Diagnostic Value of Skin Biopsy in Fatal Histoplasmosis-Associated Hemophagocytic Lymphohistiocytosis: A Case Report.

INTRODUCTION: Hemophagocytic lymphohistiocytosis (HLH) is a rare, life-threatening hyperinflammatory syndrome most commonly triggered in adults by infection, malignancy, or autoimmune disease. Disseminated histoplasmosis is an uncommon but recognized infectious trigger, particularly in immunosuppressed patients. Its cutaneous manifestations may be subtle and nonspecific but can provide accessible tissue for diagnostic evaluation. CASE REPORT: A 41-year-old man with psoriasis treated with adalimumab presented with 5 months of progressive constitutional and abdominal symptoms followed by fever,

Pretransplant infection associated with inferior survival after TCRαβ/CD19-depleted haploidentical HSCT in pediatric nonmalignant disorders.

BACKGROUND AND AIMS: TCRαβ/CD19-depleted haploidentical hematopoietic stem cell transplantation (HSCT) is associated with favorable outcomes in pediatric nonmalignant disorders when performed in clinically stable patients receiving adequate conditioning. Outcomes in patients transplanted in the setting of active infection remain poorly defined. To evaluate the impact of pretransplant infection on outcomes after TCRαβ/CD19-depleted haploidentical HSCT in pediatric nonmalignant disorders. METHODS: We retrospectively analyzed 36 pediatric patients with nonmalignant disorders who underwent haploi

Hyperinflammatory syndromes in primary mitochondrial disease: A three-center case series and focused review.

INTRODUCTION: Primary mitochondrial diseases (PMtD) are increasingly recognized to involve clinically meaningful immune dysregulation in addition to bioenergetic failure. Hyperinflammatory syndromes, including secondary hemophagocytic lymphohistiocytosis (HLH) and related cytokine-mediated inflammatory states, remain underrecognized complications of PMtD, and the mechanisms linking mitochondrial dysfunction to pathologic inflammation are incompletely understood. METHODS: We report three individuals with genetically confirmed PMtD evaluated at three academic medical centers who developed sever

Severe Plasmodium vivax Malaria Complicated by Secondary Hemophagocytic Lymphohistiocytosis, Disseminated Intravascular Coagulation, and Acute Pancreatitis: A Case Report.

Plasmodium vivax (P. vivax) malaria, once regarded as a comparatively benign infection, is increasingly recognized as a cause of severe, potentially life-threatening multisystem disease. Concurrent secondary hemophagocytic lymphohistiocytosis (HLH), disseminated intravascular coagulation (DIC), and acute pancreatitis complicating P. vivax infection is exceptionally rare. A previously healthy 19-year-old Bangladeshi woman presented with a 5-day history of high-grade fever with rigors, generalized weakness, and upper abdominal pain. Laboratory evaluation demonstrated pancytopenia, markedly eleva

Coagulation dysfunction in children with secondary hemophagocytic lymphohistiocytosis: a comprehensive analysis.

OBJECTIVE: Coagulation dysfunction plays a critical role in the pathogenesis and prognosis of secondary haemophagocytic lymphohistiocytosis (sHLH) in children. This study aims to systematically analyze the coagulation profiles in paediatric sHLH patients, evaluate their prognostic value and provide an effective basis for reducing mortality in children with HLH. METHODS: A total of 209 paediatric patients with sHLH were enrolled in this study. Coagulation parameters at admission were collected and compared across groups stratified by aetiology, prognosis and presence of disseminated intravascu

Cytomegalovirus Infection-Associated Hemophagocytic Lymphohistiocytosis and Histiocytic Necrotising Lymphadenitis Progressing to Systemic Lupus Erythematosus: A Case Report.

Systemic lupus erythematosus (SLE) is an autoimmune multisystem disorder predominantly seen in young females. Its clinical presentation is enormously heterogeneous. Genetic, epigenetic and environmental factors cause SLE. Secondary hemophagocytic lymphohistiocytosis (HLH) associated with cytomegalovirus (CMV) infection is observed in immunocompromised patients and is rare in immunocompetent patients. Histiocytic necrotising lymphadenitis (HNL) is a self-limiting hyperimmune reaction rarely associated with HLH. CMV infection can be associated with HLH, HNL, and SLE. In this case, we discuss an

Hemophagocytic Lymphohistiocytosis Secondary to Disseminated Bacillus Calmette-Guérin (BCG): A Steroid-Sparing Strategy in a Patient Awaiting Cardiac Surgery-A Case Report.

We report a case of hemophagocytic lymphohistiocytosis (HLH) secondary to disseminated Bacillus Calmette-Guérin (BCG) infection in an 80-year-old man treated with intravesical BCG for non-muscle-invasive bladder cancer. The patient presented with several weeks of constitutional symptoms including night sweats, fatigue, weight loss, confusion, and pancytopenia. Laboratory studies revealed profound inflammation, coagulopathy, and hepatocellular injury. HLH was suspected clinically and supported by a high H-score. Extensive infectious and autoimmune workup was negative. Mycobacterium bovis was su

Pneumococcal HUS in infancy masking DGKE-related thrombotic microangiopathy with secondary hemophagocytic lymphohistiocytosis.

Hemolytic uremic syndrome (HUS) in infancy may be difficult to classify because infection-associated HUS and genetic thrombotic microangiopathies (TMAs) can present with similar findings. We report a 7-month-old boy admitted with pneumonia, anemia, thrombocytopenia, and acute kidney injury. Radiologically confirmed pneumonia, elevated CRP levels, positive nasopharyngeal pneumococcal PCR testing initially suggested pneumococcal-associated HUS; however, microangiopathic hemolysis and anuria persisted despite infection control, dialysis, plasma exchange, and complement inhibition therapy. A case report of secondary haemophagocytic lymphohistiocytosis following chikungunya virus infection: Insights into post-viral immune dysregulation.

Chikungunya virus infection is typically a self limiting febrile illness; however, rare cases may trigger severe immune dysregulation. We report an elderly man who developed secondary hemophagocytic lymphohistiocytosis following confirmed chikungunya infection. He presented with persistent fever, progressive pancytopenia, splenomegaly, and marked hyperferritinemia. Bone marrow examination demonstrated hemophagocytosis, and an HScore of 191 indicated a high probability of reactive Haemophagocytic lymphohistiocytosis. Despite fulfilling diagnostic criteria, the patient remained hemodynamically s

Adult Primary Hemophagocytic Lymphohistiocytosis (HLH) Initially Suspected as Brucellosis-Associated HLH: A Case Report.

Hemophagocytic lymphohistiocytosis (HLH) is a rare hyperinflammatory syndrome. Due to overlapping symptoms, primary HLH can be challenging to distinguish from brucellosis-associated HLH. This report describes the case of a 40-year-old male farmer who presented with progressive pancytopenia, body aches, fatigue, anorexia, weight loss, and fever. Laboratory examination showed severe leukopenia, anemia, thrombocytopenia, and abnormal liver function tests. Serology was positive for Brucella, but bone marrow culture was negative. Bone marrow biopsy showed histiocytic hemophagocytosis and preserved

Secondary hemophagocytic lymphohistiocytosis in pediatric patients: a single-center experience.

BACKGROUND: Hemophagocytic lymphohistiocytosis (HLH) is a life-threatening hyperinflammatory syndrome. In children, secondary HLH may occur with infection, rheumatologic disease, malignancy, or metabolic disorders and is often difficult to diagnose because it overlaps with sepsis and other severe inflammatory states. We aimed to describe the etiologies, clinical features, treatment patterns, and outcomes of pediatric secondary HLH at a single tertiary center. METHODS: We performed a retrospective review of children diagnosed with secondary HLH between April 2007 and June 2022. Diagnosis was b

Distinct Clinical and Prognostic Features of Acute and Chronic Active Epstein-Barr Virus-Associated Hemophagocytic Lymphohistiocytosis in Adults: A Scoping Review.

Epstein-Barr virus-associated hemophagocytic lymphohistiocytosis (EBV-HLH) is a major subtype of secondary HLH and is increasingly recognized as a heterogeneous entity. However, the clinical differences between acute EBV-HLH (AEBV-HLH) and chronic active EBV infection-associated HLH (CAEBV-HLH) have not been systematically characterized. We conducted a scoping review of PubMed up to December 7, 2024, and identified 168 eligible articles involving 714 adult patients with EBV-HLH, including 584 with AEBV-HLH and 130 with CAEBV-HLH. Current outlook on the use of biological agents to improve outcomes in adult secondary hemophagocytic lymphohistiocytosis.

INTRODUCTION: Adult secondary hemophagocytic lymphohistiocytosis (asHLH) is a fulminant hyperinflammatory condition triggered by cancers, infections, autoimmune conditions or drugs. The hallmark of its pathogenesis involves dysregulated immune effector cell activation and excessive cytokine release. AREAS COVERED: This narrative review updates the reader on modern treatment options for asHLH. The traditional one-size-fits-all strategy derived from the pediatric HLH-2004 protocol is increasingly viewed as outdated today. A significant shift toward a more individualized, trigger- directed treat

Secondary hemophagocytic lymphohistiocytosis following bacterial pneumonia managed with dexamethasone monotherapy: a case report.

INTRODUCTION: Secondary hemophagocytic lymphohistiocytosis (HLH) is a rare hyperinflammatory syndrome in adults, resulting from uncontrolled immune activation and a cytokine storm, and is frequently misdiagnosed because its manifestations closely resemble severe infection or sepsis, particularly when triggered by bacterial pneumonia. CASE PRESENTATION: A 63-year-old woman presented with 1 week of fever, cough, and dyspnea despite outpatient antibiotic treatment. Investigations confirmed right-sided lobar pneumonia caused by Streptococcus pneumoniae. Despite targeted intravenous antibiotics, s

Use of dermoscopy in patients with dark skin phototypes: Practices, challenges, and diagnostic value.

Hemophagocytic lymphohistiocytosis (HLH) is an inflammatory condition that may run a rapid fatal course and calls for prompt diagnosis. Early intervention with steroids and other immunosuppressive drugs can contain the disease process and favours positive outcome. Ferritin ≥500 ng/ml is a HLH diagnostic criterion. We evaluated the diagnostic potential of admission ferritin, in children with HLH. Pediatric patients of a referral teaching hospital from Feb 2010-Oct 2013 having been investigated for ferritin on admission were included. HLH was confirmed when patients had clinical features and met
