Bile duct injuries (BDI) tend to be more complex in laparoscopic than in open cholecystectomy procedures, and frequently involve young adults with benign pathologies. The ultimate consequence may be a liver transplantation (LT), making this situation one of the most rare transplant indications. Fatal post-transplant outcome is extreme infrequently reported. Aim of this study is to report on our single-case experience and to review the literature concerning lethal outcome after LT for major BDI following cholecystectomy. A 36-year old obese caucasian woman underwent a laparoscopic cholecystecto

BACKGROUND: Accurate measurement of ankle joint dorsiflexion is clinically important as a restriction has been linked to many foot and ankle pathologies, as well as increased ulcer risk and delayed ulcer healing in people with diabetes. Use of the reliable weight bearing (WB) Lunge test is limited as normal and restricted ranges for WB ankle joint dorsiflexion are not identified. Additionally the extent of WB dorsiflexion restriction that results in clinically adverse outcomes is unclear. Therefore the aim of this investigation is to validate a proposed weight bearing equinus value (dorsiflexi

The month of March is dedicated to Colon Cancer Awareness. Worldwide, colorectal cancer (CRC) incidence has been on the rise. It is currently the third most common cancer in men (746,000 cases, 10.0% of the total) and the second in women (614,000 cases, 9.2% of the total).1 Arecent meta-analysis reported a 61% risk reduction in CRC incidence with colonoscopy.2 Unlike screening programs for breast and prostate cancers, not only has CRC screening reduced mortality from colon cancer and detected early CRC, it has also decreased the incidence of CRC through detection and removal of pre-cancerous l

The Hepatorenal Syndrome-Acute Kidney Injury (HRS-AKI) patients infected with methicillin-resistant Staphylococcus aureus (MRSA) urgently require safe and effective treatment options due to their compromised hepatic and renal functions, as well as thrombocytopenia resulting from hypersplenism. In our case, an HRS-AKI patient who underwent continuous renal replacement therapy for fluid overload developed fever with chills. His blood tests indicated elevated C-reactive protein and neutrophils, low platelet count, and bilateral lung infiltrates. Subsequently, his blood culture and catheter cultur

INTRODUCTION AND IMPORTANCE: This report describes a rare and remarkable case of partial splenectomy (PS) performed to manage the sequelae of massive malarial splenomegaly. It is likely the first reported case to date.
CASE PRESENTATION: A 40-year-old female from Papua New Guinea presented shocked to a remote hospital in the Torres Strait Islands. She had a history of hyperreactive malarial splenomegaly syndrome (HMSS) during childhood. After urgent transfer and stabilisation in a regional centre, the patient underwent a PS.
CLINICAL DISCUSSION: Indications for surgery included hypersplenism

In 296 patients with homozygous sickle cell disease (HbSS) detected during the screening of 100,000 deliveries between 1973-1981, chronic hypersplenism defined as a spleen measuring ≥4 cm below the costal margin with evidence of prolonged red cell sequestration occurred in 30 (10.1%) subjects, 23 resolved by splenectomy and 7 resolved spontaneously. Median age at splenectomy was 4.8 years and following splenectomy, median values for hemoglobin increased by 2.3 g/dL, reticulocytes fell by 8.3%, total nucleated cells fell by 2.2%, and platelets increased by 29,813 × 109/dL. Mean splenic weight a

PURPOSE: Splenic artery embolization (SAE) is a minimally invasive interventional radiology (IR) procedure that plays an important role in the management of complications in a certain subgroup of liver transplant recipients. SAE is technically of two types - proximal (P-SAE) and distal (D-SAE). There is limited data regarding the role of SAE in liver transplant recipients. The purpose of this study was to describe the role and outcomes of SAE in LDLT recipients based on the indications and the type of procedure used.
MATERIAL AND METHODS: Twenty-seven consecutive patients who underwent SAE af

BACKGROUND: Liver cirrhosis patients can develop various complications including bacteremia as the intestinal flora is heterologous. In those with low immunity, trauma, or following surgery, etc., the body is susceptible to concurrent systemic or local infections. Under these circumstances, even minimally invasive treatment methods such as interventional therapy like transjugular intrahepatic portosystemic shunt (TIPS) for liver cirrhosis patients can also result in complications such as infections.
CASE SUMMARY: A male patient with decompensated cirrhosis experienced multiple episodes of gas

Extrahepatic portal hypertension is commonly caused by portal vein thrombosis (PVT). Clinical manifestations of this may include splenomegaly, varices and ascites. Rare complications such as splenic artery aneurysms (SAAs) can present, especially in the presence of portal hypertension; however, such cases are poorly described in the literature and can therefore present a challenge in investigative work-up and management. The case of a young woman in her late 20s with chronic extrahepatic PVT from thrombophilic disease is presented in this report. She developed splenomegaly, thrombocytopaenia f

A 22-month-old male infant presented with cyanosis and stridor after a trivial fall and then developed acute respiratory distress. The respiratory status of the patient progressed rapidly to severe acute respiratory distress syndrome. Additional findings of hypersplenism prompted a comprehensive multidisciplinary approach and consideration of an inborn error of metabolism. Rapid whole-genome sequence showed a compound heterozygote mutation in the GBA1 gene involving a maternally inherited known pathogenic variant, p.L484P, and a paternally inherited novel likely pathogenic variant, p.P358l. PURPOSE OF REVIEW: In this review, we highlight conventional agents and novel emerging therapeutic strategies to treat anemia in MF.
RECENT FINDINGS: Anemia is a common and challenging feature of myelofibrosis (MF). The pathobiology of anemia is multifactorial, including progressive bone marrow fibrosis, decreased erythropoiesis due to high hepcidin levels leading to iron sequestration in the reticuloendothelial system, hypersplenism, erythropoiesis inhibition by myelosuppressive JAK inhibitors (ruxolitinib, fedratinib), and others. MF-associated anemia has a negative impact on survival. Conv

OBJECTIVE: To assess the correlation between the platelet-to-white blood cell ratio (PWR) and the severity of liver dysfunction, hepatic complications, and prognosis in Wilson disease (WD) patients.
METHODS: A retrospective analysis was conducted on medical records from January 1, 2016, to March 30, 2022. Both univariate and multivariate analyses were performed to examine the impact of a low PWR (<26.3) on WD severity, liver complications, and disease progression. Additionally, the effect of splenectomy on PWR was evaluated.
RESULTS: The study included 315 patients with WD, among whom 105 ha

Extrahepatic portal hypertension is the hypertension of the portal venous system in the absence of liver cirrhosis, and variceal bleeding is its commonly seen complication. Long-standing portal hypertension will have the risk of symptomatic hypersplenism, portal biliopathy, growth failure, and ectopic varices. Portosystemic shunt surgery can more effectively manage these complications than medical and endoscopic management. Conventional portosystemic shunts, especially the proximal splenorenal shunt (PSRS), are preferred over unconventional shunts, as the latter will have increased procedural

BACKGROUND Living donor liver transplantation (LDLT) has been shown to be safe in the curative treatment of liver cirrhosis. Portal flow modulation techniques, such as splenic artery ligation (SAL), have been used to avoid complications like small-for-size syndrome (SFSS). However, the effects of SAL on portal flow, splenic function, and hematologic outcomes remain underexplored. MATERIAL AND METHODS This retrospective study analyzed 60 LDLT recipients treated at a single center from January 2023 to December 2024. Thirty patients underwent SAL (SAL+) while 30 did not undergo SAL (SAL-). Data o
