An exploratory simulation of the optimization of leukocyte transmigration, indicating that barrier routing is governed by context-dependent biochemical cues rather than hardwired programs.

INTRODUCTION: Leukocyte infiltration across the blood-brain barrier contributes to many neuroinflammatory disorders, yet the physical mechanisms that determine how immune cells cross the barrier remain incompletely understood. In particular, it is unclear how leukocytes choose between paracellular migration through endothelial junctions and transcellular migration through the endothelial cell body. This question is especially relevant during central nervous system inflammation, where different T-lymphocyte subsets, including Th1 and Th17 cells, appear to preferentially use distinct migration r

A natural neuronal receptor-derived MRI probe targets β-amyloid oligomers for the diagnosis of Alzheimer's disease.

Early-to-mid detection of Alzheimer's disease (AD) remains challenging because clinically used amyloid positron emission tomography (PET) probes primarily visualize fibrillar β-amyloid (Aβ) deposits and require radioactive isotopes. Magnetic resonance imaging (MRI) offers a radiation-free alternative, but its application in molecular AD diagnosis is limited by insufficient blood-brain barrier (BBB) penetration and poor specificity toward early-to-mid Aβ species. Here, we developed a BBB-crossing liposomal MRI probe, Lipo@D1D2-Gd, by integrating a natural Aβ oligomer-binding receptor domain wit

Lessons learned: Digital epidemiology of eosinophilic esophagitis and what Google Trends reveals.

Eosinophilic esophagitis (EoE) is a chronic, immune-mediated disease whose incidence and prevalence have continued to rise, yet little is known about how public awareness has evolved alongside clinical progress. We used Google Trends, a freely available tool that indexes the relative popularity of search queries from 0 to 100, to examine interest in the terms 'eosinophilic esophagitis' and 'EoE' in the United States from January 2004 through December 2025, and to compare the disease term with searches for regulatory-approved EoE-relevant therapies (dupilumab/Dupixent, budesonide/Eohilia, and b

Paediatric Helicobacter pylori infection and allergic and inflammatory diseases: a contemporary systematic review with meta-analysis.

UNLABELLED: Helicobacter pylori (H. pylori) infection is prevalent worldwide, and its potential protective role in allergic and immune diseases has been a topic of speculation. This systematic review aims to analyse recent literature to establish a consensus on this relationship. Articles published in the past 12 years regarding association of H. pylori and various allergic diseases were screened. Thirty-five articles from three databases were assessed for bias using the ROBINS-E tool. Objective data from each article were collected for this review. Among the reviewed articles, 14 focused on a

Type 2 Inflammation in Airway Diseases: Eosinophilic Bronchiectasis as an Emerging Phenotype.

Bronchiectasis is a chronic and highly heterogeneous airway disease traditionally regarded as a neutrophil-dominated condition driven by persistent infection and dysregulated innate immunity. Over the last decade, this traditional view has been increasingly challenged by the identification of distinct inflammatory phenotypes with important biological and therapeutic implications.Among these, eosinophilic bronchiectasis has emerged as a clinically meaningful and mechanistically distinct phenotype, characterized by type 2 (T2) immune activation, eosinophil-driven airway inflammation, and differe

Deep Learning Blood Dosimetry Predicts Severe Lymphopenia and Survival After Craniospinal Irradiation.

PURPOSE: Craniospinal irradiation (CSI) is a standard treatment for pediatric brain tumors and is increasingly used for leptomeningeal metastases. Although effective, CSI often causes radiation-induced severe lymphopenia (RISL), which is associated with poor outcomes across various cancer types. We developed and validated a novel platform for personalized dosimetry of circulating blood cells (CBCs) and assessed its predictive value for RISL. METHODS AND MATERIALS: We combined a prospective pediatric CSI registry (initiated April 2018; n = 48) and retrospectively collected patients (January 20

Toxoplasma serology: Breaking the diagnostic boundary of schizoaffective disorder? Insights from a comparative peripheral immune marker analysis.

BACKGROUND: Toxoplasma gondii (T. gondii) infection has been implicated in the pathophysiology of psychiatric disorders, yet no study has compared its seroprevalence between schizoaffective disorder (SZA) and other psychiatric conditions. This study aimed to compare T. gondii seroprevalence between patients with SZA, schizophrenia (SZ), and bipolar disorder (BD), and to examine whether seropositivity differentially affects innate immune markers across these diagnoses. METHODS: Overall 144 patients were included in this cross-sectional study. T. gondii seroprevalence was determined and leukocy

Prospective phase I study of nivolumab to promote graft-versus-tumour in relapsed or refractory haematological malignancy post allogeneic haematopoietic stem cell transplantation.

Allogeneic haematopoietic stem cell transplantation (alloHSCT) relies on donor-derived T cells to mediate the graft-versus-tumour (GVT) effect, but relapse may result from impaired immune reconstitution, tumour antigen downregulation or T-cell exhaustion. This prospective phase I study evaluated the safety and efficacy of the PD-1 inhibitor, nivolumab, in enhancing GVT post-alloHSCT and explored the impact of baseline T-cell phenotypes on clinical outcomes. Fourteen patients with relapsed/refractory haematological malignancies post-alloHSCT received nivolumab every 2 weeks for up to 48 weeks.

Symptom clusters and help-seeking behaviours during neutropenia after autologous haematopoietic stem cell transplantation: A qualitative study to inform oncology nursing support.

PURPOSE: To explore how adult autologous haematopoietic stem cell transplantation recipients experience and appraise symptom constellations during neutropenia, and how these appraisals shape help-seeking and interactions with clinicians. METHODS: A qualitative descriptive study was conducted in an inpatient haematopoietic stem cell transplantation unit in Wuxi, China. Adults aged 18 years or older who had undergone autologous transplantation and were neutropenic were purposively sampled to maximise variation. Semi-structured interviews were audio-recorded, transcribed verbatim, and analysed u

IRF7 signalling in tanycytes controls brain-body-immune response across the ovarian cycle.

Sex-specific differences in metabolic and immune function are shaped by endocrine-immune interactions, yet the complexity of this relationship remains poorly understood. Here, we identify an immune cell-like state of tanycytes, an ependymoglial cell type lining the third ventricle that mediates brain-body communication1,2 in both mice and humans. Using single-cell transcriptomics, we provide a high-resolution cellular map of the mouse and human mediobasal hypothalamus with cross-species comparison of tanycytes. We capture dynamic sex-, oestrous cycle- and high-fat diet-dependent transcriptomic

[Livedoid vasculopathy: A case report].

Livedoid vasculopathy (LV) is a painful and thrombotic obliterative vascular disorder characterized by spontaneous thrombus formation within skin capillaries or small veins, ultimately leading to localized hypoxia and the development of skin ulcers. LV is a rare condition predominantly affecting young women, with an incidence rate of approximately 1/100 000. Clinically, it manifests as recurrent reticular green spots and painful ulcers on the distal lower extremities, which subsequently heal, leaving behind white and atrophic scars, termed "white atrophy". The skin manifestations of the diseas

Possible CPX-351-associated drug reaction with eosinophilia and systemic symptoms/drug-induced hypersensitivity syndrome-like reaction during severe lymphopenia.

Drug reaction with eosinophilia and systemic symptoms (DRESS), also known as drug-induced hypersensitivity syndrome (DIHS), is a severe delayed drug reaction characterized by skin eruption, systemic inflammation, organ involvement, and human herpesvirus 6 (HHV-6) reactivation. CPX-351 is increasingly used for high-risk acute myeloid leukemia, and skin eruptions are common adverse events; however, CPX-351-associated DRESS/DIHS-like reactions have not been well described. We report a 62-year-old man with acute erythroid leukemia who developed a progressive eruption after CPX-351 therapy. An earl

Clinical characteristics of patients with overlapping eosinophilic esophagitis and inflammatory bowel disease in a national real-world cohort.

Eosinophilic esophagitis (EoE) and inflammatory bowel disease (IBD) are immune-mediated gastrointestinal disorders with a recognized epidemiologic association, yet the clinical phenotype of overlapping disease remains incompletely characterized. We performed a retrospective cohort study using the All of Us Research Program Controlled Tier dataset mapped to the Observational Medical Outcomes Partnership common data model. Adults with EoE were identified using International Classification of Diseases, Ninth Revision (ICD-9), International Classification of Diseases, Tenth Revision (ICD-10), and

Late-onset ankylosing spondylitis presenting with severe flank and abdominal-to-back pain in a 64-year-old man: a case report.

BACKGROUND: Ankylosing spondylitis (AS) is a chronic inflammatory disease that primarily affects the spine and sacroiliac joints and is typically diagnosed in younger adults. However, AS may be overlooked in older patients when symptoms are atypically localized, such as flank or abdominal-to-back pain, rather than classic inflammatory low back pain. CASE DESCRIPTION: We report the case of a 64-year-old man who presented with persistent left-sided abdominal-to-back pain accompanied by markedly elevated inflammatory markers. Initial evaluation focused on abdominal, urinary, infectious, malignan

Eosinophilic Gastroenteritis during Tocilizumab Treatment for Castleman Disease.

Eosinophilic gastroenteritis (EGE) is a rare inflammatory disorder characterized by eosinophilic infiltration of the gastrointestinal tract and its associated symptoms. We report a case of EGE in a patient with idiopathic multicentric Castleman disease and asthma, who developed gastrointestinal symptoms and eosinophilia during tocilizumab therapy. Endoscopy revealed marked gastric mucosal injury and biopsy confirmed eosinophilic infiltration. Despite normal intestinal endoscopic findings, the biopsy specimens revealed eosinophilic infiltration. The discontinuation of tocilizumab and initiation

Functional Clinical Flow Cytometry Assays to Assess Lymphocyte Proliferation, Neutrophil Oxidative Burst, and Leukocyte Adhesion Deficiency Immune Defects.

Inborn errors of immunity (IEI) are a diverse group of inherited disorders caused by detrimental variants in the genome that compromise the immune system and lead to an increased susceptibility to infections, autoimmunity, and/or malignancy. Although the diagnostic process traditionally begins with a clinical workup and laboratory screening, such as complete blood counts, serum immunoglobulin measurements, and lymphocyte subset enumeration, functional assays serve as a vital bridge to diagnosis by providing a real-time assessment of immune system performance. Such assays include lymphocyte pro

Beyond the Induction Phase: Should We Keep the Proton Pump Inhibitor in Pediatric Eosinophilic Esophagitis?

BACKGROUND: Proton pump inhibitors (PPIs) are a common and effective management strategy for eosinophilic esophagitis (EoE) leading to successful remission in most cases. However, there is uncertainty regarding the ideal follow-up pharmacological treatment to maintain remission. We performed a cost-effectiveness and quality-adjusted life year (QALY) analysis. OBJECTIVES: To compare various approaches of pharmacologic treatments for EoE. METHODS: A Markov model was constructed to evaluate the cost-effectiveness and QALY of maintenance therapies for eosinophilic esophagitis, including PPI, top

Macrophages in non-infectious uveitis: cell origin, disease stage, evidence provenance, and therapeutic maturity.

Non-infectious uveitis (NIU) comprises a heterogeneous group of immune-mediated disorders in which myeloid cells influence leukocyte entry, inflammatory amplification, tissue injury, resolution, and relapse. The conventional dichotomy of classically activated (M1) and alternatively activated (M2) macrophages does not adequately capture this functional diversity. This narrative review organizes the evidence along three complementary dimensions: cell origin, disease stage, and evidence provenance; therapeutic maturity is evaluated separately. Experimental data support distinct roles for resident
