Vision and Quality of Life in Fuchs' Endothelial Dystrophy Using a Prototype Aberrometer: A Cross-Sectional Study.

BACKGROUND: Fuchs' endothelial corneal dystrophy (FECD) is a progressive disease that affects vision and quality of life (QoL). While best distance-corrected visual acuity (BDCVA) is commonly used to assess visual function, the role of optical aberrations and high-resolution imaging in explaining patient-reported outcomes remains unclear. This study aims to evaluate how clinical, optical, and QoL parameters interrelate in patients with FECD. METHODS: Prospective, cross-sectional study. Patients underwent ophthalmic evaluation including best distance-corrected BDCVA, refraction, central cornea

IgA Vasculitis (Henoch-Schönlein Purpura) in a Seven-Year-Old Female Child With Recurrent Respiratory Infections.

A seven-year-old female child presented with a purpuric rash, joint pain, and a recent history of streptococcal pharyngitis. Initial symptoms included fever, throat pain, and vomiting, for which she received amoxicillin. Following treatment, the patient developed a rash and joint pain, prompting further evaluation. The physical examination revealed erythematous, edematous, and purpuric lesions on the extremities, along with periarticular swelling in the knees and ankles. Laboratory investigations showed hematuria, raising concerns for differential diagnoses, including IgA vasculitis, acute urt

Immunofluorescence staining profiles of glomerular diseases: a single-center retrospective study.

Immunofluorescence (IF) staining is essential for diagnosing glomerular diseases, yet comprehensive characterization of immune deposition patterns remains limited. We retrospectively analyzed 10,489 consecutive native kidney biopsies (2011-2022) from a tertiary center. IF staining for IgA, IgG, IgM, C1q, C3, C4d, and fibronectin was semi-quantitatively graded (0-4+) and deposits classified as mesangial area only (MAO), glomerular capillary loop only (GCLO), or combined mesangial-capillary loop (MA-GCL). IgA nephropathy (IgAN) was the most frequent diagnosis (38.4 %), followed by membranous nep

Case Report: Maximizing the anti-proteinuric response: a multicenter real-world sparsentan case series in IgA disorders.

Immunoglobulin A Nephropathy (IgAN) is a prevalent form of glomerulonephritis that leads to chronic kidney disease (CKD), typically marked by ongoing proteinuria, even when treated with standard therapies such as renin-angiotensin-aldosterone system (RAAS) blockers and occasionally immunosuppression. Proteinuria is a modifiable risk factor crucial for disease advancement. Sparsentan, a dual endothelin receptor and angiotensin receptor blocker (DEARA), has been introduced as a novel therapeutic option focusing on proteinuria. We present a case series featuring seven patients - five diagnosed wi

Henoch-Schönlein purpura induced by sitagliptin: A case report.

Dipeptidyl peptidase-4 inhibitors are oral antihyperglycemic medications often used as adjuncts to other antidiabetic agents to treat type 2 diabetes mellitus. Henoch-Schönlein purpura, also known as ImmunoglobulinA (IgA) vasculitis, is a form of IgA-mediated leukocytoclastic vasculitis, which is rarely reported in adults. Herein, we report the case of a 52-year-old male with type 2 diabetes mellitus who presented with petechiae, ecchymoses, and palpable purpura below the knees bilaterally after starting sitagliptin, a dipeptidyl peptidase-4 inhibitor. A punch biopsy for direct immunofluoresce

Acute Scrotum: A Rare Manifestation of Henoch-Schönlein Purpura.

Henoch-Schönlein purpura (HSP) is a systemic vasculitis commonly seen in children. The disease is characterised by a tetrad of clinical manifestations, which includes palpable purpura, arthritis/arthralgia, abdominal pain, and kidney involvement. Henoch-Schönlein purpura scrotal involvement is rare. They can mimic an acute scrotum and require prompt diagnosis and treatment to avert significant complications. We present the case of a six-year-old male child with HSP presenting with an acute scrotum. This case report aims to highlight the diagnostic challenges and treatment approaches for HSP-as

Clinical review of adult Henoch-Schönlein purpura and analysis of predictors related to gastrointestinal involvement.

BACKGROUND: Henoch-Schönlein purpura (HSP) is a small-vessel vasculitis characterized by non-thrombocytopenic purpura, often involving the gastrointestinal tract, joints, kidneys, and other organs. While most studies on HSP focus on children, severe manifestations and poor outcomes are more frequently observed in adults. OBJECTIVE: This retrospective study aimed to evaluate predictors associated with gastrointestinal (GI) involvement in adult patients. METHODS: We included 206 adult patients with HSP, categorized into subgroups based on the presence of gastrointestinal involvement. RESULTS:

IgA Vasculitis With Nephritis Following Controlled Ovarian Stimulation and Oocyte Donation.

A woman in her 20s developed IgA vasculitis with nephritis (IgAVN), which started four days after oocyte donation (for in-vitro fertilisation (IVF) and transfer to another recipient). Prior to oocyte retrieval, the patient received a course of controlled ovarian stimulation (COS) with menotrophin, ganirelix acetate and choriogonadotropin alfa. Four days after COS, she developed a painful purpuric rash alongside knee pain and swelling. Prednisolone was started for presumed IgA vasculitis. Soon after, she developed de novo mononeuritis multiplex and renal dysfunction characterised by haematoprot

Gene-Environment Interaction: Lessons From Complement-Mediated Kidney Disease.

Atypical hemolytic uremic syndrome (aHUS) or complement-mediated thrombotic microangiopathy (CM-TMA) and C3 glomerulopathy are two prototypical diseases of complement dysregulation occurring due to genetic variants in complement proteins or acquired factors such as autoantibodies. Despite the presence of an underlying genetic etiology, an environmental trigger is often necessary to manifest disease, a phenomenon known as incomplete penetrance. These triggers could include infections, pregnancy, medication, cancers, or ischemia-reperfusion injury and antibody-mediated rejection in the setting o

A Case of Palpable Purpura Associated With Japanese Spotted Fever.

This report presents the case of a 90-year-old man with Japanese spotted fever (JSF), caused by Rickettsia japonica, who initially manifested high fever, rash, eschar, and lymphadenopathy. After successful antibiotic therapy, he developed palpable purpura similar to IgA vasculitis (IgAV). Previous reports indicate that the rash in spotted fever group of rickettsial infections, including JSF, undergoes a transformation from petechial spots after three to four days. However, in this case, the rash resolved completely after one week, and on day 10 of hospitalization, new purpura with palpable inf

New insights into kidney disease after COVID-19 infection and vaccination: histopathological and clinical findings.

In addition to its pulmonary effects, coronavirus disease 2019 (COVID-19) has also been found to cause acute kidney injury (AKI), which has been linked to high mortality rates. In this review, we collected data from 20 clinical studies on post-COVID-19-related AKI and 97 cases of AKI associated with COVID-19 vaccination. Acute tubular injury was by far the most common finding in the kidneys of patients with COVID-19-related AKI. Among patients hospitalized for COVID-19, 34.0% developed AKI, of which 59.0%, 19.1% and 21.9% were Stages 1, 2 and 3, respectively. Though kidney disease and other ad

Urinary peptidomic liquid biopsy for non-invasive differential diagnosis of chronic kidney disease.

BACKGROUND AND HYPOTHESIS: Specific urinary peptides hold information on disease pathophysiology, which, in combination with artificial intelligence, could enable non-invasive assessment of chronic kidney disease (CKD) aetiology. Existing approaches are generally specific for the diagnosis of single aetiologies. We present the development of models able to simultaneously distinguish and spatially visualize multiple CKD aetiologies. METHODS: The urinary peptide data of 1850 healthy control (HC) and CKD [diabetic kidney disease (DKD), immunoglobulin A nephropathy (IgAN) and vasculitis] particip

Histologic and Clinical Factors Associated with Kidney Outcomes in IgA Vasculitis Nephritis.

BACKGROUND: Nephritis is a common manifestation of IgA vasculitis and is morphologically indistinguishable from IgA nephropathy. While MEST-C scores are predictive of kidney outcomes in IgA nephropathy, their value in IgA vasculitis nephritis has not been investigated in large multiethnic cohorts. METHODS: Biopsies from 262 children and 99 adults with IgA vasculitis nephritis ( N =361) from 23 centers in North America, Europe, and Asia were independently scored by three pathologists. MEST-C scores were assessed for correlation with eGFR/proteinuria at biopsy. Because most patients ( N =309, 8

Standardization of the management of rheumatoid purpura nephropathy in the West of France. What are the repercussions on the renal sequelae?

INTRODUCTION: Rheumatoid purpura is the most common vasculitis in children, and its renal involvement determines the prognosis. To date, no national protocol exists for its management. A protocol was drafted for the French Grand Ouest inter-region in 2011 in order to standardize practices. OBJECTIVES: The main objective is to evaluate renal sequelae with a median follow-up of 2 years since the implementation of this protocol. The secondary objectives are to evaluate the different therapeutic and diagnostic management. METHOD: Inclusion of all children from 2006 to 2018 with nephropathy due t
