Late-onset polyvinylpyrrolidone allergy in a long-term hemodialysis patient: a case report of dialysis hypotension and gastrointestinal symptoms.

Polysulfone membranes are widely used in hemodialysis due to their excellent biocompatibility, with polyvinylpyrrolidone (PVP) serving as a key hydrophilic component. While PVP-related allergic reactions typically occur during initial exposure, delayed reactions in long-term dialysis patients present a unique diagnostic challenge due to their unexpected nature and similarity to routine complications. We report a case of delayed PVP allergy in a 69-year-old male who developed severe intradialytic hypotension and gastrointestinal symptoms after 13 months of stable hemodialysis treatment. Blood v

Prevalence and clinical implications of duodenal eosinophilia in patients with refractory functional dyspepsia: A cross-sectional analysis.

BACKGROUND AND OBJECTIVES: Functional dyspepsia (FD) is a common disorder with multi-factorial pathophysiology. It has two sub-types, post-prandial distress syndrome (PDS) and epigastric pain syndrome (EPS), which frequently overlap. Emerging evidence suggests that low-grade inflammation, particularly duodenal eosinophilia (DE), may play a pathogenic role in FD. However, Indian data on this subject remains scarce. This study aimed at evaluating the prevalence and clinical correlates of DE in patients with refractory FD. METHODS: In this prospective, cross-sectional study, Rome- IV defined FD

Drug Reaction with Eosinophilia and Systemic Symptoms Following Antibiotic Cement Spacer Placement: A Case Report.

CASE: A 44-year-old woman with an infected pilon fracture nonunion developed Drug Reaction with Eosinophilia and Systemic Symptoms (DRESS) after prolonged intravenous vancomycin and implantation of a vancomycin/tobramycin-loaded antibiotic cement spacer (ACS). Fevers, rash, and eosinophilia persisted despite stopping systemic vancomycin and starting corticosteroids. ACS explantation led to symptom resolution; however, severe thrombocytopenia developed, requiring therapeutic plasma exchange. A daptomycin-loaded spacer was reimplanted before definitive hindfoot fusion. CONCLUSION: This case of

Significance of monocyte-to-lymphocyte ratio as a prognostic factor in canine urothelial carcinoma: a retrospective study.

Canine urothelial carcinoma (cUC) is an aggressive malignancy, yet clinically accessible and reliable prognostic markers remain limited. While systemic inflammatory responses are increasingly recognized as critical determinants of cancer progression in humans, the prognostic value of white blood cell fractions in cUC remains unexplored. This retrospective cohort study investigated the association between white blood cell fractions and survival in 86 dogs with urothelial carcinoma (cUC) diagnosed at a university-affiliated veterinary hospital between 2017 and 2024. Case assignment, presence of

Symptomatic Hypercalcemia with Vomiting in a Pediatric Patient with Graves' Disease.

Graves' disease (GD) is the leading cause of childhood hyperthyroidism, resulting from excessive thyroid hormone production. In some cases, it can cause alterations in mineral homeostasis, including calcium, phosphorus, and magnesium, which are often overlooked. Hyperthyroidism increases osteoclastic bone resorption, and mild to moderate hypercalcemia occurs in approximately 20-50% of affected patients, typically resolving with appropriate therapy. Although uncommon, symptomatic hypercalcemia in the setting of GD requires immediate evaluation and management. An 8-year-11-month-old girl was bro

Heart Transplantation: Focus on Rejection and the Rare Complication of Eosinophilic Myocarditis.

Many patients with heart failure (HF) progress to an advanced stage, characterized by persistent symptoms despite maximal therapy. Heart transplantation (HT) remains the most effective treatment option for improving the survival of patients with advanced HF. This is true even with advancements in medical therapy for HF and the development of mechanical circulatory support systems. Over the past few decades, HT has undergone significant evolution, leading to greatly improved success rates. However, HT recipients face the risk of several potential complications that can negatively impact their o

Intraperitoneal Paclitaxel-Induced Eosinophil Recruitment as a Potential Mediator of Tumor Response in Peritoneal Metastases from Gastric Cancer.

BACKGROUND AND PURPOSE: The peritoneal cavity constitutes a unique immune microenvironment that critically influences the pathobiology of peritoneal metastasis (PM). This study aimed to clarify the mechanisms by which local immune alterations affect the efficacy of intraperitoneal (IP) chemotherapy for PM from gastric cancer (GC). METHOD: Peritoneal lavage or ascitic fluid was obtained from 42 patients with GC and PM treated with IP paclitaxel (PTX) in combination with systemic oxaliplatin and oral S-1. Serial samples from 31 patients were analyzed after 1-3 cycles of chemotherapy. Immune cel

Hematologic malignancies in pediatric patients with RUNX1-familial platelet disorder with associated myeloid malignancy.

Deleterious germ line RUNX1 variants cause familial platelet disorder with associated myeloid malignancy (FPDMM), characterized by thrombocytopenia, platelet dysfunction, and predisposition to hematologic malignancies (HM). We examined clinical and laboratory findings of pediatric (<19 years) patients with RUNX1-FPDMM enrolled in the National Institutes of Health RUNX1 Natural History Study. Pediatric patients have thrombocytopenia (131 ± 49 ×103/μL vs 314 ± 55 ×103/μL) and mild eosinophilia (372 ± 244/μL vs 165 ± 100/μL) similar to adults. Abnormal bleeding is less frequent in children than a

EGPA presenting as sudden cardiac arrest: a case report and review of cardiac manifestations.

Eosinophilic granulomatosis with polyangiitis (EGPA) is a rare systemic vasculitis in which cardiac involvement is a primary cause of mortality. Complete heart block presenting as Adams-Stokes syndrome is a rare but critical complication. Notably, to our knowledge, EGPA initially manifesting as Adams-Stokes syndrome has not been previously documented, based on a comprehensive review of the literature. We report a 27-year-old female presenting with recurrent syncope and seizures. Laboratory tests revealed significant eosinophilia (49.5%), and cardiac workup confirmed third-degree atrioventricul

Leukocytosis and a JAK2 mutation: The importance of expertise in somatic variant interpretation.

This case details a patient presenting with a history of chronic leukocytosis with isolated eosinophilia, who was found to have a JAK2 G571S mutation by a next generation sequencing (NGS) panel for myeloproliferative neoplasm driver mutations. Adjunct NGS testing was performed on a sample of buccal cells to demonstrate this variant was a germline alteration, occurring in the absence of additional disease-causing mutations. The result caused the patient to be re-treated for a prior Strongyloides infection with ivermectin, which resolved his eosinophilia. This patient case highlights the likely

Catastrophic antiphospholipid syndrome with eosinophilia mimicking hypereosinophilic syndromes with disseminated intravascular coagulation: a case report.

BACKGROUND: Catastrophic antiphospholipid syndrome is a rare variant of antiphospholipid syndrome where the presence of antiphospholipid antibodies and systemic inflammation leads to the rapid onset of multifocal thrombosis. Eosinophilia is known to accompany conditions such as allergy, parasite infection, malignancy, or autoimmune diseases; however, catastrophic antiphospholipid syndrome with eosinophilia has not been reported and its clinical influence remains unknown. We describe the first case of probable catastrophic antiphospholipid syndrome with eosinophilia, which mimicked eosinophil-a
