Human immunoglobulin G (IgG) can be divided into four subclasses that are selectively expressed. For instance, carbohydrate antigens preferentially elicit IgG2 antibodies, whereas protein antigens usually elicit IgG1 and IgG3. Elucidating the biological basis of the selective expression of these IgG subclasses is important to our understanding immunodeficiencies and B lymphocyte development. To investigate clinical importance of IgG subclass deficiencies, a sensitive and specific assay has been developed for IgG subclasses using particle concentration fluorescence immunoassay. Preliminary clin

Immunoglobulin replacement therapy in the form of intravenous immunoglobulin (IVIgG) is clearly of benefit in primary hypogammaglobulinaemia and related disorders involving antibody deficiency. However, its use in the prevention of infection in other conditions is controversial and needs to be clarified. IVIgG therapy may be of benefit in selected patients with IgG subclass deficiency, and with a proven history of recurrent upper respiratory tract infections. IVIgG therapy may also benefit infants with AIDS and recurrent bacterial infections, but is only rarely of value in cases of adult AIDS.

The requirements for a present-day IVIG preparation are outlined. These are mainly: fully preserved activities of the recognition and effector functions of the IgG molecule, a normal subclass distribution, and a normal half-life after infusion. The therapeutic uses of IVIG preparations are discussed as follows: Antibody substitution in cases of generalized or partial antibody deficiency in immune-compromised patients. These include the following diseases: hypogammaglobulinemia (congenital and acquired, including the neonates); drug-induced and viral immunosuppression. Antibody substitution in

IgG subclasses differ not only in their biochemical and biologic properties, but also in their occurrence as selective deficiencies, alone and in combination. The normal range of IgG subclass concentrations in children and adults is compared with deficiencies found in individuals with repeated respiratory tract infections. Concomitant IgA deficiency is often noted. It is suggested that decreased IgG2 and IgG3 levels in IgA-deficient individuals may increase the risk of recurrent respiratory infection and lung damage. Finally, the efficacy immunoglobulin. prophylaxis in reducing of infection is

19S IgM rheumatoid factors (RF) may play an important role in sustaining inflammation in rheumatoid arthritis (RA). As yet, no unique antigenic specificity for RF in RA has been identified. Because the synovium is central to the pathogenic changes in RA, RF produced therein might be pathogenically more important than serum RF. Therefore, we examined the reactivity and relative avidity of 19S IgM-RF in serum and rheumatoid synovial cells (RSC) from 20 patients with seropositive RA. Reactivities were determined by competitive inhibition of serum RF hemolytic activity and RSC RF-plaque-forming ce

Five male military recruits were studied for evidence of immunologic deficiencies. Four had recovered recently from a first-episode meningococcal meningitis and one presented with recurrent meningococcemia. A selective decrease in IgG and IgA was found in one of the first-episode patients and a selective IgG2 subclass deficiency in the recurrent case. No complement abnormalities were detected, however, in any of the five cases. These results present further proof of the crucial role of immunoglobulins in the defense against N. meningitidis and they stress the necessity of a through examination

Ten survivors of neonatal HSV encephalitis had IgG1 and IgG3 antibodies to HSV 6 or more months after their infection. Six had IgG4 antibodies but only one had IgG2 antibodies. Titers of anti-HSV antibodies were similar following neonatal and later HSV infections. Antibody deficiency is thus not part of the selective deficiency of HSV-specific immunity following neonatal infection.

We report the first specific human immunoglobulin subclass probe which was obtained by subcloning the gamma 3 hinge region. This specific gamma 3 probe allowed us to identify with certainty the C gamma 3 gene on Southern genomic blots, to describe the first C gamma 3 restriction fragment length polymorphism (EZZ gamma 3 RF) and to show that an IgG3 selective deficiency, previously described serologically, was not due to a deletion of the C gamma 3 gene. Such a probe should be particularly useful for screening libraries from individuals with IgG3 immunodeficiencies or presenting unusual C gamma

We studied 20 children with recurrent sinopulmonary infections and serum IgG levels within the normal range, who had selective IgG-subclass deficiency. Twelve of the children were IgG2 deficient, five were IgG3 deficient, and three were deficient in both IgG2 and IgG3. IgA deficiency was present in 3 of the 20 patients. In the children with IgG2 deficiency, serum antibody concentrations to the capsular polysaccharide of Hemophilus influenzae type B (Hib) were significantly lower than those in age-matched controls, both before and after immunization with the Hib capsular polysaccharide antigen,

Serum IgG subclasses and IgG, IgA and IgM were measured in 47 patients with chronic respiratory tract infection, 53 patients with recurrent acute respiratory infections and 66 healthy normal adults. Compared to the healthy control group, mean serum IgG2 was significantly lower in patients with chronic infection (P less than 0.01) and in those with recurrent infection (P less than 0.001), whereas mean serum IgG3 was significantly higher (P less than 0.001 for those with chronic and P less than 0.05 for those with recurrent infection). Five patients with recurrent infection and two with chronic

Mouse monoclonal antibodies to various human epidermal and basement membrane components were formed by immunizing Balb/c mice with ME-180, a line of human cervical carcinoma cells. The spleen cells from hyperimmunized mice were fused with a nonsecreting mouse myeloma cell line using polyethylene glycol. The resulting hybrids were selected by growth in media containing 20% fetal calf serum, hypoxanthine, thymidine, and methotrexate in RPMI-1640 in 24-well Linbro plates. Wells producing antibodies of interest were grown and eventually cloned over an HGPRT- rat fibroblast feeder layer. These cult

5 sera from a series of more than 1,000 serum samples from hospitalized patients tested by immunoelectrophoresis were selected for further examination because of a disproportional increase in the anodic part of IgG. A marked polyclonal increase in IgG4 subclass level was detected in each of these sera. While the patients suffered from a variety of diseases (e.g. Loeffler's syndrome, leiomyosarcoma, periarteritis nodosa, chronic bronchitis, and mycosis fungoides, respectively), they all had an acquired respiratory disease as the only common clinical denominator.

The results of studies in a patient with congenital deficiency of Factor XI who developed an inhibitor are presented. The patient presented with a severe, apparently spontaneous bleed into the thigh, which progressed despite infusion of fresh frozen plasma, but which responded promptly to activated prothrombin complex. During therapy with plasma his clotting time and Factor XI level were unresponsive and a Factor XI inhibitor titer of 6,000 U/ml was attained. The inhibitor was isolated and found to be polyclonal immunoglobulin G (IgG), predominantly of subclass 4. The specificity of the antibo

CBA/N mice and F1 crosses of CBA/N X BALB/c with the CBA/N phenotype respond to immunization with PC-LPS with a PC-specific and an anti-bridge antibody production. The PC-specific response in defective CBA/N and NBF1 is devoid of the IgG3 subclass and is not T15 idiotype dominant, whereas normal BALB/c and nondefective NBF1 mice express the T15 dominantly in their anti-PC-LPS response. By the criteria of responsiveness to PC-LPS only and the absence of dominant T15 expression, the precursors in defective NBF1 mice for TI-1 antigen PC-LPS can be characterized as being immature B cells similar t

Individuals with selective absence of IgG1 and IgG2 were discovered by testing for allotypes and isotypes of the respective sub-classes. These individuals were homozygous for sub-class deleted Gm-Am haplotypes, as shown by allotype studies in two families (Gm--;..;g;A2m1/Gm--;n;b;A2m1 and Gm--;n;b;A2m1/Gm--;..;b;A2m1) and by a population study of New Guineans (Gm fa;--;b;A2m2). The individuals with IgG1 sub-class deficiency showed elevation of IgG2, IgG4 and in particular of IgG3. Gene deletion can result from unequal crossing over which renders a complementary chromosome with a duplication of

Determinations of IgG subclasses were made by electroimmunoassay and crossed immunoelectrophoresis, and Gm markers were typed in sera from seventeen patients with well-defined immunodeficiency diseases. Certain IgG subclass and Gm patterns were recognized in various diseases: IgG2 deficiency and homozygosity of Gm (4,5) in the cartilage-hair-hypoplasia syndrome, in the ataxia telangiectasia syndrome and in selective IgG subclass deficiency; and IgG3 deficiency and homozygosity of Gm(1,-5) in the Wiskott-Aldrich syndrome. The findings suggest a common structural or regulator gene defect in some

Immunoglobulin levels of individual classes and IgG subclasses and the occurrence of homogeneous immunoglobulins--paraproteins--were studied longitudinally in the sera of three patients with the Wiskott-Aldrich syndrome; Common findings in all three patients were great variations in the immunoglobulin levels, restricted heterogeneity of the immunoglobulins, the frequent appearance of transient homogeneous immunoglobulins and the presence of serum antibodies against bovine milk proteins. A partial and selective deficiency involving mainly the T immune system is postulated as an explanation for
