Diagnostic Utility of Apparent Diffusion Coefficient Values of Spleen and Liver in Assessment of Severity of Portal Hypertension and Liver Cirrhosis.

BACKGROUND: Portal hypertension (PH) is a significant effect of chronic liver disease (CLD), resulting in variceal haemorrhage, ascites and hypersplenism. Apparent diffusion coefficient (ADC) and diffusion-weighted imaging (DWI) values serve as potential non-invasive indicators for evaluating the PH and liver fibrosis. AIMS: This study evaluated the impact of portal hypertension on the ADC values of the liver and spleen and related PH severity with the Child-Pugh score. MATERIALS AND METHODS: A cross-sectional study was performed with individuals exhibiting clinical and radiological indicati

Portal vein thrombosis as a clinical dilemma in Echis carinatus envenomation: a case report and review of the literature.

BACKGROUND: Snakebite envenoming is a major neglected tropical disease, particularly in rural regions of tropical and subtropical countries. Echis carinatus (saw-scaled viper; SSV), prevalent in the desert regions of Iran, is a medically significant species of the Viperidae family whose venom disrupts hemostasis through potent procoagulant activity. While venom-induced consumption coagulopathy (VICC) is well recognized as a hemorrhagic complication, thrombotic sequelae are rare and poorly documented. CASE PRESENTATION: We report the case of a 15-year-old male who presented to the emergency dep

Case Report: A case of hepatic encephalopathy secondary to a spontaneous splenorenal shunt.

This report presents a case of hepatic encephalopathy (HE) induced by a spontaneous splenorenal shunt (SSRS). A 73-year-old male patient was admitted to our medical facility due to loss of consciousness. Laboratory analyses revealed elevated blood ammonia levels and varying degrees of reduction in erythrocyte, leucocyte, and platelet levels. Portal vein imaging utilizing 320-slice CT demonstrated enlargement of the portal and splenic veins, splenomegaly, multiple varicose veins at the splenic hilum, and local protrusion of the left renal vein. An initial diagnosis of HE with SSRS and hypersple

Simultaneous Splenectomy and Partial Hepatectomy for Hepatocellular Carcinoma in a Cirrhotic Patient With Severe Hypersplenism: A Case Report.

Hepatocellular carcinoma (HCC) often arises in the setting of liver cirrhosis and portal hypertension, which complicates curative treatment strategies. Although liver transplantation represents the gold standard treatment for HCC in cirrhotic patients, severe organ shortage, particularly in Asian countries including Japan, limits its availability. Hypersplenism associated with massive splenomegaly frequently results in severe thrombocytopenia, limiting the feasibility of surgical resection. We report the case of a 55-year-old man with alcohol-related cirrhosis, portal hypertension, and marked

Overcoming transfusion-refractory anemia: partial splenic embolization to facilitate orthopedic surgery in β-thalassemia.

β-thalassemia is a hereditary hemolytic disorder. Non-transfusion-dependent patients typically present with mild anemic symptoms under normal conditions, but may develop severe anemia during stress states. Moreover, treatment is often challenging due to concurrent hypersplenism. This article reports a case of a young male patient with non-transfusion-dependent β-thalassemia who was admitted to the hospital due to a closed fracture of the right femur. After admission, the patient developed severe anemia and showed a poor response to blood transfusion therapy. Considering the association with hyp

Partial splenic embolization in patients with cirrhosis: Comparison of < 50% versus > 50% infarction strategy.

BACKGROUND: Splenomegaly commonly occurs in portal hypertension due to cirrhosis, potentially causing leukopenia, thrombocytopenia and portal hypertension. Partial splenic artery embolization (PSE) serves as a substitute for splenectomy, given the significant risks associated with surgery in cirrhotic patients. In addition to improving hematologic indices, PSE may indirectly benefit liver-related outcomes and reduce portal hypertension complications. METHODS: The objective of this study was to assess the short-term (One-month) and long-term (24-month) impact on hematological parameters and li

Safety and Feasibility of Yttrium-90 Radioembolization for Cirrhosis-Induced Splenomegaly and Thrombocytopenia: Results from an Investigational Device Exemption Study.

Cirrhosis-associated hypersplenism and thrombocytopenia lead to discomfort and bleeding risk, which can complicate necessary medical/procedural management. Current medical therapies are limited in effectiveness, whereas surgical splenectomy and partial splenic embolization are associated with unfavorable adverse event (AE) profiles and other drawbacks. This prospective study evaluated novel yttrium-90 (90Y) splenic transarterial radioembolization (TARE) for cirrhosis-associated hypersplenism and thrombocytopenia. In 7 patients, 90Y splenic TARE was associated with 2 mild AEs, and no moderate/s

Splenic Artery Embolization: Tips, Tricks, and Current Standards of Practice.

The spleen is a vital intra-abdominal organ that provides a role in hematopoietic and immunologic functions. It is highly vascular and is the most commonly injured organ in the setting of blunt trauma, potentially leading to life-threatening hemoperitoneum. Splenic artery embolization (SAE) serves as an alternative to operative management of patients with splenic trauma and has quickly become popular given the infectious risks associated with splenectomy. Since its inception, indications for SAE have evolved beyond the management of traumatic injury. The authors discuss splenic artery anatomy

Long-term outcomes and bleeding risk factors in pancreatic portal hypertension: a retrospective cohort study.

BACKGROUND: Pancreatic portal hypertension (PPH) is a distinct form of sinistral (left-sided) portal hypertension, primarily resulting from pancreatic diseases such as acute or chronic pancreatitis and pancreatic tumors. These conditions lead to splenic vein distortion, compression, and inflammatory thickening, which can progress to luminal obstruction and thrombosis. This thrombosis impairs venous return, elevates splenic vein pressure, and ultimately causes splenomegaly, hypersplenism, and left-sided portal hypertension. Current knowledge regarding the long-term prognosis of PPH is primarily

Development and validation of a machine learning model for predicting hypersplenism in Wilson disease patients.

OBJECTIVE: Wilson disease (WD) is a rare autosomal recessive copper metabolism disorder, with hypersplenism as a severe, common complication secondary to disease-related cirrhosis. Currently, there is a lack of precise early prediction tools for this complication. This study aimed to construct a hypersplenism prediction model for WD patients by integrating multidimensional clinical indicators and machine learning, providing references for early identification of high-risk individuals and personalized interventions. METHODS: A total of 524 WD patients were enrolled at the First Affiliated Hosp
