Exploring gut microbiome dysbiosis in vasculitis in adults: a systematic review.

BACKGROUND AND AIM: Although studies have confirmed that disruptions in the gut microbiota (dysbiosis) contribute to the development of autoimmune disorders, the precise pathways through which the gut microbiota influences the onset and progression of vasculitis remain largely unknown. In recent years, research has increasingly concentrated on assessing the contribution of the gut microbiota to vasculitis. This systematic study sought to assess the dominant gut microbiota profiles in vasculitis patients and compare them to those of healthy individuals. METHODS: The Cochrane methodology was us

Triple Renal Pathology in a Patient With Myeloperoxidase-Associated Vasculitis, Diabetic Nephropathy, and Immunoglobulin A Nephropathy: A Case Report.

Rapidly progressive glomerulonephritis (RPGN) is a nephrologic emergency often caused by antineutrophil cytoplasmic antibody (ANCA)-associated vasculitis. While superimposed diabetic nephropathy can alter the clinical baseline, the concurrent presentation of ANCA vasculitis, diabetic nephropathy, and immunoglobulin A (IgA) nephropathy is uncommon. We present a unique case of a triple-overlap renal pathology. A 56-year-old female patient with a history of type 2 diabetes mellitus presented with acute kidney injury (AKI) with a peak serum creatinine of 5.2 mg/dL, accompanied by macroscopic hemat

Fever and Painful Skin Lesions: When to Consider Sweet's Syndrome.

Sweet's syndrome is an acute febrile neutrophilic dermatosis characterised by fever, neutrophilia, and painful cutaneous lesions. We describe a case of a 31-year-old male admitted with a two-week history of fever up to 39°C and painful erythematous papules, scattered and coalescing into plaques over the trunk and limbs, together with pustules on an erythematous base over the palms and dorsum of the hands. No relevant epidemiological exposure or recent drug exposure was identified. Investigation showed an erythrocyte sedimentation rate of 40 mm/h, leucocytosis of 14,380/µL with 77% neutrophils

Cutaneous Adverse Reactions to IL-17 and IL-23 Inhibitors: A Clinical Review of Reaction Patterns.

INTRODUCTION: IL-17 and IL-23 inhibitors have transformed the management of moderate-to-severe psoriasis and other inflammatory diseases through targeted immune modulation. Although generally well tolerated, an expanding spectrum of cutaneous adverse reactions has been reported, ranging from common paradoxical eruptions to rare immune-mediated dermatoses that may mimic primary inflammatory or autoimmune conditions. OBJECTIVES: We present a narrative review of cutaneous adverse reactions associated with IL-17 and IL-23 inhibitors. This review aims to summarize reported reaction patterns, propo

Nephrotic range proteinuria and rapid progression to end-stage kidney disease in seronegative pauci-immune necrotizing glomerulonephritis masked by cutaneous lesions: a case-based review.

Antineutrophil cytoplasmic antibodies (ANCA)-negative pauci-immune necrotizing glomerulonephritis (PING) is a rare entity, representing a distinct subset of small-vessel vasculitis. The absence of ANCA does not exclude the diagnosis, and clinical suspicion should remain high in the presence of compatible systemic and renal findings. We present the case of a 22-year-old woman with recurrent, non-specific cutaneous manifestations since early childhood, who was hospitalized for an exacerbation of skin lesions. Laboratory results revealed nephrotic-range proteinuria of 17 g/d [N < 150 mg/d] and a

OBJECTIVES: Tuberculosis is a highly contagious disease that has a significant impact on global health. Emerging evidence suggests that tuberculosis can lead to an altered immune response. We investigated the association between tuberculosis and the onset of inflammatory arthritides (IA). METHODS: Patients with incident tuberculosis in the South Korean National Claims database from 2010 to 2021 were included, and those who had undergone appendectomy during 2010-2011 served as controls. Adult-Onset IgA Vasculitis With Concurrent Gastrointestinal and Possible Coronary Involvement.

IgA vasculitis, also known as Henoch-Schönlein purpura, is a small-vessel vasculitis that can affect multiple organ systems, most commonly the skin, GI tract, and kidneys. Although it is typically a condition involving the pediatric population, adult cases may follow a more severe, rapidly progressive course with more variable presentations. We present the case of a 50-year-old man with biopsy-proven IgA nephropathy with GI and possible coronary involvement. He initially presented with arthralgia and a purpuric rash, later developing rectal bleeding and eventually abdominal distention, nausea,

Repeat kidney biopsy reveals new-onset IgA nephropathy in a patient with ANCA-associated vasculitis.

Microscopic polyangiitis (MPA) is a subtype of antineutrophil cytoplasmic antibody (ANCA)-associated vasculitis (AAV). Although relapse during maintenance therapy is relatively common, the coexistence or subsequent development of other glomerular diseases, such as IgA nephropathy, is rare. We report the case of an 81-year-old woman diagnosed with renal-limited AAV nine years earlier. Initial presentation was characterized by elevated myeloperoxidase (MPO)-ANCA levels and crescentic glomerulonephritis. Remission was achieved with prednisolone, and her serum creatinine level remained stable at a

Prolonged skin involvement distinguishes adolescent from childhood-onset IgA vasculitis: a large multicenter study with 687 patients.

BACKGROUND: Immunoglobulin A vasculitis (IgAV), also known as Henoch- Schönlein purpura (HSP), is the most common systemic vasculitis in childhood. Potential differences in demographic characteristics, clinical presentation, laboratory findings, and treatments approaches across age groups, remain poorly explored. To the best of our knowledge, no multicenter study in Latin America has systematically addressed these features. We aimed to assess demographic, clinical and laboratory features, and treatments in children versus adolescents with (IgAV)/ (HSP) in a large multicenter study. METHODS: A

Tiny warriors, big impact: targeted immunotherapy in kidney diseases.

Recent advances in immunotherapy targeting B cells have ushered in a new era for disease treatment. The effectiveness of targeted approaches such as T-cell engagers or B cell-directed chimeric antigen receptor (CAR)-T-cell therapy in treating autoimmune diseases has been demonstrated so far only in uncontrolled, investigator-initiated trials. Several case series in patients with therapy-resistant systemic lupus erythematosus showed efficacy and safety of CD19+ CAR-T cells, showing depletion of autoreactive B cells and full clinical remission. Autoreactive B cells not only mediate pathogenesis

Surgical Pathology of Diffuse Parenchymal Lung Disease in Adults With IgA Vasculitis or IgA Nephropathy.

IgA vasculitis (IgAV) and IgA nephropathy (IgAN) are closely related disorders that usually present in childhood. Pulmonary involvement is rare and histopathologic features thereof in adults are poorly understood. Institutional archives were searched for adults with IgAV or IgAN and diffuse parenchymal lung disease. Ten patients (6 men, median age 59 years) with lung tissue sampling were enrolled. Clinical history and pathology slides were reviewed. All patients were diagnosed with IgAV or IgAN before or concurrently with lung disease. Symptoms were nonspecific but hemoptysis was common. An Atypical Pediatric Dermatologic Presentation Unmasking an Unexpected Systemic Diagnosis: A Diagnostic Challenge.

Dermatologic and musculoskeletal presentations in pediatric patients can pose significant diagnostic challenges, particularly when findings are atypical, persistent, and resistant to therapy. We present a diagnostically complex case of a 13-year-old male with a history of multiple psychiatric comorbidities who developed progressive bilateral pedal edema, bullous lower extremity lesions with excoriations, and severe pain, all symptoms seemingly refractory to antibiotics. Despite initial concern for skin and soft-tissue infection, symptoms persisted, prompting inpatient evaluation and an expande

The UK Eastern Network for Kidney Inflammatory Disease (ENKID) MDT at 24 months: advancing access to high-cost drugs, clinical trials, and complex case management in renal autoimmune diseases.

BACKGROUND: Glomerulonephritis (GN) accounts for 20%–25% of the causes of chronic and end-stage kidney disease. The evolving treatment landscape with new targeted immunosuppressants has highlighted the unmet need for equity of access to specialist GN services. Regional networks and multidisciplinary team meetings (MDTs) provide access to clinical expertise, high-cost drugs (HCDs) and clinical trials, facilitating collaborative decision-making and optimal use of therapeutics to improve patient outcomes. Launched in May 2023 in the East of England (EoE), the Eastern Network Kidney Inflammatory D

Gastrointestinal Involvement in Adult IgA Vasculitis: A Comprehensive Review.

Immunoglobulin A vasculitis (IgAV), formerly Henoch-Schönlein purpura (HSP), is a small-vessel vasculitis with IgA (immunoglobulin A) immune complex deposition that predominantly affects the skin, joints, kidneys, and gastrointestinal (GI) tract. While less common in adults than in children, IgAV (immunoglobulin A vasculitis) is more severe and less well-characterized in adults. GI involvement in adults is associated with significant morbidity, motivating a comprehensive understanding of IgAV pathophysiology, diagnosis, and management. We synthesized current literature on adult IgAV with GI in

S100 proteins in IgA vasculitis and other systemic vasculitides - from pathogenic mechanisms to clinical biomarkers: a systematic review.

BACKGROUND: IgA vasculitis (IgAV) is a small-vessel vasculitis characterized by immune complex deposition, neutrophil activation, and endothelial injury. S100 proteins are recognized mediators of inflammation and vascular damage; however, their specific biological and clinical relevance in IgAV remains incompletely understood. OBJECTIVES: This systematic review aimed to synthesize current evidence on the mechanistic and clinical roles of S100 proteins in vasculitides, focusing on IgAV, and to evaluate their potential utility as biomarkers of disease activity, organ involvement, and prognosis.

Landscape of kidney biopsy diagnoses in a Romanian pediatric cohort.

BACKGROUND: There is relatively little information available on the landscape of histological findings in kidney biopsies of pediatric populations, particularly in Eastern Europe. METHODS: Records of pediatric native kidney biopsies processed between 1996 and 2023 were extracted from the registry of the Victor Babeş National Institute of Pathology, Bucharest. Demographic characteristics, biopsy indications and histopathological findings, as well as temporal trends thereof, were analyzed and compared to those reported in other European studies. An Atypical Presentation of Pediatric IgA Vasculitis: Early Hypertension and Severe Systemic Manifestations Despite Initially Normal Urinalysis.

Immunoglobulin A vasculitis (IgAV), formerly Henoch-Schönlein purpura, is the most common systemic vasculitis of childhood and is typically self-limited. Renal risk stratification frequently relies on the presence of hematuria or proteinuria; however, early disease severity may not be detected by urinalysis alone. We present a previously healthy seven-year-old boy with palpable purpura and joint pain. The diagnosis was made clinically based on the presence of nonthrombocytopenic palpable purpura in the setting of arthralgia. He was diagnosed with IgAV after initially reassuring laboratory find
