A Diagnostic Odyssey: Mevalonate Kinase Deficiency Revealed by Genetic Testing in Adulthood.

Mevalonate kinase deficiency (MKD) is a rare autosomal recessive autoinflammatory disorder with marked clinical heterogeneity, frequently leading to delayed diagnosis. We describe a 71-year-old woman with lifelong episodic inflammatory symptoms beginning in infancy, including recurrent fevers, lymphadenopathy, and gastrointestinal and mucocutaneous manifestations, later evolving into intermittent arthralgia, myalgia, and fatigue. A unifying diagnosis was established when genetic testing identified two missense pathogenic MVK variants consistent with compound heterozygous MKD, supported by elev

Smoldering multiple myeloma in transition: redefining early myeloma in the modern era.

Smoldering multiple myeloma (SMM) has long been viewed as an intermediate precursor state, monitored until symptoms or organ dysfunction signaled the transition to active multiple myeloma (MM). Over the past decade, however, major advances in molecular biology, modern imaging, and immunotherapy have reshaped our understanding of early plasma cell disorders. The recent FDA approval of daratumumab for high-risk SMM marks a historic turning point. For the first time, early intervention is not only feasible but clinically validated. This evolving framework challenges long-standing assumptions rega

MGUS prevalence among active-duty military personnel.

Monoclonal gammopathy of undetermined significance (MGUS) is a precursor condition for multiple myeloma, and although environmental exposures have been implicated in its pathogenesis, it is unknown whether exposure to open-air burn pits used during U.S. military operations in Iraq affects prevalence of MGUS in active-duty service members (ADSMs). ADSMs represent a distinct young and physically fit population, with higher proportions of male and Black individuals compared to the general U.S. population. This retrospective cohort study evaluated MGUS prevalence among ADSMs. Participants included

Renal presentation of diffuse large B-cell lymphoma emerging after pre-existing IgG4-related disease and monoclonal gammopathy of undetermined significance.

A woman in her late 70s with a congenital solitary kidney, end-stage renal disease on haemodialysis, prior biopsy-proven IgG4-related disease (IgG4-RD) involving the liver and porta hepatis and monoclonal gammopathy of undetermined significance presented after a fall. Trauma-related CT imaging revealed a 20 cm infiltrative renal mass with bulky retroperitoneal lymphadenopathy. The laboratory data showed persistently elevated serum IgG4 levels with stable kappa-to-lambda free light chain ratios. Biopsy of the renal mass confirmed aggressive diffuse large B-cell lymphoma with a high proliferativ

Anatomy of a Cohort: 40-Year Follow-Up of a Sjögren's Cohort.

Background: Sjögren's disease (SjD) is a chronic autoimmune rheumatic disorder primarily affecting exocrine glands, leading to dryness and systemic involvement. B-cell hyperactivity and autoantibody production drive its pathogenesis and contribute to increased lymphoma risk. Although several long-term studies exist, we present a review of a closely monitored cohort assessed over 40 years. Methods: Retrospective observational study at University College London Hospital included patients fulfilling the 2016 ACR/EULAR criteria for SjD between 1986-2025. Patients with associated SjD were excluded.

Monoclonal Gammopathy of Neurological Significance: Toward an Integrated Hematologic-Neurologic Perspective-A Single-Center Retrospective Study.

Monoclonal gammopathies of clinical significance (MGCSs) are entities in which a small hematological clone produces a monoclonal immunoglobulin capable of causing organ damage. Neurological involvement remains difficult to diagnose and treat, especially in the context of incidental monoclonal gammopathy of undetermined significance (MGUS)-peripheral neuropathy (PN) associations. We conducted a single-center retrospective study at Fundeni Clinical Institute, Bucharest, from January 2015 to December 2025. The reference population included 300 patients with MGUS. The diagnosis of MGNS was establi

Peptidomic Profiling Reveals Extracellular Matrix Remodeling Signatures Discriminative of Multiple Myeloma.

Multiple myeloma (MM) evolves from monoclonal gammopathy of undetermined significance (MGUS) and smoldering multiple myeloma (SMM) with annual progression rates of 1% and 10%, respectively. Current risk models don't fully capture the underlying dynamic molecular processes. We hypothesized that urinary peptides reflect disease-specific microenvironmental alterations in plasma cell dyscrasias. To test this hypothesis, capillary electrophoresis coupled to mass spectrometry (CE-MS) was applied to profile the urinary peptidome of 314 individuals, including a discovery group (42 MGUS, 27 SMM, 14 MM)

Recognising disease progression in MGUS and smouldering myeloma: Biomarkers, symptom monitoring and imaging.

The Tackling Early Morbidity and Mortality in Myeloma trial (TEAMM) trial recruited 977 newly diagnosed myeloma patients from 93 UK hospitals to assess the advantages and disadvantages of prophylactic antibiotics for the first 12 weeks. This paper analyses the 133 (14%) patients who had previously known precursor disease including monoclonal gammopathy of undetermined significance (MGUS) and smouldering myeloma (SMM), reporting the relative importance of blood-based biomarkers, imaging findings and symptoms in their management. Prior to progression with active myeloma, patients with precursor

Machine Learning Model Predicts Monoclonal Gammopathy Using Routine Laboratory Values.

PURPOSE: Monoclonal gammopathy (MG) is a disorder defined by the presence of monoclonal immunoglobulin. Monoclonal protein (M-protein) serves as an important biomarker for the spectrum of MG that includes plasma cell myeloma (PCM) and, its precursor, MG of undetermined significance (MGUS). Recent evidence suggest early identification of MGUS before the onset of PCM is associated with improvement in overall survival. Despite these benefits, the recognition rate of MG remains inadequate. Addressing this care gap would help identify at-risk patients who may benefit from targeted evaluation to pre

Recurrent optic neuritis as the first presentation of monoclonal gammopathy of undetermined significance: a case report.

BACKGROUND: Monoclonal gammopathy of undetermined significance (MGUS), which can be asymptomatic, is a premalignant proliferative disorder of plasma cells. The peripheral nervous system may be involved in MGUS, causing a monoclonal gammopathy associated peripheral neuropathy. However, involvement of the central nervous system (CNS) in MGUS was rarely reported. CASE PRESENTATION: Here we report a rare case of recurrent optic neuritis, as the first clinical presentation of MGUS, in an otherwise healthy 54-year-old Iranian male patient. He was treated with intravenous high-dose methylprednisolon

The systemic immune-inflammation biomarkers in Sjögren's disease: associations with disease activity and extraglandular manifestations in a multicentric Italian cohort.

Sjögren's disease (SjD) is an autoimmune exocrinopathy characterized by heterogeneous extraglandular manifestations (EGM). The systemic immune-inflammation biomarkers, including the neutrophil-to-lymphocyte ratio (NLR), platelet-to-lymphocyte ratio (PLR), and systemic immune-inflammation index (SII), have been proposed as indicators of the inflammatory burden, but data in SjD remain limited. This retrospective multicenter Italian study aimed to evaluate the NLR, PLR, and SII in exocrine SjD patients and across specific EGM. Consecutive patients fulfilling the 2016 ACR/EULAR classification crit

Effective interleukin-6 inhibition in a pediatric patient with mevalonate kinase deficiency and chronic nonbacterial osteomyelitis-like bone lesions under interleukin-1 blockade.

BACKGROUND: Mevalonate kinase deficiency (MKD) is a rare autosomal recessive autoinflammatory disease. Chronic nonbacterial osteomyelitis (CNO) represents another autoinflammatory disorder characterized by sterile bone inflammation. Although musculoskeletal pain is common in MKD, CNO-like bone lesions have rarely been described. Tocilizumab has been used separately in MKD / hyper-IgD syndrome (HIDS) and in CNO; however, pediatric cases showing both conditions together and responding completely to IL-6 blockade have not been previously reported. CASE PRESENTATION: We present a 16-year-old boy

Smoldering Multiple Myeloma: From Clinical to Immunogenomic Risk Stratification and Therapeutic Implications of Early Intervention.

Smoldering multiple myeloma (SMM) is an asymptomatic precursor state that precedes the development of symptomatic active MM. Newer immunogenomic data highlight the monoclonal gammopathy of undetermined significance-SMM-MM spectrum as a continuous genomic gradient rather than a series of discrete clinical categories. Current risk stratifications to define high-risk SMM use a progression rate of ≥50% within 2 years. The International Myeloma Working Group uses the 20/2/20 model that identifies serum M-protein >2 g/dL, bone marrow plasma cells >20%, and involved/uninvolved free light chain ratio

Prevalence of monoclonal gammopathy of undetermined significance (MGUS) using a sensitive mass spectrometry assay in young individuals 10-49 years of age: a population-based study from the National Health and Nutritional Examination Survey.

The prevalence of monoclonal gammopathy of undetermined significance (MGUS) increases with age, but the study of age of onset and the prevalence of MGUS in younger individuals has been limited by lack of sensitive serum protein testing methods. In addition, there is marked racial disparity in the prevalence in Black individuals compared with White individuals and it is important to determine if MGUS has an earlier age of onset in Black individuals. In this first large mass spectrometry-based population-based screening study of MGUS, we evaluated 12,378 individuals (3598 White, 4075 Black, 4147

Distinct molecular profile in Sjögren's syndrome is associated with disease activity and clinical manifestations.

Sjögren's disease (SjD) is a systemic autoimmune condition for which reliable diagnostic and stratification biomarkers are still lacking. Several molecules have been implicated in its pathogenesis, and this study aimed to evaluate their potential as biomarkers in a monocentric cohort of SjD patients. Sera from 40 SjD patients (2016 ACR/EULAR criteria) and 41 matched controls were analyzed for 23 soluble inflammatory mediators using a Luminex® multiplex immunoassay. Clinical and laboratory correlations were examined, and multivariate logistic regression was performed. Compared with controls, Sj

Interleukin-17 in Health and Disease: Special Focus on Its Role in Autoimmune Hepatitis.

Autoimmune hepatitis (AIH) is a progressive inflammatory liver disease characterized by hypergammaglobulinemia, circulating antibodies, and distinctive histological features, with a higher prevalence in females. Immune responses targeting hepatic antigens are considered the main mechanism behind AIH. Many cytokines are involved in the inflammatory response typical of this disease. Interleukin 17 (IL-17) is a powerful pro-inflammatory protein that serves as a key link between the innate and adaptive immune systems. It plays an important role in regulating the inflammatory response in various ti
