Venetoclax-enhanced reduced-intensity conditioning in older patients aged ≥55 years with high-risk myeloid malignancies undergoing allogeneic peripheral blood stem cell transplantation: A phase II study.

Reduced-intensity conditioning (RIC) is associated with a high relapse rate in high-risk acute myeloid leukaemia (AML) and myelodysplastic syndromes (MDS) undergoing allogeneic haematopoietic stem cell transplantation (allo-HSCT). It remains unclear whether venetoclax, an anti-leukaemic agent, can enhance RIC efficacy. A prospective, single-arm, phase 2 trial enrolled patients with high-risk AML or MDS, aged ≥55 years, who underwent allogeneic peripheral blood stem cell transplantation (allo-PBSCT). All patients received venetoclax-enhanced RIC (VEN-RIC), comprising venetoclax, busulfan, fluda

Bone marrow edema as an independent imaging predictor of dedifferentiated chondrosarcoma.

BACKGROUND: Dedifferentiated chondrosarcoma is the most aggressive subtype of chondrosarcoma; however, imaging features that independently distinguish it from other subtypes remain unclear. The purpose of this study was to identify independent imaging predictors of long bone dedifferentiated chondrosarcoma to signal for expedited biopsy and treatment. METHODS: This retrospective study incorporated consecutive patients undergoing resection at a tertiary center for primary intramedullary long bone chondrosarcoma between February 2001 and January 2024. Inclusion required preoperative MRI and pos

Caregiver communication and psychosocial outcomes among adolescents and young adults with sickle cell disease.

OBJECTIVE: The transition from pediatric to adult medical care is associated with increased symptoms of anxiety, depression, and experiences with disease stigma for adolescents and young adults (AYA) with sickle cell disease (SCD). Community-informed conceptual frameworks suggest that caregiver communication could play an important role in enhancing AYA well-being during this critical period; yet this association has yet to be empirically investigated in this population. This study examined the influence of caregiver-AYA communication on psychosocial outcomes among AYA with SCD. METHODS: AYA

Germline predisposition in myeloproliferative neoplasms.

Myeloproliferative neoplasms (MPNs) were previously thought to result solely from somatic mutations; however, familial clustering and germline genetic testing have revealed hereditary MPNs. These hereditary MPNs are one type of hereditary hematopoietic malignancy (HHM) syndrome, or hereditary blood cancer, and generally display an autosomal-dominant inheritance pattern within families. Point estimates of the prevalence of hereditary MPNs vary with testing methodology and the patient population sampled, but diagnostic yields for pathogenic/likely pathogenic variants in patients with MPNs range

An integrated deep learning and molecular dynamics pipeline for the discovery of novel CDK8 inhibitors against acute myeloid leukaemia.

Cyclin-dependent kinase 8 (CDK8) has emerged as a promising therapeutic target for acute myeloid leukaemia (AML). We developed an integrated computational pipeline combining neural network-based potency prediction with molecular dynamics (MD) simulations for CDK8 inhibitor discovery. A curated dataset of 1,200 unique CDK8 inhibitors was assembled from ChEMBL. The optimal neural network architecture (two hidden layers, 512→128 units) with dropout regularization achieved a test set r2 of 0.47 and RMSE of 0.87. Y-randomization testing confirmed genuine structure-activity relationships. Using an e

Multiparameter flow cytometry versus droplet digital polymerase chain reaction for monitoring measurable residual disease in paediatric fusion gene-positive acute myeloid leukaemia.

Measurable residual disease (MRD) monitoring is crucial for prognostic stratification in paediatric acute myeloid leukaemia (AML). This study compared the performance of multiparameter flow cytometry (MFC) and droplet digital polymerase chain reaction (ddPCR) for MRD assessment in 116 paediatric AML patients at end of induction (EOI)-1, EOI-2 and end of consolidation (EOC)-1. Compared to MFC, ddPCR revealed a significantly slower rate of MRD negativity conversion, with moderate concordance between methods. Both MFC and ddPCR were significant predictors of 5-year event-free survival (EFS) at EO

Dalcetrapib decreases red blood cell sickling propensity in sickle cell disease.

The aim of the present study was to compare the effects of dalcetrapib and voxelotor on the rheology of red blood cells (RBCs) from sickle cell patients, as well as on hemoglobin oxygen affinity. Oxygen gradient ektacytometry was used to determine RBC deformability in normoxia and hypoxia, as well as the propensity of RBCs to sickle. Hemoglobin oxygen affinity was also measured following dalcetrapib and voxelotor incubation. Both voxelotor and dalcetrapib increased RBC deformability in hypoxia and decreased the propensity of RBCs to sickle under deoxygenation. However, in contrast to voxelotor

Minimal residual disease-guided ibrutinib and venetoclax in patients with chronic lymphocytic leukaemia and complex karyotype.

Complex karyotype (CK) is a strong predictor of early progression and poor survival in chronic lymphocytic leukaemia (CLL), including during targeted therapy. Minimal residual disease (MRD)-guided therapy may improve outcomes by enabling deeper remissions and limiting clonal evolution. We present the final results of a prospective MRD-guided ibrutinib and venetoclax (IVen) compared with a retrospective cohort treated with ibrutinib monotherapy (Imono). Eligible patients had CLL with high CK (≥5 aberrations) or CK with deletion 17p. The IVen cohort received 3 months of ibrutinib followed by up

Sickle cell disease and commercial aviation. A retrospective analysis of in-flight and pre-flight medical events.

The cabin of an aircraft while at cruising altitude is a hypobaric hypoxic environment. The hypoxic nature of this environment may predispose sickle cell disease (SCD) passengers to vaso-occlusive episodes (VOEs). Evidence on pre-flight and in-flight events related to SCD is limited. A retrospective analysis was undertaken of records from a ground-based medical support (GBMS) provider (1.1.22-1.10.24) for pre-flight and in-flight events relating to SCD. Passenger demographics, presenting features, oxygen/analgesia use and flight characteristics were extracted. 84 pre-flight and 76 in-flight SC

Minor Myeloid Leukemic Populations in Pediatric Acute Lymphoblastic Leukemia: A Potentially Underrecognized Mixed-Phenotype Tumor?

In "bilineal" mixed-phenotype acute leukemia (MPAL), the proportions of lymphoid and myeloid leukemic cells may differ substantially. A myeloid component may constitute <10% of bone marrow (BM) cells and show subtle immunophenotypic abnormalities detectable by multicolor flow cytometry (MFC). The significance of minor myeloid leukemic populations in otherwise acute lymphoblastic leukemia (ALL), including their association with lineage switch, remains uncertain. We retrospectively reviewed diagnostic MFC data from 5094 children with B-cell precursor or T-cell ALL. An additional minor myeloid le

[Clinical case of branch retinal artery occlusion in inherited thrombophilia].

Branch retinal artery occlusion (BRAO) is an extremely rare vascular event in the pediatric population. Unlike elderly patients, in whom atherosclerosis is the primary cause, BRAO in children and adolescents is often associated with systemic conditions, including hypercoagulation and genetic predisposition to thrombosis. Ophthalmoscopy and OCT revealed local ischemic retinal edema along the inferior temporal branch of the central retinal artery. Systemic examination and molecular genetic testing revealed combined genetic thrombophilia: heterozygous mutation of the MTHFR gene (A1298C) and homoz

Validation of HemoCue method and development of a revised hemoglobin cut-off to detect anemia in children aged 6-24-months.

BACKGROUND: Accurate hemoglobin (Hb) measurement is essential for determining anemia prevalence at the population level. The HemoCue method, commonly used in resource poor settings, has been shown to overestimate Hb levels compared to the direct cyanmethemoglobin (DCM) method, leading to an underestimation of anemia. We aimed to validate the HemoCue method against the DCM method and establish an optimal cut-off value for diagnosing anemia in children under two. METHODS: Data were collected from children under two years of age as part of the Etiology, Risk Factors, and Interactions of Enteric
