We present the case of a 61-year-old man with a 30-year history of iron-deficiency anemia and recurrent gastrointestinal bleeding, accompanied by splenomegaly and hypersplenism. Despite multiple endoscopy and imaging examinations, the source of bleeding remained elusive. The patient ultimately underwent exploratory laparotomy with splenectomy, which revealed a Meckel's diverticulum with stale hemorrhage, along with congestive changes in the spleen. Following surgery, the patient experienced a complete resolution of symptoms with normalization of hematological parameters. This case highlights M

BACKGROUND/AIMS: Various non-invasive tools (NITs) predicting first hepatic decompensation (HD) in advanced chronic liver disease (ACLD) enclose platelet (PLT) count. A relevant proportion of metabolic dysfunction-associated steatotic liver disease (MASLD)-ACLD patients do not show splenomegaly- and hypersplenism-related thrombocytopenia. We aimed to evaluate the performance of NITs in predicting HD according to ultrasound-assessed spleen size.
METHODS: In this observational study, 148 splenic and 27 asplenic (ASP) MASLD-compensated advanced chronic liver disease (cACLD) patients were enrolle

OBJECTIVE: Volume measurement software is not routinely utilized in clinical practice before splenic artery embolization. To explore the function of such software measurement in this context, we conducted a retrospective study.
METHODS: We divided patients into two groups: 38 patients in Group A underwent three-dimensional reconstruction using Revolution CT, and 37 patients in Group B underwent imaging with normal CT technology. We compared the changes in liver function test results and haematological parameters between these two groups.
RESULTS: A total of 75 patients who underwent splenic

Giant splenomegaly is an uncommon but significant complication of chronic myeloid leukemia (CML), often causing substantial morbidity. While tyrosine kinase inhibitors have revolutionized CML treatment, splenectomy remains necessary in selected cases, particularly when there is symptomatic hypersplenism, compressive symptoms, or risk of splenic rupture. This case describes a 35-year-old male with chronic-phase CML who presented with progressive abdominal distension, early satiety, and anemia due to massive splenomegaly. Abdominal ultrasound revealed a spleen measuring 40 cm, extending into the

Liver transplantation : official publication of the American Association for the Study of Liver Diseases and the International Liver Transplantation Society2025eng
While simultaneous splenectomy (SPX) is an option in selected cases of living donor liver transplantation to protect graft function, the graft-to-spleen volume ratio (GSVR) may serve as a new determinant of SPX. A prospective validation study was conducted to test our institutional strategy of SPX, based mainly on GSVR, which has been employed since 2019. This prospective study enrolled 141 recipients of living donor liver transplantation from 2019 to 2024. SPX was indicated for GSVR ≤0.70 and high-risk patients (ABO-incompatibility or donor age ≥45 y) with a final portal venous pressure >15 m

Kala-azar, or visceral leishmaniasis (VL), is a parasitic disease caused by Leishmania spp., characterised by fever, weight loss, splenomegaly, hepatomegaly, and anaemia. This study evaluated the relationship between hepcidin, inflammation, iron metabolism, and hypersplenism in VL-associated anaemia. In this cross-sectional study, confirmed VL patients without recent transfusions were assessed. Haematological and inflammatory parameters were analysed using correlation and multivariate regression tests. Anaemia was present in 95.2% of the sample, predominantly normocytic (59.5%) and normochromi

BACKGROUND Idiopathic portal hypertension (IPH) is a rare disease of unknown etiology that causes hypersplenism, splenomegaly, and portal hypertension. There have been rare reports of hepatocellular carcinoma (HCC) in patients with IPH, but no causal relationship has been confirmed. This report details the case of an 88-year-old Japanese woman who developed HCC after a 30-year history of IPH and was treated with carbon-ion radiotherapy. CASE REPORT An 88-year-old Japanese woman had presented to our hospital 33 years earlier with bleeding from esophageal varices. Liver function test results wer

BACKGROUND: Major variceal hemorrhages in children of the Indian subcontinent, are caused by extrahepatic portal vein obstruction (EHPVO). Portosystemic shunt surgery (PSS) is the recommended course of action for long-term care in refractory cases, even if endoscopic techniques constitute the initial line of treatment for acute variceal hemorrhage. The most usual surgery, particularly with splenomegaly, is the proximal splenorenal shunt (PSRS). However, in some cases, unusual or alternative nonconventional shunts are required for treatments due to anatomical variances and technical difficultie

Background/Objectives: Portal hypertension (PH) is a common complication in children with chronic liver diseases. Primary and secondary prophylaxis of variceal bleeding in these patients remains controversial. Our study aims to evaluate the management of gastrointestinal (GI) varices in children with PH in Italy. Methods: A questionnaire was sent to 21 major pediatric hepatology centers. It included 34 questions referring to the medical, endoscopic, radiological, and surgical management of GI varices. Results: Out of 21 centers, 16 returned a completed questionnaire (survey response rate 76%)

Venous thromboembolism is often underestimated in transfusion-dependent thalassemia (TDT) patients, as arterial thrombotic events are more commonly observed. Although therapeutic advancements have transformed this disease from a once-fatal childhood disease into a manageable chronic condition, some treatments may contribute to an increased risk of thrombosis. Additionally, the prolonged life expectancy of these patients further contributes to the overall thrombotic risk. Patients with thalassemia major present multiple challenges when considering anticoagulation therapy. The decision-making pr

Hypersplenism is a common complication of cirrhosis that is associated with significant impairment to patients' life quality. Splenectomy is often employed in clinical settings as a treatment for hypersplenism. While splenectomy is carried out for the purposes of alleviating hypersplenism-related adverse outcomes like thrombocytopenia or anaemia, studies have suggested alterations in the immune status, hemodynamics, and intestinal microbiota of patients following splenectomy, which may potentially influence the onset and progression of hepatocellular carcinoma (HCC). Additionally, patients hav

BACKGROUND: Primary Sjögren's syndrome (pSS) is an autoimmune disease characterized by exocrine glands involving mainly the lacrimal and salivary glands. Splenomegaly from pSS is a rare clinical feature unless the pSS is complicated by lymphoma. Splenectomy could exclude malignant lymphoma, reduce the risk of spontaneous spleen rupture and remove hypersplenism to improve pancytopenia.
CASE PRESENTATION: We reported the case of a 49-year-old female patient who was diagnosed with pSS according to dry mouth, dry eyes, splenomegaly, multiple positive autoantibodies, positive Schirmer's test and b

BACKGROUND: Cavernous transformation of the portal vein (CTPV) is one of the significant pathogenic factors of prehepatic portal hypertension in children. The Rex shunt, by reconstructing an intrahepatic portal venous pathway, not only effectively reduces portal pressure and restores physiological hepatic blood flow, but also promotes normal growth and development, making it the treatment of choice for CTPV. In contrast, traditional non-selective shunting procedures primarily alleviate symptoms of portal hypertension without restoring hepatic perfusion, thereby compromising growth potential. F

BACKGROUND: The dilemma of current treatments for symptomatic patients with portal hypertension is how to resolve the problems of the varices and hypersplenism simultaneously and meanwhile preserve the splenic function. Partial splenectomy and pericardial devascularization is a potential alternative to currently used approaches, but it is incompletely defined. This study aimed to evaluate the efficacy and safety of partial splenectomy and pericardial devascularization.
METHODS: This study followed 132 symptomatic patients with portal hypertension who underwent partial splenectomy and pericard

INTRODUCTION: We report the case of a child who underwent splenectomy and developed marked thrombocytopenia after detorsion due to wandering splenic torsion.
CASE PRESENTATION: A 7-year-old boy who underwent laparoscopic inguinal hernia surgery developed sudden abdominal pain 2 days later. Contrast-enhanced CT revealed a poor contrast effect in the spleen, and emergency surgery was performed based on the diagnosis of splenic infarction. The spleen was not fixed to the retroperitoneum and was twisted at 720° around the splenic hilum. However, the color tone improved after detorsion of the sple

The advent of targeted therapy and immunotherapy has revolutionised the management of hepatocellular carcinoma (HCC) patients with all stages, dramatically improving their survival outcomes. Currently, radical resection is still the preferred first-line treatment for early-stage HCC, nevertheless, the surgical outcomes remain unsatisfactory due to high recurrence rate of 70% within 5 years after surgery. Moreover, up to two thirds of diagnosed HCC patients are in the advanced stages of the disease, exhibiting intrahepatic or extrahepatic metastases and vascular invasion. In recent years, the c

BACKGROUND: Bleeding from esophageo-gastric varices is a serious complication related to portal hypertension. In patients with cirrhosis, liver and splenic stiffness measurements (LSM and SSM) are useful to predict high-risk varices, bleeding and decompensation. Data regarding the utility of SSM for predicting bleeding in patients with non-cirrhotic portal hypertension (NCPH) is scarce.
METHODS: Consecutive patients with NCPH (n = 150, 129 extrahepatic portal vein obstruction; 21 non-cirrhotic portal fibrosis) with baseline available LSM, SSM and HVPG values were included. Half of the patient

Transjugular intrahepatic portosystemic shunt (TIPS) placement is a well-established intervention for portal hypertension. However, some patients experience persistent complications such as encephalopathy, ascites, or thrombocytopenia, especially when further TIPS optimization is not technically possible. Partial splenic embolization (PSE), typically performed for hypersplenism or certain hematologic conditions, can reduce portal venous inflow and improve cytopenias; however, its use as an adjunct to TIPS is less well described. We present the case of a 73-year-old male with cirrhosis secondar
