Complement system in kidney transplantation: from innate immune surveillance to a therapeutic and genetic engineering target.

Kidney transplantation remains the most effective treatment for end-stage kidney disease; however, long-term graft survival is constrained by rejection and chronic graft injury, and donor shortages continue to drive interest in xenotransplantation. The complement system is increasingly recognized both as a terminal effector of antibody-mediated injury and as a regulatory network linking antibody responses, endothelial activation, ischemia-reperfusion injury, coagulation, and innate immune cell responses. In this review, we compare the role of complement in allogeneic and xenogeneic kidney tran

Mertk drives alveolar bone loss in experimental periodontitis associated with altered neutrophil function and osteoclast recruitment.

BACKGROUND: Alveolar bone loss is the hallmark of periodontitis and often remains refractory to conventional mechanical and antimicrobial therapies in chronic cases. Mer receptor tyrosine kinase (Mertk) has been implicated in bone homeostasis, yet the immune mechanisms by which Mertk modulates periodontal bone resorption remain insufficiently characterized. The aim of this study is to investigate the role of Mertk deficiency in modulating the inflammatory response during ligature-induced periodontitis and its impact on alveolar bone loss. METHODS: C57BL/6J wild-type (WT) and Mertk-∖- mice at

Case Report: IgG4-related lung disease.

Immunoglobulin G4-related disease (IgG4-RD) is a systemic immune-mediated fibroinflammatory condition, distinct from classical autoimmune disorders, that can affect multiple organs. Pulmonary involvement often presents with non-specific clinical and radiographic features, contributing to high rates of misdiagnosis and underdiagnosis. Here, we report a rare case of IgG4-related lung disease (IgG4-RLD) to enhance clinical awareness. A 71-year-old female presented with cough. Chest computed tomography (CT) revealed an irregular soft tissue mass in the right middle lobe. All autoantibodies tested

Single-cell transcriptomic atlas uncovers Immune/Structural lineage remodeling and fibrosis in Klebsiella pneumoniae-infected porcine lungs.

Klebsiella pneumoniae (KP) is a predominant pathogen in respiratory infections, whose widespread antibiotic resistance poses severe public health risks and economic burdens. As the primary target of KP infection, the lung contains diverse immune cells, while pulmonary cellular responses to KP remain poorly understood. This study integrated single-cell RNA sequencing, flow cytometry, RT-qPCR, and immunofluorescence to investigate porcine lung cell response from healthy pigs and those infected with multidrug-resistant, highly-virulent KP. Post-infection, myeloid cells (neutrophils, monocytes, an

Innate and adaptive immunity pathways linking Mycoplasma pneumoniae to pediatric asthma phenotypes.

UNLABELLED: Mycoplasma pneumoniae (Mp) is repeatedly detected during pediatric wheeze and asthma exacerbations, but its clinical significance depends on the host-response context. This review evaluates the clinical and mechanistic evidence linking Mp to pediatric asthma phenotypes and examines approaches for distinguishing biologically meaningful infection from incidental detection. We narratively synthesized clinical, experimental, immunological, toxin, and proteomic literature addressing Mp-associated wheeze, asthma exacerbation, epithelial injury, allergic amplification, and inflammatory en

Systemic immune-inflammation index predicts left atrial thrombus in non-valvular atrial fibrillation: A retrospective study of 847 patients.

Non-valvular atrial fibrillation (NVAF) serves as a primary cause of cardioembolic stroke, with left atrial thrombus (LAT) functioning as its major thrombotic substrate. The systemic immune-inflammation index (SII) has gained recognition as a dependable biomarker in cardiovascular disease. Nevertheless, its relationship with LAT in NVAF patients has not been thoroughly characterized in large-scale investigations. This retrospective study enrolled 847 NVAF patients who underwent transesophageal echocardiography (TEE). The primary outcome was TEE-detected left atrial thrombus (LAT). Patients wer

Leukocyte cell population data predict 30-day mortality in community-acquired pneumonia: A multicenter prospective study.

Early identification of patients with community-acquired pneumonia (CAP) at increased risk of death remains essential, but rapid and low-cost prognostic biomarkers are still needed. Cell population data (CPD), automatically generated during routine complete blood count (CBC) testing, reflect leukocyte morphology and functional activation. This study aimed to assess whether CPD parameters predict 30-day mortality in hospitalized patients with CAP. This multicenter, observational, prospective study included consecutive adults admitted with CAP to three teaching hospitals in northern Spain betwee

Dopamine signaling reprograms macrophage FAO to alleviate acute lung injury by inhibiting NETosis via the CXCL10-CXCR3 axis.

BACKGROUND: Dysregulated innate immunity and oxidative stress drive the pathogenesis of acute lung injury/acute respiratory distress syndrome (ALI/ARDS), yet master endogenous regulators that orchestrate inflammation resolution remain elusive. METHODS: This study employed public database mining, clinical data investigation, murine disease models, mouse bone marrow-derived macrophages and neutrophils, and human macrophages from healthy donors and ARDS patients, to investigate the dynamic changes of the dopaminergic signaling system in the acute pulmonary inflammatory environment and its role i

Targeted Degradation of STING by a Neutrophil Membrane-Coated Nanoplatform Suppresses Microglial Pyroptosis After Subarachnoid Hemorrhage.

Subarachnoid hemorrhage (SAH) is a life-threatening cerebrovascular disease in which neuroinflammation and neuronal death critically contribute to poor outcomes. Here, we identify aberrant STING activation as a key driver of microglial pyroptosis and post-SAH injury. Transcriptomic and biochemical analyses support a close association between STING signaling and microglial pyroptosis, with MAPK signaling acting as a functionally relevant downstream pathway. Based on this mechanistic insight, we engineered MG1 peptide-functionalized, neutrophil membrane-coated STING-PROTAC nanoparticles (MG1@NM-

NLR-based nomogram predicts survival in hepatocellular carcinoma treated with TACE plus targeted immunotherapy.

BACKGROUND: The neutrophil-to-lymphocyte ratio (NLR) has emerged as a potential prognostic biomarker in hepatocellular carcinoma (HCC), but its predictive value in patients with unresectable HCC receiving transarterial chemoembolization (TACE) combined with targeted agents and immune checkpoint inhibitors remains unclear. This study aimed to evaluate the prognostic significance of NLR in this setting and to develop individualized nomograms for predicting overall survival (OS) and progression-free survival (PFS). METHODS: This retrospective real-world cohort consecutively enrolled 148 patients

CD73 controls neutrophil responsiveness to type I interferon impairing antibacterial responses during secondary pneumococcal pneumonia.

Streptococcus pneumoniae (pneumococcus) are asymptomatic colonizers of the nasopharynx but can progress to pulmonary and systemic pathogens upon influenza A virus (IAV) infection. Polymorphonuclear cells (PMNs) are key for controlling S. pneumoniae , but their antibacterial function is impaired by IAV. Using a mouse model that allows transition of pneumococci from colonizers to disease-causing pathogens upon IAV co-infection, we examined the signaling pathways impairing PMN responses. We found that type I interferons (IFN) produced upon IAV infection drives impairment of PMN antibacterial resp

Association between neutrophil percentage-to-albumin ratio and all-cause mortality in stroke survivors stratified by triglyceride-glucose index: A prospective observational study based on the NHANES database from 1999 to 2018.

The neutrophil percentage-to-albumin ratio (NPAR) is a novel inflammatory biomarker with prognostic value in critical illnesses, but its association with outcomes in stroke survivors remains unclear. No prior study has explored whether this association differs across triglyceride-glucose index (TyG) strata, a surrogate marker of insulin resistance (IR) and metabolic disturbance in cardiovascular disease. Data from the National Health and Nutrition Examination Survey 1999 to 2018 were analyzed for 1187 stroke survivors aged 40 to 80 years. Kaplan-Meier curves, multivariate Cox models, restricte

Endoplasmic Reticulum Stress, Redox Signaling, and Neutrophil Effector Functions in Chronic Lung Diseases: Evidence and Research Priorities.

Endoplasmic reticulum (ER) stress and unfolded protein response (UPR) signaling intersect with inflammatory and redox pathways. Experiments in primary neutrophils link selected UPR pathways to mitochondrial reactive oxygen species, neutrophil extracellular trap (NET) formation, antimicrobial activity, and immune regulation under defined conditions. This review organizes these findings by cell type, experimental model, and the relationship tested, distinguishing them from cell-line experiments, other-cell mechanisms, clinical associations, and database connections. We examine calcium signaling,

Circulating Cit-H3 and MPO-DNA complexes reveal divergent neutrophil-associated immune profiles in multiple myeloma.

INTRODUCTION: Neutrophils are increasingly recognized as dynamic regulators of tumor inflammation and immune remodeling. Neutrophil extracellular trap (NET)-related biomarkers have been implicated in cancer progression, but their clinical and immunological interpretation remains challenging because different circulating NET components may not represent equivalent biological processes. In multiple myeloma, a malignancy shaped by complex interactions among clonal plasma cells, hematopoiesis, and the bone marrow immune microenvironment, the relevance of distinct circulating NET-related markers re

Matrix Metalloproteinase-8 Deficiency Alters the Balance Between Viral Clearance and Immunopathology in H1N1 Infection.

Matrix metalloproteinase-8 (MMP-8) regulates inflammatory responses, but its contributions to the pathogenesis of influenza A viral (IAV) disease are not known. Plasma MMP-8 levels were increased 30-fold in subjects infected with H1N1 or seasonal IAV and were inversely associated with the PaO2/FiO2 ratio, suggesting potential value as a candidate marker of lung injury severity that requires validation in larger cohorts. MMP-8 levels were elevated in the lungs of H1N1-infected wild-type (WT) mice and localized to airway epithelial cells, macrophages, and polymorphonuclear neutrophils (PMNs). H1

A national registry for female carriers of chronic granulomatous disease.

BACKGROUND: Registries for inborn errors of immunity can identify the natural history and best practices for these conditions. X-linked chronic granulomatous disease (XLCGD) is an inborn error of immunity characterized by impaired neutrophil oxidative burst, leading to autoimmunity and life-threatening infections. XLCGD carriers can develop similar symptoms and impaired neutrophil function because of skewed X-chromosome inactivation. No registry currently exists for carriers of XLCGD. OBJECTIVE: To describe findings from the Registry for Autoimmunity and Infections in Chronic Granulomatous Di

Innate immunity and hypertension: unveiling inflammatory pathways for novel therapies.

Hypertension is a leading cause of global cardiovascular morbidity and mortality, yet its primary etiology remains poorly defined. Accumulating evidence increasingly positions inflammation as a key determinant of disease progression, with both innate and adaptive immune mechanisms playing central roles. Under physiological conditions, the immune system safeguards vascular homeostasis through the coordinated activity of immune cells and soluble mediators. Cells of the innate immune system, such as natural killer cells, dendritic cells, macrophages, and neutrophils, detect pathogen- and damage-a

The emerging role of brain-derived microparticles in trauma-induced coagulopathy after traumatic brain injury.

Traumatic brain injury (TBI) is frequently complicated by trauma-induced coagulopathy (TIC), which contributes substantially to secondary brain injury and poor clinical outcomes. The mechanisms underlying traumatic brain injury induced coagulopathy (TBI-IC) are multifactorial and involve extensive cellular alterations in endothelial cells, neutrophils, and platelets. Brain-derived microparticles (BDMPs), a subgroup of extracellular vesicles derived from brain-associated cells, attracted increasing attention due to their potential involvement in coagulation regulation after TBI. This review sum

Defective Inflammation Resolution in Cystic Fibrosis: Cellular Mechanisms and Emerging Pro-resolving Therapies in the CFTR Modulator Era.

Cystic fibrosis (CF) is a fatal monogenic hereditary disease that affects approximately 100,000 people worldwide. It is caused by mutations in the cystic fibrosis transmembrane conductance regulator (CFTR) gene, impairing biosynthesis and/or function of a protein that regulates chloride and bicarbonate fluxes. CFTR loss of function is associated with aberrant, non-resolving inflammation that in combination with recurrent infection leads to respiratory insufficiency. Cellular and molecular studies have uncovered failures in key mechanisms of inflammation resolution, consequent to loss of functi
