Impact of 22q11.2 deletion syndrome on clinical and immunological status of infants with conotruncal heart defects.

BACKGROUND: Patients with 22q11.2 deletion are known to have immune abnormalities. Data on the immune profile of non-syndromic patients with conotruncal heart defects are limited. METHODS: A prospective study evaluated the genetic and immunological profiles and early to mid-term postoperative outcomes of patients with conotruncal heart defects. RESULTS: Infants with 22q11.2 deletion had low leukocyte counts, while low total lymphocyte counts were observed in all patients except infants without a genetic syndrome. Reduced CD3+, CD4+, and CD8+ cells were found in 22q11.2 deletion neonates and

A decoy oligodeoxynucleotide favors the differentiation of CpG ODN-induced B cells into IL-10-producing Breg-like cells over plasma cells by restoring IRF4/IRF8 imbalance.

The imbalanced expression of interferon regulatory factor (IRF) 4 and IRF8 in activated B cells significantly influences their differentiation and promotes the development of immune-related diseases. Restoring abnormal B cells to appropriate responses may treat these diseases. In this study, an oligodeoxynucleotide (ODN) S2, designed according to the consensus sequence recognized by IRFs in interferon-stimulated response elements, was used as an immunomodulator to investigate its effects on mouse splenic B cells stimulated with the TLR9 agonist CpG ODN, either alone or in combination with anti

Imaging Flow Cytometric Assessment of del(17p) and amp(1q21) in Circulating Plasma Cells of Myeloma Patients.

Multiple myeloma is a bone marrow-derived neoplastic proliferation of plasma cells with low levels of circulating tumor cells. Cytogenetic abnormalities are present in >90% of patients, when assessed by fluorescence in situ hybridization (FISH) on bone marrow, and the specific abnormalities provide vital prognostic information. Provided is a protocol for assessing the high-risk abnormalities del(17p) and amp(1q21) in the circulating plasma cells of myeloma patients by flow cytometry. This utilizes positive plasma cell identification by standard immunophenotyping and then chromosomal analysis b

Intrinsic neutrophil dysregulation in programmed cell death promotes neutrophilic inflammation in cystic fibrosis.

Neutrophilic inflammation constitutes a major pathology in cystic fibrosis (CF), a life-shortening genetic disorder. However, the underlying mechanisms remain incompletely understood. Here we report our investigation into CF neutrophil recruitment, apoptosis and clearance to determine if any neutrophil intrinsic defect is involved in the pathogenesis. Congenic fluorescent wild-type (WT) and CF mice were generated and used as bone marrow (BM) donors. Non-fluorescent WT and CF recipients were transplanted with a mixture of EGFP-WT and DsRed-CF BM cells and interrogated for neutrophil recruitment

Transcription factor Etv3 controls the tolerogenic function of dendritic cells.

Dendritic cells (DCs) facilitate the maintenance of immunological tolerance in the steady state. We report that transcription factor Etv3 is preferentially expressed in mature DCs, including tissue-derived migratory DCs (migDCs), and facilitates their homeostatic maturation and CCR7-dependent migration. Mice with global or DC-specific deletion of Etv3 manifested the expansion of CD25low regulatory T (Treg) cells, spontaneous activation of conventional T cells, and multiorgan T cell infiltration. Etv3 deficiency exacerbated TLR7-driven systemic lupus erythematosus (SLE)-like disease, supporting

Telomere Shortening Drives Atrial Fibrillation Through VCAM-1 Mediated Atrial Electrical and Structural Remodeling.

Telomere shortening is a hallmark of aging and has been implicated in cardiovascular disease, but its mechanistic link to atrial fibrillation (AF) remains elusive. Using a high-throughput, single-gene-calibrated dot blot assay, we developed to quantify leukocyte telomere length (LTL). In age-stratified analyses, shorter LTL was associated with AF predominantly in individuals younger than 70 years. In telomerase-deficient (TERT-/-) mice with telomere dysfunction, higher AF inducibility, atrial electrical conduction slowing, and atrial fibrosis were observed. Transcriptomic profiling revealed si

Decoding the Genetic Enigma: A Case Study on Congenital Anomalies with Developmental Delay and 9q Duplication Unveiled Via Comprehensive Whole Exome Sequencing and Cytogenetic Analysis.

OBJECTIVES: Approximately 3% of newborns worldwide are affected by Congenital Anomalies (CAs) with or without Intellectual Disability (ID)/Developmental Delay (DD), often caused by genetic factors such as single-gene disorders or chromosome aberrations. Whole Exome Sequencing (WES) has become a highly effective first-tier test for identifying these genetic factors, detecting both Copy Number Variations (CNVs) and Single Nucleotide Polymorphisms (SNPs)/Insertion/Deletion Polymorphisms (INDELs), while conventional cytogenetic analysis can provide additional valuable information to confirm result

Case Report: Fabry disease presenting with electrocardiographic findings mimicking acute myocardial infarction: a diagnostic challenge.

UNLABELLED: Fabry disease (FD) is a rare X-linked lysosomal storage disorder characterized by heterogeneous clinical manifestations. Cardiac involvement arises from progressive glycosphingolipid accumulation within cardiomyocytes, vascular endothelium, and the conduction system, resulting in left ventricular hypertrophy, conduction abnormalities, and arrhythmias. A subset of patients exhibits electrocardiographic findings that mimic acute myocardial infarction (AMI), often leading to misdiagnosis as ischemic heart disease. CASE PRESENTATION: We describe a 62-year-old man presenting with recur

From Thymic Hypoplasia to Immune Reconstitution: An Immunological Review of DiGeorge Syndrome.

DiGeorge syndrome (DGS), also known as 22q11.2 deletion syndrome, is the most prevalent chromosomal microdeletion syndrome. It presents with a heterogeneous phenotype and diverse clinical manifestations, which may include, but are not limited to, cardiovascular, endocrine, neurodevelopmental and immune function abnormalities. The immune defects seen in individuals with DGS arise from impairment in thymus development, resulting in immune dysregulation. This thymic dysfunction impairs T-cell development and maturation, leading to a spectrum of T-cell lymphopenia, a restricted T-cell receptor rep

Activated Platelet-Released Heat Shock Protein 90α Triggers Autophagy-Dependent Neutrophil Extracellular Trap Formation and Amplifies Sepsis.

Platelets are crucial to the development of thrombosis and coagulation abnormalities in sepsis, but the mechanisms by which they contribute to these pathological processes are not fully understood. Here, we identify a key role for platelet-released heat shock protein 90α (HSP90α) in driving neutrophil extracellular trap (NET) formation and supporting thromboinflammation during sepsis. Proteomic analysis of platelets from patients with sepsis showed a significant increase in HSP90α, which we traced back to trafficking pathways originating from megakaryocytes. When activated, platelets transloca

Plasma proteomic profiles of lung volume-based phenotypes in tobacco-exposed individuals without spirometric chronic obstructive pulmonary disease.

BACKGROUND: Among individuals with a history of smoking but preserved spirometry (tobacco exposed with preserved spirometry, or TEPS), lung volume-based stratification identifies 2 phenotypes at increased risk for chronic obstructive pulmonary disease (pre-COPD): those with a relatively elevated total lung capacity ([TLC]high) and those with a relatively elevated functional residual capacity-to-TLC ratio ([FRC/TLC]high). These subgroups exhibit distinct respiratory symptoms, radiographic abnormalities, and clinical trajectories. OBJECTIVE: This study aimed to determine whether these lung volu

Engineering CAR T cells to secrete VEGF-neutralizing scFvs enhances antitumor activity against solid tumors.

Chimeric antigen receptor (CAR) T cell therapy has shown limited efficacy against solid tumors, which often reside in highly immunosuppressive tumor microenvironments (TMEs). TMEs can be highly abundant in vascular endothelial growth factor A (VEGF), which contributes to immunosuppression and abnormal tumor vasculature. Here, we found that CAR T cells engineered to secrete an anti-VEGF single-chain variable fragment (CAR-αVEGF T cells) achieved superior antitumor efficacy against multiple in vivo models of ovarian cancer and glioma, outperforming conventional CAR T cells with and without combi

Alkaline Phosphatase and Infantile GM1 Gangliosidosis: A Simple Biomarker for a Complex Disease?

GM1 gangliosidosis is a lysosomal storage disease (LSD) caused by β-galactosidase deficiency, characterized by the accumulation of gangliosides in various tissues. Among different GM1 forms (infantile form, late-infantile and juvenile form, and late-onset form), the infantile form is the most severe: despite an early clinical onset with rapid neurodegeneration, coarse face, abdominal visceromegaly and skeletal abnormalities, the diagnosis is usually delayed, given the lack of recognized early disease-specific markers. We report the case of a newborn presenting with mild edema of hands and feet

Immune-metabolic plasticity in AML: Prognostic roles of aCTLs (activated Cytotoxic T Cells) and Kynurenine.

Acute myeloid leukemia (AML), traditionally risk-stratified by genetic abnormalities, is an aggressive malignancy with high relapse and modest survival. Immune dysregulation is an emerging pathogenic mechanism. We investigated cellular immune subsets and metabolic mediators of the PD-L1/PD-1 and IDO1-kynurenine-AhR pathways, including vitamin D as an immunomodulatory component in 78 newly diagnosed adult AML uniformly treated with intensive therapy. Lower TLC (<30 × 109/L) and blast percentage (<67%) were associated with higher CR rates (OR=3.9, p = 0.006; OR=2.9, p = 0.025). Higher baseline a

Effects of Cannabidiol on TAFAZZIN-Deficient B-Lymphoblastoid Cells.

Barth Syndrome (BTHS) is a debilitating X-linked genetic disorder caused by mutations in the gene encoding TAFAZZIN, an enzyme responsible for the remodeling of cardiolipin. While cyclic neutropenia is a well-recognized immunological feature of this disease, emerging evidence suggests that lymphopenia may also occur. The objective of this study was to examine the effects of cannabidiol (CBD) on growth, cardiolipin content, and mitochondrial abnormalities in BTHS patient-derived B-lymphoblastoid cells. CBD (1 μM) restored the growth of BTHS B-lymphoblastoids to healthy control levels, but did n

Identification of alterations of immunometabolism associated with Pompe disease.

Pompe disease (PD) is a neuromuscular lysosomal storage disorder caused by mutations in the GAA gene, characterized by progressive glycogen accumulation in multiple tissues and autophagy and metabolic abnormalities. While immunological changes have largely been overlooked as part of PD's symptomatology, autophagy and metabolic regulation are crucial in immune cell function. High incidence of immune reactions against therapeutic recombinant GAA (rhGAA) in PD patients suppose an important hindrance to treatment efficacy, yet the impact of GAA deficiency on the immune system remains unclear. Here

Case Report: Transient severe T cell lymphopenia in a patient with Cornelia de Lange Syndrome captured by TREC screening.

Cornelia de Lange Syndrome (CdLS) is a rare multisystem disorder characterized by craniofacial dysmorphism, growth restriction, limb anomalies, intellectual disability, and mild to moderate immune abnormalities. We present the case of a newborn female with CdLS who was found to have severe transient T-cell lymphopenia following abnormal newborn T-cell receptor excision circle (TREC) screening. She was treated with immunoglobulin replacement and antimicrobial prophylaxis. She experienced normalization of T cells and no severe infections over a two-year period. To our knowledge, this is the firs

Persistent T cell phenotypic alterations and early innate immune dysregulation as potential biomarkers of Long COVID.

Long COVID is a complex condition characterized by a broad spectrum of persistent, multisystemic symptoms that often impair daily activities and cause disability with significant socioeconomic impact. Although several hypotheses have been proposed to explain its pathophysiology-including immune dysregulation-the underlying mechanisms remain poorly understood. Indeed, there are no current treatments for this condition. Therefore, we aimed to characterize immune alterations induced by SARS-CoV-2 during the acute phase of infection and three months post-hospital discharge in a cohort of patients

A laboratory puzzle in a "blue" adolescent: Cyanosis, discordant oxygen measurements, and interference in leukocyte differential analysis.

Methemoglobinemia caused by hemoglobin M variants is a rare inherited hemoglobinopathy characterized by lifelong cyanosis and discordant oxygen measurements. We report an 18-year-old male presenting with marked cyanosis, chocolate-brown blood, markedly reduced arterial oxygen saturation, normal arterial partial pressure of oxygen, and elevated methemoglobin levels. Complete blood count analysis on a Sysmex XN hematology analyzer revealed a global downward shift of fluorescence signals on the leukocyte differential scattergram with apparent pseudoeosinophilia, whereas parallel analysis on an AD
