We have examined the secretion of IgA, IgM, and IgG by isolated human intestinal MNC, human bone marrow MNC from rib specimens, and peripheral blood MNC from patients with CD, UC, SLE, and HSP. "Normal" control intestinal MNC exhibited high spontaneous secretion of IgA, whereas intestinal MNC from UC and CD patients exhibited only modest increases in IgA secretion. Peripheral blood MNC from patients with CD, UC, SLE, and HSP exhibited markedly elevated spontaneous secretion of immunoglobulins in general and IgA in particular. Pure human bone marrow MNC exhibited high spontaneous secretion of I

We studied the distribution of autologous rosette forming cells (ARFC) in the peripheral blood of 30 healthy adult donors, 30 patients with IgA nephropathy, 20 patients with primary glomerular diseases, eight patients with systemic diseases and 25 patients with other renal diseases. The mean percentages of ARFC were markedly reduced in the IgA nephropathy patients compared with the healthy adult donors. The values for ARFC were even more significantly reduced in IgA nephropathy patients compared with patients with primary glomerular diseases, systemic diseases and other renal diseases. This me

Six cases of intramural duodenal hematoma in children less than seven years old are presented. A review of the literature found 143 cases in patients under 15 years. The etiology is traumatic in most cases, although they have also been described in patients with Schönlein-Henoch purpura or coagulation diseases. Contrast X-rays are fundamental elements for diagnosis. The "spring" sign is very valuable. Patients were conservatively treated, although three of them where operated, one for a hepatic rupture and the two other for presumed appendicitis and intussusception respectively. In none was th

Two cases of Henoch-Schönlein purpura in adults are reported. Both patients presented with severe digestive symptoms suggesting malabsorption or exudative enteropathy. Roentgenological examinations showed marked anomalies of the small intestine as well as changes suggestive in intra-mural hematoma located in the stomach in one case and in the duodenum in the other. Hypoalbuminemia occurred in both patients during the course of the disease and was highly suggestive of protein-loosing enteropathy. Disseminated intravascular coagulation was seen in one patient. Both patients recovered from their

A new conglutinin solid phase assay for the detection of immune complexes containing IgA (IgAIC) and other conglutinin tests for immune complexes containing IgG and IgM (IgGIC and IgMIC) were used in studies on 34 patients affected by Berger's GN (92 sera) and 12 affected by Henoch-Schoenlein GN (61 sera). Thirty-six patients were observed over follow-up periods of 2-43 months. Levels of IgAIC in both groups of patients were significantly higher than those in healthy people. The values obtained in patients with Henoch-Schoenlein GN were statistically higher than those obtained in patients with

Hairy cell leukemia is a rare disease, probably of B-lymphocyte origin, that has been reported to rarely occur with polyarteritis nodosa. The records of 31 patients with hairy cell leukemia were reviewed for an association with other disorders of the immune system; such an association was found more often than previously suspected. Four cases are discussed--one with hepatitis B surface antigen-positive polyarteritis nodosa and another with an IgA (kappa) monoclonal gammopathy and amyloidosis of the kidney, liver, and small bowel. The other two patients had cutaneous vasculitis, one of which wa

This study concerns 11 patients with immune complex associated cutaneous vasculitis (5 leukocytoclastic vasculitis or Gougerot Ruiter's disease, 3 essential mixed cryoglobulinaemia, 2 Henoch-Schönlein purpura and 1 Waldenström's hypergammaglobulinaemic purpura). By determining the clearance of 51Cr-labeled IgG sensitized erythrocytes we showed a slight modification in the splenic mononuclear phagocyte system. In patients with Gougerot-Ruiter's disease the clearance of the autologous IgG-coated erythrocytes was delayed in 1 patient, and normal in 2 patients. In contact, the 8 other patients sho

Different rates of renal involvement in the Henoch-Schönlein syndrome have been reported in the literature. We assessed renal involvement during the initial attack of this disease in 71 children in northern Israel, and reevaluated 69 of them after an interval of one to eight years. The mean age at onset was higher than in other reports, and there was a clustering of cases in the late summer and early autumn. During the acute phase of the illness, involvement of the kidney was apparent in only seven patients (9.8%), and only one of these (1.4% of those followed up) showed evidence of continuing

Two adult patients with Henoch-Schoenlein purpura and rapidly progressive glomerulonephritis were treated with plasmapheresis. One patient also received cyclophosphamide. Both patients recovered their renal function. Before plasmapheresis circulating IgA immune complexes were demonstrated in both patients by two assays with specificity for IgA. The level of IgA immune complexes decreased after each plasma exchange. IgA immune complexes disappeared in the patient who was treated with cyclophosphamide but remained present in the other patient. Plasma exchange may be a useful form of therapy for

Henoch-Schönlein's purpura is a systemic vasculitis involving mainly skin, joints, intestine and kidney. We report on 2 patients who presented with clinical signs and symptoms suggestive of testicular torsion. In 1 case purpuric areas after leg cast removal indicated a diagnosis of Henoch-Schölein's purpura. Surgical exploration of the other case revealed congestion of the hydatid of Morgagni, which was followed by typical systemic manifestations of Henoch-Schönlein's purpura. These and another case indicate that orchitis accompanying Henoch-Schönlein's purpura may mimic testicular torsion.

A microplate radioimmunoassay for IgG, IgA and IgM antiglobulins reactive with rabbit IgG was performed on sera from 69 patients with various connective tissue diseases. IgM antiglobulins were detected in rheumatoid arthritis, systemic sicca syndrome and some patients with systemic vasculitis. IgA antiglobulins were found in seven of 10 patients with systemic sicca syndrome and only five of 59 patients with other connective tissue diseases. There was no correlation between the levels of IgM and IgA antiglobulins in the systemic sicca syndrome or rheumatoid arthritis (r = -0.21 and 0.2 respecti

Neuromuscular manifestations of children's collagenosis are revised. The incidence varies in different entities, from exceptional to constitute the main clinical characteristic of the disease. They are owed to inespecific inflammatory infiltration, vasculitis and following ischemy-infarction lesions, compressive or vicinity phenomenons, iatrogenic factors or to other pathology from the basal disease, mainly nephropathy. Muscular pathology is manifested as weakness, pain, edema atrophy, retractions and possible calcification. The neurological pathology, less important according to our experienc

Simultaneous demonstration of circulating and tissue-fixed immune complexes was attempted in 22 patients with cutaneous necrotizing vasculitis (7 anaphylactoid purpura, 9 cutaneous allergic vasculitis, 2 livedo reticularis, 1 thrombophlebitis, 2 erythema elevatum diutinum and 1 acute generalized pustular bacterid). In 16 out of the 22 patients, particularly patients with anaphylactoid purpura and cutaneous necrotizing vasculitis, there was a high Clq-binding activity. Decreased levels of C3 and C4 were seen in 2 and 3 patients, respectively. In 11 out of 16 skin lesions, the granular deposits

In glomerulonephritis accompanying the Schönlein-Henoch syndrome (SHS) a characteristic subepithelial basement membrane change is present in 85% of cases. The subepithelial change is a reaction to subepithelial deposits and consists of a garland or dome-like new formation of thin densa lamellae. This change is much more frequent in SHS than in IgA-nephritis or idiopathic glomerulonephritis or any other systemic disease. Furthermore, subepithelial deposits (50% of cases) are nearly as frequent as subendothelial deposits (65%) and more often present than formerly assumed.

The sera of patients with dermatitis herpetiformis, Henoch-Schönlein purpura and systemic lupus erythematosus were examined for IgA-containing immune complexes using a newly described radioimmunoassay. The IgG Raji cell radioimmunoassay and the 125I-C1q binding assay were also used to detect IgG- and IgM- containing soluble immune complexes. IgA-containing immune complexes were found in the sera of twelve of forty-nine (24%) patients with dermatitis herpetiformis, four of six (67%) patients with Henoch-Schönlein purpura, and seven of ten (70%) patients with systemic lupus erythematosus. We described three cases of eosinophilic pneumonia of unknown aetiology investigated clinically and by lung biopsy. The illnesses lasted between six and 20 weeks and consisted of cough, dyspnoea, malaise, and in two cases prolonged pyrexia. All had blood eosinophilia and chest radiographs showing widespread bilateral shadowing; in two cases this had a characteristic peripheral distribution. One patient recovered spontaneously and the other two responded to steroids, with disappearance of pyrexia within 12 hours and radiological clearing within 14 days. Lung function tests during the acute illn

In Henoch-Schönlein purpura immune complexes in inflamed vessel walls characteristically contain immunoglobulin A(IgA). To determine whether IgA is also the predominant immunoglobulin in circulating immune complexes, we compared the results of three immune complex assays with specificities for different classes of immunoglobulins in a longitudinal study of 37 patients (30 children and seven adults) with Henoch-Schönlein purpura. Circulating IgA-containing immune complexes were detected by their reactivity with a low avidity anti-IgA antibody in 27 of the 37 patients. IgA was simultaneously pre

Immunologically mediated renal diseases include Goodpasture's syndrome, systemic lupus erythematosus, membranoproliferative glomerulonephritis, membranous nephropathy, poststreptococcal glomerulonephritis, IgA-IgG nephropathy and anaphylactoid purpura. In Goodpasture's syndrome, antibodies are directed against the glomerular capillary basement membrane. The other disorders appear to result from circulating immune complexes which deposit in glomerular capillary walls or in the glomerular mesangium. Treatment with steroids and/or immunosuppressives is effective in lupus nephritis; in other disea
