Safety of red blood cell transfusion despite serologic incompatibility in autoimmune hemolytic anemia: A real-world multicenter study.

Red blood cell transfusion in Autoimmune Hemolytic Anemia (AIHA) poses significant clinical challenges due to pan-reactive autoantibodies that interfere with cross-matching, often raising concerns regarding the safety of transfusing serologically "incompatible" blood. This retrospective multicenter study evaluated the frequency, management criteria, and safety profile of red blood cell transfusions in a cohort of adult patients affected by AIHA with a specific focus on transfusion management in the setting of serologic incompatibility. We analyzed 93 adult patients diagnosed with AIHA at two h

Hospital length of stay and cost associated with routine-care documentation of probable or suspected fungal infection among patients hospitalized with acute myeloid leukemia: a propensity score analysis.

BACKGROUND: Fungal infection remains a recurring concern during inpatient care for acute myeloid leukemia (AML), but many hospitals lack local estimates of the bed-day and cost burden associated with fungal-infection documentation. We estimated excess length of stay (LOS) and hospitalization cost associated with routine-care documented probable or suspected fungal infection (PFI) among AML inpatient encounters. METHODS: We conducted a retrospective encounter-level study using routinely collected 2020-2025 AML inpatient data from the electronic medical record database of Ganzhou People's Hospi

Prevalence of anaemia and associated factors among pregnant women living in Koloji camps of Fafan Zone of Somali Region, Ethiopia, 2024.

BACKGROUND: Anaemia is a major global public health problem affecting pregnant women, with a prevalence of about 40-50% in Sub-Saharan Africa and significant maternal and fetal consequences. Despite interventions, it remains highly prevalent in Ethiopia with regional variation. Internally displaced pregnant women are at higher risk due to food insecurity, poor diets, and limited health services. However, evidence on anaemia in Ethiopian IDP settings is limited. Therefore, this study assessed the prevalence and associated factors of anaemia among pregnant women in Koloji IDP camp, Somali Region

Teclistamab Induced a Profound Hematologic Response in Light Chain Deposition Disease With Probable Cardiac Involvement.

We report the first case of using teclistamab, a BCMA × CD3 bispecific antibody, in a 63-year-old male with κ-light chain deposition disease (LCDD) involving the kidneys and probably the heart. The patient initially responded to daratumumab-pomalidomide-dexamethasone but experienced hematologic relapse. Salvage therapy with teclistamab induced a rapid and sustained hematologic complete response without cytokine release syndrome or neurotoxicity. Renal and cardiac functions remained stable. This case provides critical early evidence that BCMA-directed immunotherapy may serve as an effective sal

Musculoskeletal Manifestations of Light Chain Amyloidosis: A Systematic Review.

INTRODUCTION: Diagnosing amyloid arthropathy remains challenging because its manifestations often mimic those of other inflammatory musculoskeletal diseases. This review aimed to describe the clinical, biological, and imaging characteristics of musculoskeletal involvement in light-chain (AL) Amyloidosis. METHODS: We conducted a review in accordance with the PRISMA guidelines. We searched SCOPUS and MEDLINE for case reports and series published between January 2000 and December 2023 that detailed musculoskeletal manifestations of AL amyloidosis. To minimize the risk of omitting relevant articl

Cardio-protection and monitoring for early phase clinical trials: Recommendations from the therapeutic advances in childhood leukemia and lymphoma (TACL) consortium.

BACKGROUND: Many effective anti-cancer modalities potentially induce cardiotoxicity. Pediatric patients with relapsed/refractory hematologic malignancies are often excluded from anthracycline-containing regimens based on historical data showing population-level risk of heart failure after cumulative doses of anthracyclines without considering individual patient risk variability, modernized cumulative anthracycline equivalence ratios, novel anthracycline formulations, and use of supportive cardio-protectant medications. Additionally, standard guidelines are lacking for patients with a history o

Paediatric Helicobacter pylori infection and allergic and inflammatory diseases: a contemporary systematic review with meta-analysis.

UNLABELLED: Helicobacter pylori (H. pylori) infection is prevalent worldwide, and its potential protective role in allergic and immune diseases has been a topic of speculation. This systematic review aims to analyse recent literature to establish a consensus on this relationship. Articles published in the past 12 years regarding association of H. pylori and various allergic diseases were screened. Thirty-five articles from three databases were assessed for bias using the ROBINS-E tool. Objective data from each article were collected for this review. Among the reviewed articles, 14 focused on a

Type 2 Inflammation in Airway Diseases: Eosinophilic Bronchiectasis as an Emerging Phenotype.

Bronchiectasis is a chronic and highly heterogeneous airway disease traditionally regarded as a neutrophil-dominated condition driven by persistent infection and dysregulated innate immunity. Over the last decade, this traditional view has been increasingly challenged by the identification of distinct inflammatory phenotypes with important biological and therapeutic implications.Among these, eosinophilic bronchiectasis has emerged as a clinically meaningful and mechanistically distinct phenotype, characterized by type 2 (T2) immune activation, eosinophil-driven airway inflammation, and differe

A rare but misleading cause of hematuria in hemophilia A: renal pelvic hemorrhage mimicking tumor.

Hematuria is a common manifestation in hemophilia A, whereas upper urinary tract bleeding is rare and diagnostically challenging. We report a patient with severe hemophilia A presenting with gross hematuria due to spontaneous renal pelvic hemorrhage (Antopol-Goldman lesion), initially mimicking a urothelial tumor on imaging. The diagnosis was supported by clinical context, imaging characteristics, and response to factor VIII replacement therapy. This case highlights the importance of considering benign hemorrhagic causes in hemophilia patients with atypical imaging findings to avoid unnecessar

Does electrocorticographic guidance improve seizure outcomes following cavernoma resection?

OBJECTIVE: Cerebral cavernous malformations (CCMs) are vascular anomalies frequently associated with drug-resistant epilepsy. Surgical resection is a well-established treatment; however, the optimal strategy to achieve long-term seizure freedom (SF) remains unclear. Intraoperative electrocorticography (ECoG) may enhance seizure outcome by guiding resection extent, yet comparative evidence is limited. To evaluate the efficacy of ECoG-guided resection in patients with CCMs, a single-center comparative retrospective cohort analysis and a meta-analysis were performed. METHODS: We analyzed 67 adul

Fetal demise duration and bleeding outcomes at the time of dilation and evacuation.

OBJECTIVES: To examine associations between time from second trimester intrauterine fetal demise (IUFD) to dilation and evacuation (D&E) and bleeding complications. STUDY DESIGN: We identified 197 patients scheduled for D&E for IUFD at a single academic institution between March 2011 to March 2025. IUFD duration was calculated by subtracting gestational age by fetal biometry from gestational age by obstetric dating, categorized into 0-14 days, 15-28 days, and > 28 days. We examined the associations between IUFD duration and bleeding outcomes of disseminated intravascular coagulopathy (DIC) an

Anesthesia Clinical Practice Guidelines for Ehlers-Danlos Syndrome: A Modified Delphi Survey.

Ehlers-Danlos syndrome (EDS) is a family of inherited connective tissue disorders distinguished by joint hyperextensibility, skin hyperelasticity, and increased tissue friability. No standardized anesthesia guidelines currently exist for this population, despite facing increased perioperative risks. To address this gap in literature, a three-round modified Delphi survey was conducted with 15 experts in anesthesia and EDS. Seven preliminary guidelines, developed from a comprehensive literature review, were evaluated and refined across three rounds. Consensus was achieved for all seven guideline

Identification of Macrophage Polarization-Related Biomarkers in Multiple Myeloma Using Integrated Bioinformatics.

Multiple myeloma (MM) is an incurable hematologic neoplasm with heterogeneous prognosis. Within the bone marrow microenvironment, macrophage polarization (MP) contributes to immune remodeling and may influence disease progression, yet MP-related prognostic biomarkers remain insufficiently characterized. In this study, we integrated transcriptomic data from UCSC Xena (n = 844) and GSE4581 (n = 414) to identify differentially expressed genes between immune-hot and immune-cold MM samples stratified by single-sample gene set enrichment analysis (ssGSEA)-derived immune scores. Weighted gene co-expr

Germline Genomic and DNA Methylation Differences Between MGUS and Multiple Myeloma Among Patients of African Ancestry.

Individuals of African ancestry (AA) have approximately twofold higher risk of multiple myeloma (MM) than individuals of European ancestry, yet the molecular determinants between monoclonal gammopathy of undetermined significance (MGUS) and MM remain poorly defined. We performed whole-exome sequencing (WES) and genome-wide DNA methylation profiling in 28 AA patients with MM and 28 frequency-matched AA patients with MGUS without documented progression. After quality control, the WES analysis included 28 patients with MGUS and 27 with MM, while the DNA methylation analysis included 27 patients w
