Assessing the Impact of Pandemic-Related Social Distancing Mandates on Bloodstream Infection Rates in Children With Chemotherapy-Induced Febrile Neutropenia.

Children with severe neutropenia are advised to avoid activities and public spaces where they may be exposed to potential sources of infection, due to the risk of neutropenic fever, an oncologic emergency. The influence of exposure to environmental infectious disease risk factors (IDRFs) on the incidence of bacteremia has been difficult to study. Lockdowns enforced during the COVID-19 pandemic resulted in widespread social distancing and were associated with reduced opportunities for exposure to such environmental risk factors. We performed a retrospective chart review to compare the incidence

Primary G-CSF prophylaxis in sacituzumab govitecan-treated mTNBC: real-world evidence from a multinational CEBCC-102 cohort.

BACKGROUND: Sacituzumab govitecan (SG) is a key treatment for metastatic triple-negative breast cancer (mTNBC). Neutropenia is a common toxicity, and the optimal use of G-CSF (primary prophylaxis versus reactive administration) remains uncertain. We evaluated the association of primary G-CSF prophylaxis with clinical outcomes and treatment-related toxicities. METHODS: We conducted a multinational retrospective real-world cohort study of 303 patients with mTNBC treated with SG in the second or a later line. Efficacy endpoints included progression-free survival (PFS) and overall survival (OS),

From orphan receptor to versatile regulator: Redefining LILRB3 as a context-dependent immune checkpoint and therapeutic target.

The immune system maintains homeostasis through a delicate balance between activation and inhibition. While activating pattern recognition receptors (PRRs) are well-established initiators of innate immunity, the role of inhibitory PRRs (iPRRs) in providing negative regulation is an emerging paradigm. Among these, Leukocyte Immunoglobulin-Like Receptor Subfamily B Member 3 (LILRB3), a myeloid-specific receptor, has recently undergone a dramatic conceptual transformation. Once considered a minor "orphan" receptor, LILRB3 is increasingly recognized as a context-dependent immune checkpoint with di

Expanded Umbilical Cord Blood Transplantation in Cerebral X-Linked Adrenoleukodystrophy: A Case Report.

Umbilical cord blood transplantation is a viable source of stem cells due to accessibility and low incidence of chronic graft-versus-host disease despite human-leukocyte-antigen mismatching. Disadvantages of low stem cell dose in larger recipients include delayed immune reconstitution, graft rejection, and mortality. Successful transplants with ex-vivo expanded cords resulted in approval in malignant disorders and aplastic anemia. We report the first successful expanded cord transplantation in an adolescent with cerebral adrenoleukodystrophy who had no alternative donor/treatment options and r

IFN-γ-driven skewing towards Th1 over Th17 differentiation underlies CRS and neutropenia in CAR-T therapy.

Chimeric antigen receptor T cell (CAR-T) therapy has led to significant improvements in patient survival. However, a subset of patients experience high-grade toxicities, including cytokine release syndrome (CRS) and immune cell-associated hematological toxicity (ICAHT). We utilized IL-2Ra knockout mice to model toxicities with elevated levels of IL-6, IFN-γ, and TNF-α and increased M1-like macrophages. Onset of CRS was accompanied by a reduction in peripheral blood neutrophils due to disruption of bone marrow neutrophil homeostasis characterized by an increase in apoptotic neutrophils and a de

Exploring the predictive power of a combined model of cellular indices and food allergies to screen for eosinophilic esophagitis in children.

Eosinophilic esophagitis (EoE) typically presents with dysphagia and is frequently linked to food allergies (FA), although esophagoscopy remains essential for diagnosis as no dependable diagnostic biomarkers are currently available. While cellular indices obtained from the hemogram have been evaluated as diagnostic markers in EoE, their utility when combined with FA history for screening EoE remains uninvestigated. This research explores the predictive power of combined models of cellular indices and FA history for EoE screening through a diagnostic study of patients <15 years old undergoing e

Fanning the flames: IFN-γ fuels CAR-T inflammation and cytopenia.

Chimeric antigen receptor T cell (CAR-T) therapy has transformed the treatment of hematologic malignancies, yet, severe inflammatory toxicities continue to limit its broader use. In this issue of the JCI, Goala et al. uncovered a mechanistic link between IFN-γ-driven inflammation and disrupted neutrophil homeostasis, revealing that cytokine release syndrome (CRS) and immune cell-associated hematologic toxicity (ICAHT) stem from a shared biological pathway. Using IL-2Ra-deficient mice and patient samples, they showed that IFN-γ suppressed IL-17A and granulocyte colony-stimulating factor (G-CSF)

Time Trends and Impact on Treatment Outcomes of Psychiatric Diagnoses in Patients with Eosinophilic Esophagitis.

BACKGROUND AND AIMS: While mental illness is common in eosinophilic esophagitis (EoE), it is unknown whether psychiatric diagnosis frequency has varied over time or impacts treatment outcomes. We aimed to determine time trends in psychiatric diagnoses in EoE and whether such diagnoses affect EoE treatment outcomes. METHODS: In this retrospective cohort study of newly diagnosed EoE patients, medical records were examined to determine psychiatric diagnoses and psychoactive medication use. For patients who also had treatment with either a topical corticosteroid (tCS) or diet elimination and had

Relationship Between Clozapine-Induced Inflammation and Eosinophilia: A Retrospective Cohort Study.

BACKGROUND AND HYPOTHESIS: Eosinophilia has not been highlighted in clozapine-induced adverse inflammatory events, as it is often asymptomatic and self-limiting, while drug reaction with eosinophilia and systemic symptoms (DRESS) syndrome occurs rarely. This study aimed to reveal the temporal relationships between eosinophilia and other inflammatory events during clozapine initiation. STUDY DESIGN: The temporal relationships between eosinophilia and other inflammatory events were evaluated among 241 patients with schizophrenia treated with clozapine for the first time at 7 hospitals. Risk fac

Hematologic Landscape of Adult Patients With Diamond-Blackfan Anemia Syndrome.

Information about Diamond-Blackfan anemia syndrome (DBAS), a ribosomopathy associated with anemia, congenital anomalies and cancer predisposition is limited in adults. Using the French DBAS registry, 235 adult patients with DBAS were analyzed for hematological outcomes. At last follow-up, anemia, neutropenia, thrombocytopenia, and pancytopenia affected respectively 78%, 31%, 15%, and 11% of the patients. There was no severe aplastic anemia outside of clonal evolution. Among patients without DBAS-specific treatment (e.g., steroids or red blood cell transfusions), the incidence of anemia, neutro
