Tuberculosis presenting as dual immune dysregulation: Evans syndrome and haemophagocytic lymphohistiocytosis.

We report a case of a middle-aged man with no known comorbidities who presented with a 1-month history of fever, dry cough, weight loss and progressively worsening breathlessness.Initial investigations revealed declining haemoglobin, white blood cell and platelet levels, while a chest CT showed diffuse ground-glass opacities and a small pneumothorax. The patient was diagnosed with Evans syndrome (the co-occurrence of warm autoimmune haemolytic anaemia and bicytopenia) after a positive direct Coombs test and other haematological markers. Despite starting steroids, his fever persisted, leading t

Secondary hemophagocytic lymphohistiocytosis in pediatric patients: a single-center experience.

BACKGROUND: Hemophagocytic lymphohistiocytosis (HLH) is a life-threatening hyperinflammatory syndrome. In children, secondary HLH may occur with infection, rheumatologic disease, malignancy, or metabolic disorders and is often difficult to diagnose because it overlaps with sepsis and other severe inflammatory states. We aimed to describe the etiologies, clinical features, treatment patterns, and outcomes of pediatric secondary HLH at a single tertiary center. METHODS: We performed a retrospective review of children diagnosed with secondary HLH between April 2007 and June 2022. Diagnosis was b

Lamotrigine and other therapy-associated HLH syndromes: a review.

INTRODUCTION: Hemophagocytic lymphohistiocytosis (HLH) is a syndrome of pathologic immune activation that can occur secondary to medical interventions including immunomodulatory therapies such as immune checkpoint inhibitors and stem cell transplantation. However, lamotrigine and other anti-seizure medications (ASMs), which are not thought of as overtly immunogenic, are an important emerging cause and exploring the pathophysiology of lamotrigine-associated HLH may improve our understanding of secondary HLH. AREAS COVERED: We completed a literature search in EMBASE and MEDLINE for English lang

Clinical and Immunological Overlap Between Visceral Leishmaniasis and Rheumatic Disorders: A Comparative Review on Diagnostic Challenges.

Rheumatic diseases are complex systemic disorders characterized by multi-organ involvement and diverse laboratory abnormalities. The diagnostic approach involves a comprehensive evaluation of clinical and serological markers and the exclusion of conditions with overlapping presentations, such as other rheumatic diseases and systemic infections. Visceral leishmaniasis (VL) is a protozoan infection that primarily affects the reticuloendothelial system. Direct effects of parasite infiltration of the reticuloendothelial system, combined with the subsequent host immune response, produce clinical an

Atypical presentation of Wolcott-Rallison syndrome with severe Epstein-Barr virus hepatitis, secondary peroxisomal dysfunction and novel neuroimaging features.

A female toddler born to consanguineous parents presented with severe Epstein-Barr virus hepatitis, hyperferritinaemia and severe neutropenia, initially suggesting haemophagocytic lymphohistiocytosis. Metabolic screening revealed elevated very-long-chain fatty acids and dicarboxylic aciduria, mimicking a peroxisomal disorder. Brain MRI demonstrated cortical thickening and smoothing that were more pronounced in both temporal lobes, with poorly formed gyri characteristic of pachygyria-lissencephaly, underlying subcortical white matter thinning affecting the temporo-insular parenchyma and moderat

Residential radon in Peru: challenges for health surveillance and lung cancer prevention.

The Journal of Genetics, started by William Bateson in 1910, played a distinguished role in the early years of genetics. However, it stopped publishing in 1978. The Indian Academy of Sciences revived it in 1985, and has published it regularly since then. To commemorate this landmark, I highlight one of the 17 articles published that year. 'The isolation and genetic analysis of a Caenorhabditis elegans… X-chromosome balancer' by András Fodor and Péter Deak, of the Institute of Genetics, Biological Research Centre, Hungarian Academy of Sciences, Szeged, Hungary. Harnessing Artificial Intelligence to Restore Standards in Medical and Dental Education in Pakistan.

The month of March is dedicated to Colon Cancer Awareness. Worldwide, colorectal cancer (CRC) incidence has been on the rise. It is currently the third most common cancer in men (746,000 cases, 10.0% of the total) and the second in women (614,000 cases, 9.2% of the total).1 Arecent meta-analysis reported a 61% risk reduction in CRC incidence with colonoscopy.2 Unlike screening programs for breast and prostate cancers, not only has CRC screening reduced mortality from colon cancer and detected early CRC, it has also decreased the incidence of CRC through detection and removal of pre-cancerous l

Proteomic Analyses in COVID-19-Associated Secondary Hemophagocytic Lymphohistiocytosis.

CONTEXT: COVID-19 has been associated with features of a cytokine storm syndrome with some patients sharing features with the hyperinflammatory disorder, secondary hemophagocytic lymphohistiocytosis (sHLH). HYPOTHESIS: We hypothesized that proteins associated with sHLH from other causes will be associated with COVID-sHLH and that subjects with fatal COVID-sHLH would have defects in immune-related pathways. METHODS AND MODELS: We identified two cohorts of adult patients presenting with COVID-19 at two tertiary care hospitals in Seattle, Washington in 2020 and 2021. In this observational study

Serum chitotriosidase-1 (CHIT1) as candidate biomarker for mitochondriopathies.

BACKGROUND: Neuromuscular diseases (NMDs) and mitochondriopathies are rare and heterogeneous disorders. Diagnosis is often difficult and delayed, partly due to the lack of reliable biomarkers. Chitotriosidase (CHIT1) as a candidate marker for lysosomal storage diseases is elevated in Niemann pick disease type C as a prototype of this group of diseases. Most recently, a relevant role of the lysosomal pathway in mitochondriopathies has been discussed, but markers of lysosomal involvement have not been investigated. Therefore, the aim of this study was to evaluate CHIT1 concentrations in a broad

Self-beneficial transactional social dynamics for cooperation in Shwachman-Diamond syndrome: a mixed-subject analysis using computational pragmatics.

BACKGROUND: Shwachman-Diamond Syndrome (SDS) is a rare genetic disorder with documented cognitive and behavioral challenges. However, its socio-pragmatic dynamics remain underexplored, particularly in cooperative interactions where social norms and economic considerations intersect. OBJECTIVE: This study investigates the socio-behavioral dynamics of SDS, focusing on how children with the condition navigate cooperative interactions. Using computational pragmatics, we aimed to identify the underlying principles guiding their social behavior. METHODS: A cohort of 10 children (5 SDS, 5 matched c

Simple and early prediction of severe CAR-T-related adverse events after Axi-cel infusion by initial high fever.

Chimeric antigen receptor T-cell (CAR-T)-related adverse events (CAR-AEs), such as immune effector cell-associated neurotoxicity syndrome (ICANS) and cytokine release syndrome (CRS), can be life-threatening and may require high-dose steroids. Identifying patients at high risk for severe CAR-AEs in a simplified way is crucial for early therapeutic intervention. This retrospective study analyzed 44 patients treated with axicabtagene ciloleucel (Axi-cel) to identify predictive factors for severe CAR-AEs. We found that grade ≥ 3 ICANS, hemophagocytic syndrome, and ICU admission were associated wit

Immune Effector Cell-associated Hemophagocytic Lymphohistiocytosis-like Syndrome (IEC-HS).

Immune effector cell-associated hemophagocytic lymphohistiocytosis-like syndrome (IEC-HS) is a complication of adoptive cell therapy. Presenting with clinical manifestations of hyperinflammation and surrogate indicators of hyperinflammation such as elevations in serum ferritin and hepatic transaminases, decreasing cell counts, and hypofibrinogenemia, IEC-HS resembles primary and other forms of secondary hemophagocytic lymphohistiocytosis. Nonetheless, this is an iatrogenic complication resulting from the induction of hyperinflammatory pathways during T-cell-mediated anticancer targeting. Disti

Cytokine Storms in COVID-19, Hemophagocytic Lymphohistiocytosis, and CAR-T Therapy.

IMPORTANCE: Cytokine storm (CS) is a hyperinflammatory syndrome causing multiorgan dysfunction and high mortality, especially in patients with malignant hematologic neoplasms. Triggers include malignant neoplasm-associated hemophagocytic lymphohistiocytosis (MN-HLH), cytokine release syndrome from chimeric antigen receptor T-cell therapy (CAR-T CRS), and COVID-19, but the underlying mechanisms of inflammation and their impact on outcomes are poorly understood. OBJECTIVE: To delineate the inflammatory patterns characterizing different CS etiologies and their association with clinical outcomes.

Less frequent complications following CAR T-cell therapies: hemophagocytic lymphohistiocytosis, graft-versus-host disease, thrombotic microangiopathy, coagulation disorders and secondary malignancies: best practice recommendations from the EBMT Practice Harmonization and Guidelines Committee.

CAR T-cell therapies have revolutionized the treatment of hematologic malignancies; however, alongside the well-known complications of cytokine release syndrome (CRS), neurotoxicity, immune effector cell-associated hematotoxicity (ICAHT), and infections, other non-classical toxicities are emerging. This review, developed by the EBMT Practice Harmonization and Guidelines Committee, addresses the management of other critical post-CAR T-cell complications including hemophagocytic lymphohistiocytosis (HLH), graft-versus-host disease (GvHD), thrombotic microangiopathy (TMA), coagulation disorders a

Deep insight into cytokine storm: from pathogenesis to treatment.

Cytokine storm (CS) is a severe systemic inflammatory syndrome characterized by the excessive activation of immune cells and a significant increase in circulating levels of cytokines. This pathological process is implicated in the development of life-threatening conditions such as fulminant myocarditis (FM), acute respiratory distress syndrome (ARDS), primary or secondary hemophagocytic lymphohistiocytosis (HLH), cytokine release syndrome (CRS) associated with chimeric antigen receptor-modified T (CAR-T) therapy, and grade III to IV acute graft-versus-host disease following allogeneic hematopo

The OHI index predicts early mortality from organ dysfunction and survival benefit from etoposide in patients with lymphoma.

Hemophagocytic lymphohistiocytosis (HLH) is a hyperinflammatory syndrome that complicates hematologic malignancies. The Optimized HLH Inflammatory (OHI) index, based solely on the combined elevation of soluble CD25 (sCD25; >3900 U/mL) and ferritin (1000 ng/mL) levels, predicts mortality more effectively than the conventional HLH criteria. This study aimed to determine whether mortality in OHI-positive (OHI+) patients is primarily caused by lymphoma or inflammation-related causes. In a multicenter, retrospective study of patients with newly diagnosed lymphoma with sCD25 and ferritin measurement

Perspectives on the current diagnostic and treatment paradigms in secondary hemophagocytic lymphohistiocytosis (HLH).

Improved awareness of hemophagocytic lymphohistiocytosis (HLH) among clinicians has led to an increase in its diagnosis. Often diagnosis is made based on the HLH- 2004 criteria. While these criteria have considerable strengths, they lack specificity and may be fulfilled in the setting of many pro-inflammatory disorders. Genetic defects affecting cellular cytotoxicity cause familial (primary) HLH. On the other hand, secondary HLH is more a pathophysiologic process common to many conditions, rather than a singular disease entity. Improved genetic, immunologic, and functional testing have changed

[Radiology of cysts: Analysis and classification of pulmonary and hepato-pancreatic cysts on imaging].

Cysts are common abnormalities in imaging, exhibiting a wide range of localizations and pathophysiological mechanisms. Their classification is based on morphological and clinical criteria guiding both diagnostic and therapeutic management. This article primarily focuses on pulmonary cysts, while briefly addressing hepatic and pancreatic cysts. Pulmonary cystic lesions are often the result of infectious, inflammatory, neoplastic or lymphoproliferative processes. Thoracic computed tomography is the key diagnostic tool for their evaluation, helping to differentiate these lesions from other pulmon
