Evidence-based guidelines for the diagnosis and management of adult-onset IgA vasculitis.

IgA vasculitis (IgAV) is an immune complex-mediated small-vessel vasculitis that typically affects the skin, gastrointestinal tract, kidneys and joints. Childhood-onset IgAV is a common disease and usually follows a self-limiting course, whereas adult-onset IgAV is considerably less frequent and is associated with a poorer prognosis. The diagnosis, assessment and management of adult-onset IgAV remain challenging owing to the absence of validated diagnostic criteria for adults and lack of IgAV-specific standardized disease activity scores. Short-term outcomes are mainly determined by gastrointe

Leukocytoclastic Vasculitis Presenting With Peripheral Neuropathy and Foot Drop in an Adolescent: A Case Report.

Leukocytoclastic vasculitis (LCV) is a small-vessel vasculitis that typically presents with palpable purpura on the lower extremities and is often confined to the skin. Peripheral nerve involvement is uncommon. We describe the case of a 17-year-old female who developed progressive burning paresthesias in all four limbs, asymmetric sensory loss, autonomic symptoms, and left foot drop preceding the appearance of palpable purpura and necrotic lesions on both lower limbs. Nerve conduction studies demonstrated mixed demyelinating and axonal motor neuropathy in both lower limbs, more pronounced on t

Plasma von Willebrand factor and endothelial-lineage flow-cytometry signals in acute pediatric IgA vasculitis: a cross-sectional pilot study.

INTRODUCTION: Endothelial activation is implicated in childhood IgA vasculitis, but candidate endothelial markers may reflect systemic inflammation rather than endothelial perturbation. METHODS: In this single-centre cross-sectional pilot study, we enrolled 13 children with acute IgA vasculitis and 20 age- and sex-comparable healthy controls screened to exclude acute infection, allergic disease and other inflammatory conditions. Plasma soluble fms-like tyrosine kinase 1, soluble vascular endothelial cadherin, von Willebrand factor (vWF) and vascular endothelial growth factor were measured by

Atypical Vasculitis Phenotype with Concomitant Medium- and Small-Vessel Involvement: A Case Report Highlighting Diagnostic and Pathological Challenges.

Systemic vasculitis is traditionally classified according to the predominant size of affected vessels; however, phenotypes affecting multiple vascular calibers may not be adequately captured by rigid definitions and require integrated clinicopathological interpretation. Here we report the case of a 70-year-old Caucasian man who presented with persistent systemic inflammation initially suggestive of infection. Although inflammatory markers decreased during antibiotic therapy, residual inflammation persisted, and the subsequent clinical course was characterized by myalgia, pulmonary involvement,

Adverse events following SARS-CoV-2 mRNA vaccination in norwegian adolescents.

The Norwegian COVID-19 vaccination campaign of adolescents from April 2021 necessitated surveillance of potential adverse events following immunization (AEFI). In this nationwide study of 496,432 adolescents, AEFI incidence rate ratios (IRRs) after first- and second-dose SARS-CoV-2 mRNA vaccination were compared to IRRs in unvaccinated subjects, and a self-controlled case series analysis was done as a secondary analysis. Seventeen pre-selected potential AEFIs were investigated: Anaphylactic reaction, Acute appendicitis, Lymphadenopathy, Arrhythmia, Cerebrovascular events, Death, Encephalomyeli

Breast Cancer Screening Patterns Following National Crises: Experience from a Lebanese Tertiary Center.

BACKGROUND: Lebanon's recent compounded crises-including economic collapse, the COVID-19 pandemic, and the Beirut Port explosion-have disrupted healthcare access, including breast cancer (BC) screening. This study assesses adherence to BC screening after the onset of these crises and evaluates associated imaging outcomes. METHODS: This retrospective, single-center study included women aged ≥40 who underwent screening mammography (± ultrasound) at Lebanese Hospital Geitaoui (LHG) between October 2018 and October 2019, during the year that preceded the crisis. Exclusions included known BC, symp

Immunoglobulin A (IgA)-Mediated Vasculitis as a Paraneoplastic Clue in Chronic Lymphocytic Leukemia: A Case Report.

Immunoglobulin A (IgA)-mediated vasculitis is a small-vessel vasculitis defined by IgA-dominant immune complex deposition. Although common in children, adult-onset cases are rare and may signal underlying malignancies. In this case report, we present a 60-year-old male with chronic lymphocytic leukemia (CLL) under observation for his painful purpura and non-healing leg ulcers, with a biopsy confirming IgA vasculitis. His course was further complicated by steroid-induced diabetes, poor wound healing, recurrent infections, and fatal respiratory failure. He remained steroid-dependent with recurre

Cutaneous manifestations of scrub typhus: a comprehensive review of diagnostic clues in a neglected tropical disease.

Scrub typhus, caused by Orientia tsutsugamushi, is a re-emerging zoonotic infection with significant diagnostic reliance on its cutaneous manifestations. The disease, transmitted by larval trombiculid mites, is endemic to the 'Tsutsugamushi Triangle' and extends to other regions globally. Dermatological features are pivotal in early recognition, with eschar formation being pathognomonic. Eschars, resembling 'cigarette burns', develop as necrotic lesions surrounded by erythema and often resolve with minimal scarring. Maculopapular rashes, although variably reported, predominantly affect the tru

Infective Endocarditis-Associated Glomerulonephritis Caused by Streptococcus cristatus: A Case Report.

INTRODUCTION: Infective endocarditis (IE) can trigger systemic embolic and immune-mediated complications, including infection-related glomerulonephritis. When the infectious focus is IE, the condition is termed IE-associated glomerulonephritis (IEAGN). A major diagnostic pitfall is that IEAGN may present with purpura, nephritic urinary abnormalities, systemic inflammation, and antineutrophil cytoplasmic antibody (ANCA) positivity, closely mimicking primary small-vessel vasculitis and risking inappropriate immunosuppression if infection is not promptly excluded. CASE PRESENTATION: A 52-year-ol

IgA Nephropathy in Adults: A Review.

IMPORTANCE: IgA nephropathy (IgAN) is a chronic kidney disease involving deposition of IgA-containing immune complexes in the glomerulus, causing glomerular inflammation and scarring. It is the most common immune-mediated glomerular disease worldwide, and affects an estimated 198 887 to 208 184 persons in the US. Up to 50% of patients with IgAN develop kidney failure within 10 years of diagnosis. OBSERVATIONS: IgAN typically presents with nephritic syndrome and usually occurs in younger adults, with a mean age at diagnosis of 34 to 45 years. Incidence is highest in East Asia. Approximately 60

Cutaneous Pediatric Vasculitis: A Clinico-Histopathological Overview of Common and Novel Entities.

Vasculitis encompasses a heterogeneous group of diseases characterized by the inflammation of blood vessels. In children, the diagnosis of vasculitis with cutaneous involvement relies on a combination of clinical evaluation, histopathological examination, and, in some cases, genetic investigations. Diagnosing pediatric vasculitis remains particularly challenging due to overlapping clinical manifestations, variable disease courses, and the evolving nature of histological features. Histopathological assessment aims to confirm inflammation of the vessel walls-either readily visible at low magnifi

Harnessing Artificial Intelligence to Restore Standards in Medical and Dental Education in Pakistan.

The month of March is dedicated to Colon Cancer Awareness. Worldwide, colorectal cancer (CRC) incidence has been on the rise. It is currently the third most common cancer in men (746,000 cases, 10.0% of the total) and the second in women (614,000 cases, 9.2% of the total).1 Arecent meta-analysis reported a 61% risk reduction in CRC incidence with colonoscopy.2 Unlike screening programs for breast and prostate cancers, not only has CRC screening reduced mortality from colon cancer and detected early CRC, it has also decreased the incidence of CRC through detection and removal of pre-cancerous l

IgA vasculitis nephritis-outcomes in adult-onset disease.

OBJECTIVES: IgA vasculitis (IgAV) in adults has been relatively under-investigated. Since outcomes are worse in other forms of vasculitis with increasing age, we investigated the outcomes of IgAV comparing younger adults (18-34), middle-aged adults (35-64) and elderly patients (≥64 years) focusing on kidney outcomes. METHODS: We identified patients with renal biopsy-confirmed IgAV nephritis and collected data regarding clinical features and progression to end stage kidney disease (ESKD). The relationship between patient factors and ESKD was analysed by regression. RESULTS: We identified 202

The Impact of Infectious Diseases on Clinical Characteristics and Immunological Correlations in Pediatric Henoch-Schönlein Purpura: A Five-Year Retrospective Study.

Background/Objectives: Immunoglobulin A (IgA) vasculitis (IgAV), classically known as Henoch-Schönlein purpura (HSP), is a type of nonthrombocytopenic small-vessel vasculitis. HSP is the most frequent kind of systemic vasculitis in children, characterized by purpura, arthritis or arthralgia, gastrointestinal pain, and kidney dysfunction. The aim of our research was to investigate and observe the clinical characteristics of children diagnosed with HSP and to explore the correlation between infectious diseases and HSP. Furthermore, this retrospective study considered other factors, such as demog

IgA Vasculitis Presenting as Pulmonary-Renal Syndrome.

Immunoglobulin A (IgA) vasculitis (IgAV), formerly known as Henoch-Schönlein purpura, is a small-vessel vasculitis characterized by the deposition of IgA-containing immune complexes in vessel walls. While predominantly affecting the pediatric population, adult-onset IgAV often presents with a more severe clinical course and a higher risk of renal complications. Pulmonary-renal syndrome, characterized by diffuse alveolar hemorrhage and glomerulonephritis, is an exceedingly rare manifestation of IgAV and poses substantial diagnostic and therapeutic challenges. We report a case of a 33-year-old m

Haptoglobin as a novel predictor of visceral involvement and relapse in adult IgAV patients.

INTRODUCTION: IgA vasculitis (IgAV) can present as skin-limited or systemic disease, which can be severe in adults. Predictive markers for visceral involvement are suboptimal. Considering haptoglobin's role as an acute phase reactant, we evaluated whether its differential expression in IgAV patients' skin and leukocytes is also reflected systemically in a larger cohort of adult IgAV patients. Additionally, soluble form of haptoglobin scavenger receptor CD163 was measured in IgAV patient serum. METHODS: We re-analyzed RNA sequencing data from leukocytes and skin biopsies of treatment-naïve adu

Successful outcome of a refractory IgA vasculitis nephritis in children treated with telitacicept.

IgA vasculitis (IgAV) is the most prevalent form of vasculitis in children. While most cases of IgAV present with mild clinical symptoms and generally have a favorable prognosis, some children with IgAV nephritis may experience persistent heavy proteinuria, which is at risk of progressing to end-stage renal disease. Despite the administration of various immunosuppressive agents, treatment outcomes for these children are often suboptimal. We report the case of an 8-year-and-four-month-old girl who initially presented with rashes on both lower limbs for a duration of four days and abdominal pain

Highlights from the breakout session: other forms of vasculitis.

The session on other forms of vasculitis included a masterful review on IgG4-related disease (IgG4-RD) following which two research studies evaluating therapeutic agents and one study on histopathologic findings of IgG4-RD were presented. Peyronel F., et al. discussed the results of a prospective multicentre randomized clinical trial evaluating methotrexate for patients with retroperitoneal fibrosis. Methotrexate, in combination with a tapering regimen of glucocorticoids, was effective in inducing remission and allows use of lower cumulative prednisone doses. Hobbelink J., et al. evaluated the

An Atypical Presentation of Henoch-Schönlein Purpura With Features of Acute Hemorrhagic Edema of Infancy and Koebnerization: A Case Report.

Immunoglobulin A vasculitis, also known as Henoch-Schönlein purpura (HSP), is a type of vasculitis that is most commonly found in children. Palpable purpura is a key feature and is often associated with joint, abdominal, or renal involvement. This case report presents an atypical presentation of HSP in a four-year-old male, characterized by overlapping features of Acute Hemorrhagic Edema of Infancy (AHEI) and Koebnerization (isomorphic response), which complicates the diagnosis. The patient initially presented with redness around his mouth and nose, followed by hematuria and the development of

Origin of poorly galactosylated IgA1 other than mucosa: a viewpoint from a report on patient with IgA vasculitis.

Patients presenting monoclonal gammopathy of renal significance (MGRS) and IgA vasculitis collectively have rarely been reported. This study reports one patient with monoclonal IgA and λ, and with IgA positive-mesangial proliferative glomerulonephritis. The patient manifested slight chronic nephritic syndrome, and his serums tested positive for M protein (monoclonal IgA and λ). As a result of bone wear, the plasma cell ratio of these patients was confirmed to be mildly increased in peripheral blood smears. Just like typical IgA nephropathy or IgA vasculitis patients, serum poorly galactosylate
