Case Report: MR-guided adaptive stereotactic body radiotherapy for splenic metastases: a case series of technical feasibility and early clinical outcomes.

We present a case series of four patients with five splenic metastases treated with stereotactic body radiation therapy (SBRT) using a 1.5-T Unity (Elekta AB, Stockholm, Sweden) magnetic resonance linear accelerator (MR-Linac). While SBRT is a standard of care for the treatment of oligometastatic disease, splenic SBRT is not well established due to its rarity. MRI-guided adaptive radiotherapy (MRgART) offers superior soft tissue visualization and the unique ability to adapt treatment plans in real time. In our series, splenic oligometastases originated from primary malignancies of melanoma, co

Determinants of false-positive plasma free metanephrines and methoxytyramine: the role of medications and chronic disease.

Measurements of plasma free metanephrines provide a highly sensitive test for the diagnosis of pheochromocytoma/paraganglioma (PPGL). However, false-positive results remain common, posing a diagnostic dilemma for clinicians. The aim of this study was to determine whether chronic illness and commonly prescribed drugs can falsely elevate plasma free metanephrines. This retrospective study included 460 patients without PPGL. Disease exclusion was based on negative imaging or histopathological findings, or negative biochemistry at follow-up. Sex, age, plasma free normetanephrine, metanephrine, and

Autosplenectomy in a Patient With Autoimmune Polyglandular Syndrome Type 2 (APS-2).

Autoimmune glandular syndrome type 2 is a complex genetic condition where a triad of endocrinopathies is involved, namely, Addison's disease, type 1 diabetes, and/or autoimmune thyroid disorder. The disease predisposes one to a variety of other autoimmune associations. Here, we report a rare presentation of a patient with autoimmune polyglandular syndrome type 2 (APS-2) presenting with a 7-year history of progressive splenic atrophy causing functional hyposplenism that ultimately progressed to anatomical asplenia (autosplenectomy) as demonstrated in the serial imaging. We postulate that the un

Leukocytoclastic vasculitis of the extremities secondary to pancreatic cancer and Escherichia coli abscess; An unusual case of distinct etiologies in the same patient.

Leukocytoclastic vasculitis (LCV) is a disease secondary to infections, malignancies, or drug exposure, although its pathogenesis remains unclear. Secondary LCV from different causes in the same patient has been rarely reported. We present a case of LCV secondary to pancreatic cancer that remained in remission for one year post-surgery, after which the patient suffered from a subdiaphragmatic abscess caused by Escherichia coli, followed by recurrence of LCV. This case suggests that patients with acquired hyposplenism may be susceptible to developing LCV as a manifestation of underlying disease

Purpura Fulminans and Septic Shock With Streptococcus pneumoniae Bacteraemia in a Patient With Unacknowledged Functional Hyposplenism.

Purpura fulminans is a rare but devastating life-threatening condition characterised by a subtype of disseminated intravascular coagulation with extensive tissue thrombosis and hemorrhagic skin necrosis. We report the case of a male patient in his 50s, with unacknowledged functional hyposplenism suspected after an abdominal CT scan on admission, who was admitted in septic shock secondary to Streptococcus pneumoniae bacteraemia. The patient's condition deteriorated rapidly, leading to multiorgan failure, anuric acute kidney injury, disseminated intravascular coagulation with purpura fulminans,

A Fatal Case of Capsular Serovar D of Capnocytophaga canimorsus Infection Following a Dog Bite in a Previously Healthy Adult.

We report a rapidly fatal case of Capnocytophaga canimorsus septic shock with suspected acute infectious purpura fulminans (AIPF) in a 61-year-old man following a dog bite to the finger. Although he appeared previously healthy, retrospective analysis revealed a markedly small spleen (54 mm in diameter) on computed tomography and Howell-Jolly bodies on a peripheral blood smear, indicating unrecognized functional hyposplenism. He developed systemic symptoms within 48 h and progressed to respiratory failure and refractory shock within 1 h of presentation to the emergency department. The blood

Hyposplenism in systemic lupus erythematosus: biological and radiological characteristics and its association with increased risk of infection.

OBJECTIVE: This study aimed to evaluate the prevalence of hyposplenism in patients with systemic lupus erythematosus (SLE) and its association with an increased risk of infection. METHODS: This single-centre study included patients with SLE who underwent a systematic evaluation for Howell-Jolly bodies (HJB) on peripheral blood smear, a marker of hyposplenism. Patients exhibiting HJB were classified as having hyposplenism (cases), whereas those without such inclusions were considered to have normal splenic function (controls). We collected radiological data to assess spleen morphology. The pre

Evaluating functional hyposplenism in pediatric hodgkin lymphoma: the role of marginal zone b cells post-radiotherapy.

BACKGROUND: Immune deficiencies are common in Hodgkin lymphoma (HL) patients during and after treatment, and splenic irradiation is one of the potential causes. This study investigates long-term radiation-induced functional hyposplenism and immune dysfunction in pediatric patients with HL. METHODS: This cross-sectional study included 20 patients with HL who underwent splenic irradiation, 20 patients with splenectomy, and 20 healthy controls. Demographic and clinical data were collected, and venous blood samples were analyzed for immune dysfunction, including flow cytometry, and B-lymphocyte su

Tubular Aggregate Myopathies: Genetic Heterogeneity and Diverse Clinical Features Converging on Calcium Dysregulation.

Tubular aggregate myopathy (TAM) is a rare inherited muscle disorder characterized by the abnormal accumulation of tubular aggregates (TAs) within skeletal muscle fibers. These aggregates, composed of compacted sarcoplasmic reticulum (SR) tubules, are strongly linked to disturbances in calcium (Ca2+) homeostasis. Clinically, TAM manifests with slowly progressive proximal muscle weakness, exercise intolerance, cramps, and myalgia, frequently beginning in childhood and often present with elevated serum creatine kinase levels. These symptoms can also be associated with some additional disorders,

Inflammatory bowel disease (IBD)-including Crohn's disease, ulcerative colitis, and indeterminate colitis-is a chronic immune-mediated condition that primarily affects the intestinal mucosa but often presents with extraintestinal digestive manifestations, which are important yet frequently underrecognized sources of morbidity. These heterogeneous manifestations reflect diverse genetic, microbial, immunologic, and environmental influences, highlighting the value of a personalized medicine approach. Hepatobiliary involvement affects IBD adults patients and is even more common in children, rangin

Spontaneous Splenic Rupture in Vascular Ehlers-Danlos Syndrome: a Case Report Managed by Coil Embolization of the Splenic Artery.

Vascular Ehlers-Danlos Syndrome (VEDS) is a rare genetic disorder characterized by arterial fragility and a high risk of spontaneous vascular rupture. Splenic artery rupture is an uncommon but life-threatening manifestation of this condition. We report the case of a 34-year-old male with VEDS who presented with sudden abdominal pain and rapid progression to hemorrhagic shock. Imaging confirmed active bleeding from the distal splenic artery. After multidisciplinary evaluation, endovascular embolization via a left humeral approach was performed using multiple coils. The procedure achieved hemost

Probable Acquired Autosplenectomy in Systemic Lupus Erythematosus and Sjögren's Overlap Syndrome: A Case Report With Serial Imaging Evidence.

Autosplenectomy, defined as the spontaneous loss of splenic tissue in the absence of trauma or surgery, is most commonly associated with sickle cell disease and is rarely described in patients with autoimmune conditions. We present a case of a 54-year-old woman with systemic lupus erythematosus (SLE) and Sjögren's overlap syndrome who was incidentally found to have a complete absence of the spleen on computed tomography (CT), despite prior imaging demonstrating a normal splenic anatomy. The patient presented with polyarthralgia, fatigue, and systemic inflammation. Laboratory investigations rev

Persistent non-viable Plasmodium falciparum parasitemia mimicking artesunate treatment failure and unmasking functional hyposplenism.

A 40-year-old woman was admitted with severe Plasmodium falciparum malaria. Although she improved clinically under intravenous artesunate, parasitemia unexpectedly increased. Reassessment identified previously unrecognized functional hyposplenism, leading to delayed clearance of non-viable parasites and falsely suggesting artemisinin resistance.

Current review of pediatric Fas-associated death domain protein deficiency: expanding clinical and therapeutic perspectives.

Fas-associated death domain protein (FADD) deficiency is a rare inborn error of immunity characterized by dysregulated T-cell proliferation. The clinical spectrum and management of FADD deficiency in children remain incompletely described. This review aimed to synthesize patient-level observational evidence on clinical manifestations, immunological and genetic findings, and treatment outcomes of pediatric patients with FADD deficiency. A literature search was conducted of PubMed, the Cochrane Library, and Scopus from database inception to July 2, 2026, and reference lists of eligible articles

Epidemiology and Characterization of Immunological Disorders in Children Diagnosed With Pneumococcal Invasive Disease.

AIM: A substantial proportion of children hospitalized with invasive pneumococcal infection are subjected to immune evaluations, to detect immunological disorders (ID). The literature lacks thorough analysis of the necessity and effectiveness of these evaluations for previously healthy children with such infections. We aimed to examine the occurrence and characteristics of ID in children hospitalized with invasive pneumococcal disease (IPD), and to assess when immune investigations are most effective. METHODS: In a retrospective study, we examined the immunological assessments and outcomes of

Risk of malignancy and surveillance of premalignant gastrointestinal lesions in adult celiac disease.

Celiac disease is a systemic autoimmune affliction triggered by dietary gluten in genetically predisposed subjects. Current estimation of the prevalence of celiac disease is approximately 1% for the global population. These data are consistent with current figures from the Czech Republic (prevalence of 1.06% in 2023). Celiac disease can be associated with several complications including malignancy. Exact epidemiology data on T-cell lymphoma and small intestinal adenocarcinoma in the Czech Republic over the period 2010-2023 are also provided. Unlike the no-biopsy approach in children, despite c
