A rare but misleading cause of hematuria in hemophilia A: renal pelvic hemorrhage mimicking tumor.

Hematuria is a common manifestation in hemophilia A, whereas upper urinary tract bleeding is rare and diagnostically challenging. We report a patient with severe hemophilia A presenting with gross hematuria due to spontaneous renal pelvic hemorrhage (Antopol-Goldman lesion), initially mimicking a urothelial tumor on imaging. The diagnosis was supported by clinical context, imaging characteristics, and response to factor VIII replacement therapy. This case highlights the importance of considering benign hemorrhagic causes in hemophilia patients with atypical imaging findings to avoid unnecessar

Correlation between urinary soluble CD163 and endocapillary proliferation and fibrinoid necrosis in IgA vasculitis with nephritis.

BACKGROUND: CD163 is a surface marker expressed by M2c macrophages. This study aims to evaluate the level of urinary soluble CD163 (u-sCD163) in children with IgA vasculitis with nephritis (IgAVN) and its potential diagnostic value. METHODS: U-sCD163 was analyzed in 73 children with IgAVN who underwent a kidney biopsy, 37 children with IgA nephropathy and 50 normal controls. CD163 expression was analyzed by immunohistochemistry. Correlation analyses and inter-group comparison were performed to assess the association between u-sCD163 levels and clinicopathological features of IgAVN. The compar

Does electrocorticographic guidance improve seizure outcomes following cavernoma resection?

OBJECTIVE: Cerebral cavernous malformations (CCMs) are vascular anomalies frequently associated with drug-resistant epilepsy. Surgical resection is a well-established treatment; however, the optimal strategy to achieve long-term seizure freedom (SF) remains unclear. Intraoperative electrocorticography (ECoG) may enhance seizure outcome by guiding resection extent, yet comparative evidence is limited. To evaluate the efficacy of ECoG-guided resection in patients with CCMs, a single-center comparative retrospective cohort analysis and a meta-analysis were performed. METHODS: We analyzed 67 adul

Fetal demise duration and bleeding outcomes at the time of dilation and evacuation.

OBJECTIVES: To examine associations between time from second trimester intrauterine fetal demise (IUFD) to dilation and evacuation (D&E) and bleeding complications. STUDY DESIGN: We identified 197 patients scheduled for D&E for IUFD at a single academic institution between March 2011 to March 2025. IUFD duration was calculated by subtracting gestational age by fetal biometry from gestational age by obstetric dating, categorized into 0-14 days, 15-28 days, and > 28 days. We examined the associations between IUFD duration and bleeding outcomes of disseminated intravascular coagulopathy (DIC) an

Acute Groin Pain Revealing Acquired Hemophilia A: A Case Report of Spontaneous Iliopsoas Hematoma.

Acquired hemophilia A (AHA) is a rare autoimmune bleeding disorder caused by the development of autoantibodies against coagulation factor VIII, resulting in potentially life-threatening hemorrhage. Because affected patients typically lack a personal or family history of bleeding disorders and often present with nonspecific symptoms, diagnosis may be delayed. We report the case of a previously healthy 60-year-old woman who presented with acute progressive left groin pain caused by a spontaneous iliopsoas hematoma with active arterial bleeding. Initial laboratory evaluation revealed an isolated

Anesthesia Clinical Practice Guidelines for Ehlers-Danlos Syndrome: A Modified Delphi Survey.

Ehlers-Danlos syndrome (EDS) is a family of inherited connective tissue disorders distinguished by joint hyperextensibility, skin hyperelasticity, and increased tissue friability. No standardized anesthesia guidelines currently exist for this population, despite facing increased perioperative risks. To address this gap in literature, a three-round modified Delphi survey was conducted with 15 experts in anesthesia and EDS. Seven preliminary guidelines, developed from a comprehensive literature review, were evaluated and refined across three rounds. Consensus was achieved for all seven guideline

Identification of Macrophage Polarization-Related Biomarkers in Multiple Myeloma Using Integrated Bioinformatics.

Multiple myeloma (MM) is an incurable hematologic neoplasm with heterogeneous prognosis. Within the bone marrow microenvironment, macrophage polarization (MP) contributes to immune remodeling and may influence disease progression, yet MP-related prognostic biomarkers remain insufficiently characterized. In this study, we integrated transcriptomic data from UCSC Xena (n = 844) and GSE4581 (n = 414) to identify differentially expressed genes between immune-hot and immune-cold MM samples stratified by single-sample gene set enrichment analysis (ssGSEA)-derived immune scores. Heavy menstrual bleeding in women with a bleeding disorder of unknown cause: a common symptom impacting daily life functioning.

BACKGROUND: Persons with an increased bleeding tendency without abnormalities in standard hemostasis tests are classified as having a bleeding disorder of unknown cause (BDUC). The majority are female, and the most common bleeding symptoms are mucocutaneous, including heavy menstrual bleeding (HMB). However, knowledge on HMB in BDUC and the impact on daily functioning is limited. OBJECTIVES: Investigate the prevalence of HMB and HMB-related quality of life (QoL) in women with BDUC, compared with women with mild bleeding disorders (MBDs) and no bleeding disorder (NBD). METHODS: Data from wome

Germline Genomic and DNA Methylation Differences Between MGUS and Multiple Myeloma Among Patients of African Ancestry.

Individuals of African ancestry (AA) have approximately twofold higher risk of multiple myeloma (MM) than individuals of European ancestry, yet the molecular determinants between monoclonal gammopathy of undetermined significance (MGUS) and MM remain poorly defined. We performed whole-exome sequencing (WES) and genome-wide DNA methylation profiling in 28 AA patients with MM and 28 frequency-matched AA patients with MGUS without documented progression. After quality control, the WES analysis included 28 patients with MGUS and 27 with MM, while the DNA methylation analysis included 27 patients w

The Converging Roles of DNA Damage Resolution and Redox Dysregulation in Plasma Cell Transformation During the Evolution of Multiple Myeloma.

Increasing evidence indicates that dysregulated DNA damage response (DDR) pathways and persistent oxidative stress contribute to genomic instability, increase mutational burden, and promote disease progression. This study investigated the interplay between DDR-related parameters and redox status throughout Multiple Myeloma (MM) evolution. Bone marrow plasma cells (BMPCs) from 16 patients with monoclonal gammopathy of undetermined significance (MGUS), 18 with smoldering multiple myeloma (SMM) and 15 with MM, were analyzed. Measurements included redox status, assessed by the reduced-to-oxidized

Spatial oligonucleotide immuno-reprogramming model (SOIRM): a testable framework linking bone marrow resistance states and oligonucleotide therapeutics in multiple myeloma.

This narrative review presents SOIRM as a conceptual and testable research framework rather than a systematic review, a report of new experimental data, or an established treatment model. It distinguishes findings supported by multiple myeloma studies from emerging evidence, the author's interpretation, and hypotheses that require dedicated experimental and clinical validation. Therapeutic resistance and measurable residual disease (MRD) in multiple myeloma (MM) reflect tumor-intrinsic evolution and interactions between malignant plasma cells and the bone marrow microenvironment. Recent primar

Integrating Hereditary Hemorrhagic Telangiectasia into the Diagnostic Workup of Adolescent Heavy Menstrual Bleeding.

To understand the effect of hereditary hemorrhagic telangiectasis (HHT) on uterine bleeding is paramount to improving clinical care of menstruating individuals with HHT. Heavy menstrual bleeding (HMB) in adolescents and young adults (AYAs) with HHT is underrecognized and among individuals with HMB, HHT should be considered in the differential diagnosis, especially among those with suggestive clinical features or family history. Stigmatization associated with vaginal blood loss likely contributes to the paucity of evidence to guide HMB management in individuals with HHT, as previously reported

Population Frequencies of VWD-Associated Genotypes: Cross-Resource Evidence from RGC-ME and gnomAD.

Population-scale sequencing can quantify genotypes associated with von Willebrand disease (VWD), but these frequencies cannot be translated directly into clinically manifest disease without phenotype and penetrance data. We therefore estimated subtype-specific VWD-associated genotype frequencies in RGC-ME, compared them with gnomAD, and examined which analytical assumptions limit clinical interpretation. Germline VWF variants classified in ClinVar as pathogenic or likely pathogenic (P/LP) on 1 August 2026 were normalized to GRCh38 and matched exactly to both resources. Subtypes were assigned o

Multi-Omic Integration of the miRNome and Transcriptome in Dermal Fibroblasts from Hypermobile Ehlers-Danlos Syndrome and Hypermobility Spectrum Disorders Identifies Convergent Candidate miRNA-mRNA Regulatory Networks Linking Extracellular Matrix Remodeling and Inflammation.

Hypermobile Ehlers-Danlos syndrome (hEDS) and hypermobility spectrum disorders (HSD) are clinically defined connective tissue conditions lacking molecular biomarkers. We tested whether their transcriptomic convergence extends to miRNA-mediated post-transcriptional regulation. miRNome sequencing was performed in cultured dermal fibroblasts from 12 individuals with hEDS, 12 with HSD, and 24 controls. Twenty-five differentially expressed miRNAs (DE-miRNAs) were assessed by qPCR in pooled discovery material and an independent pooled cohort. No miRNA met differential-expression threshold in the dir

Multiple Myeloma Cell-Derived Secretome Modulates Bone Marrow Mesenchymal Stromal Cell Migration In Vitro.

Multiple myeloma (MM) depends on a permissive bone marrow microenvironment that supports malignant plasma cell survival and disease progression. Bone marrow mesenchymal stromal cells (BM-MSCs) are key components of this niche, but the mechanisms by which they are recruited by MM cells remain unclear. Here, we show that the MM-derived secretome (MM-Sec) promotes BM-MSC wound closure and transwell migration-associated responses in vitro. A small extracellular vesicle (sEV)-enriched pellet was isolated from MM.1S-Sec by differential ultracentrifugation and characterized, and the relative contribu

[Multiple myeloma in Argentina: results from a Delphi panel of experts].

INTRODUCTION: In recent decades there have been significant advances in the diagnosis and treatment of multiple myeloma (MM). However, in fragmented health systems such as Argentina's, structural barriers continue to limit equitable access to diagnostic technologies and innovative therapies. MATERIALS AND METHODS: A modified Delphi process including two asynchronous rounds followed by a structured synchronous discussion was conducted to explore expert perspectives on the main barriers affecting MM management in Argentina. Nine expert hematologists with extensive clinical experience, represent

Circulating tumor plasma cells correlate with tumor burden and routine laboratory markers in newly diagnosed multiple myeloma: A retrospective single-center study.

Circulating tumor plasma cells (CTPCs) detected by multiparameter flow cytometry may serve as a minimally invasive marker of disease burden in multiple myeloma (MM). However, the relationship between CTPC burden, routine laboratory parameters, and bone marrow (BM) involvement remains insufficiently characterized in regional clinical settings. This retrospective single-center study included 51 consecutive patients with newly diagnosed MM. Peripheral blood CTPCs were quantified using ten-color flow cytometry. Among them, 41 patients with evaluable CTPC data were stratified into CTPC-higher (n =

FAS and FASL expression in autoimmune lymphoproliferative syndrome and immune thrombocytopenia: limited diagnostic utility as transcriptional biomarkers.

BACKGROUND: Autoimmune lymphoproliferative syndrome (ALPS) is an immune dysregulation disorder caused by defective lymphocyte apoptosis, most commonly involving the FAS-FAS ligand pathway. Because ALPS frequently presents with autoimmune cytopenias, particularly immune thrombocytopenia (ITP), its diagnosis may be challenging. This study evaluated whether FAS and FAS ligand gene expression could distinguish ALPS from ITP. METHODS: FAS and FAS ligand messenger RNA expression was measured by quantitative real-time polymerase chain reaction in whole-blood samples from patients with pediatric-onse
