Integrating Hereditary Hemorrhagic Telangiectasia into the Diagnostic Workup of Adolescent Heavy Menstrual Bleeding.

To understand the effect of hereditary hemorrhagic telangiectasis (HHT) on uterine bleeding is paramount to improving clinical care of menstruating individuals with HHT. Heavy menstrual bleeding (HMB) in adolescents and young adults (AYAs) with HHT is underrecognized and among individuals with HMB, HHT should be considered in the differential diagnosis, especially among those with suggestive clinical features or family history. Stigmatization associated with vaginal blood loss likely contributes to the paucity of evidence to guide HMB management in individuals with HHT, as previously reported

Small interfering RNA targeting protein Z-dependent protease inhibitor improves hemostasis in Von Willebrand factor- and Factor X-deficient mice.

Non-factor therapeutics have transformed the treatment paradigm in hemophilia A. Owing to their mode of action, these agents may also provide benefit for other rare bleeding disorders (RBDs) such as von Willebrand disease (VWD)-type 3 and Factor X (FX) deficiency. Targeting protein Z-dependent protease inhibitor (ZPI), a natural anticoagulant, is a novel strategy to enhance hemostasis. We evaluated the effects of ZPI post-transcriptional silencing using a GalNAc-conjugated siRNA (mTGM-148) in von Willebrand factor-deficient (VwfKO) mice and in mice with strongly reduced factor X levels (< 1%;

Population Frequencies of VWD-Associated Genotypes: Cross-Resource Evidence from RGC-ME and gnomAD.

Population-scale sequencing can quantify genotypes associated with von Willebrand disease (VWD), but these frequencies cannot be translated directly into clinically manifest disease without phenotype and penetrance data. We therefore estimated subtype-specific VWD-associated genotype frequencies in RGC-ME, compared them with gnomAD, and examined which analytical assumptions limit clinical interpretation. Germline VWF variants classified in ClinVar as pathogenic or likely pathogenic (P/LP) on 1 August 2026 were normalized to GRCh38 and matched exactly to both resources. Subtypes were assigned o

Suspected Seronegative Spondyloarthropathy in a Young Woman With Ulcerative Colitis and Concurrent Bleeding Symptoms.

Extraintestinal musculoskeletal manifestations, including peripheral and axial spondyloarthritis, are well recognized in ulcerative colitis (UC) and are frequently associated with human leukocyte antigen B27 (HLA-B27) positivity. We present a 20-year-old woman with UC who developed inflammatory-pattern polyarthralgia involving the knees, ankles, wrists, shoulders, and lower back, with morning stiffness improving with movement, alongside bruising, menorrhagia, and mild leukopenia. Examination showed bilateral lower extremity ecchymoses and mild joint tenderness without swelling or synovitis. La

Acute Hemorrhagic Manifestations in a Pregnant Patient With Type 1 von Willebrand Disease: Challenges in Management of Bleeding Disorders During Gestation.

This case reports a primigravida with Type 1 von Willebrand disease experiencing hematuria and gingival bleeding despite prophylaxis. Although pregnancy raises factor levels, her response was insufficient. Desmopressin successfully controlled mucosal hemorrhage, avoiding antifibrinolytics due to obstructive risk. Management requires multidisciplinary surveillance to tailor peripartum hemostatic interventions.

The Contribution of Bleeding Assessment Score and Pictorial Blood Loss Assessment Chart to the Diagnosis of Von Willebrand Disease in Adolescent Girls With Iron Deficiency Anemia.

BACKGROUND: Heavy menstrual bleeding (HMB) is common among adolescents and is an important driver of iron deficiency anemia. However, clinicians under-recognize inherited bleeding disorders. Iron deficiency transiently elevates von Willebrand factor (VWF) and factor VIII levels, both of which are acute-phase reactants, thereby masking quantitative VWF deficiency. METHODS: This prospective cross-sectional study enrolled iron-deficient adolescent girls at a single center. We used a structured bleeding history (ISTH-BAT), menstrual blood loss quantification (PBAC), and coagulation testing after

Developing an artificial intelligence-generated peptide targeting platelet-type von Willebrand disease.

Platelet-type von Willebrand disease (PT-VWD) refers to a rare bleeding disorder caused by gain-of-function mutations in platelet glycoprotein Ibα (GPIbα). These mutations lead to a hyperactive protein-protein interaction (PPI) with von Willebrand factor (VWF) and pathological platelet aggregation. Counterintuitively, patients with PT-VWD present with a bleeding diathesis as opposed to thrombosis. Despite well-defined genetic etiology, no targeted therapy exists for PT-VWD. Here, we sought to develop a peptide inhibitor that selectively targets the aberrant interaction in PT-VWD. Using the In

Modeling response to high-purity von Willebrand factor: toward tailored dosing during surgery in von Willebrand disease.

In surgical settings, managing von Willebrand disease (VWD) often requires von Willebrand factor (VWF) concentrate administration, and close monitoring of factor VIII (FVIII) and VWF to balance bleeding control with the risk of excessive factor elevation. This management is further complicated by VWD-related variability and the pharmacokinetic (PK) impact of VWF products, containing or not exogenous FVIII. We hypothesized that building a PK model based on high-purity VWF concentrates may clarify the VWF-FVIII relationship by eliminating exogenous FVIII interference. Our aim was to characterize

The interplay of von Willebrand disease and women's reproductive health.

Von Willebrand disease (VWD) is the most common inherited bleeding disorder and is often diagnosed in the setting of heavy menstrual bleeding. Because estrogen upregulates von Willebrand factor synthesis, pregnancy and management of menses may influence VWD testing and diagnosis. Throughout the life span, the management of women's reproductive health is frequently affected by VWD. We seek to describe the interplay between VWD and women's reproductive health at 3 distinct phases in life: menarche and menstruation, maternal health and fertility, and menopause and aging. Planning for care require

Allele-selective disruption of pathogenic VWF variants in type 2 von Willebrand disease using CRISPR/Cas9.

In contrast to major innovations in treating severe hemophilia, the treatment of severe von Willebrand disease (VWD) remains limited to intravenous infusion of von Willebrand factor (VWF) concentrates. To date, no gene therapy-based approaches for the treatment of VWD have been developed, largely owing to the disease's heterogeneous mutational landscape and the challenge of specifically targeting VWF production in endothelial cells. In this study, we developed a novel gene therapy strategy for patients with VWD caused by heterozygous dominant-negative VWF variants. Our strategy permanently ina

The landmark contribution by Erik von Willebrand.

One hundred years ago Professor Erik von Willebrand, working at the Deaconess Hospital in Helsinki, Finland, published his landmark paper on hereditary pseudohemophilia (the original paper, in Swedish, was entitled Hereditär pseudohemofili and was published in Finska Läkaresällskapets Handlingar). In 1924, a 5-year-old girl named Hjördis, living in Föglö in the Åland Island archipelago, was brought to Erik von Willebrand's attention; his investigations of a large family living in the Åland Islands emerged in the 1926 publication in which a new hereditary bleeding disorder, distinct from classi
