropriate management regimen. [Emerging perspectives on sideroblastic anemia].

Sideroblastic anemias (SAs) represent a diverse group of congenital and acquired disorders, characterized by anemia and the presence of ring sideroblasts in the bone marrow. Congenital sideroblastic anemia (CSA) arises from genetic mutations that disrupt heme and iron metabolism within mitochondria. The most common form of CSA is X-linked sideroblastic anemia (XLSA), caused by mutations in the erythroid-specific aminolevulinate synthase 2 (ALAS2) gene, a key enzyme in the heme biosynthesis pathway in erythroid cells. On the other hand, the most common form of acquired SA is myelodysplastic syn

Congenital-onset MLASA2 from a novel YARS2 variant: A literature review.

IntroductionMLASA (myopathy, lactic acidosis and sideroblastic anemia) is a rare, autosomal recessive mitochondrial disorder. Symptom onset typically occurs in childhood and differs considerably in disease severity. Congenital-onset disease is uncommon.MethodsWhole genome sequencing was performed which identified two heterozygous, rare YARS2 (NM_001040436.3) variants in trans, one of which was novel. The diagnosis was confirmed with muscle biopsy and mitochondrial enzyme activity testing. To compare the clinical phenotype of our patient to those previously described in the literature, we revie

Vitamin B6 Deficiency May Not Always Present As Microcytic Hypochromic Anemia.

Objective Vitamin B6 (VB6) deficiency leads to microcytic hypochromic anemia with ringed sideroblasts. Sideroblastic anemia due to VB6 deficiency is an important differential diagnosis to consider in cases with sideroblastic anemia associated with myelodysplastic syndrome (MDS). However, VB6 screening is underutilized in Japan as it is not covered by the national health insurance system, potentially leading to low screening rates. Methods This study retrospectively evaluated the relationships between VB6 (pyridoxal (PAL)) and other clinical parameters, including anemia, with univariate and mul

Prognostic impact of SF3B1 mutation and multilineage dysplasia in myelodysplastic syndromes with ring sideroblasts: a Mayo Clinic study of 170 informative cases.

The revised 4th edition of the World Health Organization (WHO4R) classification lists myelodysplastic syndromes with ring sideroblasts (MDS-RS) as a separate entity with single lineage (MDS-RS-SLD) or multilineage (MDS-RS-MLD) dysplasia. The more recent International Consensus Classification (ICC) distinguishes between MDS with SF3B1 mutation (MDS-SF3B1) and MDS-RS without SF3B1 mutation; the latter is instead included under the category of MDS not otherwise specified. The current study includes 170 Mayo Clinic patients with WHO4R-defined MDS-RS, including MDS-RS-SLD (N=83) and MDS-RSMLD (N=87
