Concurrent hereditary angioedema type I and common variable immunodeficiency presenting as suspected antibiotic hypersensitivity: Case report and literature discussion.

Hereditary angioedema (HAE) and common variable immunodeficiency (CVID) are rare disorders with heterogeneous clinical presentations that pose significant diagnostic challenges. HAE is characterized by recurrent episodes of subcutaneous or submucosal edema affecting the skin, gastrointestinal tract, or airway, whereas CVID presents with variable immune dysfunction, recurrent infections, and impaired vaccine responses. The clinical manifestations of these conditions can mimic allergic reactions, drug hypersensitivity, mast cell disorders, autoimmune diseases, and other immunodeficiencies, makin

Immunoglobulin subclass profiles in depression: Insights from a general population study.

The immune system plays a crucial role in defending the body against harmful invaders, with immunoglobulins being key players in pathogen recognition and neutralization. Immunoglobulin dysregulation is linked to various diseases, including autoimmune disorders, immunodeficiencies, and allergies. Previous studies suggest that depression can alter immune function by modifying immunoglobulin production, which may contribute to increased vulnerability to infections and autoimmune diseases. In this study, we examined the association between the immunoglobulin (Ig) subclasses IgA, IgG, IgM, and IgE

Persistently Elevated Serum IgM: A Diagnostic Red Flag for Primary Immunodeficiencies: A Case-Based Review and Clinical Approach.

INTRODUCTION/OBJECTIVE: Persistently elevated serum immunoglobulin M (IgM) is commonly interpreted as a marker of acute infection, autoimmune activity, or lymphoproliferation. Sustained IgM elevation may represent an underrecognized marker of underlying primary immunodeficiency disorders (PIDs), especially those associated with class-switch recombination defects or immune dysregulation. This article aims to highlight the diagnostic value of persistently elevated IgM levels in pediatric PIDs and to propose a practical clinical framework for the evaluation of children with unexplained IgM elevat

HAMSIAA protects cyclophosphamide induced immunosuppression by modulating gut homeostasis and purine metabolism.

INTRODUCTION: Dietary supplementation with functional starches serves as a promising intervention strategy for immune regulation, yet the immunomodulatory efficacy and underlying mechanisms of newly developed modified starch derivatives remain to be further explored. In this study, a cyclophosphamide (CTX) induced secondary immunodeficiency mouse model was adopted to investigate the immunoregulatory effects of indole 3 acetylated high amylose maize starch derivative (HAMSIAA). METHODS: Mice were fed a basal diet supplemented with 0.5% or 1% HAMSIAA for nine consecutive days, with CTX administ

Diagnostic performance of molecular and serological markers for hepatitis E virus and Toxoplasma gondii coinfection across gestation in HIV-positive pregnant women.

BACKGROUND: Accurate identification of recent hepatitis E virus (HEV) and Toxoplasma gondii infections during pregnancy is essential for clinical management, yet diagnostic performance data in HIV-positive populations remain limited. We evaluated serological and molecular diagnostic approaches for detecting recent coinfection across gestation. METHODS: In a prospective cohort of 780 HIV-positive pregnant women (CD4 + 200-400 cells/µL) in central Nigeria, we performed serial HEV and T. gondii IgG/IgM testing using recomLine immunoblot assays. A validation subset (n = 80) underwent HEV RNA dete

Toxoplasma Gondii IGG seroprevalence among individuals living with HIV: An 18-year single-center retrospective evaluation.

BACKGROUND/AIM: Toxoplasma gondii is an important opportunistic pathogen in people living with HIV (PLWH), and the risk of reactivation increases markedly when CD4 counts fall below 100/mm³ . We aimed to determine T. gondii seroprevalence in PLWH, assess its associations with demographic and clinical variables, and emphasize preventive strategies for seronegative individuals. MATERIALS AND METHODS: This retrospective cohort study analyzed data from 1419 people living with HIV who were diagnosed by Western blot and followed at a tertiary care center between January 2006 and January 2024.Demogr

Lyell´s syndrome associated with the use of topical agents: case report.

UNLABELLED: Pseudomonas aeruginosa AG1 (PaeAG1) is a Costa Rican strain that was isolated in 2010 in a major Hospital. This strain has resistance to multiple antibiotics such as β-lactams (including carbapenems), aminoglycosides, and fluoroquinolones. PaeAG1 is considered critical (Priority 1) due to its resistance to carbapenems, and it was the first report of a P. aeruginosa isolate carrying both VIM-2 and IMP-18 genes encoding for metallo-β-lactamases (MBL) enzymes (both with carbapenemase activity). Owing to these traits, we have studied this model for 10 years using diverse approaches inc

Lack of evidence of acute HEV infections as a sexually transmitted disease: Data from a German cohort of PrEP users.

BACKGROUND: While the sexual transmissibility of HAV in MSM has been extensively described, the potential for sexual transmission of HEV has not been definitively established. Although HEV has been detected in the ejaculate of chronically infected men, studies among MSM PrEP users in France did not observe an elevated anti-HEV seroprevalence as an indicator of increased exposure risk by sexual intercourse. PATIENTS AND METHODS: A total of 111 unselected PrEP users and 111 age- and sex-matched blood donors were tested for anti-HEV IgG, IgM and HEV (PCR). Of the participants 79/111 (71 %) respo

Prevalence of anti-lymphocyte IgM autoantibodies driving complement activation in COVID-19 patients.

INTRODUCTION: COVID-19 patients can develop autoantibodies against a variety of secreted and membrane proteins, including some expressed on lymphocytes. However, it is unclear what proportion of patients might develop anti-lymphocyte antibodies (ALAb) and what functional relevance they might have. METHODS: We evaluated the presence and lytic function of ALAb in the sera of a cohort of 85 COVID-19 patients (68 unvaccinated and 17 vaccinated) assigned to mild (N=63), or moderate/severe disease (N=22) groups. Thirty-seven patients were followed-up after recovery. We also analyzed in vivo complem

Refractory and Recurrent Skin Manifestations in an Adult With Selective Immunoglobulin M Deficiency.

Selective immunoglobulin M (IgM) deficiency (sIgMD) is a rare immunodeficiency disorder characterized by decreased serum levels of IgM. Symptoms of sIgMD include repeated infections and allergic manifestations such as asthma and allergic rhinitis. The etiology and pathology of sIgMD remain largely unknown. Moreover, no genetic cause of sIgMD and associated symptoms has been established. Herein, we describe a 47-year-old female with sIgMD who presented with repeated fevers of unknown cause since childhood. She was referred to our department because of recently developed severe dermatitis withou
