Metamizole-induced agranulocytosis from a German intensive care perspective.

BACKGROUND/OBJECTIVES: Metamizole (dipyrone) is a non-opioid analgesic and antipyretic widely used in Germany, with prescriptions having increased more than tenfold since 1997. Despite its popularity, metamizole remains controversial due to the risk of agranulocytosis-a potentially life-threatening adverse reaction that can occur independently of dose or duration of therapy. This study aimed to describe severe cases of metamizole-induced agranulocytosis requiring intensive care treatment and to contextualize them within current consumption data. METHODS: A retrospective analysis was conducted

Tamoxifen-induced neutropenia and leukopenia in a breast cancer patient: a case report and literature review.

BACKGROUND: Tamoxifen is a cornerstone of endocrine therapy for hormone receptor-positive breast cancer. While its common side effects are well-documented, severe hematological toxicities such as neutropenia and leukopenia are extremely rare. CASE PRESENTATION: We report a case of a 37-year-old premenopausal woman with hormone receptor-positive breast cancer who developed neutropenia and leukopenia three months after initiating adjuvant tamoxifen (20 mg/day). Despite transient improvements with supportive therapies (leucogen tablets and Compound Ejiao Jiang), cytopenias persisted. Tamoxifen w

A clinical case and pharmacokinetic analysis of an inadvertent overdose of dipyrone (metamizole) without clinical toxicity: A case report.

Dipyrone (metamizole) is a widely used non-opioid analgesic. Its use is restricted due to the risk of side effects such as liver toxicity. We report the case of a patient who accidentally received an oral dose of 12.5 g (132 mg/kg) dipyrone. The patient was monitored for the next 62 days and remained asymptomatic, showing no signs of gastrointestinal, hepatic, or other toxicity. Dipyrone was not detectable in the plasma, while the plasma concentrations of its active metabolites 4-methylaminoantipyrine and 4-aminoantipyrine were elevated. The pharmacokinetics were best described by nonlinear on

Dual oral iron chelation with deferasirox and deferiprone in transfusion-dependent β-thalassemia: a narrative review of efficacy, safety, and practical application.

BACKGROUND: For patients with transfusion-dependent β-thalassemia (TDT) and suboptimal response to iron chelator monotherapy, dual oral iron chelation (DOIC) with deferasirox (DFX) and deferiprone (DFP) represents a practical strategy to enhance iron removal. This narrative review synthesizes contemporary evidence for its use, with a focus on identifying clinical signals from real-world data and highlighting gaps requiring prospective investigation. METHODS: We conducted a narrative review informed by a structured literature search of PubMed, Scopus, and regional databases (January 2016 to Ja

Neutropenia and infectious events during off-label treatment with venetoclax in children with malignant disease: a pharmacovigilance analysis of FDA adverse event reporting system reports.

Venetoclax, a BCL-2 inhibitor, is increasingly used off-label in pediatric malignancies, but safety data remain limited. This pharmacovigilance study analyzed 488 pediatric reports from the FDA Adverse Event Reporting System (FAERS) involving neutropenia and infection with venetoclax use. Disproportionately reported events included sepsis, leukopenia/agranulocytosis, and pseudomembranous colitis. Most cases involved acute myeloid leukemia or myelodysplastic syndromes and combination therapy. Network analysis revealed co-reporting of hematologic and infectious toxicities. These findings emphasi

Racing Heart Under Stormy Skies: A Journey From Agranulocytosis to Thyroid Storm.

Thyroid storm is a rare, severe, and life-threatening exacerbation of thyrotoxicosis, characterized by dysfunction of the thermoregulatory, central nervous, gastrointestinal, hepatic, and cardiovascular systems. We present a 31-year-old female with newly diagnosed Graves' disease complicated by carbimazole-induced agranulocytosis. The delay in initiating adjunctive therapy to control the thyrotoxicosis due to suspected sepsis was followed by rapid development of thyroid storm requiring therapeutic plasma exchange (TPE) and urgent thyroidectomy. This case highlights the importance of prompt rec

Dual Antithyroid Drug-Induced Agranulocytosis in Graves' Disease With Decompensated Heart Failure: A Multidisciplinary Approach.

Antithyroid drug-induced agranulocytosis is a rare but potentially life-threatening complication, particularly in patients with limited therapeutic options. This case illustrates the complexity of managing uncontrolled thyrotoxicosis in the context of a patient with decompensated heart failure, chronic obstructive pulmonary disease (COPD), and adverse reactions to both methimazole and propylthiouracil. A 54-year-old male patient with a 2-year history of Graves' disease presented with worsening dyspnea and tachycardia in the setting of thyrotoxicosis with a TSH of 0.0007 mIU/L. He had previousl

Potential Pharmacogenomics (PGx), Human Leukocyte Antigen (HLA) Antiviral to Cephalosporin Crossover Reaction.

Patient is an HLA-B*57:01 positive, 43-year-old male with a history of diabetes type II and admission 10 weeks ago at another institution for pubic symphysis osteomyelitis and associated pseudomonas. Cefepime IV was started and the patient presented with fevers, aches, agranulocytosis, thrombocytopenia and rash as a suspected side effect of cefepime after 3 weeks of treatment. Cefepime was replaced with piperacillin-tazobactam and symptoms resolved. Question to be resolved is whether an HLA-B*57:01 positive, abacavir sensitive patient could have an agranulocytosis cross reaction to cefepime.

Risk factors associated with blood dyscrasia during clozapine treatment: a systematic review and meta-analysis.

Clozapine is the only effective treatment for treatment-resistant schizophrenia, but concerns over blood dyscrasia and need for monitoring limit its use. Evidence suggests many hematological abnormalities may result from surveillance bias, with agranulocytosis being the primary adverse effect directly induced by clozapine. This systematic review (PROSPERO: CRD42024487199) investigates non-genetic risk factors associated with blood dyscrasia during clozapine treatment, focusing on neutropenia and agranulocytosis. Random-effect meta-analyses were performed on studies reporting quantitative risk

ACKR1/Duffy-null genotype testing for clozapine: A guideline developed by the UK Centre of Excellence in Regulatory Science and Innovation in Pharmacogenomics (CERSI-PGx).

Clozapine is licenced for treatment-resistant schizophrenia and psychosis in Parkinson's disease. In the United Kingdom, there is a mandatory requirement for absolute neutrophil count (ANC) and white blood cell count (WBC) monitoring to safeguard against agranulocytosis. Some people have naturally low ANCs without increased infection risk, caused by a homozygous T > C variant in ACKR1, commonly called the Duffy-null genotype. This condition is known as ADAN (ACKR1/DARC-associated neutropenia) and synonyms include DANC (Duffy-null associated neutrophil count) and BEN (benign ethnic neutropenia)

Anlotinib plus whole-brain radiotherapy for NSCLC brain metastases: a prospective, non-randomized, single-center cohort study.

This study focuses on the vessel normalization window of anlotinib to preliminarily explore the optimal intervention timing for combining anlotinib with whole brain radiotherapy (WBRT) in treating brain metastases from non-small cell lung cancer (NSCLC). We aimed to explore the feasibility of combining anlotinib with WBRT based on the hypothesized vascular normalization window, and to investigate potential associations with intracranial tumor control, iPFS, and quality of life in patients with NSCLC brain metastases. This study was designed as a prospective, non-randomized, single-center cohor

Clozapine-Induced Acute Interstitial Nephritis: A Systematic Review.

BACKGROUND: Clozapine, an atypical antipsychotic used primarily for treatment-resistant schizophrenia, has been associated with a wide range of side effects, including the rare but serious complication of acute interstitial nephritis (AIN). This systematic review aimed to summarise published cases of clozapine-induced AIN, focusing on clinical presentations, diagnostic findings, treatment approaches, and patient outcomes to improve awareness and management of this adverse reaction. METHODS: A systematic search of the databases Medline, Embase, Cochrane Library, and PsycINFO was conducted acco

Mirtazapine-Induced Bicytopenia in a Patient With Multifactorial Delirium: A Case Report.

Mirtazapine is often preferably used in geriatric patients with mood disorders due to its favourable side effects such as somnolence and weight gain. However, mirtazapine is associated with a rare but fatal side effect, agranulocytosis, which is not widely described. We present a case of mirtazapine-induced bicytopenia (moderate anaemia and leukopenia) in a geriatric patient. Notwithstanding scant available reports of mirtazapine-induced neutropenia, a case of iatrogenic anaemia from the drug is absent as per our literature review, although underreporting and undocumented cases are possibiliti

Clozapine-induced neutropenia: Prevalence and associated factors.

INTRODUCTION: Clozapine is the gold-standard therapy for resistant schizophrenia. However, its use is limited by risks such as clozapine-induced agranulocytosis (CIA) and neutropenia (CIN), requiring strict monitoring. This study aimed to estimate the prevalence and mortality of CIA and CIN at GHU Paris Psychiatrie & Neurosciences and identify risk factors. METHODS: This retrospective study used data from the health data warehouse. We selected all patients treated with clozapine between January 1, 2019, and December 31, 2024. Based on the absolute neutrophil count (ANC), we identified cases o

Clozapine-Induced "Pseudo-Agranulocytosis": Bone Marrow Suppression or Increased Neutrophil Margination?

INTRODUCTION: Clozapine is the gold-standard antipsychotic for treatment-resistant psychosis (TRP). Agranulocytosis is an important, potentially life-threatening adverse effect of clozapine treatment, and typically occurs during the first 6 months of treatment. Clozapine-induced agranulocytosis is thought to reflect bone marrow suppression; however, alternative hypotheses have not been reported or investigated. CASE REPORT: We present the case of a 49-year-old female with TRP who experienced late-onset agranulocytosis after 8 years of clozapine treatment. She was subsequently evaluated by hem

An in vitro approach to predict idiosyncratic drug-induced agranulocytosis using myeloperoxidase-derived reactive metabolites.

Idiosyncratic drug-induced agranulocytosis (iDIAG) is a rare but potentially life-threatening adverse reaction characterized by an absolute neutrophil count of fewer than 500 cells/μL. Owing to its complex and poorly understood mechanisms, predicting the risk of iDIAG remains a major challenge. We hypothesized that the amount of reactive metabolite (RM) formation mediated by myeloperoxidase (MPO), an enzyme abundantly expressed in neutrophils, may be correlated with iDIAG risk. To test this hypothesis, we developed a novel trapping assay using [3H]glutathione and [14C]cyanide as soft and hard
