[T-cell lymphoproliferative disorders: Epidemiology, pathophysiology, and diagnosis].

Mature T-cell lymphoproliferative disorders are rare and aggressive hematologic malignancies characterized by marked clinical and biological heterogeneity. Their incidence is rising worldwide, with significant geographic disparities, particularly between Western countries and Asia. Several environmental, occupational, and infectious risk factors have been suggested, although no causal link has been firmly established. Diagnosis is often delayed due to nonspecific initial presentations and requires close collaboration among hematologists, internists, pathologists, and other specialists. The 202

Newborn screening for severe combined immunodeficiency in the NHS newborn blood spot screening programme: a systematic review.

BACKGROUND: Severe combined immunodeficiency is an inherited condition arising from mutations in at least 19 known genes. Severe combined immunodeficiency can be identified through screening, family history or clinical presentation. Severe combined immunodeficiency is usually asymptomatic at birth and presents, in infancy, as recurrent and frequently severe infections. Without treatment, severe combined immunodeficiency is usually fatal in the first year of life. OBJECTIVES: To summarise the available evidence relevant to newborn screening for severe combined immunodeficiency in the UK NHS ne

Peripheral biochemical parameters for discrimination between bipolar disorder and major depressive disorder in female patients of reproductive age: a CRP-stratified exploratory study.

BACKGROUND: Bipolar disorder (BD) is often misdiagnosed as major depressive disorder (MDD) due to overlapping clinical syndromes. Although numerous studies have investigated biomarkers to differentiate these two disorders, no clinically applicable differential diagnostic panel has been established. Notably, sex-specific differences play a critical role in this context. Females are more prone to experience depressive episodes and exhibit more pronounced immune-inflammatory dysregulation, which further increases the risk of misdiagnosis. This study aimed to explore whether C-reactive protein (CR

Serum sPD-1 and sTIM-3 as T-cell exhaustion biomarkers in systemic lupus erythematosus: The IL-6-associated exhaustion axis and the SLE Immune Exhaustion Index.

BACKGROUND: T-cell exhaustion marked by soluble PD-1 (sPD-1) and soluble TIM-3 (sTIM-3) has been implicated in systemic lupus erythematosus (SLE), yet the cytokine mechanisms driving checkpoint-receptor upregulation, the relationship with complement consumption, and the clinical utility of these markers remain incompletely characterised. METHODS: In a case-control with cross-sectional study of 120 participants (40 active SLE, 40 quiescent SLE, 40 healthy controls), serum sPD-1, sTIM-3, IL-6, ferritin, complement proteins (C3, C4, C1q), anti-dsDNA, CRP, ESR, and urinary protein-to-creatinine r

Reduced FOXP3 expression and its association with genetic ancestry in neuromyelitis Optica spectrum disorder: a Colombian cohort.

BACKGROUND: Autoimmune disorders are characterized by impaired immune tolerance, largely mediated by CD4⁺ regulatory T cells (Tregs), whose function depends on the transcription factor FOXP3. OBJECTIVES: To compare FOXP3 expression levels between patients with neuromyelitis optica spectrum disorder (NMOSD) and healthy controls from Bogotá, Colombia, and to explore their association with genetic ancestry. METHODS: FOXP3 expression was quantified from peripheral blood mononuclear cells using RNA-based analysis. Genomic ancestry proportions were estimated using ancestry-informative markers. RE

Association between leukocyte telomere length and neurodegenerative diseases: a prospective cohort in the UK Biobank.

BACKGROUND: This study aims to investigate the association between leukocyte telomere length (LTL) and the risk of incident NDDs using a large-scale cohort from the UK Biobank. METHODS: Data from 459,902 subjects were analyzed using Cox proportional hazards models and machine learning (ML) algorithms to assess LTL's association with NDD risk. RESULTS: Shorter LTL was associated with an increased risk of NDDs, including Alzheimer's disease (HR: 0.52, 95% CI 0.40-0.67, P < 0.001), dementia in AD (HR: 0.53, 95% CI 0.39-0.73, P < 0.001), unspecified dementia (HR: 0.74, 95% CI 0.58-0.95, P < 0.05

Diagnostic challenges of the idiopathic plasmacytic lymphadenopathy (IPL) subtype of idiopathic multicentric Castleman disease (iMCD): Factors to differentiate from IgG4-related disease.

AIMS AND METHODS: Idiopathic multicentric Castleman disease (iMCD) is currently considered to be classified into three clinical subtypes, including idiopathic plasmacytic lymphadenopathy (IPL), thrombocytopaenia, anasarca, fever, reticulin fibrosis/renal dysfunction, organomegaly (TAFRO) and not otherwise specified (NOS). Among the three, iMCD-IPL closely mimics IgG4-related disease (IgG4-RD). In diagnosing IgG4-RD, it is sometimes challenging to distinguish iMCD-IPL patients that also meet the histological diagnostic criteria for IgG4-RD. In this study, we focused on the number of IgG4-positi

Personalized, autologous neoantigen-specific T cell therapy in metastatic melanoma: a phase 1 trial.

New treatment approaches are warranted for patients with advanced melanoma refractory to immune checkpoint blockade (ICB) or BRAF-targeted therapy. We designed BNT221, a personalized, neoantigen-specific autologous T cell product derived from peripheral blood, and tested this in a 3 + 3 dose-finding study with two dose levels (DLs) in patients with locally advanced or metastatic melanoma, disease progression after ICB, measurable disease (Response Evaluation Criteria in Solid Tumors version 1.1) and, where appropriate, BRAF-targeted therapy. Primary and secondary objectives were evaluation of

Untargeted proteomics enables ultra-rapid variant prioritisation in mitochondrial and other rare diseases.

BACKGROUND: Only half of individuals with suspected rare diseases receive a genetic diagnosis following genomic testing. A genetic diagnosis allows access to appropriate care, restores reproductive confidence and reduces the number of potentially unnecessary interventions. A major barrier is the lack of disease agnostic functional tests suitable for implementation in routine diagnostics that can provide evidence supporting pathogenicity of novel variants, especially those refractory to RNA sequencing. METHODS: Focusing on mitochondrial disease, we describe an untargeted mass-spectrometry base

The Potential Role of Serum Tissue Inhibitor of Metalloproteinase-2 (TIMP-2) as a Biomarker of Fibrosis in Patients With Metabolic Dysfunction-Associated Fatty Liver Disease (MAFLD).

BACKGROUND: Activation of hepatic stellate cells (HSCs) in response to liver injury increases extracellular matrix (ECM) deposition and upregulation of tissue inhibitor of metalloproteinase-2 (TIMP-2), promoting fibrogenesis, a key feature of metabolic dysfunction-associated fatty liver disease (MAFLD). This study was conducted to determine the diagnostic potential of serum TIMP-2 levels in patients with MAFLD/nonalcoholic fatty liver disease (NAFLD). METHODS: This case-control study included 100 MAFLD patients and 100 healthy controls. Demographic profiling, ultrasonography, and FibroScan wi

Follicular Helper T-Cell Lymphomas With Epstein-Barr Virus-Positive Neoplastic Cells: A Rare Scenario With Diagnostic Implications.

Follicular helper T-cell lymphomas (TFHLs) of the angioimmunoblastic type (AITL) and other TFHL variants often contain Epstein-Barr virus (EBV)-positive B blasts, but EBV infection of the neoplastic T cells has rarely been documented. Here, we report 10 cases of TFHL (9 AITLs and 1 TFHL not otherwise specified) associated with EBV infection in neoplastic T cells. The patients (5 men and 5 women), aged 56 to 81 years, presented with polyadenopathy (8/8), B symptoms (7/7), and skin lesions (4/7). EBV was confirmed in neoplastic follicular helper T-cell (TFH) by double labeling for EBV, T-cell ma

Secondary Cardiorenal Syndrome in a Cohort of Septic Patients Treated in a Medical Intensive Care Unit: A Single-Center Experience.

Context Sepsis is a life-threatening condition caused by an abnormal host response to infection, resulting in multiple organ failure and high mortality rates. Secondary or type five cardiorenal syndrome (CRS) emerges as a consequence of systemic infection, resulting in both cardiac and renal dysfunction. The synergistic impact of inflammation and hemodynamic instability results in an adverse cycle of vital organs damage during which renal impairment further aggravates cardiac dysfunction and vice versa. Objectives The objective of this study is to assess the influence of specified demographic,

SOX17 enables immune evasion of early colorectal adenomas and cancers.

A hallmark of cancer is the avoidance of immune destruction. This process has been primarily investigated in locally advanced or metastatic cancer1-3; however, much less is known about how pre-malignant or early invasive tumours evade immune detection. Here, to understand this process in early colorectal cancers (CRCs), we investigated how naive colon cancer organoids that were engineered in vitro to harbour Apc-null, KrasG12D and Trp53-null (AKP) mutations adapted to the in vivo native colonic environment. Comprehensive transcriptomic and chromatin analyses revealed that the endoderm-specifyi

Differential activity of MAPK signalling defines fibroblast subtypes in pancreatic cancer.

Fibroblast heterogeneity is increasingly recognised across cancer conditions. Given their important contribution to disease progression, mapping fibroblasts' heterogeneity is critical to devise effective anti-cancer therapies. Cancer-associated fibroblasts (CAFs) represent the most abundant cell population in pancreatic ductal adenocarcinoma (PDAC). Whether CAF phenotypes are differently specified by PDAC cell lineages remains to be elucidated. Here, we reveal an important role for the MAPK signalling pathway in defining PDAC CAF phenotypes. We show that epithelial MAPK activity promotes the m

A CRISPR/Cas9-mediated screen identifies determinants of early plasma cell differentiation.

INTRODUCTION: The differentiation of B cells into antibody-secreting plasma cells depends on cell division-coupled, epigenetic and other cellular processes that are incompletely understood. METHODS: We have developed a CRISPR/Cas9-based screen that models an early stage of T cell-dependent plasma cell differentiation and measures B cell survival or proliferation versus the formation of CD138+ plasmablasts. Here, we refined and extended this screen to more than 500 candidate genes that are highly expressed in plasma cells. RESULTS: Among known genes whose deletion preferentially or mostly aff

Understanding the Roles of the Hedgehog Signaling Pathway during T-Cell Lymphopoiesis and in T-Cell Acute Lymphoblastic Leukemia (T-ALL).

The Hedgehog (HH) signaling network is one of the main regulators of invertebrate and vertebrate embryonic development. Along with other networks, such as NOTCH and WNT, HH signaling specifies both the early patterning and the polarity events as well as the subsequent organ formation via the temporal and spatial regulation of cell proliferation and differentiation. However, aberrant activation of HH signaling has been identified in a broad range of malignant disorders, where it positively influences proliferation, survival, and therapeutic resistance of neoplastic cells. Inhibitors targeting t

The emerging family of RORγt+ antigen-presenting cells.

Antigen-presenting cells (APCs) are master regulators of the immune response by directly interacting with T cells to orchestrate distinct functional outcomes. Several types of professional APC exist, including conventional dendritic cells, B cells and macrophages, and numerous other cell types have non-classical roles in antigen presentation, such as thymic epithelial cells, endothelial cells and granulocytes. Accumulating evidence indicates the presence of a new family of APCs marked by the lineage-specifying transcription factor retinoic acid receptor-related orphan receptor-γt (RORγt) and d

Follicular helper T-cell lymphomas: disease spectrum, relationship with clonal hematopoiesis, and mimics. A report of the 2022 EA4HP/SH lymphoma workshop.

Follicular helper T-cell lymphomas (TFH lymphomas) were discussed in session V of the lymphoma workshop of the European Association for Haematopathology (EA4HP)/Society for Hematopathology (SH) 2022 meeting in Florence, Italy. The session focused on the morphologic spectrum of TFH lymphoma, including its three subtypes: angioimmunoblastic-type (AITL), follicular-type, and not otherwise specified (NOS). The submitted cases encompassed classic examples of TFH lymphoma and unusual cases such as those with early or indolent presentations, associated B-cell proliferations, or Hodgkin/Reed-Sternberg

Evaluation of QuantiFERON SARS-CoV-2 interferon-γ release assay following SARS-CoV-2 infection and vaccination.

T cells are important in preventing severe disease from SARS-CoV-2, but scalable and field-adaptable alternatives to expert T-cell assays are needed. The interferon-gamma release assay QuantiFERON platform was developed to detect T-cell responses to SARS-CoV-2 from whole blood with relatively basic equipment and flexibility of processing timelines. Forty-eight participants with different infection and vaccination backgrounds were recruited. Whole blood samples were analysed using the QuantiFERON SARS-CoV-2 assay in parallel with the well-established 'Protective Immunity from T Cells in Healthc
